A low frequency multibeam assessment: Spatial mapping of shallow gas by enhanced penetration and angular response anomaly
This study highlights the potential of using a low frequency multibeam echosounder for detection and visualization of shallow gas occurring several meters beneath the seafloor. The presence of shallow gas was verified in the Bornholm Basin, Baltic Sea, at 80 m water depth with standard geochemical core analysis and hydroacoustic subbottom profiling. Successively, this area was surveyed with a 95 kHz and a 12 kHz multibeam echosounder (MBES). The bathymetric measurements with 12 kHz provided depth values systematically deeper by several meters compared to 95 kHz data. This observation was attributed to enhanced penetration of the low frequency signal energy into soft sediments. Consequently, the subbottom geoacoustic properties contributed highly to the measured backscattered signals. Those appeared up to 17 dB higher inside the shallow gas area compared to reference measurements outside and could be clearly linked to the shallow gas front depth down to 5 meter below seafloor. No elevated backscatter was visible in 95 kHz MBES data, which in turn highlights the superior potential of low frequency MBES to image shallow sub-seafloor features. Small gas pockets could be resolved even on the outer swath (up to 65°). Strongly elevated backscattering from gassy areas occurred at large incidence angles and a high gas sensitivity of the MBES is further supported by an angular response analysis presented in this study. We conclude that the MBES together with subbottom profiling can be used as an efficient tool for spatial subbottom mapping in soft sediment environments

It’s all about the money? A qualitative study of healthcare worker motivation in urban China
Background
China’s healthcare reform programme continues to receive much attention. Central to these discussions has been how the various financial incentives underpinning reform efforts are negatively impacting on the healthcare workforce. Research continues to document these trends, however, qualitative analysis of how these incentives impact on the motivation of healthcare workers remains underdeveloped. Furthermore, the application of motivational theories to make sense of healthcare worker experiences has yet to be undertaken.
Methods
The purpose of our paper is to present a comparative case study account of healthcare worker motivation across urban China. It draws on semi structured interviews (n = 89) with a range of staff and organisations across three provinces. In doing so, the paper analyses how healthcare worker motivation is influenced by a variety of financial incentives; how motivation is influenced by the opportunities for career development; and how motivation is influenced by the day to day pressures of meeting patient expectations.
Results
The experience of healthcare workers in China highlights how a reliance on financial incentives has challenged their ability to maintain the values and ethos of public service. Our findings suggest greater attention needs to be paid to the motivating factors of improved income and career development. Further work is also needed to nurture and develop the motivation of healthcare workers through the building of trust between fellow workers, patients, and the public.
Conclusions
Through the analysis of healthcare worker motivation, our paper presents a number of ways China can improve its current healthcare reform efforts. It draws on the experience of other countries in calling for policy makers to support alternative approaches to healthcare reform that build on multiple channels of motivation to support healthcare workers
Pelvic organ prolapse and collagen-associated disorders

109010.pdf (publisher's version ) (Open Access)INTRODUCTION AND HYPOTHESIS: Pelvic organ prolapse (POP) and other disorders, such as varicose veins and joint hypermobility, have been associated with changes in collagen strength and metabolism. We hypothesized that these various disorders were more prevalent in both POP patients and their family members. METHODS: In this study, the prevalence of various collagen-associated disorders, including POP, was compared between POP patients (n = 110) and control patients (n = 100) and their first and second degree family members. RESULTS: POP patients reported a higher prevalence of varicose veins, joint hypermobility and rectal prolapse and were more likely to have family members with POP as compared to the control group (p < 0.01). In contrast, the family members of the POP group did not report a higher prevalence of collagen-associated disorders compared to the family members of the control group (p = 0.82). Changes in the gastric enteric nervous system and muscle: A case report on two patients with diabetic gastroparesis
<p>Abstract</p> <p>Background</p> <p>The pathophysiological basis of diabetic gastroparesis is poorly understood, in large part due to the almost complete lack of data on neuropathological and molecular changes in the stomachs of patients. Experimental models indicate various lesions affecting the vagus, muscle, enteric neurons, interstitial cells of Cajal (ICC) or other cellular components. The aim of this study was to use modern analytical methods to determine morphological and molecular changes in the gastric wall in patients with diabetic gastroparesis.</p> <p>Methods</p> <p>Full thickness gastric biopsies were obtained laparoscopically from two gastroparetic patients undergoing surgical intervention and from disease-free areas of control subjects undergoing other forms of gastric surgery. Samples were processed for histological and immunohistochemical examination.</p> <p>Results</p> <p>Although both patients had severe refractory symptoms with malnutrition, requiring the placement of a gastric stimulator, one of them had no significant abnormalities as compared with controls. This patient had an abrupt onset of symptoms with a relatively short duration of diabetes that was well controlled. By contrast, the other patient had long standing brittle and poorly controlled diabetes with numerous episodes of diabetic ketoacidosis and frequent hypoglycemic episodes. Histological examination in this patient revealed increased fibrosis in the muscle layers as well as significantly fewer nerve fibers and myenteric neurons as assessed by PGP9.5 staining. Further, significant reduction was seen in staining for neuronal nitric oxide synthase, heme oxygenase-2, tyrosine hydroxylase as well as for c-KIT.</p> <p>Conclusion</p> <p>We conclude that poor metabolic control is associated with significant pathological changes in the gastric wall that affect all major components including muscle, neurons and ICC. Severe symptoms can occur in the absence of these changes, however and may reflect vagal, central or hormonal influences. Gastroparesis is therefore likely to be a heterogeneous disorder. Careful molecular and pathological analysis may allow more precise phenotypic differentiation and shed insight into the underlying mechanisms as well as identify novel therapeutic targets.</p
