    Children and COVID-19: State Data Report, Version 8-12-21

    State-level reports are the best publicly available and timely data on child COVID-19 cases in the United States. The American Academy of Pediatrics and the Children's Hospital Association are collaborating to collect and share all publicly available data from states on child COVID-19 cases (definition of "child" case is based on varying age ranges reported across states; see report Appendix for details and links to all data sources).As of August 12, over 4.41 million children have tested positive for COVID-19 since the onset of the pandemic. Over 121,000 cases were added the past week, a continuing substantial increase. After declining in early summer, child cases have steadily increased since the beginning of July.The age distribution of reported COVID-19 cases was provided on the health department websites of 49 states, New York City, the District of Columbia, Puerto Rico, and Guam. Since the pandemic began, children represented 14.4% of total cumulated cases. For the week ending August 12, children were 18.0% of reported weekly COVID-19 cases.A smaller subset of states reported on hospitalizations and mortality by age; the available data indicate that COVID-19-associated hospitalization and death is uncommon in children.At this time, it appears that severe illness due to COVID-19 is uncommon among children. However, there is an urgent need to collect more data on longer-term impacts of the pandemic on children, including ways the virus may harm the long-term physical health of infected children, as well as its emotional and mental health effects

    Assessing the Extent of Carbonate Deposition in Early Rift Settings

    Select lacustrine and marine depositional settings show a spectrum of styles of carbonate deposition and illustrate the types of carbonates, with an emphasis on microbialites and tufa, to be expected in early rift settings. Early rift lake examples examined in this review article are all from East Africa: Lakes Turkana, Bogoria, Natron and Magadi, Manyara, and Tanganyika. Other lake examples include four from the western United States (Great Salt Lake and high lake level Lake Bonneville, Mono Lake and high lake level Russell Lake, Pyramid Lake and high lake level Lake Lahontan, and Searles Lake) and two from Australia (Lakes Clifton and Thetis). Marine basin examples are the Hamelin Pool part of Shark Bay from Australia (marginal marine) and the Red Sea (marine rift). Landsat images and digital elevation models for each example are used to delineate present and past lake-basin margins based on published lake-level elevations, and for some examples, the shorelines representing different lake levels can be compared to evaluate how changes in size, shape, and lake configuration might have impacted carbonate development. The early rift lakes show a range of characteristics to be expected in lacustrine settings during the earliest stages of continental extension and rifting, whereas the Red Sea shows well advanced rifting with marine incursion and reef–skeletal sand development. Collectively, the lacustrine examples show a wide range of sizes, with several of them being large enough that they could produce carbonate deposits of potential economic interest. Three of the areas—Great Salt Lake and high lake level Lake Bonneville, Pyramid Lake and high lake level Lake Lahontan, and the Red Sea—are exceedingly complex in that they illustrate a large degree of potential depositional facies heterogeneity because of their size, irregular pattern, and connectivity of subbasins within the overall basin outline

    Progressive renal disease: Role of race and antihypertensive medications. Hypertension is associated with an accelerated rate of decline in renal function in patients with chronic renal disease. To identify factors that might alter the rate of decline in renal function, we reviewed records of 200 patients from the Nephrology Clinic of the Durham VA Medical Center who had chronic progressive renal insufficiency. The average rate of decline in renal function (slope of reciprocal plasma creatinine versus time) was -0.80 ± 0.62 (SD) ml/mg month in 112 black patients and -0.84 ± 0.59 in 88 white patients. Multiple regression analysis indicates that the patient's age, level of diastolic blood pressure and type of antihypertensive treatment had a significant effect on the slope of reciprocal creatinine whereas the patient's race and diagnosis did not. In individual patients, addition of minoxidil or a calcium channel blocker to other medications significantly lowered blood pressure and slope of reciprocal creatinine. Some antihypertensive medications lowered blood pressure without significantly affecting the slope. These data suggest that specific medications may have a favorable effect on the progression of chronic renal disease by mechanisms in addition to reduction of blood pressure

    Mars Sedimentary Geology: Key Concepts and Outstanding Questions

    Processes that operate at planetary surfaces have the potential to record a history of planetary evolution in the form of sedimentary rocks. This is important because our experience on Earth shows that sediments and sedimentary rocks are the dominant archive of high-resolution proxies of present and past tectonic, climatic, and biological processes. Our understanding of the evolution of Earth’s very ancient climate and paleobiological records derives from detailed examination of the mineralogical, textural, and geochemical signatures preserved in the sedimentary rock record. Sedimentary rocks were first recognized on Mars less than a decade ago (Malin and Edgett, 2000). Recent interpretations of data collected by the Mars Express and Mars Reconnaissance Orbiter spacecraft have confirmed the surprising abundance of these sedimentary rocks, the past role of water on the martian surface, and the similarity—in some cases—to sedimentary rocks formed on Earth. Thick sulfaterich deposits invite comparison to terrestrial evaporites (Grotzinger et al., 2005). In other cases, clay-rich strata are interpreted as the terminal deposits of source-to-sink systems with well-developed fluvial networks in the upper reaches of watersheds that date back to a much wetter period in Mars’ earliest history (Ehlmann et al., 2008; Metz et al., 2009). However, these Earth-like depositional systems contrast with other deposits that may be unique in the Solar System: for example, vast terrains as large as Earth’s continents covered by thick veneers of strata that may derive entirely from settling out of wind-transported dust (Bridges et al., 2010). Whatever their origin, it is now clear that the sedimentary rocks of Mars represent a new frontier for research. Mars science is in its golden era of exploration—the past decade of orbiter and landed missions has produced an extraordinary amount of new data relevant to the analysis of sediments and sedimentary rocks, and robust international programs exist for future missions. To help stimulate discussion of these data, the First International Conference on Mars Sedimentology and Stratigraphy was convened in El Paso, Texas, in April 2010. The contents of this white paper represent the most significant findings of the conference, with additional information provided by the coauthors, and focus on seven key questions for future investigation by the sedimentary geology community

    Men Presenting With Sexual Thoughts of Children or Coercion: Flights of Fancy or Plans for Crime?

