    A lower limb assessment tool for athletes at risk of developing patellar tendinopathy

    Achilles tendinopathy is a common injury in running sports however the exact etiology of Achilles injury is still unclear. In recent years, altered neuromotor recruitment patterns of the triceps surae have been hypothesized to create differential intra-tendinous loads leading to pathology; however, this hypothesis has not been investigated. Further, the effect foot orthoses may have on neuromotor recruitment of the triceps surae in Achilles tendinopathy has not been investigated. Methods: The electromyographic activity of the triceps surae was recorded during an over-ground running task. Fifteen Achilles injured participants and 19 asymptomatic controls were assessed in a footwear only condition. The Achilles injured participants were also assessed running in a pre-fabricated foot orthoses. Results: In Achilles injured participants, there was a significant difference between soleus and lateral gastrocnemius offset times during running compared to the asymptomatic controls (p \u3c 0.05). There were no significant differences in triceps surae muscle activity between the footwear only and footwear and orthoses condition in the Achilles injured participants. Conclusions: The finding that triceps surae activity is altered in participants with Achilles tendinopathy may have clinical importance as it suggests that intra-tendinous loads are altered which may contribute to pathological changes. Further, foot orthoses have no immediate effect on the neuromotor control of the triceps surae

    CAN WE ALTER YOUTH ATHLETE’S LANDING STRATEGY IN A STOP-JUMP MOVEMENT?

    The purpose of this study was to determine the effect of four different neuromuscular training programs on the kinematic landing patterns of pre elite youth athletes during a stop-jump movement. Eighty-nine pre-elite youth athletes from the Western Region Academy of Sports Basketball, Netball, Softball, Triathlete and Hockey squads were recruited for biomechanical analysis before and after the completion of one of four randomly assigned 12-week training intervention programs, in conjunction with a strength and conditioning program. Results of this study identify youth athletes with poor movement competency acquire similar alterations in their kinematic landing pattern regardless of the type of training intervention completed, identifying that a basic strength and conditioning program can implemented to alter landing technique in a stop-jump task

    An analysis of long-term relationships among count statistics and metrics of synthetic tropical cyclones downscaled from CMIP5 models

    In a changing climate, the impact of tropical cyclones on the United States Atlantic and Gulf Coasts will be affected both by how intense and how frequent these storms become. The observational record of tropical cyclones in the Atlantic Basin is too short (A.D. 1851 to present) to allow for accurate assessment of low-frequency variability in storm activity. In order to overcome the limitations of the short observational record, we downscale four Coupled Model Intercomparison Project Phase 5 models to generate synthetic tropical cyclone data sets for the Atlantic Basin that span the interval of A.D. 850–2005. Using these long-term synthetic tropical cyclone data sets, we investigate the relationship between power dissipation and ocean temperature metrics, as well as the relationship between basin-wide and landfalling tropical cyclone count statistics over the past millennium. Contrary to previous studies, we find only a very weak relationship between power dissipation and main development region sea surface temperature in the Atlantic Basin. Consistent with previous studies, we find that basin-wide and landfalling tropical cyclone counts are significantly correlated with one another, lending further support for the use of paleohurricane landfall records to infer long-term basin-wide tropical cyclone trends.United States.     Movement Competency Screens Can Be Reliable In Clinical Practice By A Single Rater Using The Composite Score

    Background: Movement competency screens (MCSs) are commonly used by coaches and clinicians to assess injury risk. However, there is conflicting evidence regarding MCS reliability. Purpose: This study aimed to: (i) determine the inter- and intra-rater reliability of a sport specific field-based MCS in novice and expert raters using different viewing methods (single and multiple views); and (ii) ascertain whether there were familiarization effects from repeated exposure for either raters or participants. Study Design: Descriptive laboratory study Methods: Pre-elite youth athletes (n=51) were recruited and videotaped while performing a MCS comprising nine dynamic movements in three separate trials. Performances were rated three times with a minimal four-week wash out between testing sessions, each in randomized order by 12 raters (3 expert, 9 novice), using a three-point scale. Kappa score, percentage agreement and intra-class correlation were calculated for each movement individually and for the composite score. Results: Fifty-one pre-elite youth athletes (15.0±1.6 years; n=33 athletics, n=10 BMX and n=8 surfing) were included in the study. Based on kappa score and percentage agreement, both inter- and intra-rater reliability were highly variable for individual movements but consistently high (>0.70) for the MCS composite score. The composite score did not increase with task familiarization by the athletes. Experts detected more movement errors than novices and both rating groups improved their detection of errors with repeated viewings of the same movement. Conclusions: Irrespective of experience, raters demonstrated high variability in rating single movements, yet preliminary evidence suggests the MCS composite score could reliably assess movement competency. While athletes did not display a familiarization effect after performing the novel tasks within the MCS for the first time, raters showed improved error detection on repeated viewing of the same movement

    Impact of climate change on New York City’s coastal flood hazard: Increasing flood heights from the preindustrial to 2300 CE

