Association of anorexia nervosa with risk of cancer. A systematic review and meta-analysis

However, the risk of cancer in people with anorexia nervosa remains uncertain.
OBJECTIVE To evaluate the association of anorexia nervosa with the risk of developing or dying
of cancer.
DATA SOURCES MEDLINE, Scopus, Embase, and Web of Science from database inception to
January 9, 2019.
STUDY SELECTION Published observational studies in humans examining the risk of cancer in
people with anorexia nervosa compared with the general population or those without anorexia
nervosa. Studies needed to report incidence or mortality rate ratios (RRs).
DATA EXTRACTION AND SYNTHESIS Screening, data extraction, and methodological quality
assessment were performed by at least 2 researchers independently. A random-effects model was
used to synthesize individual studies. Heterogeneity (I

) was assessed and 95% prediction intervals
(PIs) were calculated.
MAIN OUTCOMES AND MEASURES All cancer incidence and cancer mortality associated with
anorexia nervosa. Secondary outcomes were site-specific cancer incidence and mortality.
RESULTS Seven cohort studies published in 10 articles (42 602 participants with anorexia nervosa)
were included. Anorexia nervosa was not associated with risk of developing any cancer (4 studies in
women; RR, 0.97; 95% CI, 0.89-1.06; P = .53; I

, 0%; 95% PI, 0.80-1.18; moderate confidence).
Anorexia nervosa was associated with decreased breast cancer incidence (5 studies in women; RR,
0.60; 95% CI, 0.50-0.80; P < .001; I

, 0%; 95% PI, 0.44-0.83; high confidence). Conversely,
anorexia nervosa was associated with increased risk of developing lung cancer (3 studies in women;
RR, 1.50; 95% CI, 1.06-2.12; P = .001; I

, 0%; 95% PI, 0.19-16.46; low confidence) and esophageal
cancer (2 studies in women; RR, 6.10; 95% CI, 2.30-16.18; P < .001; I

CONCLUSIONS AND RELEVANCE Among people with anorexia nervosa, risk of developing cancer
did not differ compared with the general population, but a significantly reduced risk of breast cancer

Physical and psychological paths toward less severe fibromyalgia: A structural equation model

fibromyalgia severity. Integration of physical and psychological, experienced and observed, modifiable
factors associated with fibromyalgia severity in a single model will reveal therapeutic paths toward less
severity of disease. We aimed to examine an encompassing model of determinants of fibromyalgia
severity.
Methods: This observational, population-based cross-sectional study included 569 people with
fibromyalgia. An integrative model of fibromyalgia severity was tested by using structural equation
modelling. This model included 8 factors: resilience, catastrophizing, active lifestyle, declarative
memory, subjective fitness, objective fitness, psychological distress, and physical fatigue.
Results: Two core paths were associated with reduced fibromyalgia severity: 1) a psychological path
connecting high resilience and low catastrophizing with low distress and 2) a physical path, connecting a
more active lifestyle (directly and via high objective and subjective physical fitness) with low fatigue.
Additional interconnecting paths especially suggested a connection from the psychological to physical
path. Our model explained 83% of the fibromyalgia severity.
Conclusions: The present model integrated the complexity of mutually influencing factors of fibromyalgia
severity, which may help to better understand the disease. It emphasised the importance of: 1) physical
factors and psychological factors and their interconnections, 2) patients’ experiences and clinical
measurements, and 3) positive and negative signs such as physical fitness and distress. Future
longitudinal and experimental research should aim at testing the causal direction of the associations in
the model as well as the clinical implications suggested by the model. For instance, to reduce fatigue,
exercise should enhance not only objective fitness but also fitness-related perceptions. Reducing distress

