    Historical Photogrammetry and Terrestrial Laser Scanning for the 3d Virtual Reconstruction of Destroyed Structures: a Case Study in Italy

    The current dramatic episodes of destruction of archaeological sites have again highlighted the problem of the safeguarding the threatened heritage and, if possible, recovering those damaged by all the armed conflicts of the past. The historical photogrammetry offers the possibility to recover a posteriori the geometrical and material properties of destroyed structures, reconstructing their 3D model to document, study and maintain their memory, until to support their real anastylosis. The presented work is about the 3D reconstruction of the civic tower of the little town of Sant'Alberto, near the city of Ravenna, Italy. The tower, as a symbol of resistance and pride of the town's population, was destroyed in December 1944 by German troops in retaliation, when they were forced to leave the area. A city committee has subsequently collected all the historical evidence concerning the tower, including a series of photographic images that can be used for the photogrammetric reconstruction; the images calibration and orientation have been solved using the geometric information derived by a terrestrial laser scanner survey realized in the area where the tower was originally located. Despite the scarcity and very poor quality of the available images, the conducted photogrammetric procedure has allowed a complete and qualitatively satisfying object reconstruction, also thanks to the use of geometric constraint tools offered by the chosen software. The integration between the obtained model of the old tower and the 3D TLS survey of the square made it possible to reconstruct the ancient situation of the area

    Studying a hospital distribution network with a stochastic end-uses demand model

    The schematization of a hospital water network through a probabilistic model of end-uses is the focus of this study. The idea is to realize a model to evaluate the water demand related to the use of individual water-demanding devices to define a strategy for the prevention and control of legionella risk. The modelling of hospital network has been carried out through EPANET 2.0 software [1], in which the hot and cold water networks can be implemented separately. The suggested probabilistic model has already been applied to several case studies [2], [3], [4] and [5], but never to a hospital network

    Assessing the value of BMI and aerobic capacity as surrogate markers for the severity of left ventricular diastolic dysfunction in patients with type 2 diabetes who are obese

    Left ventricular diastolic dysfunction (LVDD) is one of the earliest signs for abnormal cardiac function in patients with type 2 diabetes (T2DM). It is important to explore the risk factors that will assist in identifying the severity of the LVDD in this population. We examined the influences of fitness and fatness on the level of left ventricular (LV) impairment in patients with T2DM. Twenty-five patients (age: 64.0 ± 2.5 years, body mass index [BMI] = 36.0 ± 1.5 kg/m2, mean ± standard error of measurement) with T2DM and preserved systolic function, but impaired diastolic function, mitral valve (MV) E/e\u27, participated in the study. LV function was assessed using a stress echocardiograph, aerobic power was assessed with a sign- and symptom-limited graded exercise test, and the fatness level was assessed using Dual-energy X-ray absorptiometry and BMI. Patients in the higher 50% of BMI had higher lateral and septal MV E/e\u27 (∼34% and ∼25%, respectively, both P < 0.001), compared to those in the lower 50% of BMI, with no difference in LV ejection fraction (LVEF) (P > 0.05). In addition, a higher BMI correlated with a higher lateral (r = 0.62, P < 0.001) and septal (r = 0.56, P < 0.01) E/e\u27. There was no such relationship for VO2peak. BMI and VO2peak were not correlated with LV systolic function (ejection fraction). In individuals with T2DM and diastolic dysfunction, a higher BMI was associated with worsening diastolic function independent of their aerobic capacity. The data provide a simple and practical approach for clinicians to assist in the early identification and diagnostics of functional changes in the heart diastolic function in this population

For the first time, the Symposium brought together representatives from the European Reference Network on rare adult solid cancer (EURACAN) joined by sarcoma experts from the USA, Japan and patient advocacy groups, to share insights and discuss future directions in this rare condition.     Diagnosis and management of tropomyosin receptor kinase (TRK) fusion sarcomas : expert recommendations from the World Sarcoma Network

    Sarcomas are a heterogeneous group of malignancies with mesenchymal lineage differentiation. The discovery of neurotrophic tyrosine receptor kinase (NTRK) gene fusions as tissue-agnostic oncogenic drivers has led to new personalized therapies for a subset of patients with sarcoma in the form of tropomyosin receptor kinase (TRK) inhibitors. NTRK gene rearrangements and fusion transcripts can be detected with different molecular pathology techniques, while TRK protein expression can be demonstrated with immunohistochemistry. The rarity and diagnostic complexity of NTRK gene fusions raise a number of questions and challenges for clinicians. To address these challenges, the World Sarcoma Network convened two meetings of expert adult oncologists and pathologists and subsequently developed this article to provide practical guidance on the management of patients with sarcoma harboring NTRK gene fusions. We propose a diagnostic strategy that considers disease stage and histologic and molecular subtypes to facilitate routine testing for TRK expression and subsequent testing for NTRK gene fusions.Peer reviewe

    Adjuvant Imatinib in Patients with GIST Harboring Exon 9 KIT Mutations : Results from a Multi-institutional European Retrospective Study

    Purpose: The effect of high-dose imatinib (800 mg/day) on survival in the adjuvant treatment of patients with resected KIT exon 9-mutated gastrointestinal stromal tumors (GIST) is not established. Here, the association of dose and other clinicopatho-logic variables with survival was evaluated in a large multi-institutional European cohort. Experimental Design: Data from 185 patients were retrospec-tively collected in 23 European GIST reference centers. Propen-sity score matching (PSM) and inverse-probability of treatment weighting (IPTW) were used to account for confounders. Uni-variate and multivariate unweighted and weighted Cox propor-tional hazard regression models were estimated for relapse-free survival (RFS), modified-RFS (mRFS) and imatinib failure-free survival (IFFS). Univariate Cox models were estimated for overall survival. Results: Of the 185 patients, 131 (70.8%) received a starting dose of 400 mg/d and the remaining 54 (29.2%) a dose of 800 mg/d. Baseline characteristics were partially unbalanced, suggesting a potential selection bias. PSM and IPTW analyses showed no advantage of imatinib 800 mg/d. In the weighted multivariate Cox models, high-dose imatinib was not associated with the survival outcomes [RFS: hazard ratio (HR), 1.24; 95% confidence interval (CI), 0.79-1.94; mRFS: HR, 1.69; 95% CI, 0.92-3.10; IFFS: HR, 1.35; 95% CI, 0.79- 2.28]. The variables consistently associated with worse survival out-comes were high mitotic index and nongastric tumor location. Conclusions: In this retrospective series of patients with KIT exon 9-mutated GIST treated with adjuvant imatinib, a daily dose of 800 mg versus 400 mg did not show better results in terms of survival outcomes. Prospective evaluation of the more appropriate adjuvant treatment in this setting is warranted.Peer reviewe
