El régimen jurídico del control ambiental del Estado español sobre la pesca en el Mar Mediterráneo
Sustainable fishing is an increasingly pressing political and social claim ordered by the environmental awareness of Western society and by the alarming lack of fishing resources. With the aim of reinforcing marine biodiversity and the economic sustainability of fisheries, new regulations are being sanctioned which, although they affect all fishing grounds, the importance of the Mediterranean, greatly affected by millennia of human activity, is usually mentioned. It is therefore necessary to review and review the existing administrative controls and the resources available to the executive branch to understand the situation of Mediterranean fishing, its environmental and economic effects, and the real possibilities of effective control.La pesca sostenible es un reclamo político y social cada vez más acuciante ordenado por las sensibilizaciones ambientales de la sociedad occidental y por la cada vez más alarmante falta de recursos pesqueros. Con el objetivo de reforzar la biodiversidad marina y la sostenibilidad económica pesquera se están aprobando normativas que, si bien afectan a todos los caladeros, se suele mencionar especialmente la importancia del Mediterráneo, gran afectado por milenios de actividad humana. Es necesario, por tanto, un repaso y revisión de los controles administrativos existentes y los recursos de los que dispone el poder ejecutivo a fin de conocer la situación de la pesca mediterránea, sus afectaciones ambientales y económicas y las posibilidades reales de un control eficaz
Fruit detection in an apple orchard using a mobile terrestrial laser scanner
The development of reliable fruit detection and localization systems provides an opportunity to improve the crop value and management by limiting fruit spoilage and optimised harvesting practices. Most proposed systems for fruit detection are based on RGB cameras and thus are affected by intrinsic constraints, such as variable lighting conditions. This work presents a new technique that uses a mobile terrestrial laser scanner (MTLS) to detect and localise Fuji apples. An experimental test focused on Fuji apple trees (Malus domestica Borkh. cv. Fuji) was carried out. A 3D point cloud of the scene was generated using an MTLS composed of a Velodyne VLP-16 LiDAR sensor synchronised with an RTK-GNSS satellite navigation receiver. A reflectance analysis of tree elements was performed, obtaining mean apparent reflectance values of 28.9%, 29.1%, and 44.3% for leaves, branches and trunks, and apples, respectively. These results suggest that the apparent reflectance parameter (at 905 nm wavelength) can be useful to detect apples. For that purpose, a fourstep fruit detection algorithm was developed. By applying this algorithm, a localization success of 87.5%, an identification success of 82.4%, and an F1-score of 0.858 were obtained in relation to the total amount of fruits. These detection rates are similar to those obtained by RGB-based systems, but with the additional advantages of providing direct 3D fruit location information, which is not affected by sunlight variations. Early detection of deterioration in COVID-19 patients by continuous ward respiratory rate monitoring: a pilot prospective cohort study
BackgroundTachypnea is among the earliest signs of pulmonary decompensation. Contactless continuous respiratory rate monitoring might be useful in isolated COVID-19 patients admitted in wards. We therefore aimed to determine whether continuous monitoring of respiratory patterns in hospitalized patients with COVID-19 predicts subsequent need for increased respiratory support.MethodsSingle-center pilot prospective cohort study in COVID-19 patients who were cared for in routine wards. COVID-19 patients who had at least one escalation of pulmonary management were matched to three non-escalated patients. Contactless respiratory monitoring was instituted after patients enrolled, and continued for 15 days unless hospital discharge, initiation of invasive mechanical ventilation, or death occurred. Clinicians were blinded to respiratory rate data from the continuous monitor. The exposures were respiratory features over rolling periods of 30 min, 24 h, and 72 h before respiratory care escalation. The primary outcome was a subsequent escalation in ventilatory support beyond a Venturi mask.ResultsAmong 125 included patients, 13 exhibited at least one escalation and were each matched to three non-escalated patients. A total of 28 escalation events were matched to 84 non-escalation episodes. The 30-min mean respiratory rate in escalated patients was 23 breaths per minute (bpm) ranging from 13 to 40 bpm, similar to the 22 bpm in non-escalated patients, although with less variability (range 14 to 31 bpm). However, higher respiratory rate variability, especially skewness over 1 day, was associated with higher incidence of escalation events. Our overall model, based on continuous data, had a moderate accuracy with an AUC 0.81 (95%CI: 0.73, 0.88) and a good specificity 0.93 (95%CI: 0.87, 0.99).ConclusionOur pilot observational study suggests that respiratory rate variability as detected with continuous monitoring is associated with subsequent care escalation during the following 24 h. Continuous respiratory monitoring thus appears to be a valuable increment over intermittent monitoring.Strengths and limitationsOur study was the initial evaluation of Circadia contactless respiratory monitoring in COVID-19 patients who are at special risk of pulmonary deterioration. The major limitation is that the analysis was largely post hoc and thus needs to be confirmed in an out-of-sample population
Consumption of caffeinated beverages and kidney function decline in an elderly Mediterranean population with metabolic syndrome
It remains unclear whether caffeinated beverages could have deleterious renal effects in elderly population with underlying comorbid conditions. We investigated the associations between coffee, tea, or caffeine intake and 1-year changes in glomerular filtration rate (eGFR) in a large Spanish cohort of overweight/obese elderly with metabolic syndrome (MetS). This prospective analysis includes 5851 overweight/obese adults (55-75 years) with MetS from the PREDIMED-Plus study. We assessed coffee, tea, and caffeine consumption from a validated food-frequency questionnaire and creatinine-based eGFR using the Chronic Kidney Disease Epidemiology Collaboration equation. Multivariate-adjusted regression models were applied to test associations between baseline coffee, tea, or caffeine intake and 1-year eGFR changes. Caffeinated coffee (> 2 cups/day) and tea (at least 1 cup/day) drinkers had 0.88 and 0.93 mL/min/1.73 m2 greater eGFR decrease respectively, compared to those with less than 1 cup/day of coffee consumption or non-tea drinkers. Furthermore, caffeinated coffee consumption of > 2 cups/day was associated with 1.19-fold increased risk of rapid eGFR decline > 3 mL/min/1.73 m2 (95% CI 1.01-1.41). Similarly, individuals in the highest (median, 51.2 mg/day) tertile of caffeine intake had a 0.87 mL/min/1.73 m2 greater eGFR decrease. Decaffeinated coffee was not associated with eGFR changes. In conclusion, higher consumption of caffeinated coffee, tea, and caffeine was associated with a greater 1-year eGFR decline in overweight/obese adults with MetS

