    Perfil inmunohistoquímico de lesiones melanocíticas palmoplantares: estudio de casos y controles.

    Las lesiones melanocíticas tanto benignas como malignas con un verdadero para el clínico, incluso para el patólogo en caso de que una lesión sea resecada para su estudio histológico. El melanoma acral lentiginoso es de gran importancia en la población latina, debido a que es una forma prevalente de neoplasias en este grupo de individuos. Aunque existen una gran cantidad de herramientas desde las no invasivas -como la dermatoscopia- tanto invasivas como el estudio histpatológico y las tinciones inmunohistoquímicas. Estas tinciones imnunohistoquímicas pueden definir un perfil de las células predominantes en las neoplasias, y tener valor en el diagnóstico o incluso en el pronóstico. Conocer el perfil de expresión de marcadores como nestin, SOX2 y Ki67 en el caso particular del melanoma acral lentiginoso nos puede aportar datos relevantes en su comportamiento biológico y su pronóstico, además de su adecuada diferenciación de la contraparte benigna como los nevos melanocíticos en la piel acral, que tanta confusión pueden generar en el médico tratante

    bstract: The coiled and dotted vessels in a serpiginous arrangement or “string of pearls” is considered a classical vascular pattern associated with clear cell acanthoma. We present three cases of epidermal tumors different from clear cell acanthoma that have the same “string of pearls” vascular pattern. Even though most authors keep considering the “string of pearls” vascular pattern an almost pathognomonic sign of clear-cell acanthoma, the cases presented here suggest that some other epidermal tumors can also show this pattern.     El virus del papiloma humano (VPH) pertenece al grupo de virus con tropismo por los epitelios; infectan predominantemente la piel y las membranas mucosas y producen proliferaciones benignas o papilomas, que bajo ciertas circunstancias pueden experimentar transformación maligna. El VPH es considerado el agente causal más importante del carcinoma del cérvix uterino y el conocimiento de su biología es fundamental para el entendimiento de la carcinogénesis cervical. Existe evidencia epidemiológica y molecular sobre la estrecha relación del VPH en el desarrollo del carcinoma cervical y sus precursores. La infección por el virus del papiloma humano es un importante problema de salud pública en nuestro país, para lo cual se ha identificado a la población con claros factores de riesgo, tales como: inicio temprano de vida sexual activa y múltiples parejas sexuales, hacia quienes se dirigen todos los esfuerzos de prevención y diagnóstico temprano, donde ya se incluyen estudios avanzados y cada vez más sensibles y específicos, con el fin de detectar más oportunamente esta mortal enfermedad

    Dermoscopic features of acral melanocytic nevi in a case series from Mexico

    Background: Pigmented lesions on acral sites are common; clinical differentiation of nevi and early melanoma can be challenging. In these cases, dermoscopy can provide a more accurate diagnosis. Most dermoscopic patterns on acral skin have been described in Asian and European populations, while there are few studies in Latin American populations. Objectives: To determine the frequency of pigmented lesions in volar skin and their dermoscopic patterns in a Mexican population. Methods: An observational, descriptive, cross-sectional study was performed in Hispanic patients with the presence of at least one pigmented lesion on acral skin. Clinical and dermoscopic images were obtained. These were subsequently evaluated independently by two dermatologists trained and experienced in dermoscopy. Results: A total of 582 pigmented lesions on volar skin were diagnosed in 321 patients. Overall, prevalence of acral pigmented lesions on volar skin was 6.8%. For both observers, parallel furrows were the most frequent pattern described, but for observer 2, a lattice-like pattern was prevalent on the toes and a homogeneous pattern on the sides of the feet. There was lower inter-observer agreement, with a kappa index of 0.144. Study limitations: The lesions were not biopsied, so clinical-histological correlation could not be performed. The study did not correlate dermoscopic patterns with age. Conclusions.: As previously reported by other authors, parallel furrows were the most frequently found dermoscopic pattern on palmoplantar skin

