    Feasibility of the Target Aerobic Movement Test in Children and Adolescents with Spina Bifida

    The purpose of this study was to determine the feasibility of the Target Aerobic Movement Test (TAMT)’ in a group of children and adolescents with spina bifida (n = 32). Thirty-two children (11 subjects-thoracic lesion, 21 subjects-lumbar lesion) volunteered for the study. Results indicated there were no significant differences in the proportion of subjects who passed Test 1 or Test 2 (p \u3e .05). Twenty-seven out of 28 eligible subjects (96%) on Test 1 and 25 of 27 eligible subjects (93%) on Test 2 met the criteria for successful completion of the TAMT. The TAMT appears to be a reliable and feasible test for measuring aerobic behavior in children and adolescents with spina bifida. Future research should focus on studying the feasibility of the TAMT with other populations with disabilities and to also determine if the test can become a more refined discriminator of aerobic behavior and aerobic capacity

    Asteroseismic determination of helium abundance in stellar envelopes

    Intermediate degree modes of the solar oscillations have previously been used to determine the solar helium abundance to a high degree of precision. However, we cannot expect to observe such modes in other stars. In this work we investigate whether low degree modes that should be available from space-based asteroseismology missions can be used to determine the helium abundance, Y, in stellar envelopes with sufficient precision. We find that the oscillatory signal in the frequencies caused by the depression in \Gamma_1 in the second helium ionisation zone can be used to determine the envelope helium abundance of low mass main sequence stars. For frequency errors of 1 part in 10^4, we expect errors \sigma_Y in the estimated helium abundance to range from 0.03 for 0.8M_sun stars to 0.01 for 1.2M_sun stars. The task is more complicated in evolved stars, such as subgiants, but is still feasible if the relative errors in the frequencies are less than 10^{-4}.Comment: To appear in MNRAS. 11 pages, 11 figure

    Opicapone, a Novel Catechol-O-methyl Transferase Inhibitor, for Treatment of Parkinson\u27s Disease Off Episodes

    Parkinson\u27s Disease (PD) is a common neurodegenerative disorder and the leading cause of disability. It causes significant morbidity and disability through a plethora of symptoms, including movement disorders, sleep disturbances, and cognitive and psychiatric symptoms. The traditional pathogenesis theory of PD involves the loss of dopaminergic neurons in the substantia nigra (SN). Classically, treatment is pursued with an assortment of medications that are directed at overcoming this deficiency with levodopa being central to most treatment plans. Patients taking levodopa tend to experience off episodes with decreasing medication levels, causing large fluctuations in their symptoms. These off episodes are disturbing and a source of morbidity for these patients. Opicapone is a novel, peripherally acting Catechol-O-methyl transferase (COMT) inhibitor that is used as adjunctive therapy to carbidopa/levodopa for treatment and prevention of off episodes. It has been approved for use as an adjunct to levodopa since 2016 in Europe and has recently (April 2020) gained FDA approval for use in the USA. By inhibiting COMT, opicapone slows levodopa metabolism and increases its availability. Several clinical studies demonstrated significant improvement in treatment efficacy and reduction in duration of off episodes. The main side effect demonstrated was dyskinesia, mostly with the 100mg dose, which is higher than the approved, effective dose of 50mg. Post-marketing surveillance and analysis are required to further elucidate its safety profile and contribute to patient selection. This paper reviews the seminal and latest evidence in the treatment of PD off episodes with the novel drug Opicapone, including efficacy, safety, and clinical indications

