    Características radiográficas dos portadores de osteoporose e o papel do cirurgião-dentista no diagnóstico

    Osteoporosis is a disease characterized by low bone mass and microarchitectural deterioration of bone tissue, leading to enhanced bone fragility and a consequent increase in fracture risk. Oral signs might be manifested by excessive alveolar ridge resorption, tooth loss, chronic destructive periodontal disease, referred maxillary sinus pain, or fracture. The diagnosis is based on bone mineral density as measured by dual energy X-ray absorptiometry. However, this method may not be of easy access in some countries. Then it has been proposed the use of oral radiography in the diagnosis, once they are habitually used in the planning of treatments. Many investigators have developed radiographic measures to evaluate osteoporotic manifestations in oral cavity, for example the Mandibular Cortical Index, Panoramic Mandibular Index and the technique described by Wical and Swoope. All this techniques use the basal area of the mandible posterior to the mental foramen as a standard area (smallest possible inter and intraindividual variations in anatomical size, shape, bone structure and function). Then the dentists may be able to refer patients to medical professionals for further examination on the basis of incidental findings on dental panoramic radiographs. Besides have access to bone quality, that is essential for a successful oral implant planning.A osteoporose é uma doença sistêmica dos ossos, caracterizada pela diminuição da massa óssea e deterioração da microarquitetura do tecido ósseo, o que leva a um aumento da fragilidade óssea e da susceptibilidade à fratura. Suas manifestações, na cavidade oral, são caracterizadas por: reabsorção do processo alveolar, perda dentária, doença periodontal crônica destrutiva, dores relacionadas ao seio maxilar ou fraturas. O principal método de diagnóstico da osteoporose é a densitometria óssea, embora esse exame seja de difícil acesso para a população, em alguns países. Sendo assim, propõe-se a utilização de radiografias panorâmicas dentárias nessa avaliação, já que elas costumam ser procedimentos de rotina na clínica odontológica. Vários pesquisadores propuseram índices para avaliar as alterações osteoporóticas na cavidade oral, a partir de radiografias panorâmicas, como o Índice da Cortical Mandibular, o Índice Panorâmico Mandibular e a Técnica proposta por Wical e Swoope. Todas essas técnicas avaliam a região basal da mandíbula, posterior ao forame mentual, por ser uma área que preenche os requisitos para que ocorra uma menor variação inter e intra-indivíduos em relação à anatomia, forma, estrutura óssea e função. Dessa forma, os dentistas, em geral, poderiam fazer um correto encaminhamento dos pacientes para um tratamento específico, além de ter acesso à qualidade óssea do paciente, que é de grande importância no planejamento de implantes dentários

    Desfecho atípico de colesteatoma de orelha média: relato de caso

    O presente estudo objetivou descrever um caso de colesteatoma de orelha média com recidiva após remoção cirúrgica, seguido por desfecho atípico. Paciente do sexo masculino, 40 anos, procurou atendimento médico com queixa de otalgia e plenitude auricular em ouvido esquerdo. Foi realizada ressonância magnética que evidenciou conteúdo líquido e células mastoideas ocupando a cavidade timpânica esquerda, com focos de restrição à difusão na caixa timpânica e antro medindo cerca de 1,4 cm, sendo compatível com colesteatoma de orelha média esquerda. O paciente foi, então, submetido a dois procedimentos cirúrgicos (uma timpanoplastia endoscópica e uma mastoidectomia fechada), num intervalo de quatro meses, para remoção total do tumor. Após 3 anos e 6 meses, o paciente apresentou recidiva para o colesteatoma. No dia anterior à cirurgia marcada para remoção do tumor recidivado, foi constatado, por ressonância magnética, a ausência do colesteatoma, o que sugere uma cura espontânea. A cura ou desaparecimento natural do tumor abre novas perspectivas de investigação para as ciências médicas

    Pervasive gaps in Amazonian ecological research

    Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4 While the increasing availability of global databases on ecological communities has advanced our knowledge of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by 2050.     Síndrome de Chiari e Hidrossiringomielia com comprometimento neurológico: um relato de caso

