    Efficacy and Tolerability of Adjunctive Intravenous Sodium Nitroprusside Treatment for Outpatients With Schizophrenia: A Randomized Clinical Trial

    Importance: Antipsychotic medications for the treatment of schizophrenia have limitations, and new treatments are needed. A prior pilot investigation suggested that adjunctive sodium nitroprusside (SNP) administered intravenously had rapid efficacy in the treatment of patients with schizophrenia. Objective: To determine the efficacy and tolerability of intravenous SNP infused at a rate of 0.5 mug/kg/min for 4 hours in patients with schizophrenia with some degree of treatment resistance. Design, Setting, and Participants: Multicenter, randomized, double-blind acute treatment study using a sequential parallel comparison design conducted in two 2-week phases at 4 academic medical centers beginning May 20, 2015, and ending March 31, 2017. Participants were adults 18 to 65 years of age with a diagnosis of schizophrenia as confirmed by the Structured Clinical Interview for DSM-IV, taking antipsychotic medication for at least 8 weeks, and had at least 1 failed trial of an antipsychotic medication within the past year. A total of 90 participants consented, 60 participants enrolled, and 52 participants were included in the analyses. A modified intent-to-treat analysis was used. Interventions: Participants were randomized in a 1:1:1 ratio to 1 of 3 treatment sequences: SNP and SNP, placebo and SNP, and placebo and placebo. The SNP and SNP group received SNP in phase 1 and SNP in phase 2 for the purpose of blinding, but the data from phase 2 were not included in the results. The placebo and SNP group received placebo in phase 1 and SNP in phase 2. If there was no response to placebo in phase 1, data from phase 2 were included in the analyses. The placebo and placebo group received placebo in both phases; if there was no response to placebo in phase 1, data from phase 2 were included in the analyses. Main Outcomes and Measures: Effectiveness of SNP compared with placebo in improving Positive and Negative Syndrome Scale (PANSS) total, positive, and negative scores across each 2-week phase. Results: Fifty-two participants (12 women and 40 men) were included in the study. In the SNP and SNP group, the mean (SD) age was 47.1 (10.5) years. In the placebo and SNP group, the mean (SD) age was 45.9 (12.3) years. In the placebo and placebo group, the mean (SD) age was 40.4 (11.0) years. There were no significant differences between the SNP and placebo groups at baseline or in change from baseline for PANSS-total (weighted beta = -1.04; z = -0.59; P = .57), PANSS-positive (weighted beta = -0.62; z = -0.93; P = .35), or PANSS-negative (weighted beta = -0.12; z = -0.19; P = .85) scores. No significant differences in safety or tolerability measures were identified. Conclusions and Relevance: Although intravenous SNP is well tolerated, it was not an efficacious adjunctive treatment of positive or negative symptoms of psychosis among outpatients with schizophrenia with prior history of treatment resistance.     COBE's Galactic Bar and Disk

    A model of the bar and old stellar disk of the Galaxy has been derived from the survey of the Diffuse Infrared Background Experiment (DIRBE) of the Cosmic Background Explorer at wavelengths of 1.25, 2.2, 3.5, and 4.9 microns. It agrees very well with the data, except in directions in which the near- infrared optical depth is high. Among the conclusions drawn from the model: The Sun is located approximately 16.5 pc above the midpoint of the Galactic plane. The disk has an outer edge four kpc from the Sun, and is warped like the HI layer. It has a central hole roughly the diameter of the inner edge of the "three-kiloparsec" molecular cloud ring, and within that hole lies a bright, strong, "early-type" bar, tilted approximately 14 degrees from the Sun-Galactic center line. The model has 47 free parameters. The model is discussed in detail and contour plots and images of the residuals presented.Comment: 30 pages, LaTeX, with 15 figures, 3 JPG and 12 GIF.     The EROS2 search for microlensing events towards the spiral arms: the complete seven season results

    The EROS-2 project has been designed to search for microlensing events towards any dense stellar field. The densest parts of the Galactic spiral arms have been monitored to maximize the microlensing signal expected from the stars of the Galactic disk and bulge. 12.9 million stars have been monitored during 7 seasons towards 4 directions in the Galactic plane, away from the Galactic center. A total of 27 microlensing event candidates have been found. Estimates of the optical depths from the 22 best events are provided. A first order interpretation shows that simple Galactic models with a standard disk and an elongated bulge are in agreement with our observations. We find that the average microlensing optical depth towards the complete EROS-cataloged stars of the spiral arms is τˉ=0.51±.13×106\bar{\tau} =0.51\pm .13\times 10^{-6}, a number that is stable when the selection criteria are moderately varied. As the EROS catalog is almost complete up to IC=18.5I_C=18.5, the optical depth estimated for the sub-sample of bright target stars with IC<18.5I_C<18.5 (τˉ=0.39±>.11×106\bar{\tau}=0.39\pm >.11\times 10^{-6}) is easier to interpret. The set of microlensing events that we have observed is consistent with a simple Galactic model. A more precise interpretation would require either a better knowledge of the distance distribution of the target stars, or a simulation based on a Galactic model.     The intrinsic shape of galaxy bulges

