    The reciprocal value of Doctoral Design Research when housed within a Creative Business Center: a case study in Porto, Portugal

    [EN] This paper presents the case study of a pilot partnership between a Doctoral Program in Design and a Science and Technology Park located in Porto, Portugal. The partnership has been in effect since 2013; the Doctoral Program has resided within the premises of the Science and Technology Park, as an immersive research environment, in order to foster knowledge transfer and collaborations through gradual, mutual and informal recognition of reciprocal assets. We contextualise the premises and stakeholders in this partnership, outline the empirical methodologies employed, and provide examples of selected projects that illustrate the implementation of these methodologies. Furthermore, we provide an inventory of challenges detected throughout the development of the partnership, as well as information on how these challenges are being overcome. We conclude by summarising the main accomplishments of this partnership, as well as outlining validation by various external agents and opening up prospects for future development. These will include thorough materials that may be of use for implementation in other academic, scientific and entrepreneurial environments.     Self-initiated practices in the urban community of Balteiro: design challenges in a post-pandemic setting

    This research aims to identify, document, interpret and disseminate self-initiated practices in social housing neighborhoods, in the city of Vila Nova de Gaia, Portugal. The study aims to assess the consequences that the COVID-19 pandemic had on these practices and how Design can contribute to the resonance of these initiatives in other equivalent social contexts. Two local initiatives were identified: Recreational Association Clube Balteiro Jovem (ARCBJ) and School Workshop of cartoning and sewing (Escola Oficina). Preliminary work has been carried out with the Vila Nova de Gaia City Council and the aforementioned residents' associations ARCBJ and Escola Oficina. Emic data is being collected through methods of direct and indirect observation, including ethnographic interviews, focus groups, and participant observation.     Stress and psychological health: testing the mediating role of cognitive appraisal

    This study tested the mediating role of primary (e.g., threat and challenge perceptions) and secondary (e.g., coping potential and control perception) cognitive appraisal in the relationship between occupational stress and psychological health. This mediation was tested using a cross-sectional study based on self-reported measures. The total sample consisted of 2,302 nurses, 1,895 females (82.3%) and 407 males (17.7%), who completed an evaluation protocol with measures of occupational stress, cognitive appraisal, and psychological health. To test the mediating role of cognitive appraisal in the relationship between cognitive appraisal and psychological health, we used Structural Equation Modeling (SEM). The results confirmed that primary and secondary cognitive appraisals partially mediated the relationship between occupational stress and psychological health; however, the direct effects of stress on psychological health cannot be ignored.     Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial

    Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.     Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries
