Feasibility studies of time-like proton electromagnetic form factors at PANDA at FAIR
Simulation results for future measurements of electromagnetic proton form
factors at \PANDA (FAIR) within the PandaRoot software framework are reported.
The statistical precision with which the proton form factors can be determined
is estimated. The signal channel is studied on the basis
of two different but consistent procedures. The suppression of the main
background channel, , is studied.
Furthermore, the background versus signal efficiency, statistical and
systematical uncertainties on the extracted proton form factors are evaluated
using two different procedures. The results are consistent with those of a
previous simulation study using an older, simplified framework. However, a
slightly better precision is achieved in the PandaRoot study in a large range
of momentum transfer, assuming the nominal beam conditions and detector
performance
Experiencia de uso de antimicrobianos en un servicio de hematologÃa en el contexto de un programa de optimización de antibióticos

Introducción: Se implanta un Programa de Optimización de Antibióticos (PROA) en el Servicio de HematologÃa (SH) de un hospital de tercer nivel, compuesto por un equipo multidisciplinar (HematologÃa, Farmacia, Infecciosas). Para entender la situación inicial y detectar posibles acciones de mejora, se realizó un análisis del consumo de antimicrobianos (AM). Material y
Métodos: Estudio descriptivo, observacional y retrospectivo de utilización de AM en el SH durante el perÃodo de 2014 a 2017. Variables recogidas: número de ingresos, número de estancias, dosis diaria definida (DDD)/100 estancias de antimicrobianos (antibióticos y antifúngicos) y DDD/100 estancias por grupo clÃnico (G1: antibióticos de espectro reducido, G2: empÃrico en infecciones comunitarias, G3: bacterias Gram (-) resistentes, G4: bacterias gram (+) resistentes, G5: antifúngicos de espectro reducido, G6: antifúngicos de amplio espectro, G7: otros). Fuente de datos: Programa de Gestión Económica.
Resultados: El número de ingresos en el SH en los años 2014, 2015, 2016 y 2017 fue de 765, 662, 756 y 572 respectivamente. Las estancias de 6839, 6280, 6547 y 5841, respectivamente. La evolución de las DDD/ 100 estancias de AM fue: 114.73, 100.91, 122.31, 133.04, de las DDD/100 estancias de antibióticos fue: 86, 38, 75, 04, 88, 05, 89, 01 y de las DDD/100 estancias de antifúngicos fue: 28.35, 25.87, 34.27 y 44.03. El G3 es el que más consumo presenta, el meropenem el principio activo más prescrito: 42.38, 33.02, 39.08 y 32.92 (DDD/100 estancias en los cuatro años del estudio). En el grupo G4 el uso de la vancomicina se ha incrementado (DDD/100 estancias 0.22, 0.04, 0.97 y 5.37) y el consumo de daptomicina ha disminuido (DDD/100 estancias 6.95, 7.42, 12.96, 8.69). El G6 ha sido el que más incremento ha presentado en 2017, siendo posaconazol (DDD/100 estancias 6.14, 7.25, 5.33 y 12.49) y anfotericina B liposomal (DDD/100 estancias 3.35, 3.22, 12.82 y 12.49) los más prescritos. Cabe destacar en el Grupo G1 el incremento del uso de sulfametoxazol-trimetroprim con DDD/100 estancias de 1.51, 2.46, 4.33, 6.00.
Conclusiones: Se observa un incremento en DDD/100 estancias de los AM en el año 2017, sin embargo, los ingresos han disminuido. Puede ser que el paciente que ingresa sea más complejo. Meropenem es el principio activo más prescrito en el SH. La vancomicina ha incrementado su uso debido a la monitorización de niveles con recomendación posológica por parte del Servicio de Farmacia y como consecuencia, daptomicina ha disminuido. En los antifúngicos, aumenta el consumo de los de amplio espectro. Sulfametoxazol-trimetroprim ha aumentado su consumo como profilaxis en nuevos tratamientos. Se ha detectado variabilidad en sus dosis prescritas, por lo que se desarrollarán protocolos para su homogeneización
Experiencia clÃnica en el manejo de pacientes con leucemia linfática crónica en tratamiento con IBRUTINIB

Introducción: Analizar la respuesta y la tolerancia a Ibrutinib en pacientes con Leucemia Linfática Crónica (LLC) en un hospital de tercer nivel. Material y
Métodos: Estudio descriptivo, observacional, retrospectivo y unicéntrico en pacientes con LLC en tratamiento con Ibrutinib en un perÃodo comprendido entre Marzo 2015 a Abril 2018. Variables recogidas: demográficas (sexo y edad), citogenética, número de lÃneas previas de tratamiento, tiempo de evolución (desde el diagnóstico hasta inicio de Ibrutinib), linfocitos totales (al inicio y a los 6 meses de Ibrutinib), tiempo en el que se objetiva el recuento linfocitario menor, durabilidad del tratamiento, tipo de respuesta según criterios de la National Comprehensive Cancer Network (NCCN), motivo de suspensión y eventos adversos (EA) reportados.
