    Caffeine Does Not Augment Markers of Muscle Damage or Leukocytosis Following Resistance Exercise

    The purpose of this study was to evaluate the effects of caffeine ingestion before a resistance exercise session on markers of muscle damage (CK, LDH, ALT, AST) and leukocyte levels. Methods: Fifteen soccer athletes completed two resistance exercise sessions that differed only in the ingestion of caffeine or a placebo preworkout. Results: CK concentration increased significantly following the caffeine session (415.8 ± 62.8 to 542.0 ± 73.5) and the placebo session (411.5 ± 43.3 to 545.8 ± 59.9), with no significant differences between sessions. Similarly, LDH concentration increased significantly following the caffeine session (377.5 ± 18.0 to 580.5 ± 36.1) and the placebo session (384.8 ± 13.9 to 570.4 ± 36.1), with no significant differences between sessions. Both sessions resulted in significant increases in the total leukocyte count (caffeine = 6.24 ± 2.08 to 8.84 ± 3.41; placebo = 6.36 ± 2.34 to 8.77 ± 3.20), neutrophils (caffeine = 3.37 ± 0.13 to 5.15 ± 0.28; placebo = 3.46 ± 0.17 to 5.12 ± 0.24), lymphocytes (caffeine = 2.19 ± 0.091 to 2.78 ± 0.10; placebo = 2.17 ± 0.100 to 2.75 ± 0.11), and monocytes (caffeine = 0.53 ± 0.02 to 0.72 ± 0.06; placebo = 0.56 ± 0.03 to 0.69 ± 0.04), with no significant differences between sessions. Conclusion: Ingestion of caffeine at 4.5 mg⋅kg−1 did not augment markers of muscle damage or leukocyte levels above that which occurs through resistance exercise alone

    The beneficial effects of honey on human health have long been recognized. Today, many of those positive effects have been studied to elucidate its mode of action. This review briefly summarizes the best studied features of honey, highlighting it as an appealing alternative medicine. In these reports, the health benefits of honey range from antioxidant, immunomodulatory, and anti-inflammatory activity to anticancer action, metabolic and cardiovascular benefits, prebiotic properties, human pathogen control, and antiviral activity. These studies also support that the honey's biological activity is mainly dependent on its floral or geographic origin. In addition, some promising synergies between honey and antibiotics have been found, as well as some antiviral properties that require further investigation.     Cryotherapy for low risk prostate cancer, oncological and functional medium term outcomes : A three center prospective study

    Objectives: Analyze the oncologic and functional outcomes in patients affected by low risk prostate cancer underwent prostate cryotherapy. Materiasl and methods: It's a prospective tricentric study of 434 patients treated with prostate cryoablation for low risk prostate cancer. By low risk we refer to the D'Amico's risk classification. Two cycles of freezing/thawing are run for each patient following the technique described by Onik. Results: For the 434 patients, the median age was 66 years with a standard deviation of \uc2\ub1 6.68, the average PSA was 6.17 ng/d/L, the median 5.55 with a standard deviation of \uc2\ub1 2.13, the mean prostate volume was 35.59 cc, the median 34.00 cc, with a standard deviation of \uc2\ub1 7.89. Biochemical failure occurred in 67 patients (15.4%). Pre-operative erectile function in men was distributed as follows: severe in 95 patients (19.2%), moderate in 95 (19.2%), medium-moderate in 180 (36.4%), mild in 92 (18.6%), with no dysfunction in 32 (6.5%) patients. Post-operative erectile function, measured 1 month after cryotherapy, was distributed as follows: severe in 321 (65%) patients, moderate in 69 (14%), medium-moderate in 79 (16%), mild in 23 (4.7%), and no dysfunction in only 2 patients (0.4%). Post-operative erectile function after 3 months was distributed as follows: severe in 233 (47.2%) patients, moderate in 66 (13.4%), medium-moderate in 122 (24.7%), mild in 65 (13.2%), and no dysfunction in 8 patients (1.6%). Urinary incontinence was present in 21 patients (4.8%) after 3 months while it dropped to 13 patients (2.9%) after 6 months. Conclusions: Cryotherapy in the treatment of prostate cancer remains a viable alternative. The availability of new cryoprobes and the use of new diagnostic means such as fusion magnetic resonance will make this more precise and more effective method

    Overeating, caloric restriction and breast cancer risk by pathologic subtype: the EPIGEICAM study

    This study analyzes the association of excessive energy intake and caloric restriction with breast cancer (BC) risk taking into account the individual energy needs of Spanish women. We conducted a multicenter matched case-control study where 973 pairs completed lifestyle and food frequency questionnaires. Expected caloric intake was predicted from a linear regression model in controls, including calories consumed as dependent variable, basal metabolic rate as an offset and physical activity as explanatory. Overeating and caloric restriction were defined taking into account the 99% confidence interval of the predicted value. The association with BC risk, overall and by pathologic subtype, was evaluated using conditional and multinomial logistic regression models. While premenopausal women that consumed few calories (>20% below predicted) had lower BC risk (OR = 0.36; 95% CI = 0.21–0.63), postmenopausal women with an excessive intake (≥40% above predicted) showed an increased risk (OR = 2.81; 95% CI = 1.65–4.79). For every 20% increase in relative (observed/predicted) caloric intake the risk of hormone receptor positive (p-trend < 0.001) and HER2+ (p-trend = 0.015) tumours increased 13%, being this figure 7% for triple negative tumours. While high energy intake increases BC risk, caloric restriction could be protective. Moderate caloric restriction, in combination with regular physical activity, could be a good strategy for BC prevention

