    Relationship between self-reported dietary intake and physical activity levels among adolescents: The HELENA study

    Background Evidence suggests possible synergetic effects of multiple lifestyle behaviors on health risks like obesity and other health outcomes. Therefore it is important to investigate associations between dietary and physical activity behavior, the two most important lifestyle behaviors influencing our energy balance and body composition. The objective of the present study is to describe the relationship between energy, nutrient and food intake and the physical activity level among a large group of European adolescents. Methods The study comprised a total of 2176 adolescents (46.2% male) from ten European cities participating in the HELENA (Healthy Lifestyle in Europe by Nutrition in Adolescence) study. Dietary intake and physical activity were assessed using validated 24-h dietary recalls and self-reported questionnaires respectively. Analyses of covariance (ANCOVA) were used to compare the energy and nutrient intake and the food consumption between groups of adolescents with different physical activity levels (1st to 3rd tertile). Results In both sexes no differences were found in energy intake between the levels of physical activity. The most active males showed a higher intake of polysaccharides, protein, water and vitamin C and a lower intake of saccharides compared to less active males. Females with the highest physical activity level consumed more polysaccharides compared to their least active peers. Male and female adolescents with the highest physical activity levels, consumed more fruit and milk products and less cheese compared to the least active adolescents. The most active males showed higher intakes of vegetables and meat, fish, eggs, meat substitutes and vegetarian products compared to the least active ones. The least active males reported the highest consumption of grain products and potatoes. Within the female group, significantly lower intakes of bread and cereal products and spreads were found for those reporting to spend most time in moderate to vigorous physical activity. The consumption of foods from the remaining food groups, did not differ between the physical activity levels in both sexes. Conclusion It can be concluded that dietary habits diverge between adolescents with different self-reported physical activity levels. For some food groups a difference in intake could be found, which were reflected in differences in some nutrient intakes. It can also be concluded that physically active adolescents are not always inclined to eat healthier diets than their less active peers.The HELENA study took place with the financial support of the European Community Sixth RTD Framework Programme (Contract FOOD-CT: 2005-007034).     Evaluation of iron status in European adolescents through biochemical iron indicators: the HELENA Study

    BACKGROUND/OBJECTIVES: To assess the iron status among European adolescents through selected biochemical parameters in a cross-sectional study performed in 10 European cities. SUBJECTS/METHODS: Iron status was defined utilising biochemical indicators. Iron depletion was defined as low serum ferritin (SF8.5 mg/l) plus iron depletion. Iron deficiency anaemia (IDA) was defined as ID with haemoglobin (Hb) below the WHO cutoff for age and sex: 12.0 g/dl for girls and for boys aged 12.5-14.99 years and 13.0 g/dl for boys aged ≄15 years. Enzyme linked immunosorbent assay was used as analytical method for SF, sTfR and C-reactive protein (CRP). Subjects with indication of inflammation (CRP >5 mg/l) were excluded from the analyses. A total of 940 adolescents aged 12.5-17.49 years (438 boys and 502 girls) were involved. RESULTS: The percentage of iron depletion was 17.6%, significantly higher in girls (21.0%) compared with boys (13.8%). The overall percentage of ID and IDA was 4.7 and 1.3%, respectively, with no significant differences between boys and girls. A correlation was observed between log (SF) and Hb (r = 0.36, P < 0.01), and between log (sTfR) and mean corpuscular haemoglobin (r = -0.30, P < 0.01). Iron body stores were estimated on the basis of log (sTfR/SF). A higher percentage of negative values of body iron was recorded in girls (16.5%) with respect to boys (8.3%), and body iron values tended to increase with age in boys, whereas the values remained stable in girls.     Dietary animal and plant protein intakes and their associations with obesity and cardio-metabolic indicators in European adolescents: The HELENA cross-sectional study

    Background: Previous studies suggest that dietary protein might play a beneficial role in combating obesity and its related chronic diseases. Total, animal and plant protein intakes and their associations with anthropometry and serum biomarkers in European adolescents using one standardised methodology across European countries are not well documented. Objectives: To evaluate total, animal and plant protein intakes in European adolescents stratified by gender and age, and to investigate their associations with cardio-metabolic indicators (anthropometry and biomarkers). Methods: The current analysis included 1804 randomly selected adolescents participating in the HELENA study (conducted in 2006-2007) aged 12.5-17.5 y (47% males) who completed two non-consecutive computerised 24-h dietary recalls. Associations between animal and plant protein intakes, and anthropometry and serum biomarkers were examined with General linear Model multivariate analysis. Results: Average total protein intake exceeded the recommendations of World Health Organization and European Food Safety Authority. Mean total protein intake was 96 g/d (59% derived from animal protein). Total, animal and plant protein intakes (g/d) were significantly lower in females than in males and total and plant protein intakes were lower in younger participants (12.5-14.9 y). Protein intake was significantly lower in underweight subjects and higher in obese ones; the direction of the relationship was reversed after adjustments for body weight (g/(kg.d)). The inverse association of plant protein intakes was stronger with BMI z-score and body fat percentage (BF%) compared to animal protein intakes. Additionally, BMI and BF% were positively associated with energy percentage of animal protein. Conclusions: This sample of European adolescents appeared to have adequate total protein intake. Our findings suggest that plant protein intakes may play a role in preventing obesity among European adolescents. Further longitudinal studies are needed to investigate the potential beneficial effects observed in this study in the prevention of obesity and related chronic diseases

    Assessing children&apos;s diet quality in Crete based on healthy eating index: The children study

