Efecto del tiempo hasta el inicio de la terapia antibiótica adecuada sobre la mortalidad intrahospitalaria a treinta días en pacientes de cuidado intensivo con bacteriemia por Pseudomonas aeruginosa
Introducción: Pseudomonas aeruginosa es uno de los patógenos más importantes en las infecciones nosocomiales. Objetivo: Estimar la asociación entre el tiempo desde el diagnóstico hasta el inicio de la terapia antibiótica adecuada y la mortalidad intrahospitalaria en pacientes con bacteriemia por Pseudomonas aeruginosa en cuidado intensivo. Métodos: Cohorte retrospectiva multicéntrica. Se usó análisis de supervivencia bivariado y regresión paramétrica. Resultados y conclusiones: Se estudiaron 178 pacientes, un 55.6% recibieron al menos un antibiótico adecuado con una media de inicio de 2.4 días. La mortalidad a 30 días fue de 42%. No se encontró una asociación entre el tiempo hasta el inicio de la terapia antibiótica adecuada y la mortalidad intrahospitalaria (razón de tiempo al evento, métrica de tiempo acelerado 1.08, intervalo de confianza del 95% 0.89-1.3). Se encontró una tendencia hacia el efecto protector de la terapia adecuada (razón de tiempo al evento 1.78, intervalo de confianza del 95% 0.92-3.43). / Abstract. Background: Pseudomonas aeruginosa is one of the most important pathogens in hospital-acquired infections. Objective: To estimate the association between time from diagnosis to onset of adequate antimicrobial therapy and hospital mortality in critical care patients with Pseudomonas aeruginosa bacteremia. Methods: Multicenter historic cohort study. Survival analysis and parametric survival regression was used. Results and conclusions: 178 patients were studied, 55.6% of them received at least one antibiotic considered adequate, with a mean onset of 2.4 days. 30-day mortality was 42%. An association between time to onset of adequate antimicrobial therapy and mortality was not found (time ratio, accelerated failure time metric 1.08, 95% confidence interval 0.89-1.3). Evaluation method in field of the blue mold resistance in garlic accessions
Con el objeto de ajustar un método de
evaluación a campo para facilitar la selección
de clones de ajo (Allium sativum L.) con
resistencia a Penicillium allii, se realizó un
ensayo replicado en el cual se midió la respuesta
de tres clones experimentales a la
infección, en dos épocas de plantación. Los
tratamientos incluyeron combinaciones de
presencia o ausencia de: a) desinfección de
la "semilla", b) heridas artificiales en los
bulbillos, c) inoculación artificial. Los bulbillos
con la hoja envolvente se desinfectaron con
una solución diluida de hipoclorito de sodio.
La inoculación se realizó mediante la inmersión
en una solución de 106 esporas•ml-1.
Las heridas se realizaron con una lanceta
(blood lancet). Los bulbillos se incubaron en
cámara húmeda durante 24 horas a 20 °C,
antes de plantación. Las variables respuestas
computadas fueron las siguientes: número
de plantas muertas, y vivas con síntomas
y sin síntomas, a los 152 días de plantación
en la primera época y 118 días en la
segunda; número de bulbos por calibres y
rendimiento a cosecha. La información obtenida
se analizó a través de correspondencia
simple, diferencia de proporciones y análisis
paramétricos. Se detectaron mayores
diferencias entre los tratamientos en la tasa
de sobrevivencia y en la proporción de calibres
comerciales en la época tardía. No se
observaron diferencias (p < 0,05) del rendimiento
en la plantación temprana. La herida
fue determinante en el ingreso del patógeno.
Los tratamientos quedaron limitados a
un testigo: desinfectado, no inoculado y sin
herida, y a un tratamiento: desinfectado, con
inoculación y herida. Con respecto a la época,
resultó conveniente la evaluación en plantaciones
tardías porque se expresan mejor
las diferencias entre los tratamientos.In order to establish a method of
selection in the field of clones of garlic
(Allium sativum L.) resistance to Penicillium
allii, a test was conducted to measure the
response of three clones to the infection, at
two times of plantation that corresponded to
cycles of 152 (early plantation) and 118 days
(late plantation). The treatments included
combinations of presence or absence of:
a) disinfection of the "seed", b) artificial
wounds in the cloves, c) artificial inoculation.