    The flood hazard in New York City depends on both storm surges and rising sea levels. We combine modeled storm surges with probabilistic sea-level rise projections to assess future coastal inundation in New York City from the preindustrial era through 2300 CE. The storm surges are derived from large sets of synthetic tropical cyclones, downscaled from RCP8.5 simulations from three CMIP5 models. The sea-level rise projections account for potential partial collapse of the Antarctic ice sheet in assessing future coastal inundation. CMIP5 models indicate that there will be minimal change in storm-surge heights from 2010 to 2100 or 2300, because the predicted strengthening of the strongest storms will be compensated by storm tracks moving offshore at the latitude of New York City. However, projected sea-level rise causes overall flood heights associated with tropical cyclones in New York City in coming centuries to increase greatly compared with preindustrial or modern flood heights. For the various sea-level rise scenarios we consider, the 1-in-500-y flood event increases from 3.4 m above mean tidal level during 1970–2005 to 4.0–5.1 m above mean tidal level by 2080–2100 and ranges from 5.0–15.4 m above mean tidal level by 2280–2300. Further, we find that the return period of a 2.25-m flood has decreased from ∼500 y before 1800 to ∼25 y during 1970–2005 and further decreases to ∼5 y by 2030–2045 in 95% of our simulations. The 2.25-m flood height is permanently exceeded by 2280–2300 for scenarios that include Antarctica’s potential partial collapse

    Attempts to Image the Early Inflammatory Response during Infection with the Lymphatic Filarial Nematode Brugia pahangi in a Mouse Model

    Helminth parasites remain a major constraint upon human health and well-being in many parts of the world. Treatment of these infections relies upon a very small number of therapeutics, most of which were originally developed for use in animal health. A lack of high throughput screening systems, together with limitations of available animal models, has restricted the development of novel chemotherapeutics. This is particularly so for filarial nematodes, which are long-lived parasites with a complex cycle of development. In this paper, we describe attempts to visualise the immune response elicited by filarial parasites in infected mice using a non-invasive bioluminescence imaging reagent, luminol, our aim being to determine whether such a model could be developed to discriminate between live and dead worms for in vivo compound screening. We show that while imaging can detect the immune response elicited by early stages of infection with L3, it was unable to detect the presence of adult worms or, indeed, later stages of infection with L3, despite the presence of worms within the lymphatic system of infected animals. In the future, more specific reagents that detect secreted products of adult worms may be required for developing screens based upon live imaging of infected animals

    Psychotropic medication use among nursing home residents in Austria: a cross-sectional study

    <p>Abstract</p> <p>Background</p> <p>The use of psychotropic medications and their adverse effects in frail elderly has been debated extensively. However, recent data from European studies show that these drugs are still frequently prescribed in nursing home residents. In Austria, prevalence data are lacking. We aimed to determine the prevalence of psychotropic medication prescription in Austrian nursing homes and to explore characteristics associated with their prescription.</p> <p>Methods</p> <p>Cross-sectional study and association analysis in forty-eight out of 50 nursing homes with 1844 out of a total of 2005 residents in a defined urban-rural region in Austria. Prescribed medication was retrieved from residents' charts. Psychotropic medications were coded according to the Anatomical Therapeutic Chemical Classification 2005. Cluster-adjusted multiple logistic regression analysis was performed to investigate institutional and residents' characteristics associated with prescription.</p> <p>Results</p> <p>Residents' mean age was 81; 73% of residents were female. Mean cluster-adjusted prevalence of residents with at least one psychotropic medication was 74.6% (95% confidence interval, CI, 72.0–77.2). A total of 45.9% (95% CI 42.7–49.1) had at least one prescription of an antipsychotic medication. Two third of all antipsychotic medications were prescribed for bedtime use only. Anxiolytics were prescribed in 22.2% (95% CI 20.0–24.5), hypnotics in 13.3% (95% CI 11.3–15.4), and antidepressants in 36.8% (95% CI 34.1–39.6) of residents. None of the institutional characteristics and only few residents' characteristics were significantly associated with psychotropic medication prescription. Permanent restlessness was positively associated with psychotropic medication prescription (AOR 1.54, 95% CI 1.32–1.79) whereas cognitive impairment was inversely associated (AOR 0.70, 95% CI 0.56–0.88).</p> <p>Conclusion</p> <p>Frequency of psychotropic medication prescription is high in Austrian nursing homes compared to recent published data from other countries. Interventions should aim at reduction and optimisation of prescriptions.</p

    GA4GH: International policies and standards for data sharing across genomic research and healthcare.

    The Global Alliance for Genomics and Health (GA4GH) aims to accelerate biomedical advances by enabling the responsible sharing of clinical and genomic data through both harmonized data aggregation and federated approaches. The decreasing cost of genomic sequencing (along with other genome-wide molecular assays) and increasing evidence of its clinical utility will soon drive the generation of sequence data from tens of millions of humans, with increasing levels of diversity. In this perspective, we present the GA4GH strategies for addressing the major challenges of this data revolution. We describe the GA4GH organization, which is fueled by the development efforts of eight Work Streams and informed by the needs of 24 Driver Projects and other key stakeholders. We present the GA4GH suite of secure, interoperable technical standards and policy frameworks and review the current status of standards, their relevance to key domains of research and clinical care, and future plans of GA4GH. Broad international participation in building, adopting, and deploying GA4GH standards and frameworks will catalyze an unprecedented effort in data sharing that will be critical to advancing genomic medicine and ensuring that all populations can access its benefits