The aim of our study is to assess the relationship between fruit and vegetable (F&V) consumption and plasma FAs and
their subtypes.
Methods and Results: Plasma FAs are assessed in a cross-sectional analysis of a subsample of 240 subjects from the
PREDIMED-Plus study. Participants are categorized into four groups of fruit, vegetable, and fat intake according to the
food frequency questionnaire. Plasma FA analysis is performed using gas chromatography. Associations between FAs
and F&V consumption are adjusted for age, sex, physical activity, bodymass index (BMI), total energy intake, and alcohol
consumption. Plasma saturated FAs are lower in groups with high F&V consumption (-1.20 mg cL−1 [95% CI: [-2.22, -
0.18], p-value = 0.021), especially when fat intake is high (-1.74 mg cL−1 [95% CI: [-3.41, -0.06], p-value = 0.042). Total
FAs and n-6 polyunsaturated FAs tend to be lower in high consumers of F&V only in the high-fat intake groups.
Conclusions: F&V consumption is associated with lower plasma saturated FAs when fat intake is high. These findings
suggest that F&V consumption may have different associations with plasma FAs depending on their subtype and on the

Adopting a High-Polyphenolic Diet Is Associated with an Improved Glucose Profile: Prospective Analysis within the PREDIMED-Plus Trial
Previous studies suggested that dietary polyphenols could reduce the incidence and complications of type-2 diabetes (T2D); although the evidence is still limited and inconsistent. This work analyzes whether changing to a diet with a higher polyphenolic content is associated with an improved glucose profile. At baseline, and at 1 year of follow-up visits, 5921 participants (mean age 65.0 ± 4.9, 48.2% women) who had overweight/obesity and metabolic syndrome filled out a validated 143-item semi-quantitative food frequency questionnaire (FFQ), from which polyphenol intakes were calculated. Energy-adjusted total polyphenols and subclasses were categorized in tertiles of changes. Linear mixed-effect models with random intercepts (the recruitment centers) were used to assess associations between changes in polyphenol subclasses intake and 1-year plasma glucose or glycosylated hemoglobin (HbA1c) levels. Increments in total polyphenol intake and some classes were inversely associated with better glucose levels and HbA1c after one year of follow-up. These associations were modified when the analyses were run considering diabetes status separately. To our knowledge, this is the first study to assess the relationship between changes in the intake of all polyphenolic groups and T2D-related parameters in a senior population with T2D or at high-risk of developing T2
Effect of a Nutritional and Behavioral Intervention on Energy-Reduced Mediterranean Diet Adherence Among Patients With Metabolic Syndrome: Interim Analysis of the PREDIMED-Plus Randomized Clinical Trial

ObjectiveTo assess the effect of a nutritional and physical activity education program on dietary quality. Design, Setting, and ParticipantsPreliminary exploratory interim analysis of an ongoing randomized trial. In 23 research centers in Spain, 6874 men and women aged 55 to 75 years with metabolic syndrome and no cardiovascular disease were enrolled in the trial between September 2013 and December 2016, with final data collection in March 2019. InterventionsParticipants were randomized to an intervention group that encouraged an energy-reduced Mediterranean diet, promoted physical activity, and provided behavioral support (n=3406) or to a control group that encouraged an energy-unrestricted Mediterranean diet (n=3468). All participants received allotments of extra-virgin olive oil (1 L/mo) and nuts (125 g/mo) for free. Main Outcomes and MeasuresThe primary outcome was 12-month change in adherence based on the energy-reduced Mediterranean diet (er-MedDiet) score (range, 0-17; higher scores indicate greater adherence; minimal clinically important difference, 1 point). ResultsAmong 6874 randomized participants (mean [SD] age, 65.0 [4.9] years; 3406 [52%] men), 6583 (96%) completed the 12-month follow-up and were included in the main analysis. The mean (SD) er-MedDiet score was 8.5 (2.6) at baseline and 13.2 (2.7) at 12 months in the intervention group (increase, 4.7 [95% CI, 4.6-4.8]) and 8.6 (2.7) at baseline and 11.1 (2.8) at 12 months in the control group (increase, 2.5 [95% CI, 2.3-2.6]) (between-group difference, 2.2 [95% CI, 2.1-2.4]; P<.001). Conclusions and RelevanceIn this preliminary analysis of an ongoing trial, an intervention that encouraged an energy-reduced Mediterranean diet and physical activity, compared with advice to follow an energy-unrestricted Mediterranean diet, resulted in a significantly greater increase in diet adherence after 12 months. Further evaluation of long-term cardiovascular effects is needed.
