    Quality Of Antenatal Care In Rural Southern Tanzania: A Reality Check.

    Counselling on the danger signs of unpredictable obstetric complications and the appropriate management of such complications are crucial in reducing maternal mortality. The objectives of this study were to identify gaps in the provision of ANC services and knowledge of danger signs as well as the quality of care women receive in case of complications. The study took place in the Rufiji District of Tanzania in 2008 and was conducted in seven health facilities. The study used (1) observations from 63 antenatal care (ANC) sessions evaluated with an ANC checklist, (2) self-assessments of 11 Health workers, (3) interviews with 28 pregnant women and (4) follow-up of 12 women hospitalized for pregnancy-related conditions.Blood pressure measurements and abdominal examinations were common during ANC visits while urine testing for albumin or sugar or haemoglobin levels was rare which was often explained as due to a lack of supplies. The reasons for measuring blood pressure or abdominal examinations were usually not explained to the women. Only 15/28 (54%) women were able to mention at least one obstetric danger sign requiring medical attention. The outcomes of ten complicated cases were five stillbirths and three maternal complications. There was a considerable delay in first contact with a health professional or the start of timely interventions including checking vital signs, using a partograph, and detailed record keeping. Linking danger signs to clinical and laboratory examination results during ANC with the appropriate follow up and avoiding delays in emergency obstetric care are crucial to the delivery of coordinated, effective care interventions

    Potentiation of thrombus instability: a contributory mechanism to the effectiveness of antithrombotic medications

    © The Author(s) 2018The stability of an arterial thrombus, determined by its structure and ability to resist endogenous fibrinolysis, is a major determinant of the extent of infarction that results from coronary or cerebrovascular thrombosis. There is ample evidence from both laboratory and clinical studies to suggest that in addition to inhibiting platelet aggregation, antithrombotic medications have shear-dependent effects, potentiating thrombus fragility and/or enhancing endogenous fibrinolysis. Such shear-dependent effects, potentiating the fragility of the growing thrombus and/or enhancing endogenous thrombolytic activity, likely contribute to the clinical effectiveness of such medications. It is not clear how much these effects relate to the measured inhibition of platelet aggregation in response to specific agonists. These effects are observable only with techniques that subject the growing thrombus to arterial flow and shear conditions. The effects of antithrombotic medications on thrombus stability and ways of assessing this are reviewed herein, and it is proposed that thrombus stability could become a new target for pharmacological intervention.Peer reviewedFinal Published versio

    The psychological well-being of Norwegian adolescents exposed in utero to radiation from the Chernobyl accident

    <p>Abstract</p> <p>Background</p> <p>On 26 April 1986, the Chernobyl nuclear power plant suffered an accident. Several areas of central Norway were heavily affected by far field radioactive fallout. The present study focuses on the psychological well-being of adolescents who were exposed to this radiation as fetuses.</p> <p>Methods</p> <p>The adolescents (n = 53) and their mothers reported their perceptions of the adolescents' current psychological health as measured by the Youth Self Report and Child Behaviour Checklist.</p> <p>Results</p> <p>In spite of previous reports of subtle cognitive deficits in these exposed adolescents, there were few self-reported problems and fewer problems reported by the mothers. This contrasts with findings of studies of children from the former Soviet Union exposed in utero, in which objective measures are inconsistent, and self-reports, especially by mothers, express concern for adolescents' cognitive functioning and psychological well-being.</p> <p>Conclusion</p> <p>In the current paper, we explore possible explanations for this discrepancy and suggest that protective factors in Norway, in addition to perceived physical and psychological distance from the disaster, made the mothers less vulnerable to Chernobyl-related anxiety, thus preventing a negative effect on the psychological health of both mother and child.</p

    The effect of exposure to synthetic pheromone lures on male Zygaena filipendulae mating behaviour: implications for monitoring species of conservation interest

    Pheromone based monitoring of insects of conservation value has the potential to revolutionise the way in which surveys are carried out. However, due to their effective use in pest management, concerns have been raised about potential negative effects of pheromone exposure on populations of rare insects. The effect of exposure to synthetic pheromone lures on male mating behaviour was examined in laboratory and field conditions using the six spot burnet moth Zygaena filipendulae (Linnaeus, 1758). For the laboratory experiment larvae were collected and cultured separately under controlled conditions. Virgin males were exposed to a synthetic pheromone lure for 24 h; then tested for responsiveness immediately after this exposure, 1 and 24 h later. Control males were tested three times: initially, 1 h later and 24 h later. The time taken for males to detect females, shown by exposure of their anal claspers, and the time taken for males to locate females were recorded. No significant difference was found between the time taken for control and exposed males to detect or locate females, and no significant difference between the proportions of males that successfully located females in exposed and control groups was found. In the field experiment the time males spent in the presence of contained females, both with and without a pheromone lure present, was recorded. Males spent more time in the presence of the females when the pheromone lure was present. Both experiments indicate male Z. filipendulae mating behaviour is not adversely affected by exposure to synthetic pheromone lures

    Sustainable Phosphorus Loadings from Effective and Cost-Effective Phosphorus Management Around the Baltic Sea

