    A simple model for quasar density evolution

    It is widely agreed upon that AGN and Quasars are driven by gas accretion onto a supermassive black hole. The origin of the latter however still remains an open question. In this work we present the results of an extremely simple cosmological model combined with an evolutionary scenario in which both the formation of the black hole as well as the gas accretion onto it are triggered by major mergers of gas-rich galaxies.     Summary Economic evaluations are increasingly used to assess the value of health interventions, but variable quality and heterogeneity limit the use of these evaluations by decision-makers. These recommendations provide guidance for the design, conduct, and reporting of economic evaluations in osteoporosis to improve their transparency, comparability, and methodologic standards. Introduction This paper aims to provide recommendations for the conduct of economic evaluations in osteoporosis in order to improve their transparency, comparability, and methodologic standards. Methods A working group was convened by the European Society for Clinical and Economic Aspects of Osteoporosis and Osteoarthritis to make recommendations for the design, conduct, and reporting of economic evaluations in osteoporosis, to define an osteoporosis-specific reference case to serve a minimum standard for all economic analyses in osteoporosis, to discuss methodologic challenges and initiate a call for research. A literature review, a face-to-face meeting in New York City (including 11 experts), and a review/approval by a larger group of experts worldwide (including 23 experts in total) were conducted. Results Recommendations on the type of economic evaluation, methods for economic evaluation, modeling aspects, base-case analysis and population, excess mortality, fracture costs and disutility, treatment characteristics, and model validation were provided. Recommendations for reporting economic evaluations in osteoporosis were also made and an osteoporosis-specific checklist was designed that includes items to report when performing an economic evaluation in osteoporosis. Further, 12 minimum criteria for economic evaluations in osteoporosis were identified and 12 methodologic challenges and need for further research were discussed. Conclusion While the working group acknowledges challenges and the need for further research, these recommendations are intended to supplement general and national guidelines for economic evaluations, improve transparency, quality, and comparability of economic evaluations in osteoporosis, and maintain methodologic standards to increase their use by decision-makers

    Constraints on the Local Sources of Ultra High-Energy Cosmic Rays

    Ultra high-energy cosmic rays (UHECRs) are believed to be protons accelerated in magnetized plasma outflows of extra-Galactic sources. The acceleration of protons to ~10^{20} eV requires a source power L>10^{47} erg/s. The absence of steady sources of sufficient power within the GZK horizon of 100 Mpc, implies that UHECR sources are transient. We show that UHECR "flares" should be accompanied by strong X-ray and gamma-ray emission, and that X-ray and gamma-ray surveys constrain flares which last less than a decade to satisfy at least one of the following conditions: (i) L>10^{50} erg/s; (ii) the power carried by accelerated electrons is lower by a factor >10^2 than the power carried by magnetic fields or by >10^3 than the power in accelerated protons; or (iii) the sources exist only at low redshifts, z<<1. The implausibility of requirements (ii) and (iii) argue in favor of transient sources with L>10^{50} erg/s.Comment: 7 pages, 1 figure, submitted to JCA

    The power of suggestion: examining the impact of presence or absence of shared first language in the antenatal clinic

    © 2019 Foundation for the Sociology of Health & Illness Healthcare encounters involving participants from diverse linguistic backgrounds are becoming more common due to the globalisation of health care and increasing migration levels. Research suggests that this diversity has a significant impact on health outcomes; however less is known about how it is managed in the actual consultation process. This article presents an analysis of antenatal screening consultations video recorded in Hong Kong, using conversation analysis. We consider how the use of a second or subsequent language impacts on these consultations, and on discussions and decisions about further action. The presence or absence of shared first language did not appear to affect the extent to which particular courses of action were promoted or recommended. Recommendations were a common occurrence across consultations with and without shared first language. However, we argue that the routine use of recommendations can be consequential, as second language speakers may have more limited resources to interrogate or contest these. This finding has implications for the ability for professionals to maximise patient involvement in decision-making

