Individualization of attention-deficit/hyperactivity disorder treatment: Pharmacotherapy considerations by age and co-occurring conditions

Attention-deficit/hyperactivity disorder (ADHD) is a neurodevelopmental disorder that manifests in childhood and can persist into adolescence and adulthood. Impairments associated with ADHD can impact quality of life, social interactions, and increase the risk of morbidity and mortality; however, for many patients, effective treatment can lessen these effects. Pharmacotherapy with stimulants or nonstimulants is recommended in conjunction with psychosocial therapy for most patients. Determining the optimal pharmacotherapy can be complex, and the clinician needs to consider many factors such as the patient\u27s age, comorbidities, and lifestyle. Furthermore, the needs of the patient with ADHD will change over time, with specific challenges to consider at each stage of life. A variety of Food and Drug Administration (FDA)-approved stimulant and nonstimulant formulations are available with different modes of delivery and durations of effect. This armamentarium of ADHD medications can be used to individualize ADHD treatment for each patient\u27s needs. This article combines current information from the literature and the first-hand experience of the authors to provide guidance on ADHD treatment options for patients of different ages and for some of the more common comorbidities

CAPACITAÇÃO DE ADOLESCENTES DO ENSINO MÉDIO À PARTICIPAÇÃO SOCIAL NO SISTEMA ÚNICO DE SAÚDE

A Participação Social é constitucionalmente garantida a qualquer cidadão, sendo parte fundamental para a gestão e controle de políticas públicas. O objetivo deste estudo é relatar a experiência na execução de atividades de capacitação de adolescentes do ensino médio ao exercício da cidadania e controle social em saúde.  Realizou-se, no período de execução do projeto, rodas de conversas, apresentações orais e dinâmicas na escola Estadual de Ensino Médio Álvaro Adolfo da Silveira, município de Santarém, Pará, com a participação de 41 alunos do primeiro ano do Ensino Médio, do turno vespertino. As oficinas aconteceram semanalmente, no período de março a agosto de 2017. As experiências foram relacionadas à execução de métodos que promovessem reflexões -acerca dos Princípios e Diretrizes do Sistema Único de Saúde, dando ênfase à participação social e levantando questões relacionadas à Saúde e seus determinantes. Este processo gerou debates e construção mútua de conhecimento, favorecendo a apropriação de saberes que estimularam empoderamento e luta diária por direitos e espaço no controle social na saúde. Portanto, todas essas experiências mostram a necessidade de se produzir mais atividades e ações voltadas a multiplicar informações sobre o papel de cada um na sociedade, relacionadas, principalmente, aos direitos constitucionais. Este processo de formação pode gerar maiores reflexões do papel dos indivíduos para com o Estado, ampliando a percepção das responsabilidades mútuas na elaboração e controle das políticas públicas de saúde. Macroscopic-Imaging Technique for Subsurface Quantification of Near-Infrared Markers During Surgery

Obtaining accurate quantitative information on the concentration and distribution of fluorescent markers lying at a depth below the surface of optically turbid media, such as tissue, is a significant challenge. Here, we introduce a fluorescence reconstruction technique based on a diffusion light transport model that can be used during surgery, including guiding resection of brain tumors, for depth-resolved quantitative imaging of near-infrared fluorescent markers. Hyperspectral fluorescence images are used to compute a topographic map of the fluorophore distribution, which yields structural and optical constraints for a three-dimensional subsequent hyperspectral diffuse fluorescence reconstruction algorithm. Using the model fluorophore Alexa Fluor 647 and brain-like tissue phantoms, the technique yielded estimates of fluorophore concentration within ±25% of the true value to depths of 5 to 9 mm, depending on the concentration. The approach is practical for integration into a neurosurgical fluorescence microscope and has potential to further extend fluorescence-guided resection using objective and quantified metrics of the presence of residual tumor tissue

PLANEJAMENTO FINANCEIRO FAMILIAR DA POPULAÇÃO URBANA DE PONTE SERRADA

Com o conhecimento em finanças pessoais, e o auxílio de ferramentas como o orçamento doméstico é possível realizar planejamento financeiro familiar. Em face disso, o trabalho propôs verificar se as famílias urbanas do município de Ponte Serrada possuem planejamento financeiro familiar, utilizando-no para aplicação de investimentos de seus rendimentos, e quais as consequências que a falta de um planejamento pode causar às finanças da família. A metodologia utilizada foi pesquisa de campo, de caráter quantitativo, com finalidade descritiva com amostragem aleatória. A pesquisa foi realiza com 358 famílias e foram utilizados questionários. A tabulação dos dados foi realizada por meio de planilha eletrônica que geraram gráficos e tabelas. Com os resultados obtidos através da coleta e tabulação pode-se perceber que há famílias que utilizam controles para gerir seus gastos, destinam parte de seus rendimentos em investimentos, com destaque para a poupança, como também há famílias que não planejam e estão como uma situação financeira ruim, por não possuírem controle de seus gastos. O planejamento auxilia as famílias na melhor utilização dos recursos recebidos e na dedicação em investimentos, trazendo estabilidade frente aos imprevistos e na garantia de um futuro melhor.

