Birational self-maps and piecewise algebraic geometry
Let X be a smooth projective complex variety, of dimension 3, whose Hodge
numbers h^{3,0}(X), h^{1,0}(X) both vanish. Let f: X--> X be a birational map
that induces an isomorphism on (dense) open subvarieties U,V of X. Then we show
that the complex reduced varieties (X \ U), (X \ V) are piecewise isomorphic
Periodic behaviour of coronal mass ejections, eruptive events, and solar activity proxies during solar cycles 23 and 24
We report on the parallel analysis of the periodic behaviour of coronal mass
ejections (CMEs) based on 21 years [1996 -- 2016] of observations with the
SOHO/LASCO--C2 coronagraph, solar flares, prominences, and several proxies of
solar activity. We consider values of the rates globally and whenever possible,
distinguish solar hemispheres and solar cycles 23 and 24. Periodicities are
investigated using both frequency (periodogram) and time-frequency (wavelet)
analysis. We find that these different processes, in addition to following the
11-year Solar Cycle, exhibit diverse statistically significant
oscillations with properties common to all solar, coronal, and heliospheric
processes: variable periodicity, intermittence, asymmetric development in the
northern and southern solar hemispheres, and largest amplitudes during the
maximum phase of solar cycles, being more pronounced during solar cycle 23 than
the weaker cycle 24. However, our analysis reveals an extremely complex and
diverse situation. For instance, there exists very limited commonality for
periods of less than one year. The few exceptions are the periods of 3.1--3.2
months found in the global occurrence rates of CMEs and in the sunspot area
(SSA) and those of 5.9--6.1 months found in the northern hemisphere. Mid-range
periods of 1 and 2 years are more wide spread among the
studied processes, but exhibit a very distinct behaviour with the first one
being present only in the northern hemisphere and the second one only in the
southern hemisphere. These periodic behaviours likely results from the
complexity of the underlying physical processes, prominently the emergence of

Etude d'évaluation du risque de mobilité des métaux dans l'agrosystÚme contaminé de Pierrelaye-Bessancourt : prospections du site, fonctionnement des sols et tests de lixiviation.
Ce sont les mĂ©canismes de fixation et de libĂ©ration des mĂ©taux dans ces sols polluĂ©s qui ont Ă©tĂ© approchĂ©s. Dans ce sens, l'Ă©tude se veut un outil d'aide Ă la dĂ©cision pour les amĂ©nagements futurs du territoire. 1- Les de structures polygonales reconnues par gĂ©ophysique conditionnent les modes et lieux d'infiltration des eaux. 2- L'irrigation a conduit Ă la dĂ©stabilisation des argiles des sols qui de ce fait ne jouent plus le rĂŽle de rĂ©tention. Toutes les phases porteuses de mĂ©taux sont plus ou moins directement liĂ©es aux anciennes conditions d'hydromorphie (irrigation). L'arrĂȘt de l'irrigation va dĂ©stabiliser progressivement ces complexes porteurs et libĂ©rer mĂ©taux retenus. 3- Des antibiotiques ont Ă©tĂ© mis en Ă©vidence dans les sols. Ils sont susceptibles d'agir sur les microorganismes des sols, et de ce fait prĂ©senter des risques environnementaux. 4- Les tests de lixiviation (lessivage/mobilisation) montrent la sensibilitĂ© des mĂ©taux Ă l'acidification du milieu et Ă l'oxydation de la matiĂšre organique. En raison de l'arrĂȘt de l'irrigation il y aura accĂ©lĂ©ration de la migration des mĂ©taux. 5- Les options de rĂ©amĂ©nagement sont discutĂ©es sous le regard sols/paysages : (1) en cas de boisement, l'acidification entraĂźnera leur Ă©vacuation ; (2) les sols en place gĂ©nĂšrent des poussiĂšres (dispersion dans l'environnement et inhalation humaine) ; (3) La dispersion incontrĂŽlĂ©e des terres lors de terrassements est certainement un risque majeur
ABiMed: An intelligent and visual clinical decision support system for medication reviews and polypharmacy management
Background: Polypharmacy, i.e. taking five drugs or more, is both a public
health and an economic issue. Medication reviews are structured interviews of
the patient by the community pharmacist, aiming at optimizing the drug
treatment and deprescribing useless, redundant or dangerous drugs. However,
they remain difficult to perform and time-consuming. Several clinical decision
support systems were developed for helping clinicians to manage polypharmacy.