Resultados: 9 pacientes (mujeres 44.4 %) recibieron tratamiento con Ibrutinib. Con una mediana de edad al diagnóstico de 65 años (49 – 76). Al inicio del tratamiento, 2 pacientes presentaron delección 11q, 6 delección 13q, 4 delección 17p (mutación TP53) y 1 trisomÃa del 12. La mediana de lÃneas de tratamiento recibidas previo a ibrutinib fue de 1 (0-4), administrándose en primera lÃnea a un paciente con delección 17p (mutación TP53). El tiempo medio de evolución fue de 83 meses. La media de linfocitosis al inicio y a los 6 meses fue 186 y 41 mil/mm3 respectivamente. La mediana de tiempo en alcanzar el recuento linfocitario menor fue de 6 meses (4-24). La media de duración de dicho tratamiento fue de 280 dÃas. Tras > 6 meses de tratamiento la respuesta fue: 5 pacientes respuesta parcial, 3 completa y 1 progresión (transformación a sÃndrome de Ritcher). Suspendieron el tratamiento 6 pacientes. Los motivos fueron: 3 por eventos adversos (2 cambiaron a Idelalisib), 2 por éxitus (infección y problema cardiovascular) y 1 por progresión clÃnica. Los EA registrados en las historias clÃnicas fueron: 3 eventos hemorrágicos, 2 gastrointestinales, 2 cardiovasculares (fibrilación auricular e insuficiencia cardÃaca congestiva) y 1 astenia.
Conclusiones: En nuestra experiencia clÃnica Ibrutinib es un fármaco eficaz en el tratamiento de LLC. A pesar de su buena tolerancia, el principal motivo de discontinuación fue la aparición de efectos adversos moderados/ graves de tipo hemorrágicos y cardiovasculares. Lo que nos indica la necesidad de realizar una minuciosa selección del paciente más idóneo y una estrecha monitorización durante el tratamiento con Ibrutinib

This article formulates nine hypotheses concerning the proof of facts in criminal proceedings, which are analysed by nine judges of Chilean criminal Courts (Tribunal de Juicio Oral y de GarantÃa). Seven hypotheses aimed at describing different practices that take place in the courts and two of them concern how judges should act in certain hard cases. The matters are grouped in the following categories: a) scopes of the compromise that judges should have in order to operate with true stories as support of their decisions; b) difficulties that arise as consequence of the epistemic weakness of the evidence available in trials and the possible slants in their interpretation; c) requirements of the evaluation of the proof according to the sana crÃtica; and d) meaning that should have the standard of proof beyond reasonable doubt. In this way, a dialogue between the system of beliefs of judges and of jurists takes place
La infusión de linfocitos efectores autólogos en combinación con rituximab de mantenimiento

Introducción: La combinación de anticuerpos monoclonales anti CD20 con varios regimenes de quimioterapia se considera el tratamiento estándar en pacientes con linfoma folicular (LF). A pesar de los buenos resultados de este tratamiento, un porcentaje importante de los pacientes no se beneficia de esta terapia ensombreciendo de manera considerable su pronóstico. Nuestra hipótesis es que el tratamiento con una suspensión de linfocitos efectores autólogos expandidos ex vivo (células LAK) podrÃa potenciar el efecto biológico del tratamiento con rituximab (R) mediante el incremento de la actividad ADCC. Investigamos la toxicidad y eficacia de dicho tratamiento con un ensayo clÃnico fase II.