The cloves and their surrounding leaf were
disinfected with diluted sodium hypochlorite.
The inoculation consisted of dipping the
cloves in a solution of 106 spores•ml-1. The
wounds were produced by a blood lancet.
The cloves were incubated in humid chamber
for 24 hours at 20 °C, before plantation. The
variables measured were number of plants
dead, alive with symptoms and alive without
symptoms, 152 and 118 days after plantation
and bulb diameter and yield. Results were
analyzed statistically using non parametric
and parametric tests. Significant differences
between treatments were found 118 days
after plantation regarding the rate of survival
and the proportion of commercial calibers.
No differences in yield (p < 0.05) were
detected in the early plantation. Wounding
was determinant for the entrance of the
pathogen. It is concluded that treatments can
be limited to a control (disinfection, not
inoculated and not wounded) and a treatment
with disinfection, inoculation and wounding.
Late plantations are more sensible to the

Evolución clínica de pacientes hospitalizados por dengue en una institución de salud de Bucaramanga, Colombia
Introduction. Dengue hemorrhagic fever has extended to every tropical and subtropical area of the world, resulting in a half million hospitalizations every year. This disease appears to affect increasing numbers of adolescents and young adults. Objective. The clinical characteristics were described for adult and pediatric dengue inpatients to establish risk factors associated with bad prognosis. Materials and methods. A cohort of dengue inpatients of years 2006 and 2007 was evaluated retrospectively at “Clínica Chicamocha”, a high level hospital in Bucaramanga, Colombia. Results. Of 328 patients evaluated, 165 were female and 163 were male with a median age of 25 years. Dengue hemorrhagic fever was diagnosed in 116 patients, of which 113 were classified grade II. Of the 212 patients with dengue fever, 156 developed signs of plasma leakage, bleeding or thrombocytopenia. A positive serology was indicated in 82.4% of the patients. Inpatients with dengue hemorrhagic fever were younger (20.1 vs. 25.7 years, p<0.0054). Both lowest level of platelets and highest hematocrit were reached at the sixth day of illness. Children presented the typical symptoms of dengue less frequently, but demonstrated a greater proportion of ascites, pleural effusion and bleeding, and a higher risk of developing respiratory distress (RR=3.59, 95%CI 1.3-9.9, p<0.014) and hypotension (RR=10.77, 95%CI 5.56-20.86, p<0.001). Conclusions. Age was the most determinant factor of severity in dengue inpatients. In addition, a combination of particular symptoms and laboratory data at the day of admission may predict the development of complications.Introducción. El dengue hemorrágico abarca todas las áreas tropicales y subtropicales del planeta, y causa medio millón de hospitalizaciones al año en el mundo. Se ha descrito que esta enfermedad afecta a un grupo cada vez mayor de adolescentes y adultos jóvenes. Objetivo. Describir las características clínicas de la población, tanto adulta como pediátrica, hospitalizada por dengue durante un periodo endémico y determinar los factores de riesgo asociados a mal pronóstico. Materiales y métodos. Cohorte retrospectiva de pacientes hospitalizados por dengue en los años 2006 y 2007 en la Clínica Chicamocha. Resultados. Se evaluaron 328 pacientes, 165 mujeres y 163 hombres, con mediana de edad de 25 años. Se encontraron 116 casos de dengue hemorrágico, de los cuales, 113 eran de grado II. De los 212 pacientes con dengue clásico, 156 presentaron extravasación, sangrado o trombocitopenia. El 82,4% tuvieron serología positiva. Los pacientes con dengue hemorrágico eran más jóvenes (20,1 contra 25,7 años, p=0,0054). El número mínimo de plaquetas y el valor máximo del hematocrito se alcanzaron alrededor del sexto