However, most were limited to the implementation of clinical practice
guidelines. In this work, our objective is to design an innovative clinical
decision support system for medication reviews and polypharmacy management,
named ABiMed.
Methods: ABiMed associates several approaches: guidelines implementation, but
the automatic extraction of patient data from the GP's electronic health record
and its transfer to the pharmacist, and the visual presentation of
contextualized drug knowledge using visual analytics. We performed an ergonomic
assessment and qualitative evaluations involving pharmacists and GPs during
focus groups and workshops.
Results: We describe the proposed architecture, which allows a collaborative
multi-user usage. We present the various screens of ABiMed for entering or
verifying patient data, for accessing drug knowledge (posology, adverse
effects, interactions), for viewing STOPP/START rules and for suggesting
modification to the treatment. Qualitative evaluations showed that health
professionals were highly interested by our approach, associating the automatic
guidelines execution with the visual presentation of drug knowledge.
Conclusions: The association of guidelines implementation with visual
presentation of knowledge is a promising approach for managing polypharmacy.
Future works will focus on the improvement and the evaluation of ABiMed.Comment: 10 pages, 7 figure
Routine molecular profiling of cancer: results of a one-year nationwide program of the French Cooperative Thoracic Intergroup (IFCT) for advanced non-small cell lung cancer (NSCLC) patients.
International audienceBackground: The molecular profiling of patients with advanced non-small-cell lung cancer (NSCLC) for known oncogenic drivers is recommended during routine care. Nationally, however, the feasibility and effects on outcomes of this policy are unknown. We aimed to assess the characteristics, molecular profiles, and clinical outcomes of patients who were screened during a 1-year period by a nationwide programme funded by the French National Cancer Institute. Methods This study included patients with advanced NSCLC, who were routinely screened for EGFR mutations, ALK rearrangements, as well as HER2 (ERBB2), KRAS, BRAF, and PIK3CA mutations by 28 certified regional genetics centres in France. Patients were assessed consecutively during a 1-year period from April, 2012, to April, 2013. We measured the frequency of molecular alterations in the six routinely screened genes, the turnaround time in obtaining molecular results, and patients' clinical outcomes. This study is registered with ClinicalTrials.gov, number NCT01700582. Findings 18â679 molecular analyses of 17â664 patients with NSCLC were done (of patients with known data, median age was 64·5 years [range 18â98], 65% were men, 81% were smokers or former smokers, and 76% had adenocarcinoma). The median interval between the initiation of analysis and provision of the written report was 11 days (IQR 7â16). A genetic alteration was recorded in about 50% of the analyses; EGFR mutations were reported in 1947 (11%) of 17â706 analyses for which data were available, HER2 mutations in 98 (1%) of 11â723, KRAS mutations in 4894 (29%) of 17â001, BRAF mutations in 262 (2%) of 13â906, and PIK3CA mutations in 252 (2%) of 10â678; ALK rearrangements were reported in 388 (5%) of 8134 analyses. The median duration of follow-up at the time of analysis was 24·9 months (95% CI 24·8â25·0). The presence of a genetic alteration affected first-line treatment for 4176 (51%) of 8147 patients and was associated with a significant improvement in the proportion of patients achieving an overall response in first-line treatment (37% [95% CI 34·7â38·2] for presence of a genetic alteration vs 33% [29·5â35·6] for absence of a genetic alteration; p=0·03) and in second-line treatment (17% [15·0â18·8] vs 9% [6·7â11·9]; p<0·0001). Presence of a genetic alteration was also associated with improved first-line progression-free survival (10·0 months [95% CI 9·2â10·7] vs 7·1 months [6·1â7·9]; p<0·0001) and overall survival (16·5 months [15·0â18·3] vs 11·8 months [10·1â13·5]; p<0·0001) compared with absence of a genetic alteration. Interpretation Routine nationwide molecular profiling of patients with advanced NSCLC is feasible. The frequency of genetic alterations, acceptable turnaround times in obtaining analysis results, and the clinical advantage provided by detection of a genetic alteration suggest that this policy provides a clinical benefit