Hookworm-Related Anaemia among Pregnant Women: A Systematic Review
Anaemia affects large numbers of pregnant women in developing countries and increases their risk of dying during pregnancy and delivering low birth weight babies, who in turn are at increased risk of dying. Human hookworm infection has long been recognized among the major causes of anaemia in poor communities, but understanding of the benefits of the management of hookworm infection in pregnancy has lagged behind the other major causes of maternal anaemia. Low coverage of anthelmintic treatment in maternal health programmes in many countries has been the result. After systematically reviewing the available literature we observed that increasing hookworm infection intensity is associated with lower haemoglobin levels in pregnant women. We also estimate that between a quarter and a third of pregnant women in sub-Saharan Africa are infected with hookworm and at risk of preventable hookworm-related anaemia. However, all identified intervention studies showed a benefit of deworming for maternal or child health and we argue that increased efforts should be made to increase the coverage of anthelmintic treatment among pregnant women
Known and unknown requirements in healthcare
We report experience in requirements elicitation of domain knowledge from experts in clinical and cognitive neurosciences. The elicitation target was a causal model for early signs of dementia indicated by changes in user behaviour and errors apparent in logs of computer activity. A Delphi-style process consisting of workshops with experts followed by a questionnaire was adopted. The paper describes how the elicitation process had to be adapted to deal with problems encountered in terminology and limited consensus among the experts. In spite of the difficulties encountered, a partial causal model of user behavioural pathologies and errors was elicited. This informed requirements for configuring data- and text-mining tools to search for the specific data patterns. Lessons learned for elicitation from experts are presented, and the implications for requirements are discussed as “unknown unknowns”, as well as configuration requirements for directing data-/text-mining tools towards refining awareness requirements in healthcare applications
Does sex matter in the associations between classic risk factors and fatal coronary heart disease in populations from the Asia-Pacific region?
Background: There is much interest in promoting healthy heart awareness among women. However, little is known about the reasons behind the lower rates of heart disease among women compared with men, and why this risk difference diminishes with age. Previous comparative studies have generally had insufficient numbers of women to quantify such differences reliably. Methods: We carried out an individual participant data meta-analysis of 39 cohort studies (32 from Asian countries and 7 from Australia and New Zealand). Cox models were used to estimate hazard ratios (HR) for coronary death, comparing men to women. Further adjustments were made for several proven coronary risk factors to quantify their contributions to the sex differential. Sex interactions were tested for the same risk factors. Results: During 4 million person-years of follow-up, there were 1989 (926 female) deaths from coronary heart disease (CHD). The age-adjusted and study-adjusted male/female HR (95% confidence interval [95% CI]) was 2.05 (1.89-2.22). At baseline, 54% of men vs. 7% of women were current smokers; hence, adjustment for smoking explained the largest component (20%) of this HR. A significant sex interaction was observed between systolic blood pressure (SBP) and CHD mortality such that a 10 mm Hg increase was associated with a 15% greater increase in the relative risk (RR) of coronary death in women compared with men (p = 0.002). Conclusions: Only a small amount of the sex differential in coronary death could be explained by differences in the prevalence of classic risk factors. Mathematical modeling of microRNA-mediated mechanisms of translation repression
MicroRNAs can affect the protein translation using nine mechanistically
different mechanisms, including repression of initiation and degradation of the
transcript. There is a hot debate in the current literature about which
mechanism and in which situations has a dominant role in living cells. The
worst, same experimental systems dealing with the same pairs of mRNA and miRNA
can provide ambiguous evidences about which is the actual mechanism of
translation repression observed in the experiment. We start with reviewing the
current knowledge of various mechanisms of miRNA action and suggest that
mathematical modeling can help resolving some of the controversial
interpretations. We describe three simple mathematical models of miRNA
translation that can be used as tools in interpreting the experimental data on
the dynamics of protein synthesis. The most complex model developed by us
includes all known mechanisms of miRNA action. It allowed us to study possible
dynamical patterns corresponding to different miRNA-mediated mechanisms of
translation repression and to suggest concrete recipes on determining the
dominant mechanism of miRNA action in the form of kinetic signatures. Using
computational experiments and systematizing existing evidences from the
literature, we justify a hypothesis about co-existence of distinct
miRNA-mediated mechanisms of translation repression. The actually observed
mechanism will be that acting on or changing the limiting "place" of the
translation process. The limiting place can vary from one experimental setting
to another. This model explains the majority of existing controversies