    A Malformação de Chiari (MC) pertence a um amplo grupo de raras deformidades estruturais da junção craniocerebelomedular. O tipo I da doença caracteriza-se pela herniação tonsilar ou amigdaliana cerebelar devido à anomalia da base do crânio e da parte superior da coluna cervical, além de a porção medial do lobo inferior do cerebelo pelo canal cervical também se protuberar através do forame magno, impedindo que o líquor flua normalmente através do canal. A real prevalência da doença é desconhecida, pois muitos pacientes com herniação cerebelar são assintomáticos e o problema agrava-se na fase adulta, com queixas de cefaleia intensa e, por vezes, parestesia. O objetivo deste estudo é relatar um caso de síndrome de Chiari (SC) em uma paciente de 53 anos, ao abordar sua apresentação clínica, diagnóstico e tratamento. Paciente do sexo feminino, 53 anos, foi admitida em um hospital da rede pública de referência se queixando de cefaleia occipital intensa e cervicalgia com irradiação da dor para os membros superiores, acompanhada de parestesia nos quatro segmentos. Relatou já sentir dor há 2 anos, mas apresentou piora do quadro clínico há 8 meses. Foi, também, observada incontinência urinária devido à dissinergia detrusora-esfincteriana por provável bexiga neurogênica. Foi, então, realizado exame de imagem de ressonância magnética (RNM) do crânio e da coluna cervical, com obtenção de sequências ponderadas em T1, T2 e STIR, nos planos sagital e transverso com contraste, o qual evidenciou leve alargamento medular, além de sinais de hidrossiringomielia difusa, com hipossinal na sequência T2 intramedular na altura de D1-D2 (coluna dorsal). Foi notada discreta herniação das tonsilas cerebelares junta ao forame magno, típica da SC, sendo, por fim, confirmado o diagnóstico. A paciente, no entanto, não apresentava hidrocefalia, mesmo com a interrupção do fluxo do líquido cefalorraquidiano (LCR) para o canal vertebral. Ela encaixou- se nos parâmetros de indicação cirúrgica, sendo realizada craniotomia occipital, com acesso ao plexo coroide do quarto ventrículo do tronco encefálico com o intuito de elevar as tonsilas cerebelares baixas, herniadas no canal espinhal cervical e bloqueando o fluxo do LCR. Após a descompressão craniocervical, o curso do líquor foi restaurado e a paciente foi, por fim, encaminhada à sala de recuperação pós-operatória. A SC é uma rara doença que apresenta quadro clínico e alterações radiológicas complexas e extensas e, por vezes, o diagnóstico é retardado devido à inespecificidade dos sintomas confundidos com cervicalgias e cefaleias comuns. A hipótese diagnóstica deve ser embasada nas queixas do paciente, na anamnese minuciosa, exame clínico e nos exames de imagens, sendo a prevalência desta patologia de difícil definição e com faixas etárias distintas

    The complete genome sequence of Chromobacterium violaceum reveals remarkable and exploitable bacterial adaptability

    Chromobacterium violaceum is one of millions of species of free-living microorganisms that populate the soil and water in the extant areas of tropical biodiversity around the world. Its complete genome sequence reveals (i) extensive alternative pathways for energy generation, (ii) ≈500 ORFs for transport-related proteins, (iii) complex and extensive systems for stress adaptation and motility, and (iv) wide-spread utilization of quorum sensing for control of inducible systems, all of which underpin the versatility and adaptability of the organism. The genome also contains extensive but incomplete arrays of ORFs coding for proteins associated with mammalian pathogenicity, possibly involved in the occasional but often fatal cases of human C. violaceum infection. There is, in addition, a series of previously unknown but important enzymes and secondary metabolites including paraquat-inducible proteins, drug and heavy-metal-resistance proteins, multiple chitinases, and proteins for the detoxification of xenobiotics that may have biotechnological applications

    ATLANTIC EPIPHYTES: a data set of vascular and non-vascular epiphyte plants and lichens from the Atlantic Forest

    Epiphytes are hyper-diverse and one of the frequently undervalued life forms in plant surveys and biodiversity inventories. Epiphytes of the Atlantic Forest, one of the most endangered ecosystems in the world, have high endemism and radiated recently in the Pliocene. We aimed to (1) compile an extensive Atlantic Forest data set on vascular, non-vascular plants (including hemiepiphytes), and lichen epiphyte species occurrence and abundance; (2) describe the epiphyte distribution in the Atlantic Forest, in order to indicate future sampling efforts. Our work presents the first epiphyte data set with information on abundance and occurrence of epiphyte phorophyte species. All data compiled here come from three main sources provided by the authors: published sources (comprising peer-reviewed articles, books, and theses), unpublished data, and herbarium data. We compiled a data set composed of 2,095 species, from 89,270 holo/hemiepiphyte records, in the Atlantic Forest of Brazil, Argentina, Paraguay, and Uruguay, recorded from 1824 to early 2018. Most of the records were from qualitative data (occurrence only, 88%), well distributed throughout the Atlantic Forest. For quantitative records, the most common sampling method was individual trees (71%), followed by plot sampling (19%), and transect sampling (10%). Angiosperms (81%) were the most frequently registered group, and Bromeliaceae and Orchidaceae were the families with the greatest number of records (27,272 and 21,945, respectively). Ferns and Lycophytes presented fewer records than Angiosperms, and Polypodiaceae were the most recorded family, and more concentrated in the Southern and Southeastern regions. Data on non-vascular plants and lichens were scarce, with a few disjunct records concentrated in the Northeastern region of the Atlantic Forest. For all non-vascular plant records, Lejeuneaceae, a family of liverworts, was the most recorded family. We hope that our effort to organize scattered epiphyte data help advance the knowledge of epiphyte ecology, as well as our understanding of macroecological and biogeographical patterns in the Atlantic Forest.