    Introduction. There is limited evaluation of clinical and theoretical claims that sexual thoughts of children and coercing others facilitate sexual offending. The nature of these thoughts (what they contain) is also unknown. Aims. To examine the relationship between child/coercive sexual thoughts and sexual offending, and to determine the nature of these thoughts and any differences between sexual offending (SO), non-sexual offending (NSO) and non-offending (NO) men. Methods. In a cross-sectional computerized survey, anonymous qualitative and quantitative self-reported sexual thought and experience data were collected from 279 adult volunteers, comprising equal numbers of SO, NSO and NO men recruited from a medium-security UK prison and a community sample of 6081 men. Main Outcome Measures. Computerized Interview for Sexual Thoughts and Computerized Inventory of Sexual Experiences. Results. Three analytical approaches found child sexual thoughts were related to sexual offending; sexual thoughts with coercive themes were not. Latent class analyses identified three types of child sexual thought (primarily differentiated by interpersonal context: the reporting of own emotions, emotions of others or both) and four types of sexual thoughts of coercing others (chiefly discriminated by the other person’s response: no emotional states reported, consent, non-consent, mixed). Type of child sexual thought and participant group were not significantly related. Type of coercive sexual thought and group were marginally related; the consensual type was more common for the NO group, the non-consensual type more common for the SO group, than expected statistically. Conclusions. Child sexual thoughts are a risk factor for sexual offending and should be assessed by clinicians. Generally, sexual thoughts with coercive themes are not a risk factor, though thought type may be important (i.e. thoughts where the other person expresses an enduring lack of consent). Exploring the dynamic risk factors associated with each type of child/coercive thought may lead to more targeted treatment

    The human capital transition and the role of policy

    Along with information and communication technology, infrastructure, and the innovation system, human capital is a key pillar of the knowledge economy with its scope for increasing returns. With this in mind, the purpose of this chapter is to investigate how industrialized economies managed to achieve the transition from low to high levels of human capital. The first phase of the human capital transition was the result of the interaction of supply and demand, triggered by technological change and boosted by the demands for (immaterial) services. The second phase of the human capital transition (i.e., mass education) resulted from enforced legislation and major public investment. The state’s aim to influence children’s beliefs appears to have been a key driver in public investment. Nevertheless, the roles governments played differed according to the developmental status and inherent socioeconomic and political characteristics of their countries. These features of the human capital transition highlight the importance of understanding governments’ incentives and roles in transitions

    Drosophila DNA polymerase theta utilizes both helicase-like and polymerase domains during microhomology-mediated end joining and interstrand crosslink repair

    Double strand breaks (DSBs) and interstrand crosslinks (ICLs) are toxic DNA lesions that can be repaired through multiple pathways, some of which involve shared proteins. One of these proteins, DNA Polymerase theta (Pol theta), coordinates a mutagenic DSB repair pathway named microhomology-mediated end joining (MMEJ) and is also a critical component for bypass or repair of ICLs in several organisms. Pol theta contains both polymerase and helicase-like domains that are tethered by an unstructured central region. While the role of the polymerase domain in promoting MMEJ has been studied extensively both in vitro and in vivo, a function for the helicase-like domain, which possesses DNA-dependent ATPase activity, remains unclear. Here, we utilize genetic and biochemical analyses to examine the roles of the helicase-like and polymerase domains of Drosophila Pol theta. We demonstrate an absolute requirement for both polymerase and ATPase activities during ICL repair in vivo. However, similar to mammalian systems, polymerase activity, but not ATPase activity, is required for ionizing radiation-induced DSB repair. Using a site-specific break repair assay, we show that overall end-joining efficiency is not affected in ATPase-dead mutants, but there is a significant decrease in templated insertion events. In vitro, Pol theta can efficiently bypass a model unhooked nitrogen mustard crosslink and promote DNA synthesis following microhomology annealing, although ATPase activity is not required for these functions. Together, our data illustrate the functional importance of the helicase-like domain of Pol theta and suggest that its tethering to the polymerase domain is important for its multiple functions in DNA repair and damage tolerance

    How parents choose to use CAM: a systematic review of theoretical models

    Background: Complementary and Alternative Medicine (CAM) is widely used throughout the UK and the Western world. CAM is commonly used for children and the decision-making process to use CAM is affected by numerous factors. Most research on CAM use lacks a theoretical framework and is largely based on bivariate statistics. The aim of this review was to identify a conceptual model which could be used to explain the decision-making process in parental choice of CAM. Methods: A systematic search of the literature was carried out. A two-stage selection process with predetermined inclusion/exclusion criteria identified studies using a theoretical framework depicting the interaction of psychological factors involved in the CAM decision process. Papers were critically appraised and findings summarised. Results: Twenty two studies using a theoretical model to predict CAM use were included in the final review; only one examined child use. Seven different models were identified. The most commonly used and successful model was Andersen's Sociobehavioural Model (SBM). Two papers proposed modifications to the SBM for CAM use. Six qualitative studies developed their own model. Conclusion: The SBM modified for CAM use, which incorporates both psychological and pragmatic determinants, was identified as the best conceptual model of CAM use. This model provides a valuable framework for future research, and could be used to explain child CAM use. An understanding of the decision making process is crucial in promoting shared decision making between healthcare practitioners and parents and could inform service delivery, guidance and policy
