Burnout syndrome and work engagement in nursing staff: a systematic review and meta-analysis
BackgroundA difficult and demanding work environment, such as that often experienced in healthcare, can provoke fatigue, anxiety, distress, and discomfort. This study considers factors that may influence levels of burnout and work engagement among nurses and seeks to determine the relationship between these conditions.MethodA systematic scoping review was performed, in accordance with the PRISMA Extension for Scoping Reviews, based on data obtained from a search of the PubMed/MEDLINE and Scopus databases carried out in 2022 using the search equation: âwork engagement AND nurs* AND burnout.â This search identified nine quantitative primary studies suitable for inclusion in our analysis.ResultsWork overload, type of shift worked, and/or area of hospital service, among other elements, are all relevant to the development of burnout. This syndrome can be countered by social support and appropriate personal resources and values, which are all positively associated with work engagement. Our analysis revealed a significant correlation between work engagement and the different domains of burnout. The correlation effect size between burnout and work engagement was â0.46 (95% CI â0.58, â0.31), with p < 0.001.ConclusionWell-targeted interventions in the healthcare work environment can reduce burnout levels, strengthen work engagement, and enhance the quality of healthcare
Clinical utility of urinary gluten immunogenic peptides in the follow-up of patients with coeliac disease
[Background] Gluten-free diet (GFD) is the only treatment for patients with coeliac disease (CD) and its compliance should be monitored to avoid cumulative damage.[Aims] To analyse gluten exposures of coeliac patients on GFD for at least 24âmonths using different monitoring tools and its impact on duodenal histology at 12-month follow-up and evaluate the interval of determination of urinary gluten immunogenic peptides (u-GIP) for the monitoring of GFD adherence.[Methods] Ninety-four patients with CD on a GFD for at least 24âmonths were prospectively included. Symptoms, serology, CDAT questionnaire, and u-GIP (three samples/visit) were analysed at inclusion, 3, 6, and 12âmonths. Duodenal biopsy was performed at inclusion and 12âmonths.[Results] At inclusion, 25.8% presented duodenal mucosal damage; at 12âmonths, this percentage reduced by half. This histological improvement was indicated by a reduction in u-GIP but did not correlate with the remaining tools. The determination of u-GIP detected a higher number of transgressions than serology, regardless of histological evolution type. The presence of >4 u-GIP-positive samples out of 12 collected during 12âmonths predicted histological lesion with a specificity of 93%. Most patients (94%) with negative u-GIP in â„2 follow-up visits showed the absence of histological lesions (pâ<â0.05).[Conclusion] This study suggests that the frequency of recurrent gluten exposures, according to serial determination of u-GIP, could be related to the persistence of villous atrophy and that a more regular follow-up every 6 months, instead of annually, provides more useful data about the adequate adherence to GFD and mucosal healing.This study was funded in part by FundaciĂłn Progreso y Salud, ConsejerĂa de Salud, Junta de AndalucĂa (PI-0427-2017 and PI-0053-2018).Peer reviewe
Role of age and comorbidities in mortality of patients with infective endocarditis
[Purpose]: The aim of this study was to analyse the characteristics of patients with IE in three groups of age and to assess the ability of age and the Charlson Comorbidity Index (CCI) to predict mortality.
[Methods]: Prospective cohort study of all patients with IE included in the GAMES Spanish database between 2008 and 2015.Patients were stratified into three age groups:<65âŻyears,65 to 80âŻyears,andâŻâ„âŻ80âŻyears.The area under the receiver-operating characteristic (AUROC) curve was calculated to quantify the diagnostic accuracy of the CCI to predict mortality risk.
[Results]: A total of 3120 patients with IE (1327âŻ<âŻ65âŻyears;1291 65-80âŻyears;502âŻâ„âŻ80âŻyears) were enrolled.Fever and heart failure were the most common presentations of IE, with no differences among age groups.Patients â„80âŻyears who underwent surgery were significantly lower compared with other age groups (14.3%,65âŻyears; 20.5%,65-79âŻyears; 31.3%,â„80âŻyears). In-hospital mortality was lower in the <65-year group (20.3%,<65âŻyears;30.1%,65-79âŻyears;34.7%,â„80âŻyears;pâŻ<âŻ0.001) as well as 1-year mortality (3.2%, <65âŻyears; 5.5%, 65-80âŻyears;7.6%,â„80âŻyears; pâŻ=âŻ0.003).Independent predictors of mortality were ageâŻâ„âŻ80âŻyears (hazard ratio [HR]:2.78;95% confidence interval [CI]:2.32â3.34), CCIâŻâ„âŻ3 (HR:1.62; 95% CI:1.39â1.88),and non-performed surgery (HR:1.64;95% CI:11.16â1.58).When the three age groups were compared,the AUROC curve for CCI was significantly larger for patients aged <65âŻyears(pâŻ<âŻ0.001) for both in-hospital and 1-year mortality.