    Investigating the basis of substrate recognition in the pC221 relaxosome

    The nicking of the origin of transfer (oriT) is an essential initial step in the conjugative mobilization of plasmid DNA. In the case of staphylococcal plasmid pC221, nicking by the plasmid-specific MobA relaxase is facilitated by the DNA-binding accessory protein MobC; however, the role of MobC in this process is currently unknown. In this study, the site of MobC binding was determined by DNase I footprinting. MobC interacts with oriT DNA at two directly repeated 9 bp sequences, mcb1 and mcb2, upstream of the oriT nic site, and additionally at a third, degenerate repeat within the mobC gene, mcb3. The binding activity of the conserved sequences was confirmed indirectly by competitive electrophoretic mobility shift assays and directly by Surface Plasmon Resonance studies. Mutation at mcb2 abolished detectable nicking activity, suggesting that binding of this site by MobC is a prerequisite for nicking by MobA. Sequential site-directed mutagenesis of each binding site in pC221 has demonstrated that all three are required for mobilization. The MobA relaxase, while unable to bind to oriT DNA alone, was found to associate with a MobC–oriT complex and alter the MobC binding profile in a region between mcb2 and the nic site. Mutagenesis of oriT in this region defines a 7 bp sequence, sra, which was essential for nicking by MobA. Exchange of four divergent bases between the sra of pC221 and the related plasmid pC223 was sufficient to swap their substrate identity in a MobA-specific nicking assay. Based on these observations we propose a model of layered specificity in the assembly of pC221-family relaxosomes, whereby a common MobC:mcb complex presents the oriT substrate, which is then nicked only by the cognate MobA

    The effect of active galactic nuclei on the cold interstellar medium in distant star-forming galaxies

    In the framework of a systematic study with the ALMA interferometer of IR-selected main-sequence and starburst galaxies at z ∼ 1 − 1.7 at typical ∼1″ resolution, we report on the effects of mid-IR- and X-ray-detected active galactic nuclei (AGN) on the reservoirs and excitation of molecular gas in a sample of 55 objects. We find widespread detectable nuclear activity in ∼30% of the sample. The presence of dusty tori influences the IR spectral energy distribution of galaxies, as highlighted by the strong correlation among the AGN contribution to the total IR luminosity budget (fAGN = LIR,  AGN/LIR), its hard X-ray emission, and the Rayleigh-Jeans to mid-IR (S1.2 mm/S24 μm) observed color, with evident consequences on the ensuing empirical star formation rate estimates. Nevertheless, we find only marginal effects of the presence and strength of AGN on the carbon monoxide CO (J = 2, 4, 5, 7) or neutral carbon ([C I](3P1  −  3P0), [C I](3P2  −  3P1)) line luminosities and on the derived molecular gas excitation as gauged by line ratios and the full spectral line energy distributions. The [C I] and CO emission up to J = 5, 7 thus primarily traces the properties of the host in typical IR luminous galaxies. However, our analysis highlights the existence of a large variety of line luminosities and ratios despite the homogeneous selection. In particular, we find a sparse group of AGN-dominated sources with the highest LIR,  AGN/LIR,  SFR ratios, ≳3, that are more luminous in CO (5−4) than what is predicted by the L′CO(5-4)−LIR, SFR relation, which might be the result of the nuclear activity. For the general population, our findings translate into AGN having minimal effects on quantities such as gas and dust fractions and star formation efficiencies. If anything, we find hints of a marginal tendency of AGN hosts to be compact at far-IR wavelengths and to display 1.8 times larger dust optical depths. In general, this is consistent with a marginal impact of the nuclear activity on the gas reservoirs and star formation in average star-forming AGN hosts with LIR > 5 × 1011 L⊙, typically underrepresented in surveys of quasars and submillimeter galaxies

    Epidemiology of bladder cancer: A systematic review and contemporary update of risk factors in 2018

    CONTEXT: Bladder cancer (BC) is a significant health problem, and understanding the risk factors for this disease could improve prevention and early detection. OBJECTIVE: To provide a systematic review and summary of novel developments in epidemiology and risk factors for BC. EVIDENCE ACQUISITION: A systematic review of original articles was performed by two pairs of reviewers (M.G.C., I.J., F.E., and K.P.) using PubMed/Medline in December 2017, updated in April 2018. To address our primary objective of reporting contemporary studies, we restricted our search to include studies from the last 5yr. We subdivided our review according to specific risk factors (PICO [Population Intervention Comparator Outcome]). EVIDENCE SYNTHESIS: Our search found 2191 articles, of which 279 full-text manuscripts were included. We separated our manuscripts by the specific risk factor they addressed (PICO). According to GLOBOCAN estimates, there were 430000 new BC cases and 165000 deaths worldwide in 2012. Tobacco smoking and occupational exposure to carcinogens remain the factors with the highest attributable risk. The literature was limited by heterogeneity of data. CONCLUSIONS: Evidence is emerging regarding gene-environment interactions, particularly for tobacco and occupational exposures. In some populations, incidence rates are declining, which may reflect a decrease in smoking. Standardisation of reporting may help improve epidemiologic evaluation of risk. PATIENT SUMMARY: Bladder cancer is common worldwide, and the main risk factors are tobacco smoking and exposure to certain chemicals in the working and general environments. There is ongoing research to identify and reduce risk factors, as well as to understand the impact of genetics on bladder cancer risk
