    The Sensewheel: An Adjunct to Wheelchair Skills Training

    The purpose of this study was to investigate the influence of real time verbal feedback to optimise push arc during over ground manual wheelchair propulsion. 10 healthy non wheelchair users pushed a manual wheelchair for a distance of 25 metres on level paving, initially with no feedback and then with real time verbal feedback aimed at controlling push arc within a range of 85Ëš-100Ëš. The real time feedback was provided by a physiotherapist walking behind the wheelchair, viewing real time data on a tablet personal computer received from the Sensewheel, a lightweight instrumented wheelchair wheel. The real time verbal feedback enabled the participants to significantly increase their push arc. This increase in push arc resulted in a non-significant reduction in push rate and a significant increase in peak force application. The intervention enabled participants to complete the task at a higher mean velocity using significantly fewer pushes. This was achieved via a significant increase in the power generated during the push phase. This study identifies that a lightweight instrumented wheelchair wheel such as the Sensewheel is a useful adjunct to wheelchair skills training. Targeting the optimisation of push arc resulted in beneficial changes in propulsion technique

    COMPLIANCE TESTING OF IOWA’S SKID-MOUNTED SIGN DEVICE

    A wide variety of traffic control devices are used in work zones, some of which are nont ormally found on the roadside or in the traveled way outsideofthe work zones. These devices are used to enhance the safety of the work zones by controlling the traffic through these areas. Due to the placement of the traffic control devices, the devices themselves may be potentially hazardous to both workers and errant vehicles. The impact performance of many work zone traffic control devices is mainly unknown and to date limited crash testing has been conducted under the criteria of National Cooperative Highway Research Program (NCHRP) Report No. 350, Recommended Procedures for the Safety Performance Evaluation of Highway Features. The objective of the study was to evaluatethe safety performance of existing skid-mounted sign supports through full- scale crash testing. Two full-scale crash tests were conducted on skid-mounted sign supports to determine their safety performance according to the Test Level 3 (TL-3) criteria set forth in the NCHRP Report No. 350. The safety performancevaluations indicate that these skid-mounted sign supports did not perform satisfactorily in the full-scale crash tests. The results of the crash tests were documented, and conclusions and recommendations pertaining tothe safety performance of the existing work zone traffic control devices were made

    Influence network linkages across implementation strategy conditions in a randomized controlled trial of two strategies for scaling up evidence-based practices in public youth-serving systems.

    BackgroundGiven the importance of influence networks in the implementation of evidence-based practices and interventions, it is unclear whether such networks continue to operate as sources of information and advice when they are segmented and disrupted by randomization to different implementation strategy conditions. The present study examines the linkages across implementation strategy conditions of social influence networks of leaders of youth-serving systems in 12 California counties participating in a randomized controlled trial of community development teams (CDTs) to scale up use of an evidence-based practice.MethodsSemi-structured interviews were conducted with 38 directors, assistant directors, and program managers of county probation, mental health, and child welfare departments. A web-based survey collected additional quantitative data on information and advice networks of study participants. A mixed-methods approach to data analysis was used to create a sociometric data set (n = 176) to examine linkages between treatment and standard conditions.ResultsOf those network members who were affiliated with a county (n = 137), only 6 (4.4%) were directly connected to a member of the opposite implementation strategy condition; 19 (13.9%) were connected by two steps or fewer to a member of the opposite implementation strategy condition; 64 (46.7%) were connected by three or fewer steps to a member of the opposite implementation strategy condition. Most of the indirect steps between individuals who were in different implementation strategy conditions were connections involving a third non-county organizational entity that had an important role in the trial in keeping the implementation strategy conditions separate. When these entities were excluded, the CDT network exhibited fewer components and significantly higher betweenness centralization than did the standard condition network.ConclusionAlthough the integrity of the RCT in this instance was not compromised by study participant influence networks, RCT designs should consider how influence networks may extend beyond boundaries established by the randomization process in implementation studies.Trial registrationNCT00880126

    Occupy: 'struggles for the common or an 'anti-politics of dignity? Reflections on Hardt and Negri and John Holloway

    This article provides a critical examination of Michael Hardt and Antonio Negri’s and John Holloway’s theory of revolutionary subjectivity, and does so by applying their theories to the Occupy movement of 2011. Its central argument is that one should avoid collapsing ‘autonomist’ and ‘open’ Marxism, for whilst both approaches share Tronti’s (1979) insistence on the constituent role of class struggle, and also share an emphasis on a prefigurative politics which engages a non-hierarchical and highly participatory politics, there nevertheless remain some significant differences between their approaches. Ultimately, when applied to Occupy Movement whilst their theory isn’t entirely unproblematic, I will argue that Hardt and Negri’s ‘autonomist’ approach offers the stronger interpretation, due mainly to their revised historical materialism

