Acute coronary syndromes amongst type 2 diabetics with ischaemic electrocardiograms presenting to accident and emergency department of a Kenyan tertiary institution
Objective: To determine the prevalence of acute coronary syndromes among type 2 diabetic patients presenting to Accident and Emergency department. Design: Prospective cross-sectional study. Setting: Kenyatta National Hospital, a tertiary teaching and referral hospital. Subjects: Type 2 diabetic patients with ischaemic electrocardiograms (ECG). Main outcome measures: Demographics, clinical symptoms, cardiovascular status and risk factors - central obesity, hypertension, dyslipidaemia, smoking. Results: From 12,307 accident and emergency attendees, 400 ( 33%) diabetics aged >30 years were screened with a resting ECG and 95 (24%) with ischaemic ECG were recruited; age range 41-87 years, 60% were male; diabetes duration ranged 0-30 years with 8.4% being newly diagnosed. The commonest enrolling ECG feature was nonspecific ST-T changes. The commonest presenting complaint were fatigue and dyspnoea. Majority had three coronary artery disease (CAD) risk factors: obesity 86%, elevated LDL 73% and hypertension 60%. Therapy in use was OHA 43%, insulin 42%, insulin and OHA 1%; prophylactic aspirin 14.7% and statins 8.4%. Thirty four (35.8%) were classified as acute coronary syndrome (ACS); 29 ( 30.5%) acute myocardial infarction (ACS-AMI) and five (5.2%) unstable angina (ACS-UA). Majority (79.4%) of the ACS presented more than six hours after symptom onset and majority had features of acute left ventricular failure. Conclusions: Acute coronary syndrome accounted for 35% of the morbidity in type 2 diabetics with ischaemic ECG’s presenting to KNH accident and emergency department;patients presented late and 80% were not on CAD prophylactic therapy
Analytic and Asymptotic Methods for Nonlinear Singularity Analysis: a Review and Extensions of Tests for the Painlev\'e Property
The integrability (solvability via an associated single-valued linear
problem) of a differential equation is closely related to the singularity
structure of its solutions. In particular, there is strong evidence that all
integrable equations have the Painlev\'e property, that is, all solutions are
single-valued around all movable singularities. In this expository article, we
review methods for analysing such singularity structure. In particular, we
describe well known techniques of nonlinear regular-singular-type analysis,
i.e. the Painlev\'e tests for ordinary and partial differential equations. Then
we discuss methods of obtaining sufficiency conditions for the Painlev\'e
property. Recently, extensions of \textit{irregular} singularity analysis to
nonlinear equations have been achieved. Also, new asymptotic limits of
differential equations preserving the Painlev\'e property have been found. We

Semiclassical Instability of the Cauchy Horizon in Self-Similar Collapse
Generic spherically symmetric self-similar collapse results in strong
naked-singularity formation. In this paper we are concerned with particle
creation during a naked-singularity formation in spherically symmetric
self-similar collapse without specifying the collapsing matter. In the generic
case, the power of particle emission is found to be proportional to the inverse
square of the remaining time to the Cauchy horizon (CH). The constant of
proportion can be arbitrarily large in the limit to marginally naked
singularity. Therefore, the unbounded power is especially striking in the case
that an event horizon is very close to the CH because the emitted energy can be
arbitrarily large in spite of a cutoff expected from quantum gravity. Above
results suggest the instability of the CH in spherically symmetric self-similar
spacetime from quantum field theory and seem to support the existence of a
semiclassical cosmic censor. The divergence of redshifts and blueshifts of
emitted particles is found to cause the divergence of power to positive or
negative infinity, depending on the coupling manner of scalar fields to
gravity. On the other hand, it is found that there is a special class of
self-similar spacetimes in which the semiclassical instability of the CH is not
efficient. The analyses in this paper are based on the geometric optics
approximation, which is justified in two dimensions but needs justification in

Generating Neutrino Mass in the 331 Model
A mechanism for generating small tree-level Majorana mass for neutrinos is
implemented in the 331 Model. No additional fermions or scalars need to be