    Migraine is a common form of primary headache, affecting up to 1 in every 6 Americans. The pathophysiology is an intricate interplay of genetic factors and environmental influence and is still being elucidated in ongoing studies. The trigeminovascular system is now known to have a significant role in the initiation of migraines, including the release of pain mediators such as CGRP and substance P. Traditional treatment of migraine is usually divided into acute and preventive treatment. Acute therapy includes non-specific therapy, such as NSAIDs and other analgesics, which may provide relief in mild to moderate migraines. 5-HT1 agonists may provide relief in severe migraine, but are not universally effective and carry a significant side-effect profile with frequent redosing requirement. Prophylactic therapy may reduce the occurrence of acute migraine attacks in selected patients, but does not completely eliminate it. More recently, CGRP antagonism has been studied and shown to be effective in both abortion and prevention of migraine. Novel medications, targeting CGRP, divide into CGRP antibodies and receptor antagonists (gepants). Rimegepant, a second-generation gepant, has shown efficacy in several clinical trials in treating acute migraine. Ongoing trials are also evaluating its role in migraine prophylaxis, and results are promising. It is also generally safer for use than existing options, does not appear to increase the chance of developing chronic migraines, and carries a very tolerable side effects profile. It is a part of a growing arsenal in migraine treatment, and may present the silver bullet for treatment of this disease

    The impact of neogene grassland expansion and aridification on the isotopic composition of continental precipitation

    The late Cenozoic was a time of global cooling, increased aridity, and expansion of grasslands. In the last two decades numerous records of oxygen isotopes have been collected to assess plant ecological changes, understand terrestrial paleoclimate, and to determine the surface history of mountain belts. The δ¹⁸(O) values of these records, in general, increase from the mid-Miocene to the Recent. We suggest that these records record an increase in aridity and expansion of grasslands in midlatitude continental regions. We use a nondimensional isotopic vapor transport model coupled with a soil water isotope model to evaluate the role of vapor recycling and transpiration by different plant functional types. This analysis shows that increased vapor recycling associated with grassland expansion along with biomechanistic changes in transpiration by grasses themselves conspires to lower the horizontal gradient in the δ¹⁸(O) of atmospheric vapor as an air mass moves into continental interiors. The resulting signal at a given inland site is an increase in δ¹⁸(O) of precipitation with the expansion of grasslands and increasing aridity, matching the general observed trend in terrestrial Cenozoic δ¹⁸(O) records. There are limits to the isotopic effect that are induced by vapor recycling, which we refer to here as a “hydrostat.” In the modern climate, this hydrostatic limit occurs at approximately the boundary between forest and grassland ecosystems

    Seismology of the Sun : Inference of Thermal, Dynamic and Magnetic Field Structures of the Interior

    Recent overwhelming evidences show that the sun strongly influences the Earth's climate and environment. Moreover existence of life on this Earth mainly depends upon the sun's energy. Hence, understanding of physics of the sun, especially the thermal, dynamic and magnetic field structures of its interior, is very important. Recently, from the ground and space based observations, it is discovered that sun oscillates near 5 min periodicity in millions of modes. This discovery heralded a new era in solar physics and a separate branch called helioseismology or seismology of the sun has started. Before the advent of helioseismology, sun's thermal structure of the interior was understood from the evolutionary solution of stellar structure equations that mimicked the present age, mass and radius of the sun. Whereas solution of MHD equations yielded internal dynamics and magnetic field structure of the sun's interior.     Multi-Element Abundance Measurements from Medium-Resolution Spectra. IV. Alpha Element Distributions in Milky Way Dwarf Satellite Galaxies

    We derive the star formation histories of eight dwarf spheroidal (dSph) Milky Way satellite galaxies from their alpha element abundance patterns. Nearly 3000 stars from our previously published catalog (Paper II) comprise our data set. The average [alpha/Fe] ratios for all dSphs follow roughly the same path with increasing [Fe/H]. We do not observe the predicted knees in the [alpha/Fe] vs. [Fe/H] diagram, corresponding to the metallicity at which Type Ia supernovae begin to explode. Instead, we find that Type Ia supernova ejecta contribute to the abundances of all but the most metal-poor ([Fe/H] < -2.5) stars. We have also developed a chemical evolution model that tracks the star formation rate, Types II and Ia supernova explosions, and supernova feedback. Without metal enhancement in the supernova blowout, massive amounts of gas loss define the history of all dSphs except Fornax, the most luminous in our sample. All six of the best-fit model parameters correlate with dSph luminosity but not with velocity dispersion, half-light radius, or Galactocentric distance.Comment: 28 pages, 14 figures; accepted for publication in ApJ; very minor editorial corrections in v