Objective and subjective measures of physical functioning in women with fibromyalgia: what type of measure is associated most clearly with subjective well-being?
To find modifiable factors that are related to subjective well-being would be valuable for improving
interventions in fibromyalgia. Physical activity, sedentary behaviour, and physical fitness may represent potential
areas to optimize treatment regimens. In fibromyalgia, there is a discordance between clinical observations and
patient-reported outcomes (objective and subjective assessments). Therefore, the present study aims at analyzing
the associations of objective and subjective evaluations of physical activity, sedentary behaviour, and physical
fitness with subjective well-being and determine if and how objective and subjective associations differ In the most conservative multivariate analysis, we found independent associations of the objective
measures of physical activity with positive affect and life satisfaction and sedentary behaviour with
positive affect. No such relationship was seen with subjective measures of the same behaviours.
Moreover, we observed that objective and subjective physical fitness evaluations were independent of
each other related to subjective well-being. Independent associations of the objectivemeasures (but not the subjective assessments) of physical
activity with positive affect and life satisfaction, and of sedentary behaviour with positive affect were observed.
However, objective measures and subjective appraisals of physical fitness appear to be independently related to

ALDH4A1 is an atherosclerosis auto-antigen targeted by protective antibodies
Cardiovascular disease (CVD) is the leading cause of mortality in the world, with most CVD-related deaths resulting from myocardial infarction or stroke. The main underlying cause of thrombosis and cardiovascular events is atherosclerosis, an inflammatory disease that can remain asymptomatic for long periods. There is an urgent need for therapeutic and diagnostic options in this area. Atherosclerotic plaques contain autoantibodies, and there is a connection between atherosclerosis and autoimmunity. However, the immunogenic trigger and the effects of the autoantibody response during atherosclerosis are not well understood. Here we performed high-throughput single-cell analysis of the atherosclerosis-associated antibody repertoire. Antibody gene sequencing of more than 1,700 B cells from atherogenic Ldlr and control mice identified 56 antibodies expressed by in-vivo-expanded clones of B lymphocytes in the context of atherosclerosis. One-third of the expanded antibodies were reactive against atherosclerotic plaques, indicating that various antigens in the lesion can trigger antibody responses. Deep proteomics analysis identified ALDH4A1, a mitochondrial dehydrogenase involved in proline metabolism, as a target antigen of one of these autoantibodies, A12. ALDH4A1 distribution is altered during atherosclerosis, and circulating ALDH4A1 is increased in mice and humans with atherosclerosis, supporting the potential use of ALDH4A1 as a disease biomarker. Predictive factors and early biomarkers of response in multiple sclerosis patients treated with natalizumab
There are an increasing number of treatments available for multiple sclerosis (MS). The early identification of optimal responders to individual treatments is important to achieve individualized therapy. With this aim, we performed a multicenter retrospective longitudinal study including 186 MS patients treated with natalizumab who were followed for 2 years. We analyzed the following variables at recruitment: sex, current age, age at disease onset, disease duration, EDSS, number of T2 and Gd + lesions, IgG and IgM oligoclonal bands, HLA class II (DR, DRB, DQA, DQB, and DRB1*15:01), IgG and IgM antibody titers against human herpesvirus 6 (HHV-6) and the antibody response to Epstein–Barr virus (EBV) through the measurement of the anti-EBNA-1 and anti-VCA IgG titers, in relation to clinical response (no relapses or disability progression), and to NEDA-3 (no evidence of disease activity in terms of clinical response and no changes in MRI scans either) after 2-years follow-up. Baseline EDSS score, baseline EBNA-1 IgG titers and percentage change of HHV6 IgG titers between baseline and 6 month visits were significantly different in clinical responders and in NEDA-3 status (all of them remained significant in the multivariate analysis). VAMOS: a Pathfinder for the HAWC Gamma-Ray Observatory
VAMOS was a prototype detector built in 2011 at an altitude of 4100m a.s.l.
in the state of Puebla, Mexico. The aim of VAMOS was to finalize the design,
construction techniques and data acquisition system of the HAWC observatory.
HAWC is an air-shower array currently under construction at the same site of
VAMOS with the purpose to study the TeV sky. The VAMOS setup included six water
Cherenkov detectors and two different data acquisition systems. It was in
operation between October 2011 and May 2012 with an average live time of 30%.
Besides the scientific verification purposes, the eight months of data were
used to obtain the results presented in this paper: the detector response to
the Forbush decrease of March 2012, and the analysis of possible emission, at
energies above 30 GeV, for long gamma-ray bursts GRB111016B and GRB120328B.Comment: Accepted for pubblication in Astroparticle Physics Journal (20 pages,

