    Immunisation of migrants in EU/EEA countries: Policies and practices

    In recent years various EU/EEA countries have experienced an influx of migrants from low and middle-income countries. In 2018, the “Vaccine European New Integrated Collaboration Effort (VENICE)” survey group conducted a survey among 30 EU/EEA countries to investigate immunisation policies and practices targeting irregular migrants, refugees and asylum seekers (later called “migrants” in this report). Twenty-nine countries participated in the survey. Twenty-eight countries reported having national policies targeting children/adolescent and adult migrants, however vaccinations offered to adult migrants are limited to specific conditions in seven countries. All the vaccinations included in the National Immunisation Programme (NIP) are offered to children/adolescents in 27/28 countries and to adults in 13/28 countries. In the 15 countries offering only certain vaccinations to adults, priority is given to diphtheria-tetanus, measles-mumps-rubella and polio vaccinations. Information about the vaccines given to child/adolescent migrants is recorded in 22 countries and to adult migrants in 19 countries with a large variation in recording methods found across countries. Individual and aggregated data are reportedly not shared with other centres/institutions in 13 and 15 countries, respectively. Twenty countries reported not collecting data on vaccination uptake among migrants; only three countries have these data at the national level. Procedures to guarantee migrants’ access to vaccinations at the community level are available in 13 countries. In conclusion, although diversified, strategies for migrant vaccination are in place in all countries except for one, and the strategies are generally in line with international recommendations. Efforts are needed to strengthen partnerships and implement initiatives across countries of origin, transit and destination to develop and better share documentation in order to guarantee a completion of vaccination series and to avoid unnecessary re-vaccination. Development of migrant-friendly strategies to facilitate migrants' access to vaccination and collection of vaccination uptake data among migrants is needed to meet existing gaps

    Immunity to mumps virus in children population in Bulgaria

    AbstractMumps is an acute, contagious, viral vaccine-preventable disease caused by mumps virus (MuV). The measles, mumps, rubella (MMR) vaccine that is used in many countries is considered highly effective with decreased MuV incidence, but suboptimal MuV long-term immunity. This study assessed the MuV seropositivity and antibody titre among vaccinated children in Bulgaria to provide evidence for a better understanding of MuV circulation and immunity in Bulgaria. The samples from 734 immunized children (369 females and 365 males) aged 1 to 16, divided into four age groups (≥1, 2–6, 7–11, and 12–16) were tested. Qualitative and quantitative indirect enzyme-linked immunosorbent assay (Anti-Mumps IgG ELISA, Euroimmun, Germany) was performed to determine the mumps IgG antibody levels in sera. Among all participants, protective MuV immunity was identified in 93%. MuV IgG antibody positivity ranged between 87% in the age group between 1 and 2 years and 96% in the age group 12–16 years, but no statistically significant difference was found among age groups. At the same time, there was a statistically significant difference (p = 0.01) between seropositivity in male and female participants, with male participants having an overall seropositivity of 90% and female 95%. Among the antibody-positive samples, the quantitative measurements of median anti-MuV IgG concentrations showed that titres decreased with increasing age. A slightly waning immunity following vaccination was observed, but positivity remained high among vaccinated children over the years. Similar studies show that maintaining high immunity is crucial to prevent mumps outbreaks

    Different transmission patterns in the early stages of the influenza A(H1N1)v pandemic: a comparative analysis of 12 European countries.

    Following the emergence of a novel strain of influenza A(H1N1) in Mexico and the United States in April 2009, its epidemiology in Europe during the summer was limited to sporadic and localised outbreaks. Only the United Kingdom experienced widespread transmission declining with school holidays in late July. Using statistical modelling where applicable we explored the following causes that could explain this surprising difference in transmission dynamics: extinction by chance, differences in the susceptibility profile, age distribution of the imported cases, differences in contact patterns, mitigation strategies, school holidays and weather patterns. No single factor was able to explain the differences sufficiently. Hence an additive mixed model was used to model the country-specific weekly estimates of the effective reproductive number using the extinction probability, school holidays and weather patterns as explanatory variables. The average extinction probability, its trend and the trend in absolute humidity were found to be significantly negatively correlated with the effective reproduction number - although they could only explain about 3% of the variability in the model. By comparing the initial epidemiology of influenza A (H1N1) across different European countries, our analysis was able to uncover a possible role for the timing of importations (extinction probability), mixing patterns and the absolute humidity as underlying factors. However, much uncertainty remains. With better information on the role of these epidemiological factors, the control of influenza could be improved