día de enfermedad. Los menores de 13 años presentaron con menor frecuencia los síntomas típicos de dengue, aunque con un mayor porcentaje de ascitis, derrame y sangrado; además, tuvieron mayor riesgo de desarrollar dificultad respiratoria (riesgo relativo (RR)=3,59, IC95% 1,3-9,9, p=0,014) e hipotensión (RR=10,77, IC95% 5,56-20,86, p<0,001). Conclusiones. La edad continúa siendo el factor predominante en la gravedad intrahospitalaria del dengue. Independientemente de ésta, un grupo de signos, síntomas y hallazgos de laboratorio al ingreso permite predecir la aparición de complicaciones
Applying Model-Driven Web Engineering to the Testing Phase of the ADAGIO Project
The Model-Driven Engineering (MDE) has been used in recent years
to promote better results in the development of Web Applications, in the field
that has been called Model-Driven Web Engineering (MDWE). One of the
advantages of applying MDWE is that it offers a solution to reduce the cost of
the tests without affecting their quality execution. This paper presents the
application of a MDWE methodology (Navigational Development Techniques,
NDT) that provides support for all the phases of the lifecycle of a software
project development proposing transformations between these phases, to manage
the test phase of a real-world case study named ADAGIO. This project,
among other goals, proposes the development of a web application whose main
objective is to offer researchers the possibility of integrating and consolidating
heterogeneous data sources, showing a unified vision of them, allowing to
simplify the search task in different repositories as well as the relationship

The execution of an Engineering Project - whose requirements and characteristics implies an exhaustive analysis of needs, environments, operational risks, regulations, equipment and instruments, physical spaces, documentation, and budgetary resources - involves the development of three sequential and complementary phases: Conceptual, Basic and Detail; whose results are: a State of the Art of the system-solution feasible to install, the Architecture and characteristics of equipment and elements of the system-solution, and the specifications of Equipment, instruments, materials and resources to implement the system-solution, respectively. This article reviews, taking as a reference a mathematical model of the three components of the Air Conditioning Units: Compressor, Condenser and Evaporator, the corresponding phases to project Air Conditioning Units in Data Center (ACUDC) according to requirements, norms and technical and engineering characteristics of implementation. Finally, success cases of Data Center implemented and its respective air conditioning systems are presented.La ejecución de un proyecto de ingeniería -cuyos requerimientos y características supone el análisis exhaustivo de necesidades, entornos, riesgos de operación, normatividad, equipos e instrumentos, espacios físicos, documentación, y recursos presupuestales- involucra el desarrollo de tres fases secuenciales y complementarias: conceptual, básica y de detalle; cuyos resultados son: un Estado del Arte del sistema-solución factible a instalar, la Arquitectura y características de equipos y elementos del sistema-solución, y las especificaciones de Equipos, instrumentos, materiales y recursos para implementar el sistema-solución, respectivamente. En el presente artículo se revisan, tomando como referencia un modelo matemático de los tres componentes de las Unidades de Climatización: compresor, condensador y evaporador, las fases correspondientes para proyectar Unidades de Climatización en Centro de Datos (UCDC, por sus siglas en ingles) de acuerdo con requerimientos, normas, y características técnicas e ingenieriles de implementación. Towards the Improvement of the Software Quality: An Enterprise 2.0 Architecture for Distributed Software Developments.
