    Genome-wide meta-analysis of cerebral white matter hyperintensities in patients with stroke.

    OBJECTIVE: For 3,670 stroke patients from the United Kingdom, United States, Australia, Belgium, and Italy, we performed a genome-wide meta-analysis of white matter hyperintensity volumes (WMHV) on data imputed to the 1000 Genomes reference dataset to provide insights into disease mechanisms. METHODS: We first sought to identify genetic associations with white matter hyperintensities in a stroke population, and then examined whether genetic loci previously linked to WMHV in community populations are also associated in stroke patients. Having established that genetic associations are shared between the 2 populations, we performed a meta-analysis testing which associations with WMHV in stroke-free populations are associated overall when combined with stroke populations. RESULTS: There were no associations at genome-wide significance with WMHV in stroke patients. All previously reported genome-wide significant associations with WMHV in community populations shared direction of effect in stroke patients. In a meta-analysis of the genome-wide significant and suggestive loci (p < 5 × 10(-6)) from community populations (15 single nucleotide polymorphisms in total) and from stroke patients, 6 independent loci were associated with WMHV in both populations. Four of these are novel associations at the genome-wide level (rs72934505 [NBEAL1], p = 2.2 × 10(-8); rs941898 [EVL], p = 4.0 × 10(-8); rs962888 [C1QL1], p = 1.1 × 10(-8); rs9515201 [COL4A2], p = 6.9 × 10(-9)).     Anticoagulant selection in relation to the SAMe-TT2R2 score in patients with atrial fibrillation. the GLORIA-AF registry

    Aim: The SAMe-TT2R2 score helps identify patients with atrial fibrillation (AF) likely to have poor anticoagulation control during anticoagulation with vitamin K antagonists (VKA) and those with scores &gt;2 might be better managed with a target-specific oral anticoagulant (NOAC). We hypothesized that in clinical practice, VKAs may be prescribed less frequently to patients with AF and SAMe-TT2R2 scores &gt;2 than to patients with lower scores. Methods and results: We analyzed the Phase III dataset of the Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation (GLORIA-AF), a large, global, prospective global registry of patients with newly diagnosed AF and ≥1 stroke risk factor. We compared baseline clinical characteristics and antithrombotic prescriptions to determine the probability of the VKA prescription among anticoagulated patients with the baseline SAMe-TT2R2 score &gt;2 and ≤ 2. Among 17,465 anticoagulated patients with AF, 4,828 (27.6%) patients were prescribed VKA and 12,637 (72.4%) patients an NOAC: 11,884 (68.0%) patients had SAMe-TT2R2 scores 0-2 and 5,581 (32.0%) patients had scores &gt;2. The proportion of patients prescribed VKA was 28.0% among patients with SAMe-TT2R2 scores &gt;2 and 27.5% in those with scores ≤2. Conclusions: The lack of a clear association between the SAMe-TT2R2 score and anticoagulant selection may be attributed to the relative efficacy and safety profiles between NOACs and VKAs as well as to the absence of trial evidence that an SAMe-TT2R2-guided strategy for the selection of the type of anticoagulation in NVAF patients has an impact on clinical outcomes of efficacy and safety. The latter hypothesis is currently being tested in a randomized controlled trial.     Comparative effectiveness and safety of non-vitamin K antagonists for atrial fibrillation in clinical practice: GLORIA-AF Registry

    Background and purpose: Prospectively collected data comparing the safety and effectiveness of individual non-vitamin K antagonists (NOACs) are lacking. Our objective was to directly compare the effectiveness and safety of NOACs in patients with newly diagnosed atrial fibrillation (AF). Methods: In GLORIA-AF, a large, prospective, global registry program, consecutive patients with newly diagnosed AF were followed for 3&nbsp;years. The comparative analyses for (1) dabigatran vs rivaroxaban or apixaban and (2) rivaroxaban vs apixaban were performed on propensity score (PS)-matched patient sets. Proportional hazards regression was used to estimate hazard ratios (HRs) for outcomes of interest. Results: The GLORIA-AF Phase III registry enrolled 21,300 patients between January 2014 and December 2016. Of these, 3839 were prescribed dabigatran, 4015 rivaroxaban and 4505 apixaban, with median ages of 71.0, 71.0, and 73.0&nbsp;years, respectively. In the PS-matched set, the adjusted HRs and 95% confidence intervals (CIs) for dabigatran vs rivaroxaban were, for stroke: 1.27 (0.79–2.03), major bleeding 0.59 (0.40–0.88), myocardial infarction 0.68 (0.40–1.16), and all-cause death 0.86 (0.67–1.10). For the comparison of dabigatran vs apixaban, in the PS-matched set, the adjusted HRs were, for stroke 1.16 (0.76–1.78), myocardial infarction 0.84 (0.48–1.46), major bleeding 0.98 (0.63–1.52) and all-cause death 1.01 (0.79–1.29). For the comparison of rivaroxaban vs apixaban, in the PS-matched set, the adjusted HRs were, for stroke 0.78 (0.52–1.19), myocardial infarction 0.96 (0.63–1.45), major bleeding 1.54 (1.14–2.08), and all-cause death 0.97 (0.80–1.19). Conclusions: Patients treated with dabigatran had a 41% lower risk of major bleeding compared with rivaroxaban, but similar risks of stroke, MI, and death. Relative to apixaban, patients treated with dabigatran had similar risks of stroke, major bleeding, MI, and death. Rivaroxaban relative to apixaban had increased risk for major bleeding, but similar risks for stroke, MI, and death. Registration: URL: https://www.clinicaltrials.gov.     Atrial fibrillation genetic risk differentiates cardioembolic stroke from other stroke subtypes