    Background: This study aimed to assess children&apos;s diet quality in Crete and the potential role of several socio-demographic factors related to it. Methods: Between October 2005 and March 2006, 481 primary school children were recruited from Crete. Dietary intake data was obtained using a combination of techniques comprising a 24 h recall and 3 food diaries. The Healthy Eating Index (HEI) was calculated summing the individual scores (0-10) assigned to 10 index components. Results: The majority of participants (84.5%) had diet that &apos;needs improvement&apos; (HEI score: 51-80). Twelve percent of participants had &apos;poor diet&apos; and only 3.5% of schoolchildren had &apos;good diet&apos; (HEI score &gt;80). The overall mean of the HEI score was 60.5. The mean values of the HEI components score ranged between 2.8 (that is, the HEI component that measures vegetables intake) and 9.9 (that is, component that expresses the variety of consumed foods). Low mean values were found in the total fat and saturated fat components (3.7 and 3.4, respectively). Moreover, the HEI score was strongly associated with dietary macronutrients and micronutrients intake. Conclusion: On the basis of the HEI score, the vast majority of children were found to have a diet that &apos;needs improvement&apos;. This indicates the increased necessity of drawing and implementing nutrition education programs targeting both children and their families and aiming to increase vegetables and fruits intake and reduce fat intake

    Diet quality of preschool children and maternal perceptions/misperceptions: The GENESIS study

    Objectives: To quantify maternal perceptions regarding the quality of their child&apos;s diet, and to identify factors associated with misperceptions. Study design: A representative sample of 2287 children aged 2-5 years from a cross-sectional study (GENESIS study) was used. Methods: Maternal perceptions of the quality of their child&apos;s diet, child&apos;s and mother&apos;s anthropometric characteristics, and other characteristics (i.e. socio-demographic and lifestyle) were recorded. The actual quality of each child&apos;s diet was estimated using the Healthy Eating Index (HEI) score. Results: Based on the HEI score, 18.3% of participants had a &apos;poor&apos; diet, 81.5% had a diet which &apos;needs improvement&apos; and only 0.2% had a &apos;good&apos; diet. Almost 83% of mothers overestimated the quality of their child&apos;s diet. The overestimation rate was 86% among mothers who declared that they choose their child&apos;s food based on what they consider to be healthy, and 72% among those who reported that other factors play the predominant role in food choices for their child (P &lt; 0.001). Moreover, total energy intake as well as the intake of fruits, grains, vegetables, meat and milk was significantly higher among children whose mothers overestimated the quality of their diet. Conclusion: The vast majority of mothers overestimate the quality of their child&apos;s diet. Given that maternal perceptions regarding the quality of their child&apos;s diet are likely to be one of the predominant factors determining the child&apos;s food intake, health professionals should make mothers aware of the existence of particular dietary recommendations that their children should meet in order to eat a healthy diet. © 2009 The Royal Society for Public Health

    Obesity and television watching in preschoolers in greece: The genesis study

    The aim of the current work was to evaluate the effect of preschoolers&apos; television (TV) watching time on the prevalence of obesity even after controlling for their total energy intake and their physical activity status. A representative sample of 2,374 Greek children aged 1-5 years was examined (Growth, Exercise and Nutrition Epidemiological Study in preSchoolers, GENESIS study). Children&apos;s TV watching time on a usual weekday and at a usual weekend was recorded. The overall mean of children&apos;s TV viewing time was 1.32 h/day. The majority of participants (74.0%) spent 2 h/day watching TV whereas only 3.1% spent 4 h/day in front of a TV set. Overall, 65.2% of participants were normal weight, 17.2% were overweight, and the rest 17.6% were obese. The prevalence of obesity was significantly higher among those with TV viewing time 2 h/day (21.7%) compared to those watching TV 2 h/day (16.1%, P = 0.003). TV viewing time remained significantly associated with the likelihood of being obese even after controlling for potential confounders (i.e., socio demographic and other characteristics and physical activity status) only among children aged 3-5 years. However, further adjusting for children&apos;s total energy intake revealed that the association between the TV viewing time and the probability of being obese was no longer statistically significant. On the other hand, physical activity status continued to be an independent factor of being obese. The current findings support the hypothesis that the effect of TV viewing time on childhood obesity is independent of physical activity status and may be attributed to the increased total energy intake during TV watching

    The aim of the present study was to investigate the interaction between the angiotensin-converting enzyme 1 (ACE) I/D polymorphism and energy and macronutrient intakes on adiposity-related phenotypes among toddlers and preschoolers. A representative sample of 2374 Greek children aged 1 to 5 years old was examined (Growth, Exercise and Nutrition Epidemiological Study in preSchoolers (GENESIS)). Dietary and anthropometric (i.e. BMI, waist circumference (WC)) assessments were carried out using standard procedures. DNA samples were obtained from 2102 children and were genotyped for the ACE I/D polymorphism. Among the entire population, 17% were &apos;at risk of overweight&apos; and a similar percentage were &apos;overweight&apos;. The frequencies of the II, ID and DD genotypes were 16, 46 and 38 %, respectively. Significant interactions were found between the ACE I/D polymorphism and total energy intake on WC (P = 0.004 for interaction) and the ACE I/D polymorphism and protein intake on BMI and being overweight (P &lt; 0.05 for interaction). Furthermore, it was found that the ACE I/D polymorphism may modify the effect of fat intake on WC and BMI, but this interaction disappeared after adjustment for additional potential confounders. Stratified analyses revealed that total energy is correlated with WC and protein intake is associated with BMI and being overweight only among carriers of the D-allele (i.e. DD or ID genotypes). These results suggest that the ACE I/D polymorphism may act as a modifying factor in the response of adiposity-related phenotypes to diet. © The Authors 2008
