Soccer and homosexuality: the conflicts that lie within the affective game of the coach-adolescent athlete relationship
Discussions regarding homosexuality within a sports context are mediated by emotional nuances, especially in the world of soccer, where practitioners are required to have attributes such as strength, masculinity and aggressiveness. As a consequence, this work aimed to analyze the symbolic representation of homosexual relationships between
adolescent soccer players and their coaches; and, furthermore, to understand the intention behind homoerotic relationships that are established between members of the management team and its players. In this study, researchers used the life history methodology, with participation from three adolescent athletes, ages 14, 15 and 16, and three former athletes, ages 26, 28 and 45, who we called speakers. Based on the collection and analysis of research participantsâ testimonies, homosexual relations were found to have occurred between athletes and coaches, or between athletes and other members of the management team, as a part of a âsocial contractâ that is exclusively linked to their professional objectives. These relationships appear to be purely objective; authentic affective links are absent, due apparently to the transient nature of the athletes in these groups. Too, this context is usually marked by the athleteâs physical and emotional vulnerability, and the agreements he makes within this context are accompanied with moral dilemmas, because such adolescents idealize the coach as a paternal archetype.âFutebol e homossexualidade: Os conflitos que se encontram dentro do jogo afetivo da relação treinador-atleta

e a tĂ©cnica adotada foi a âhistĂłria de vidaâ e a participação de trĂȘs atletas adolescentes em atividade, com idade de
14, 15 e 16 anos e trĂȘs ex-atletas profissionais, com idades 26, 28 e 45 anos serviram de base metodolĂłgica. A partir da
anålise e tratamento dos depoimentos verificou-se que ocorreram relaçÔes homossexuais nas categorias de base envolvendo
técnico e atleta, ou com outros membros da equipe diretiva, como parte dos contratos sociais, ligados exclusivamente a
objetivos profissionais. Estas relaçÔes parecem ser puramente objetais, nĂŁo parecendo existir vĂnculos afetivos duradouros,
aparentemente, devido à natureza transitória de atletas nesses grupos. Também, nesses ambientes, muitas vezes marcados

Risk profiles and one-year outcomes of patients with newly diagnosed atrial fibrillation in India: Insights from the GARFIELD-AF Registry.
BACKGROUND: The Global Anticoagulant Registry in the FIELD-Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. METHODS AND RESULTS: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012-2016). In India, the mean age was 65.8 years at diagnosis of NVAF. Hypertension was the most prevalent risk factor for AF, present in 68.5% of patients from India and in 76.3% of patients globally (P < 0.001). Diabetes and coronary artery disease (CAD) were prevalent in 36.2% and 28.1% of patients as compared with global prevalence of 22.2% and 21.6%, respectively (P < 0.001 for both). Antiplatelet therapy was the most common antithrombotic treatment in India. With increasing stroke risk, however, patients were more likely to receive oral anticoagulant therapy [mainly vitamin K antagonist (VKA)], but average international normalized ratio (INR) was lower among Indian patients [median INR value 1.6 (interquartile range {IQR}: 1.3-2.3) versus 2.3 (IQR 1.8-2.8) (P < 0.001)]. Compared with other countries, patients from India had markedly higher rates of all-cause mortality [7.68 per 100 person-years (95% confidence interval 6.32-9.35) vs 4.34 (4.16-4.53), P < 0.0001], while rates of stroke/systemic embolism and major bleeding were lower after 1 year of follow-up. CONCLUSION: Compared to previously published registries from India, the GARFIELD-AF registry describes clinical profiles and outcomes in Indian patients with AF of a different etiology. The registry data show that compared to the rest of the world, Indian AF patients are younger in age and have more diabetes and CAD. Patients with a higher stroke risk are more likely to receive anticoagulation therapy with VKA but are underdosed compared with the global average in the GARFIELD-AF. The Cholecystectomy As A Day Case (CAAD) Score: A Validated Score of Preoperative Predictors of Successful Day-Case Cholecystectomy Using the CholeS Data Set
Background
Day-case surgery is associated with significant patient and cost benefits. However, only 43% of cholecystectomy patients are discharged home the same day. One hypothesis is day-case cholecystectomy rates, defined as patients discharged the same day as their operation, may be improved by better assessment of patients using standard preoperative variables.
Methods
Data were extracted from a prospectively collected data set of cholecystectomy patients from 166 UK and Irish hospitals (CholeS). Cholecystectomies performed as elective procedures were divided into main (75%) and validation (25%) data sets. Preoperative predictors were identified, and a risk score of failed day case was devised using multivariate logistic regression. Receiver operating curve analysis was used to validate the score in the validation data set.
Results
Of the 7426 elective cholecystectomies performed, 49% of these were discharged home the same day. Same-day discharge following cholecystectomy was less likely with older patients (OR 0.18, 95% CI 0.15â0.23), higher ASA scores (OR 0.19, 95% CI 0.15â0.23), complicated cholelithiasis (OR 0.38, 95% CI 0.31 to 0.48), male gender (OR 0.66, 95% CI 0.58â0.74), previous acute gallstone-related admissions (OR 0.54, 95% CI 0.48â0.60) and preoperative endoscopic intervention (OR 0.40, 95% CI 0.34â0.47). The CAAD score was developed using these variables. When applied to the validation subgroup, a CAAD score of â€5 was associated with 80.8% successful day-case cholecystectomy compared with 19.2% associated with a CAAD score >5 (pâ<â0.001).
Conclusions
The CAAD score which utilises data readily available from clinic letters and electronic sources can predict same-day discharges following cholecystectomy
Advancing Systems Thinking in Evaluation: A Review of Four Publications
ReviewThis article reviews four books published in 2010 and 2011 on the topic of systems thinking in evaluation: Developmental Evaluation: Applying Complexity Concepts to Enhance Innovation and Use by Michael Q. Patton; Purposeful Program Theory: Effective Use of Theories of Change and Logic Models by Sue Funnell and Patricia Rogers; Systems Concepts in Action: A Practitioner's Toolkit by Bob Williams and Richard Hummelbrunner, and Evaluation in the Face of Uncertainty: Anticipating Surprise and Responding to the Inevitable by Jonathan Morell. These books describe the interdisciplinary landscape of systems theories, encompassing general systems theory, systems dynamics, complexity theory, network theory, and learning systems, and how they can be used to improve evaluation practice. The books appeal to many different and sometimes overlapping audiences. Separately, they address specific information needs about how to incorporate systems concepts into different aspects of evaluation design, data collection, analysis, and reporting. Together, the books complement and reinforce each other, creating a cohesive picture of the state of the art of systems thinking by encouraging thinking about situational dynamics in an often complicated and complex world. The books' authors also advance systems thinking in evaluation by inviting their readers to test out the books' ideas and methods in their own evaluation work. © The Author(s) 2012
Hydrodynamic modelling of hydrostatic magnesium extrusion
Wilsonâs hydrodynamic model of the hydrostatic extrusion process is extended to meet the geometry found on residual billets. The transition from inlet to work zone of the process is not considered sharp as in the model of Wilson but as a rounded edge, modelled by a parabolic function. It is shown that this rounded edge has a considerable influence on the predicted film thickness. Furthermore, it is shown that for hydrostatic extrusion of magnesium with castor oil as pressure medium, it is not possible to generate full film lubrication in the work zone of the hydrostatic extrusion process