    Dietary responses to a multiple sclerosis diagnosis: a qualitative study

    Background/objectives: Multiple sclerosis (MS) is an immune-mediated disease with no known cure and insufficient evidence to support a special therapeutic diet to alter symptom management or disease progression. Several studies have reported dietary changes made by people with MS, but there has been limited investigation into experiences surrounding diet in those recently diagnosed. This study explored responses to diet after a recent diagnosis of MS in people living in Western Australia. Subjects/methods: Eleven adults with MS (mean time since diagnosis 8 months) participated in semi-structured interviews focusing on responses to diet since MS diagnosis. Interviews were transcribed, coded and analysed using grounded theory principles. Results: Three theme responses emerged; (1) the perceived incompatibility of lack of/or generalised dietary advice with disease seriousness at the time of diagnosis; (2) extensive personal research and information seeking with difficulty judging credibility, and (3) self-experimentation with diet to either control MS symptoms or to cure MS. Conclusions: Given the seriousness of the disease, there is a perceived gap in dietary information provided at the time of diagnosis. Healthcare professionals should address concerns with alternative therapeutic diets advertised to treat or cure MS, and clearly convey the reasoning for the general healthy dietary recommendations. This would better align advice with the perceptions about the role of diet in MS, assist people with MS in need of information and minimise dietary self-experimentation. Future research should explore the importance of diet for those who have had MS for a longer period of time

Any other use of these PDF files is subject to reprint fees.Over a 20 year period 1996–2016, a new 223 ha town is being developed 10 miles west of Dublin's city centre on the south side of Lucan, County Dublin, in the Republic of Ireland (ROI). This €4 billion ‘Adamstown’ development is the first of four planning schemes in ROI to be approved as a strategic development zone – an integrated planning framework deemed suitable for creating sustainable neighbourhoods in sites of strategic economic or social importance to the state. The creation of sustainable neighbourhoods in ROI is facilitated through the implementation of a checklist of 60 indicators. This paper critically examines the attempts being made to consider sustainability within the development's overall infrastructure plan, specifically: transport, energy and water services, information technology and waste. Inadequacies in the existing development are linked to shortfalls in the sustainability checklist, by way of a comparison of infrastructure-related indicators from the ROI checklist with those derived for the UK and exemplar European projects (i.e. Bedzed, UK and Freiberg, Germany). The subsequent legacy for future residents of Adamstown is then considered in the context of ‘what if’ scenarios

    Developing an agenda for research about policies to improve access to healthy foods in rural communities: a concept mapping study

    Background Policies that improve access to healthy, affordable foods may improve population health and reduce health disparities. In the United States most food access policy research focuses on urban communities even though residents of rural communities face disproportionately higher risk for nutrition-related chronic diseases compared to residents of urban communities. The purpose of this study was to (1) identify the factors associated with access to healthy, affordable food in rural communities in the United States; and (2) prioritize a meaningful and feasible rural food policy research agenda. Methods This study was conducted by the Rural Food Access Workgroup (RFAWG), a workgroup facilitated by the Nutrition and Obesity Policy Research and Evaluation Network. A national sample of academic and non-academic researchers, public health and cooperative extension practitioners, and other experts who focus on rural food access and economic development was invited to complete a concept mapping process that included brainstorming the factors that are associated with rural food access, sorting and organizing the factors into similar domains, and rating the importance of policies and research to address these factors. As a last step, RFAWG members convened to interpret the data and establish research recommendations. Results Seventy-five participants in the brainstorming exercise represented the following sectors: non-extension research (n = 27), non-extension program administration (n = 18), “other� (n = 14), policy advocacy (n = 10), and cooperative extension service (n = 6). The brainstorming exercise generated 90 distinct statements about factors associated with rural food access in the United States; these were sorted into 5 clusters. Go Zones were established for the factors that were rated highly as both a priority policy target and a priority for research. The highest ranked policy and research priorities include strategies designed to build economic viability in rural communities, improve access to federal food and nutrition assistance programs, improve food retail systems, and increase the personal food production capacity of rural residents. Respondents also prioritized the development of valid and reliable research methodologies to measure variables associated with rural food access. Conclusions This collaborative, trans-disciplinary, participatory process, created a map to guide and prioritize research about polices to improve healthy, affordable food access in rural communities

    Measures of the Consumer Food Store Environment: A Systematic Review of the Evidence 2000–2011

    Description of the consumer food environment has proliferated in publication. However, there has been a lack of systematic reviews focusing on how the consumer food environment is associated with the following: (1) neighborhood characteristics; (2) food prices; (3) dietary patterns; and (4) weight status. We conducted a systematic review of primary, quantitative, observational studies, published in English that conducted an audit of the consumer food environment. The literature search included electronic, hand searches, and peer-reviewed from 2000 to 2011. Fifty six papers met the inclusion criteria. Six studies reported stores in low income neighborhoods or high minority neighborhoods had less availability of healthy food. While, four studies found there was no difference in availability between neighborhoods. The results were also inconsistent for differences in food prices, dietary patterns, and weight status. This systematic review uncovered several key findings. (1) Systematic measurement of determining availability of food within stores and store types is needed; (2) Context is relevant for understanding the complexities of the consumer food environment; (3) Interventions and longitudinal studies addressing purchasing habits, diet, and obesity outcomes are needed; and (4) Influences of price and marketing that may be linked with why people purchase certain items