Métodos: Entre 2010 y 2012 se reclutaron 20 pacientes con LF en respuesta tras una primera lÃnea de tratamiento con R-CHOP. El objetivo primario de este ensayo, multicéntrico, prospectivo y de un solo brazo fue evaluar la seguridad y eficacia de la infusión de células LAK (administradas cuatrimestralmente en los ciclos pares de R) en combinación con la pauta estándar de R de mantenimiento bimestral durante 2 años. La producción de células LAK se realizó a partir de células de sangre periférica extraÃdas en los ciclos impares de R para aislar células mononucleadas y estimularlas y expandirlas en cultivo con interleukina-2 durante 8 semanas. Uno de los objetivos secundarios del ensayo fue la evaluación de los fenotipos celulares y de la potencia biológica de las células LAK midiendo su actividad citotóxica. We report on hallmarks of its discovery, its structural and functional characterization and localization over time, and point to present days’ knowledge on its position as a member of a large family. We focus on the rather puzzling number of diverse functions as proposed for actin as a dual compartment protein. Finally, we venture on some speculations as to its origin
Visualization of acetylcholine distribution in central nervous system tissue sections by tandem imaging mass spectrometry
Metabolite distribution imaging via imaging mass spectrometry (IMS) is an increasingly utilized tool in the field of neurochemistry. As most previous IMS studies analyzed the relative abundances of larger metabolite species, it is important to expand its application to smaller molecules, such as neurotransmitters. This study aimed to develop an IMS application to visualize neurotransmitter distribution in central nervous system tissue sections. Here, we raise two technical problems that must be resolved to achieve neurotransmitter imaging: (1) the lower concentrations of bioactive molecules, compared with those of membrane lipids, require higher sensitivity and/or signal-to-noise (S/N) ratios in signal detection, and (2) the molecular turnover of the neurotransmitters is rapid; thus, tissue preparation procedures should be performed carefully to minimize postmortem changes. We first evaluated intrinsic sensitivity and matrix interference using Matrix Assisted Laser Desorption/Ionization (MALDI) mass spectrometry (MS) to detect six neurotransmitters and chose acetylcholine (ACh) as a model for study. Next, we examined both single MS imaging and MS/MS imaging for ACh and found that via an ion transition from m/z 146 to m/z 87 in MS/MS imaging, ACh could be visualized with a high S/N ratio. Furthermore, we found that in situ freezing method of brain samples improved IMS data quality in terms of the number of effective pixels and the image contrast (i.e., the sensitivity and dynamic range). Therefore, by addressing the aforementioned problems, we demonstrated the tissue distribution of ACh, the most suitable molecular specimen for positive ion detection by IMS, to reveal its localization in central nervous system tissues
Prognostic significance of FLT3-ITD length in AML patients treated with intensive regimens
FLT3-ITD mutations are detected in approximately 25% of newly diagnosed adult acute myeloid leukemia (AML) patients and confer an adverse prognosis. The FLT3-ITD allelic ratio has clear prognostic value. Nevertheless, there are numerous manuscripts with contradictory results regarding the prognostic relevance of the length and insertion site (IS) of the FLT3-ITD fragment. We aimed to assess the prognostic impact of these variables on the complete remission (CR) rates, overall survival (OS) and relapse-free survival (RFS) of AML patients with FLT3-ITDmutations. We studied the FLT3-ITD length of 362 adult AML patients included in the PETHEMA AML registry. We tried to validate the thresholds of ITD length previously published (i.e., 39 bp and 70 bp) in intensively treated AML patients (n = 161). We also analyzed the mutational profile of 118 FLT3-ITD AML patients with an NGS panel of 39 genes and correlated mutational status with the length and IS of ITD. The AUC of the ROC curve of the ITD length for OS prediction was 0.504, and no differences were found when applying any of the thresholds for OS, RFS or CR rate. Only four out of 106 patients had ITD IS in the TKD1 domain. Our results, alongside previous publications, confirm that FLT3-ITD length lacks prognostic value and clinical applicability. © 2021, The Author(s)
Inhibitor of Kappa B Epsilon (IκBε) Is a Non-Redundant Regulator of c-Rel-Dependent Gene Expression in Murine T and B Cells
Inhibitors of kappa B (IκBs) -α, -β and -ε effect selective regulation of specific nuclear factor of kappa B (NF-κB) dimers according to cell lineage, differentiation state or stimulus, in a manner that is not yet precisely defined. Lymphocyte antigen receptor ligation leads to degradation of all three IκBs but activation only of subsets of NF-κB-dependent genes, including those regulated by c-Rel, such as anti-apoptotic CD40 and BAFF-R on B cells, and interleukin-2 (IL-2) in T cells. We report that pre-culture of a mouse T cell line with tumour necrosis factor-α (TNF) inhibits IL-2 gene expression at the level of transcription through suppressive effects on NF-κB, AP-1 and NFAT transcription factor expression and function. Selective upregulation of IκBε and suppressed nuclear translocation of c-Rel were very marked in TNF-treated, compared to control cells, whether activated via T cell receptor (TCR) pathway or TNF receptor. IκBε associated with newly synthesised c-Rel in activated cells and, in contrast to IκBα and -β, showed enhanced association with p65/c-Rel in TNF-treated cells relative to controls. Studies in IκBε-deficient mice revealed that basal nuclear expression and nuclear translocation of c-Rel at early time-points of receptor ligation were higher in IκBε−/− T and B cells, compared to wild-type. IκBε−/− mice exhibited increased lymph node cellularity and enhanced basal thymidine incorporation by lymphoid cells ex vivo. IκBε−/− T cell blasts were primed for IL-2 expression, relative to wild-type. IκBε−/− splenic B cells showed enhanced survival ex vivo, compared to wild-type, and survival correlated with basal expression of CD40 and induced expression of CD40 and BAFF-R. Enhanced basal nuclear translocation of c-Rel, and upregulation of BAFF-R and CD40 occurred despite increased IκBα expression in IκBε−/− B cells. The data imply that regulation of these c-Rel-dependent lymphoid responses is a non-redundant function of IκBε