What to consider when pseudohypoparathyroidism is ruled out: IPPSD and differential diagnosis
Background: Pseudohypoparathyroidism (PHP) is a rare disease whose phenotypic features are rather difficult to identify in some cases. Thus, although these patients may present with the Albright''s hereditary osteodystrophy (AHO) phenotype, which is characterized by small stature, obesity with a rounded face, subcutaneous ossifications, mental retardation and brachydactyly, its manifestations are somewhat variable. Indeed, some of them present with a complete phenotype, whereas others show only subtle manifestations. In addition, the features of the AHO phenotype are not specific to it and a similar phenotype is also commonly observed in other syndromes. Brachydactyly type E (BDE) is the most specific and objective feature of the AHO phenotype, and several genes have been associated with syndromic BDE in the past few years. Moreover, these syndromes have a skeletal and endocrinological phenotype that overlaps with AHO/PHP. In light of the above, we have developed an algorithm to aid in genetic testing of patients with clinical features of AHO but with no causative molecular defect at the GNAS locus. Starting with the feature of brachydactyly, this algorithm allows the differential diagnosis to be broadened and, with the addition of other clinical features, can guide genetic testing. Methods: We reviewed our series of patients (n = 23) with a clinical diagnosis of AHO and with brachydactyly type E or similar pattern, who were negative for GNAS anomalies, and classify them according to the diagnosis algorithm to finally propose and analyse the most probable gene(s) in each case. Results: A review of the clinical data for our series of patients, and subsequent analysis of the candidate gene(s), allowed detection of the underlying molecular defect in 12 out of 23 patients: five patients harboured a mutation in PRKAR1A, one in PDE4D, four in TRPS1 and two in PTHLH. Conclusions: This study confirmed that the screening of other genes implicated in syndromes with BDE and AHO or a similar phenotype is very helpful for establishing a correct genetic diagnosis for those patients who have been misdiagnosed with "AHO-like phenotype" with an unknown genetic cause, and also for better describing the characteristic and differential features of these less common syndromes

3′,4′-Dihydroxyflavonol Antioxidant Attenuates Diastolic Dysfunction and Cardiac Remodeling in Streptozotocin-Induced Diabetic m(Ren2)27 Rats
Diabetic cardiomyopathy (DCM) is an increasingly recognized cause of chronic heart failure amongst diabetic patients. Both increased reactive oxygen species (ROS) generation and impaired ROS scavenging have been implicated in the pathogenesis of hyperglycemia-induced left ventricular dysfunction, cardiac fibrosis, apoptosis and hypertrophy. We hypothesized that 3',4'-dihydroxyflavonol (DiOHF), a small highly lipid soluble synthetic flavonol, may prevent DCM by scavenging ROS, thus preventing ROS-induced cardiac damage.Six week old homozygous Ren-2 rats were randomized to receive either streptozotocin or citrate buffer, then further randomized to receive either DiOHF (1 mg/kg/day) by oral gavage or vehicle for six weeks. Cardiac function was assessed via echocardiography and left ventricular cardiac catheterization before the animals were sacrificed and hearts removed for histological and molecular analyses. Diabetic Ren-2 rats showed evidence of diastolic dysfunction with prolonged deceleration time, reduced E/A ratio, and increased slope of end-diastolic pressure volume relationship (EDPVR) in association with marked interstitial fibrosis and oxidative stress (all P<0.05 vs control Ren-2). Treatment with DiOHF prevented the development of diastolic dysfunction and was associated with reduced oxidative stress and interstitial fibrosis (all P<0.05 vs untreated diabetic Ren-2 rats). In contrast, few changes were seen in non-diabetic treated animals compared to untreated counterparts.Inhibition of ROS production and action by DiOHF improved diastolic function and reduced myocyte hypertrophy as well as collagen deposition. These findings suggest the potential clinical utility of antioxidative compounds such as flavonols in the prevention of diabetes-associated cardiac dysfunction