[Conclusion]: There were no differences in the clinical presentation of IE between the groups. AgeâŻâ„âŻ80âŻyears, high comorbidity (measured by CCI),and non-performance of surgery were independent predictors of mortality in patients with IE.CCI could help to identify those patients with IE and surgical indication who present a lower risk of in-hospital and 1-year mortality after surgery, especially in the <65-year group
Nanosilica to improve the flowability of fine limestone powders in thermochemical storage units
Fine powders are the cornerstone of new energy storage solutions to assist concentrated solar power plants. Though, their ability to behave like fluid can be seriously affected at high temperatures. This work investigates the use of nanosilica in fine limestone (calcium carbonate, CaCO) powders to mitigate the promotion of cohesion forces at high temperatures. Experiments were conducted over limestone powder samples with particle sizes around . The analysis was performed monitoring the tensile yield strength as the samples were subjected to different temperatures and consolidation stresses while varying the nanosilica content up until 0.82 wt%. Temperatures reached a maximum of 500 °C (close to the Tamman temperature in limestone), whereas consolidation stresses were increased up to 2 kPa. Results show that nanosilica coating is an efficient solution to inhibit the enhancement of powder cohesiveness at high temperatures and consolidations. Safety Evaluation of an Oak-Flavored Milk Powder Containing Ellagitannins upon Oral Administration in the Rat
9 pages, 4 figures, 3 tables.Ellagitannins are phenolic compounds that occur in a number of dietary sources such as walnuts, raspberries, pomegranate, strawberries, and oak-aged wines. Ellagitannins have been acknowledged to have antioxidant and cancer chemopreventive activities. However, ellagitannins have also been considered to be âantinutrientsâ and have been reported to provoke adverse effects in animals. In this context, risk assessment studies are demanded to ensure that chemicals present in food do not compromise food safety. Oak-flavored milk can be obtained by thermal treatment of milk in the presence of oak shavings. The oak-flavored milk can be used to impart wood-like flavor to different recipes. The present study evaluated, under a subchronic approach, the safety of an oak-flavored milk powder containing ellagitannins in rats upon repeated oral administration for 96 days. The effect of 30% oak-flavored milk powder in the diet was evaluated. This amount involved a margin of safety equivalent to the daily intake of 134 yogurts by a 70 kg person for 96 days. Growth rate, food utility index, 9 hematological, and 13 serobiochemical parameters as well as the metabolism and tissue distribution of ellagitannins and derived metabolites were studied. Overall, no significant differences were found in oak-milk-fed rats in any blood parameter analyzed. No effect was found on growth rate and food utility index. Urolithin A was identified in feces of rats fed oak-flavored milk and the glucuronide derivative in urine. No metabolites were found in the liver, kidney, or uterus. Histopathological analysis of liver and kidney corroborated the absence of toxicity. Therefore, these results suggest that the intake of the oak-flavored milk powder assayed in this study is safe and does not involve potential hazard to human health.M.A.-O. has a fellowship from the Spanish âConsejo Superior de Investigaciones

Negative predictive value of the repeated absence of gluten immunogenic peptides in the urine of treated celiac patients in predicting mucosal healing: New proposals for follow-up in celiac disease
[Background]: The treatment of celiac disease (CD) is a lifelong gluten-free diet (GFD). The current methods for monitoring GFD conformance, such as a dietary questionnaire or serology tests, may be inaccurate in detecting dietary transgressions, and duodenal biopsies are invasive, expensive, and not a routine monitoring technique.[Objectives]: Our aim was to determine the clinical usefulness of urine gluten immunogenic peptides (GIP) as a biomarker monitoring GFD adherence in celiac patients and to evaluate the concordance of the results with the degree of mucosal damage.[Methods]: A prospective observational study was conducted involving 22 de novo CD patients, 77 celiac patients consuming a GFD, and 13 nonceliac subjects. On 3 d of the week, urine samples were collected and the GIP concentrations were tested. Simultaneously, anti-tissue transglutaminase antibodies, questionnaire results, clinical manifestations, and histological findings were analyzed.[Results]: Approximately 24% (18 of 76) of the celiac patients consuming a GFD exhibited Marsh IIâIII mucosal damage. Among this population, 94% (17 of 18) had detectable urine GIP; however, between 60% and 80% were asymptomatic and exhibited negative serology and appropriate GFD adherence based on the questionnaire. In contrast, 97% (31 of 32) of the celiac patients without duodenal damage had no detectable GIP. These results demonstrated the high sensitivity (94%) and negative predictive value (97%) of GIP measurements in relation to duodenal biopsy findings. In the de novo CD-diagnosed cohort, 82% (18 of 22) of patients had measurable amounts of GIP in the urine.[Conclusions]: Determining GIP concentrations in several urine samples may be an especially convenient approach to assess recent gluten exposure in celiac patients and appears to accurately predict the absence of histological lesions. The maternal genetic make-up of the Iberian Peninsula between the Neolithic and the Early Bronze Age
Abstract Agriculture first reached the Iberian Peninsula around 5700 BCE. However, little is known about the genetic structure and changes of prehistoric populations in different geographic areas of Iberia. In our study, we focus on the maternal genetic makeup of the Neolithic (~ 5500â3000 BCE), Chalcolithic (~ 3000â2200 BCE) and Early Bronze Age (~ 2200â1500 BCE). We report ancient mitochondrial DNA results of 213 individuals (151 HVS-I sequences) from the northeast, central, southeast and southwest regions and thus on the largest archaeogenetic dataset from the Peninsula to date. Similar to other parts of Europe, we observe a discontinuity between hunter-gatherers and the first farmers of the Neolithic. During the subsequent periods, we detect regional continuity of Early Neolithic lineages across Iberia, however the genetic contribution of hunter-gatherers is generally higher than in other parts of Europe and varies regionally. In contrast to ancient DNA findings from Central Europe, we do not observe a major turnover in the mtDNA record of the Iberian Late Chalcolithic and Early Bronze Age, suggesting that the population history of the Iberian Peninsula is distinct in character