    AbstractObjectiveWe sought to assess whether genetic risk factors for atrial fibrillation can explain cardioembolic stroke risk.MethodsWe evaluated genetic correlations between a prior genetic study of AF and AF in the presence of cardioembolic stroke using genome-wide genotypes from the Stroke Genetics Network (N = 3,190 AF cases, 3,000 cardioembolic stroke cases, and 28,026 referents). We tested whether a previously-validated AF polygenic risk score (PRS) associated with cardioembolic and other stroke subtypes after accounting for AF clinical risk factors.ResultsWe observed strong correlation between previously reported genetic risk for AF, AF in the presence of stroke, and cardioembolic stroke (Pearson’s r=0.77 and 0.76, respectively, across SNPs with p &lt; 4.4 × 10−4 in the prior AF meta-analysis). An AF PRS, adjusted for clinical AF risk factors, was associated with cardioembolic stroke (odds ratio (OR) per standard deviation (sd) = 1.40, p = 1.45×10−48), explaining ∼20% of the heritable component of cardioembolic stroke risk. The AF PRS was also associated with stroke of undetermined cause (OR per sd = 1.07, p = 0.004), but no other primary stroke subtypes (all p &gt; 0.1).ConclusionsGenetic risk for AF is associated with cardioembolic stroke, independent of clinical risk factors. Studies are warranted to determine whether AF genetic risk can serve as a biomarker for strokes caused by AF.</jats:sec

    Assessing geotechnical centrifuge modelling in addressing variably saturated flow in soil and fractured rock

    The vadose zone typically comprises soil underlain by fractured rock. Often, surface water and groundwater parameters are readily available, but variably saturated flow through soil and rock are oversimplified or estimated as input for hydrological models. In this paper, a series of geotechnical centrifuge experiments are conducted to contribute to the knowledge gaps in: (i) variably saturated flow and dispersion in soil and (ii) variably saturated flow in discrete vertical and horizontal fractures. Findings from the research show that the hydraulic gradient, and not the hydraulic conductivity, is scaled for seepage flow in the geotechnical centrifuge. Furthermore, geotechnical centrifuge modelling has been proven as a viable experimental tool for the modelling of hydrodynamic dispersion as well as the replication of similar flow mechanisms for unsaturated fracture flow, as previously observed in literature. Despite the imminent challenges of modelling variable saturation in the vadose zone, the geotechnical centrifuge offers a powerful experimental tool to physically model and observe variably saturated flow. This can be used to give valuable insight into mechanisms associated with solid–fluid interaction problems under these conditions. Findings from future research can be used to validate current numerical modelling techniques and address the subsequent influence on aquifer recharge and vulnerability, contaminant transport, waste disposal, dam construction, slope stability and seepage into subsurface excavations.The Water Research Commission of South Africahttp://link.springer.com/journal/113562018-05-31hb2017Geolog

    Disseminating Policy and Environmental Change Interventions: Insights from Obesity Prevention and Tobacco Control

    PURPOSE: Evidence-based interventions are increasingly called for as a way to improve health behaviors such as tobacco use, physical inactivity, and poor diet. Numerous organizations are disseminating interventions that target individual-level behavioral change. Fewer are disseminating interventions that target the policy and environmental changes required to support healthier behaviors. This paper aims to describe the distinct features of policy and environmental change and the lessons learned by two Centers for Disease Control and Prevention-funded dissemination projects, the Center for Training and Research Translation (Center TRT) and Counter Tobacco. METHODS: Both Center TRT and Counter Tobacco have conducted formative research with their target audiences to customize dissemination to address practitioner-reported needs and preferences. The Centers’ have developed the following approach to disseminating policy and environmental change interventions: (1) Identify the best available evidence rather than waiting for the best possible evidence, (2) disseminate menus of broad intervention strategies, (3) provide implementation guidance, (4) incorporate stories from the field, (5) build practitioners’ capacity, and, (6) integrate dissemination into practitioners’ existing professional and social networks. In 2012, over 26,000 unique visitors accessed the Center TRT website and downloaded over 12,400 documents. The Counter Tobacco website has had 10,907 unique visitors since its launch in August 2011, and the number of visitors is increasing rapidly. CONCLUSIONS: Both Centers have had success reaching their intended audiences. Research is now needed to assess the extent of practitioners’ use of disseminated recommendations, guidance, and tools in practice and the impact of the resulting interventions
