    Pulmonary Edema following Phenylephrine Intranasal Spray Administration during the Induction of General Anesthesia in a Child

    Topical phenylephrine, an agent used to facilitate nasotracheal intubation and prevent nasal mucosal bleeding, can cause severe hypertension in some patients, secondary to its stimulation of alpha-adrenergic receptors. Moreover, a high incidence of pulmonary edema is found in patients whose phenylephrine administration is followed by treatment with beta-blocking agents. We report a case of acute pulmonary edema in a pediatric patient who developed severe hypertension after the inadvertent administration of a large dose of topical nasal phenylephrine, followed by beta-adrenergic antagonists (esmolol)

    Self-Transcendent Emotions and Social Media: Exploring the Content and Consumers of Inspirational Facebook Posts

    Although a great deal of research has examined the potential negative effects of Facebook, studies also show that Facebook use can lead to various positive effects. This study builds on this positive effects scholarship: together, the two studies presented herein aim to provide an understanding of the inspirational content available on Facebook and the way social media users in the United States encounter, recall, and interact with this content. Results from the quantitative content analysis in Study 1 show that inspirational Facebook posts contain similar frequencies of hope and appreciation of beauty and excellent elicitors when compared with other forms of media and social media. Results from the national survey conducted in Study 2 show that social media users are most often inspired by portrayals of kindness and overcoming obstacles and that Facebook users did not report different sharing behavior as compared with users of other social media sites

    On the effect of pre-strain and pre-fatigue on the monotonic behaviour of ultra-high strength steels

    The mechanical behaviour of three ultra-high strength steels has been assessed; AerMet100, 300M and the recently developed corrosion resistant high strength steel, MLX-17. Material heat treatment profiles have been utilised to provide performance optimised for the aerospace industry and specimens have been tested to explore tensile and fatigue properties, in particular when combined with pre-strain to simulate the effects of overload. Testing of this kind has not been reported within the literature, particularly amongst ultra-high strength and corrosion-resistant steels. Baseline mechanical performance for all three materials in their heat-treated conditions has been established and properties such as yield strength and ultimate tensile strength have been assessed following a 75% and 95% pre-strain as well as fatigue in combination with a 75% and 95% pre-strain. Under all loading conditions, resultant tensile mechanical properties are not seen to witness a substantial degradation in performance, but an improvement in terms of yield strength and UTS, due to the role of work hardening. An alloy comparison has been carried out and responses are seen to vary slightly as a result of material microstructure. Correlation of pre-strain and pre-fatigue results with respect to baseline properties and microstructure has contributed to advancing the understanding of the mechanical behaviour of the aforementioned ultra-high strength steels

    Factuality Challenges in the Era of Large Language Models

    The emergence of tools based on Large Language Models (LLMs), such as OpenAI's ChatGPT, Microsoft's Bing Chat, and Google's Bard, has garnered immense public attention. These incredibly useful, natural-sounding tools mark significant advances in natural language generation, yet they exhibit a propensity to generate false, erroneous, or misleading content -- commonly referred to as "hallucinations." Moreover, LLMs can be exploited for malicious applications, such as generating false but credible-sounding content and profiles at scale. This poses a significant challenge to society in terms of the potential deception of users and the increasing dissemination of inaccurate information. In light of these risks, we explore the kinds of technological innovations, regulatory reforms, and AI literacy initiatives needed from fact-checkers, news organizations, and the broader research and policy communities.     Genome-wide identification and expression analysis of the C2H2-zinc finger transcription factor gene family and screening of candidate genes involved in floral development in Coptis teeta Wall. (Ranunculaceae)

Although a large number of C2H2 zinc-finger proteins (C2H2-ZFPs) have been well characterized in a number of model plant species, little is known about their expression and function in Coptis teeta. C. teeta displays two floral phenotypes (herkogamy phenotypes). It has been proposed that the C2H2-zinc finger transcription factor family may play a crucial role in the formation of floral development and herkogamy observed in C. teeta. As such, we performed a genome-wide analysis of the C2H2-ZFP gene family in C. teeta. Results: The complexity and diversity of C. teeta C2H2 zinc finger proteins were established by evaluation of their physicochemical properties, phylogenetic relationships, exon-intron structure, and conserved motifs. Chromosome localization showed that 95 members of the C2H2 zinc-finger genes were unevenly distributed across the nine chromosomes of C. teeta, and that these genes were replicated in tandem and segmentally and had undergone purifying selection. Analysis of cis-acting regulatory elements revealed a possible involvement of C2H2 zinc-finger proteins in the regulation of phytohormones. Transcriptome data was then used to compare the expression levels of these genes during the growth and development of the two floral phenotypes (F-type and M-type). These data demonstrate that in groups A and B, the expression levels of 23 genes were higher in F-type flowers, while 15 genes showed higher expressions in M-type flowers. qRT-PCR analysis further revealed that the relative expression was highly consistent with the transcriptome data.     LY294002 may overcome 5-FU resistance via down-regulation of activated p-AKT in Epstein-Barr virus-positive gastric cancer cells

    <p>Abstract</p> <p>Background</p> <p>As EBV-associated gastric cancer has unique features that are different from EBV (-) gastric cancer, EBV is considered to have a key role in gastric carcinogenesis. It has been reported that viral latent membrane protein 2A (LMP2A) in EBV-transformed tumor cells activates the phosphatidylinositol 3-kinase (PI3K)/AKT pathway, which provides a survival signal and chemo-resistance to cytotoxic anti-cancer drugs. This study was to evaluate anti-proliferative effect and cell cycle change when 5-FU and LY294002 (LY), a selective inhibitor of PI3K, were treated separately or combined with different schedules in EBV positive gastric cancer cell line, SNU-719.</p> <p>Methods</p> <p>After single treatment and sequential combination of 5-FU and LY, cytotoxic activity was measured by MTS assay. When 5-FU and LY were treated in single and sequential combinations, the expression of p-AKT, p-NFkB, p-p53 and bcl-2 was observed on different concentrations by Western blot analysis. We also investigated the effect on apoptosis and cell cycle distribution using flow cytometry. The LMP2A siRNA inhibition was done to confirm the reversal of decreased 5-FU activity and p-AKT.</p> <p>Results</p> <p>When 5-FU was sequentially combined with LY, the combination index (CI) value indicated synergistic anti-proliferative effect. The expression of p-AKT and p-NFκB was upregulated by 5-FU alone but sequential treatment of 5-FU and LY decreased the expression of both p-AKT and p-NFκB. When 5-FU was combined with LY, G0/G1 and sub G1 cell population (%) increased. When 5-FU was added to the cells transfected with LMP2A siRNA, its anti-proliferative effect increased and the expression of p-AKT decreased. In sequential combination of 5-FU and LY, the expression of p-p53 was increased and bcl-2 expression was diminished compared to 5-FU alone.</p> <p>Conclusion</p> <p>These data suggest that sequential combination of 5-FU and LY induce synergistic cytotoxicity and overcome intrinsic and acquired resistance of 5-FU via downregulation of activated p-AKT and mitochondria-dependent apoptosis in EBV gastric cancer cell line, SNU-719.</p