Search for Higgs bosons decaying to tautau pairs in ppbar collisions at sqrt(s) = 1.96 TeV
We present a search for the production of neutral Higgs bosons decaying into
tautau pairs in ppbar collisions at a center-of-mass energy of 1.96 TeV. The
data, corresponding to an integrated luminosity of 5.4 fb-1, were collected by
the D0 experiment at the Fermilab Tevatron Collider. We set upper limits at the
95% C.L. on the product of production cross section and branching ratio for a
scalar resonance decaying into tautau pairs, and we then interpret these limits
as limits on the production of Higgs bosons in the minimal supersymmetric
standard model (MSSM) and as constraints in the MSSM parameter space.Comment: 7 pages, 5 figures, submitted to PL
Measurement of the photon-jet production differential cross section in collisions at \sqrt{s}=1.96~\TeV
We present measurements of the differential cross section dsigma/dpT_gamma
for the inclusive production of a photon in association with a b-quark jet for
photons with rapidities |y_gamma|< 1.0 and 30<pT_gamma <300 GeV, as well as for
photons with 1.5<|y_gamma|< 2.5 and 30< pT_gamma <200 GeV, where pT_gamma is
the photon transverse momentum. The b-quark jets are required to have pT>15 GeV
and rapidity |y_jet| < 1.5. The results are based on data corresponding to an
integrated luminosity of 8.7 fb^-1, recorded with the D0 detector at the
Fermilab Tevatron Collider at sqrt(s)=1.96 TeV. The measured cross
sections are compared with next-to-leading order perturbative QCD calculations
using different sets of parton distribution functions as well as to predictions
based on the kT-factorization QCD approach, and those from the Sherpa and

Limits on anomalous trilinear gauge boson couplings from WW, WZ and Wgamma production in pp-bar collisions at sqrt{s}=1.96 TeV
We present final searches of the anomalous gammaWW and ZWW trilinear gauge
boson couplings from WW and WZ production using lepton plus dijet final states
and a combination with results from Wgamma, WW, and WZ production with leptonic
final states. The analyzed data correspond to up to 8.6/fb of integrated
luminosity collected by the D0 detector in pp-bar collisions at sqrt{s}=1.96
TeV. We set the most stringent limits at a hadron collider to date assuming two
different relations between the anomalous coupling parameters
Delta\kappa_\gamma, lambda, and Delta g_1^Z for a cutoff energy scale Lambda=2
TeV. The combined 68% C.L. limits are -0.057<Delta\kappa_\gamma<0.154,
-0.015<lambda<0.028, and -0.008<Delta g_1^Z<0.054 for the LEP parameterization,
and -0.007<Delta\kappa<0.081 and -0.017<lambda<0.028 for the equal couplings
parameterization. We also present the most stringent limits of the W boson

Measurement of three-jet differential cross sections d sigma-3jet / d M-3jet in p anti-p collisions at sqrt(s)=1.96 TeV
We present the first measurement of the inclusive three-jet differential
cross section as a function of the invariant mass of the three jets with the
largest transverse momenta in an event in p anti-p collisions at sqrt(s) = 1.96
TeV. The measurement is made in different rapidity regions and for different
jet transverse momentum requirements and is based on a data set corresponding
to an integrated luminosity of 0.7 fb^{-1} collected with the D0 detector at
the Fermilab Tevatron Collider. The results are used to test the three-jet
matrix elements in perturbative QCD calculations at next-to-leading order in
the strong coupling constant. The data allow discrimination between

Search for pair production of the scalar top quark in muon+tau final states
We present a search for the pair production of scalar top quarks
(), the lightest supersymmetric partners of the top quarks, in
collisions at a center-of-mass energy of 1.96 TeV, using data
corresponding to an integrated luminosity of {7.3 } collected with the
\dzero experiment at the Fermilab Tevatron Collider. Each scalar top quark is
assumed to decay into a quark, a charged lepton, and a scalar neutrino
(). We investigate final states arising from and
. With no significant excess of events observed above the
background expected from the standard model, we set exclusion limits on this