    Vitruvius+: An area-efficient RISC-V decoupled vector coprocessor for high performance computing applications

    The maturity level of RISC-V and the availability of domain-specific instruction set extensions, like vector processing, make RISC-V a good candidate for supporting the integration of specialized hardware in processor cores for the High Performance Computing (HPC) application domain. In this article,1 we present Vitruvius+, the vector processing acceleration engine that represents the core of vector instruction execution in the HPC challenge that comes within the EuroHPC initiative. It implements the RISC-V vector extension (RVV) 0.7.1 and can be easily connected to a scalar core using the Open Vector Interface standard. Vitruvius+ natively supports long vectors: 256 double precision floating-point elements in a single vector register. It is composed of a set of identical vector pipelines (lanes), each containing a slice of the Vector Register File and functional units (one integer, one floating point). The vector instruction execution scheme is hybrid in-order/out-of-order and is supported by register renaming and arithmetic/memory instruction decoupling. On a stand-alone synthesis, Vitruvius+ reaches a maximum frequency of 1.4 GHz in typical conditions (TT/0.80V/25°C) using GlobalFoundries 22FDX FD-SOI. The silicon implementation has a total area of 1.3 mm2 and maximum estimated power of ~920 mW for one instance of Vitruvius+ equipped with eight vector lanes.This research has received funding from the European High Performance Computing Joint Undertaking (JU) under Framework Partnership Agreement No 800928 (European Processor Initiative) and Specific Grant Agreement No 101036168 (EPI SGA2).     DVINO: A RISC-V vector processor implemented in 65nm technology

    This paper describes the design, verification, implementation and fabrication of the Drac Vector IN-Order (DVINO) processor, a RISC-V vector processor capable of booting Linux jointly developed by BSC, CIC-IPN, IMB-CNM (CSIC), and UPC. The DVINO processor includes an internally developed two-lane vector processor unit as well as a Phase Locked Loop (PLL) and an Analog-to-Digital Converter (ADC). The paper summarizes the design from architectural as well as logic synthesis and physical design in CMOS 65nm technology.The DRAC project is co-financed by the European Union Regional Development Fund within the framework of the ERDF Operational Program of Catalonia 2014-2020 with a grant of 50% of total eligible cost.     Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    Perfil de expresión de Ki67 en lesiones melanocíticas palmoplantares: estudio de casos y controles

    El melanoma acral lentiginoso es una neoplasia maligna que afecta a población predominantemente no caucásica. Debido al diagnóstico tardío suele tener mal pronóstico, además de que se considera una neoplasia biológicamente más agresiva, incluso cuando se detecta tempranamente. Objetivo: Determinar la expresión de Ki67 en el melanoma acral lentiginoso invasor y compararla con los nevos acrales. Método: Estudio transversal, descriptivo, observacional. Se realizó inmunohistoquímica con marcador Ki67 en 17 biopsias de melanoma acral lentiginoso invasor (casos) y 17 biopsias de nevos palmoplantares (controles). Se determinó la expresión nuclear de Ki-67 y se comparó entre ambos grupos. Resultados: La media de expresión de Ki67 fue del 8.5% en el grupo control y del 34% en el grupo de melanomas, siendo esta diferencia estadísticamente significativa (p < 0.0001). Discusión: La expresión de Ki67 en los melanomas acrales es considerablemente mayor que en los nevos acrales. El valor pronóstico del marcador Ki67 sigue siendo considerado controversial. Sin embargo, hay estudios en los que en combinación con otros marcadores se refuerza su valor pronóstico. Conclusiones: Por la gran diferencia en inmunorreactividad de Ki67 entre melanomas y nevos, la expresión de Ki67, referida como índice proliferativo, podría ser considerada como factor pronóstico incluso más objetivo que el índice mitótico

    An academic RISC-V silicon implementation based on open-source components

The preDRAC processor is the first RISC-V processor designed and fabricated by a Spanish or Mexican academic institution, and will be the basis of future RISC-V designs jointly developed by these institutions.