    The knowledge of the intrinsic three-dimensional (3D) structure of galaxy components provides crucial information about the physical processes driving their formation and evolution. In this paper I discuss the main developments and results in the quest to better understand the 3D shape of galaxy bulges. I start by establishing the basic geometrical description of the problem. Our understanding of the intrinsic shape of elliptical galaxies and galaxy discs is then presented in a historical context, in order to place the role that the 3D structure of bulges play in the broader picture of galaxy evolution.     Evidence for a deficit of young and old stars in the Milky Way inner in-plane disc

    We give independent proof of the deficit of stars in the in-plane central disc with respect to the predictions of a pure exponential density distribution. We use three different methods: 1) the inversion of the red clump giant distribution in near-infrared colour-magnitude diagrams to obtain the star density along the line of sight; 2) the determination of the density distribution of 1612 MHz sources by means of the distance determination of OH/IR sources from their kinematical information; 3) an analysis of near and mid-infrared star counts and comparison with models. All the tests give the same result: a defect of stars in the inner disc with respect to an exponential disc (either with constant scaleheight or extrapolated from the outer disc), but only in near plane regions. This deficit might be interpreted as a flare in the vertical distribution. The in-plane density is almost independent of R and not an exponential law. Further away from the plane, however, the density increases towards the centre due to the increase of the scaleheight. Tests also show that this result cannot be due to extinction. The deficit affects both the young and the old populations, so it is probably a rather stable feature of the disc, and might be due to the existence of an in-plane bar sweeping the near-plane stars.Comment: 17 pages with 27 postscript figures, to be published in A&

    EEG alterations during treatment with olanzapine

    The aim of this naturalistic observational study was to investigate EEG alterations in patients under olanzapine treatment with a special regard to olanzapine dose and plasma concentration. Twenty-two in-patients of a psychiatric university ward with the monodiagnosis of paranoid schizophrenia (ICD-10: F20.0), who received a monotherapy of olanzapine were included in this study. All patients had a normal alpha-EEG before drug therapy, and did not suffer from brain-organic dysfunctions, as verified by clinical examination and cMRI scans. EEG and olanzapine plasma levels were determined under steady-state conditions (between 18 and 22 days after begin of treatment). In 9 patients (40.9%), pathological EEG changes (one with spike-waves) consecutive to olanzapine treatment were observed. The dose of olanzapine was significantly higher in patients with changes of the EEG than in patients without changes (24.4 mg/day (SD: 8.1) vs. 12.7 mg/day (SD: 4.8); T = −4.3, df = 21, P < 0.001). In patients with EEG changes, the blood plasma concentration of olanzapine (45.6 μg/l (SD: 30.9) vs. 26.3 μg/l (SD: 21.6) tended to be also higher. The sensitivity of olanzapine dosage to predict EEG changes was 66.7%, the specificity 100% (Youden-index: 0.67). EEG abnormalities during olanzapine treatment are common. These are significantly dose dependent. Thus, EEG control recordings should be mandatory during olanzapine treatment with special emphasis on dosages exceeding 20 mg per day, although keeping in mind that EEGs have only a limited predictive power regarding future epileptic seizures

    Study of Spin and Decay-Plane Correlations of W Bosons in the e+e- -> W+W- Process at LEP

    Data collected at LEP at centre-of-mass energies \sqrt(s) = 189 - 209 GeV are used to study correlations of the spin of W bosons using e+e- -> W+W- -> lnqq~ events. Spin correlations are favoured by data, and found to agree with the Standard Model predictions. In addition, correlations between the W-boson decay planes are studied in e+e- -> W+W- -> lnqq~ and e+e- -> W+W- -> qq~qq~ events.     The fields of rapidly moving sources are studied within nonlinear electrodynamics by boosting the fields of sources at rest. As a consequence of the ultrarelativistic limit the delta-like electromagnetic shock waves are found. The character of the field within the shock depends on the theory of nonlinear electrodynamics considered.