    Research shows that exposure to nature has benefits for people's mental and physical health and that ubiquitous and mobile technologies encourage engagement with nature. However, existing research in this area is primarily focused on people without visual impairments and is not inclusive of blind and partially sighted individuals. To address this gap in research, we interviewed seven blind people (without remaining vision) about their experiences when exploring and experiencing the outdoor natural environment to gain an understanding of their needs and barriers and how these needs can be addressed by technology.     The Lore of Low Methane Livestock:Co-Producing Technology and Animals for Reduced Climate Change Impact

    Methane emissions from sheep and cattle production have gained increasing profile in the context of climate change. Policy and scientific research communities have suggested a number of technological approaches to mitigate these emissions. This paper uses the concept of co-production as an analytical framework to understand farmers’ evaluation of a 'good animal’. It examines how technology and sheep and beef cattle are co-produced in the context of concerns about the climate change impact of methane. Drawing on 42 semi-structured interviews, this paper demonstrates that methane emissions are viewed as a natural and integral part of sheep and beef cattle by farmers, rather than as a pollutant. Sheep and beef cattle farmers in the UK are found to be an extremely heterogeneous group that need to be understood in their specific social, environmental and consumer contexts. Some are more amenable to appropriating methane reducing measures than others, but largely because animals are already co-constructed from the natural and the technical for reasons of increased production efficiency

    Accident reports play a key role in the safety of complex systems. These reports present the recommendations that are intended to help avoid any recurrence of past failures. However, the value of these findings depends upon the causal analysis that helps to identify the reasons why an accident occurred. Various techniques have been developed to help investigators distinguish root causes from contributory factors and contextual information. This paper presents the results from a study into the individual differences that can arise when a group of investigators independently apply the same technique to identify the causes of an accident. This work is important if we are to increase the consistency and coherence of investigations following major accidents

    SIGNIFICANT POSTOPERATIVE BLEEDING is a common risk of cardiac surgery, with approximately 3.5% of patients requiring surgical re-exploration.1 Re-exploration is associated with adverse outcomes, including infections, ischemia, and increased 30-day mortality.2 Similar adverse outcomes are related to erythrocyte transfusions associated with cardiac surgery,3 in addition to the immunologic and administrative hazards of transfusion.4 These risks are important because the majority of patients undergoing cardiac surgery receive a blood transfusion despite the lack of evidence to support liberal transfusion strategies.5 The frequency and significance of bleeding after cardiac surgery warrant investigation of the hematologic changes throughout the procedure.     Ideas and Enhancements Related to Mobile Applications to Support Type 1 Diabetes

    Background: Mobile devices have become increasingly important to young people who now use them to access a wide variety of health-related information. Research and policy related to the integration of health information and support with this technology do not effectively consider the viewpoint of a younger patient. Views of young people with type 1 diabetes are vital in developing quality services and improving their own health-related quality of life (HRQOL), yet research on their lifestyle and use of Web and mobile technology to support their condition and in non–health-related areas is sparse. Objective: To develop insight into young people with type 1 diabetes and their current use of Web and mobile technology and its potential impact on HRQOL. This can be achieved by constructing an in-depth picture of their day-to-day experiences from qualitative interviewing and exploring how they make use of technology in their lives and in relation to their condition and treatment. The goal was then to build something to help them, using the researcher’s technical expertise and seeking users’ opinions during the design and build, utilizing sociotechnical design principles. Methods: Data were collected by semistructured, in-depth qualitative interviews (N=9) of young people with type 1 diabetes aged 18-21. Interviews were transcribed and loaded onto NVivo for theme identification. Data analysis was undertaken during initial interviews (n=4) to locate potential ideas and enhancements for technical development. Latter interviews (n=5) assisted in the iterative sociotechnical design process of the development and provided additional developmental ideas. Results: Six themes were identified providing an understanding of how participants lived with and experienced their condition and how they used technology. Four technological suggestions for improvement were taken forward for prototyping. One prototype was developed as a clinically approved app. A number of ideas for new mobile apps and enhancements to currently existing apps that did not satisfactorily cater to this age group’s requirements for use in terms of design and functionality were suggested by interviewees but were not prototyped. Conclusions: This paper outlines the nonprototyped suggestions from interviewees and argues that young people with type 1 diabetes have a key role to play in the design and implementation of new technology to support them and improve HRQOL. It is vital to include and reflect on their suggestions as they have a radically different view of technology than either their parents or practitioners. We need to consider the relationship to technology that young people with type 1 diabetes have, and then reflect on how this might make a difference to them and when it might not be a suitable mechanism to use