    Nutrient over-enrichment of the Baltic Sea, accompanied by intensified algal blooms and decreasing water clarity, has aroused widespread concern in the surrounding countries during the last four decades. This work has used a well-tested dynamic mass-balance model to investigate which decrease in total phosphorus loading would be required to meet the environmental goal to restore the trophic state in the Baltic Sea to pre-1960s levels. Furthermore, the extent to which various abatement options may decrease the phosphorus loading in a cost-effective manner has been studied. Upgrading urban sewage treatment in the catchment could, alone or in combination with banning phosphates in detergents, be sufficient to meet the set environmental goal, at an estimated annual basin-wide cost of 0.21–0.43 billion euro. Such a plan would potentially decrease the total phosphorus loading to the Baltic Sea with 6,650–10,200 tonnes per year

    Space-time code diversity by phase rotation in multi-carrier multi-user systems

    Code diversity using space-time block codes was developed for single-carrier and single-receiver systems. In this paper, the extension of code diversity by phase rotation to multi-user and multi-carrier systems is proposed and analyzed. We show that code diversity with reduced feedback is possible in this new scenario and the coding gain has a mild logarithmic decrease with the number of users and the number of sub-carriers. In addition, we develop an analytical upper bound for the average error probability whose accuracy is verified by simulation

    <p>Abstract</p> <p>Background</p> <p>Studies have shown that metabolic syndrome and its consequent biochemical derangements in the various phases of diabetes may contribute to carcinogenesis. A part of this carcinogenic effect could be attributed to hyperinsulinism. High levels of insulin decrease the production of IGF-1 binding proteins and hence increase levels of free IGF-1. It is well established that bioactivity of free insulin growth factor 1 (IGF-1) increases tumor turnover rate. The objective is to investigate the role of insulin resistance/sensitivity in carcinogenesis by studying the relation between insulin resistance/sensitivity and IGF-1 levels in cancer patients. We postulate that hyperinsulinaemia which prevails during initial phases of insulin resistance (condition prior to overt diabetes) increases bioactivity of free IGF-1, which may contribute to process of carcinogenesis.</p> <p>Methods/Design</p> <p>Based on our pilot study results and power analysis of the same, we have designed a two group case-control study. 800 proven untreated cancer patients (solid epithelial cell tumors) under age of 50 shall be recruited with 200 healthy subjects serving as controls. Insulin resistance/sensitivity and free IGF-1 levels shall be determined in all subjects. Association between the two parameters shall be tested using suitable statistical methods.</p> <p>Discussion</p> <p>Well controlled studies in humans are essential to study the link between insulin resistance, hyperinsulinaemia, IGF-1 and carcinogenesis. This study could provide insights to the role of insulin, insulin resistance, IGF-1 in carcinogenesis although a precise role and the extent of influence cannot be determined. In future, cancer prevention and treatment strategies could revolve around insulin and insulin resistance.</p

    Persistence within dendritic cells marks an antifungal evasion and dissemination strategy of Aspergillus terreus

    Aspergillus terreus is an airborne human fungal pathogen causing life-threatening invasive aspergillosis in immunocompromised patients. In contrast to Aspergillus fumigatus, A. terreus infections are associated with high dissemination rates and poor response to antifungal treatment. Here, we compared the interaction of conidia from both fungal species with MUTZ-3-derived dendritic cells (DCs). After phagocytosis, A. fumigatus conidia rapidly escaped from DCs, whereas A. terreus conidia remained persisting with long-term survival. Escape from DCs was independent from DHN-melanin, as A. terreus conidia expressing wA showed no increased intracellular germination. Within DCs A. terreus conidia were protected from antifungals, whereas A. fumigatus conidia were efficiently cleared. Furthermore, while A. fumigatus conidia triggered expression of DC activation markers such as CD80, CD83, CD54, MHCII and CCR7, persistent A. terreus conidia were significantly less immunogenic. Moreover, DCs confronted with A. terreus conidia neither produced pro-inflammatory nor T-cell stimulating cytokines. However, TNF-α addition resulted in activation of DCs and provoked the expression of migration markers without inactivating intracellular A. terreus conidia. Therefore, persistence within DCs and possibly within other immune cells might contribute to the low response of A. terreus infections to antifungal treatment and could be responsible for its high dissemination rates

    A change in the optical polarization associated with a gamma-ray flare in the blazar 3C 279

    It is widely accepted that strong and variable radiation detected over all accessible energy bands in a number of active galaxies arises from a relativistic, Doppler-boosted jet pointing close to our line of sight. The size of the emitting zone and the location of this region relative to the central supermassive black hole are, however, poorly known, with estimates ranging from light-hours to a light-year or more. Here we report the coincidence of a gamma-ray flare with a dramatic change of optical polarization angle. This provides evidence for co-spatiality of optical and gamma-ray emission regions and indicates a highly ordered jet magnetic field. The results also require a non-axisymmetric structure of the emission zone, implying a curved trajectory for the emitting material within the jet, with the dissipation region located at a considerable distance from the black hole, at about 10^5 gravitational radii.Comment: Published in Nature issued on 18 February 2010.