Mapeamento do fluxo de valor aplicado a uma indústria de bolos artesanais

As a result of a more competitive scenario, the productivity in the bakery and confectionery sector have dropped making entrepreneurs looking for alternatives to make the business more profitable. Lean manufacturing allows us to adopt a new production approach and use more efficient resources, avoiding waste. The objective of this research was chart the value stream map of current and the future state in cakes manufacturing of a food company in São Paulo. In this case study, active observation, interviews, process analysis and documents were adopted, resulting in mapping the current state, identifying the processes, people and materials involved in the manufacturing. As a result of this mapping process, proposals for improvements have been made to reduce the lead time in production, using the kanban system, improving the manufacturing flow. In this way, this research collaborated in a practical way for a more efficient cakes production process.Como decorrência de um cenário mais competitivo, o setor de panificação e confeitaria sofreu queda na produtividade fazendo com que os empresários do segmento buscassem alternativas para tornar o negócio mais rentável. A manufatura enxuta permite adotar uma nova abordagem de produção e uso dos recursos mais eficientes evitando assim o desperdício. O objetivo desta pesquisa foi mapear o fluxo de valor atual e futuro do processo produtivo de fabricação de bolos de uma empresa alimentícia da grande São Paulo. No estudo de caso realizado foram adotadas as técnicas de observação ativa, entrevistas e análise de processos e documentos, resultando no mapeamento do estado atual, o que possibilitou a identificação dos processos, pessoas e materiais envolvidos na fabricação. A partir desse mapeamento surgiram propostas de melhorias que permitissem diminuir o lead time na produção, com o uso do sistema kanban, melhorando o fluxo da manufatura. Desta forma, esta pesquisa colaborou de forma prática para um processo de produção mais eficiente no atendimento da demanda

In-flight calibration and verification of the Planck-LFI instrument

In this paper we discuss the Planck-LFI in-flight calibration campaign. After a brief overview of the ground test campaigns, we describe in detail the calibration and performance verification (CPV) phase, carried out in space during and just after the cool-down of LFI. We discuss in detail the functionality verification, the tuning of the front-end and warm electronics, the preliminary performance assessment and the thermal susceptibility tests. The logic, sequence, goals and results of the in-flight tests are discussed. All the calibration activities were successfully carried out and the instrument response was comparable to the one observed on ground. El objetivo de este estudio fue establecer la efectividad del aula invertida para mejorar las habilidades de pensamiento de orden superior en la Taxonomía de Bloom. Esta fue una investigación cuali-cuantitativa que involucró la recopilación y conversión de datos en forma numérica, también fue una investigación cuasiexperimental y la población estuvo conformada por estudiantes de inglés de cuarto nivel del Centro de Inglés de la Escuela Superior Politécnica de Chimborazo. Los instrumentos aplicados fueron: encuesta inicial, encuesta de satisfacción, una prueba previa y una prueba posterior. La muestra se eligió al azar y los grupos estuvieron previamente establecidos. Los métodos científicos, inductivos, deductivos y descriptivos se aplicaron durante la intervención, misma que se desarrolló durante tres meses. Para analizar los resultados se siguió este procedimiento: organización de la información, análisis de resultados de la prueba inicial, análisis de las evidencias recopiladas, análisis de la prueba posterior, diseño de tablas, cuadros y gráficos, análisis de resultados estadísticos, tendencias de la hipótesis y conclusiones extraídas. Finalmente, se concluyó que las actividades de aula invertida en el aula de inglés como Lengua Extranjera mejoraron las habilidades de pensamiento de orden superior de los estudiantes

Compilado de documentos históricos: campanha civil contra o comunismo e o nazifascismo no arquivo de Orestes Augusto Alves, Curitiba (1941-1942)

This study tried to present the compilation of historical documents, produced in the city of Curitiba in the period between 1941 and 1942, entitled campaign against Nazism. As specific objectives, we tried to present Orestes Augusto Alves; describe what compiles this compiled; contextualize the historical moment in which the documents were elaborated. We counted on the studies of Bacellar (2015) for the treatment of the archives. It was possible to perceive that Orestes and his group were imbued with the sense of nationalism promoted in the Era Vargas. In addition, the large number of immigrants in the south of the country and the repercussions of the Second World War reinforced the fear of the foreign threat.Este estudo procurou apresentar o compilado de documentos históricos, produzidos na cidade de Curitiba no período entre 1941 e 1942, intitulados campanha contra o nazismo. Como objetivos específicos, procurou-se apresentar Orestes Augusto Alves; descrever o que compõe esse compilado; contextualizar o momento histórico em que os documentos foram elaborados. Contou-se com os estudos de Bacellar (2015) para o tratamento dos arquivos. Foi possível perceber que Orestes e seu grupo estavam imbuídos do sentimento de nacionalismo promovido na Era Vargas, além disso, o grande número de imigrantes no sul do país e a repercussão da 2º Guerra Mundial reforçavam o medo da ameaça estrangeira

Targeted Therapy for Metastatic Renal Carcinoma: An Update

Conventional chemotherapy is associated with poor outcomes in metastatic renal cell carcinoma (RCC). Advances in the understanding of tumor molecular biology and the implementation of new drugs that target these molecular pathways have increased the arsenal against advanced RCC and improved outcomes in these patients. Herein, we briefly describe the latest data on targeted therapies used in the treatment of advanced renal cell carcinoma. Search strategy was performed according to PRISMA guidelines. Abstracts of relevant studies published in PubMed between 2000 and 2014 were analyzed by two authors. Abstracts were selected if they were published in English, data reported was of phase II or III clinical trials, and outcomes followed FDA approval.  If consensus between the two authors was achieved, they were included in the review. Key words used were “target therapy” and “metastatic renal cell carcinoma”. The results of the studies analyzed in this review support the benefits of targeted therapy in metastatic RCC. These include improved progression-free survival, overall survival, and quality of life as well as reduced toxicities compared to immunotherapy. The improvement in outcomes in metastatic RCC makes these drugs a preferred option as a primary treatment for these patients.