Software development is tightly dependent on the tools available for supporting its processes. Organizational and sociotechnical peculiarities such as indefinition of roles, geographically distributed development teams, new business models and diverse cultural interactions steer these tools. Software development supported by web-based services, built on top of Web 2.0 technologies, is emerging as a new paradigm for distributed software development. New generation software forges (web-based development environments) such as EzForge are becoming the infrastructure that provides the required features for hosting collections of software development projects. They are composed of an integrated set of tools, interacting in a mashup-like environment, each one suited for a specific task, and therefore simple enough to keep total complexity low. An adequate selection of tools helps developers to focus on the implementation of the requirements, while at the same time they cope with complex information coming from many individuals and organizations. The complexity of distributed software development requires a controlled and a strong collaboration amongst developers, which has to be supported by the selected architecture. Moreover, an increased demand on quality assurance is required by the many organizations aiming to achieve a certain quality level. A new architecture based on the Web 2.0 core ideas and methods overcomes these challenges in software development, representing a cornerstone to achieve satisfactory results in this ambitious environment
Inactivation efficacy and reactivation of fecal bacteria with a flow-through LED ultraviolet reactor: Intraspecific response prevails over interspecific differences
Treatment with ultraviolet (UV) light is a common option for inactivating waterborne organisms. The mercury vapor lamps conventionally used as a source of UV-C light for water disinfection are eventually replaced by light emitter diodes (LEDs) in the middle term due to their higher efficiency and lack of hazardous materials. Nonetheless, biological mechanisms for repairing UV damage caused by the UV treatment are some of its significant undesirable features. The objective of this study is to evaluate and compare the UV-resistance and the reactivation degree in different strains of E. coli and E. faecalis treated with a flow-through reactor equipped with LEDs with an emission range between 265 and 285 nm. The treated organisms were subjected to various illumination regimes after the UV irradiation. The results obtained indicated that intraspecific differences between the strains of E. coli were greater than the interspecific differences with respect to E. faecalis in terms of UV-resistance and repairing potential. The UV doses necessary to achieve four log-reductions ranged from 10.2 to 16.3 mJ cm−2 for E. coli and from 11.1 to 11.4 for mJ cm−2 for E. faecalis. Dark repair was not observed within 24 h after the UV irradiation whereas the degree of photorepair depended on both the bacteria strain and the applied UV dose. The exposure of the irradiated organisms to an illuminated environment entailed and increasing between the 18 % and the 160 % of the UV dose required to achieve four log-reductions. © 2023 The Author(s
Evaluación de la transferencia metálica y estabilidad del proceso GMAW
The metal transfer, as a particularity of the process with consumable electrodes, has a direct influence on different aspects of the welding process. Hence, the operation performance is affected by the quantity of spatter and mostly, in the stability where the electrical phenomena happen. This work established a methodology to compare techniques for the definition of stability in the forms of metal transfer presented in the GMAW process. For that, welding tests were carried out in the flat position, establishing parameters, and modifying the voltage between a range of 14 to 32 V. In addition, it was analyzed the behavior of the process using histograms, stability indicators, and weld bead geometry characterization. As a result, high process instability and splashing of low voltages were obtained, improving these conditions for higher voltages. In conclusion, the best conditions for the welding process are established at the point with the highest frequency of short circuits, exhibiting a minimum of spatter and the best welding bead.La transferencia metálica, como particularidad del proceso con electrodos consumibles, ejerce influencia directa sobre variados aspectos del proceso de soldadura; de este modo, el desempeño operacional de la técnica se ve afectado por un mayor o menor nivel de salpicaduras y, en general, por la estabilidad con que los fenómenos eléctricos ocurren. En el presente artículo fue establecida una metodología que permitió comparar técnicas para la definición de la estabilidad, en los tipos de transferencia metálica presentados en el proceso GMAW. Para esto, se realizaron ensayos de soldadura en la posición plana, estableciendo parámetros y variando el voltaje entre un rango de 14 a 32 V. Además, fue analizado el comportamiento del proceso, mediante el uso de histogramas, índices de estabilidad y la caracterización de la geometría del cordón. Como resultado, se obtuvo alta inestabilidad del proceso y salpicaduras en bajas tensiones, mejorando estas condiciones para las tensiones más altas. En conclusión, en el punto de mayor frecuencia de cortos circuitos se establecen las mejores condiciones para el proceso de soldadura, exhibiendo un mínimo de salpicaduras y el cordón de soldadura con mejor formato.
