Estudo comparativo das condições higiênicosanitárias em unidades de nutrição e dietética em Belém, Pará

The meal offered in a hospital environment is essential for the recovery of the patient’s health. Thus, inappropriate hygienic conditions expose the food to sources of contamination, and may be linked to foodborne diseases, becoming hospital pathways of infection. This study aimed to analyze the hygienic-sanitary conditions of two Units of Nutrition and Dietetics (UND) of the city of Belém, Brazil. A checklist was used as an evaluation instrument, adapted from Annex II of the Resolution n. 275/2002 of the Brazilian Ministry of Health, and in accordance with the compliance standards of the Resolution n. 216/2004. In this study, the private UND presented, in the global parameter, 62.06% of conformities and 37.94% of non-conformities, while the public UND showed 60.24% of conformities and 39.76% of non-conformities. According to RDC 275/02 both hospital kitchens were classified in group 2, since more than 51% of conforming items were observed. However, although both present average health risk, there is a need to repair some faults that may compromise directly or indirectly the recovery of the patients treated in these hospitals.A refeição ofertada em ambiente hospitalar é indispensável para a recuperação da saúde do paciente. Assim, condições higiênicas inapropriadas expõem o alimento a fontes de contaminação, e podem estar vinculadas a doenças transmitidas por alimentos, tornando-se vias de infecção hospitalar. Este estudo teve como objetivo analisar as condições higiênico-sanitárias de duas Unidades de Nutrição e Dietética (UND) do município de Belém (PA). Como instrumento de avaliação foi utilizado uma ficha de verificação (checklist) adaptada do anexo II da RDC nº 275 de 2002, do Ministério da Saúde, e de acordo com os padrões de conformidade da RDC nº 216 de 2004. No presente estudo, a UND particular apresentou, no parâmetro global, 62,06% de conformidades e 37,94% de não conformidades. Já a UND pública apresentou 60,24% de conformidades e 39,76% de não conformidades. Segundo a RDC 275/02 as duas cozinhas hospitalares estão classificadas no grupo 2, pois constataram mais de 51% de itens conformes. Apesar de ambas apresentarem médio risco sanitário, recomenda-se a necessidade de reparar algumas falhas que podem comprometer direto ou indiretamente a recuperação dos pacientes atendidos nesses hospitais

Aortic Thromboembolism in a Cat

Background: The aortic thromboembolism in cats is usually associated with cardiomyopathy, when a thrombus or a clot is formed in the heart, transported through the bloodstream and fixed at somewhere. According to Virchow’s triad, changes in the endocardial surface or in the blood flow/composition can result in thrombus formation. The most common clinical signs are: hind limb paralysis, lack of femoral pulse, cold and cyanotic extremities. The treatment should be performed as soon as possible and it is based on antiplatelet agents, anticoagulants, thrombolytics agents or surgical procedures. It is reported the case of a cat presenting aortic thromboembolism.Case: Macroscopically it was observed that the hind limb extremities were with a dark red color and with a bad odor on cut. There were in the subcutaneous tissue of the hind limbs a severe and diffuse accumulation of a reddish material, translucent, shiny, gelatinous (intense diffuse edema) and the skeletal muscles of the hind limbs had extensive pale and friable areas. Inside the medial saphenous vein lumen there was a greyish-white and soft material which adhered to the vessel wall. The lungs were not fully collapsed, it had a smooth and shiny surface and an extensive dark red area in the right middle lobe (moderate extensive bleeding). The spleen was with slightly bulging edges and on cut flowed moderate amount of blood (moderate congestive splenomegaly). In the kidneys there was numerous whitish and slightly depressed areas measuring approximately 0.5 cm in diameter, which was deep and wedge-shaped on cut (multifocal infarction). Microscopically it was observed mild diffuse congestion in the spleen; mild diffuse congestion and moderate focal bleeding in the lungs; intense multifocal neutrophilic infiltrate associated with coagulation necrosis in the cortical and medullary with multifocal cysts in moderate amounts in the kidneys; abdominal aorta artery with intense accumulation of fibrin, red blood cells and cellular debris organized and stuck in the tunica intima featuring a multifocal thrombosis area. The saphenous artery contained fibrin organization in the lumen, which was compatible with embolus.Discussion: The patient presented paralysis of hind limbs, absence of superficial and deep pain, cyanotic and cold hind limb extremities, loss of proprioception of the limbs and absence of detectable pulse in the vascular Doppler, which is characteristic of thromboembolic disease, but no specific sign of the underlying cause. Thromboembolic episodes are compatible with thrombus in the trifurcation of the abdominal aorta artery and the embolus in the saphenous artery. The focal lung bleeding can be the clinical sign of pulmonary thromboembolism. The unilateral renal infarction may have been induced after surgical excision of the thrombus, as the patient had no change in renal biochemical parameters prior to the surgical procedure. This is a complication, because small thrombus fragments can break off and cause thromboembolism elsewhere. The surgical procedure is generally non indicated because most domestic cats that have arterial thrombus have significant heart disease and more than half have congestive heart failure, making them poor candidates for surgery, which did not occur in the clinical case described. The repetitions of the events are common, and significant embolization of the kidney, intestines or other organs worsen the prognosis.Keywords: aortic thromboembolism, feline

Políticas públicas e vulnerabilidade social: Uma reflexão teórica a partir de experiência de estágio

This article provides a theoretical reflection on the relationship between public policies and the social vulnerability of children and adolescents, based on the supervised internship of students of Psychology at UNESP/Campus Assis. The work revealed the need to examine the implications that social vulnerability concepts and public policies have in this area of professional activity. The study indicated the importance of addressing the ways that social vulnerability arises, and suggested possible contributions that psychology could make in the social sphere. The work highlights the need for the future psychologist to deepen his/her knowledge of these issues in order to develop a critical professional attitude that breaks from the approach in which individuals and/or their families are considered responsible for the processes that maintain them at risk.Este artículo propone una reflexión teórica sobre la relación entre la política pública y la vulnerabilidad social a la que son sometidos los niños y adolescentes, a partir de una experiencia de la pasantía supervisada de estudiantes del Curso de Psicología de la UNESP / Assis.. Tal experiencia suscita la necesidad de analizar las implicaciones que los conceptos de vulnerabilidad social y políticas públicas asumen en este campo de atención profesional. El estudio indica la importancia de cuestionar la producción de la condición de vulnerabilidad social y señaló también las posibles contribuciones que la Psicología puede ofrecer en el campo social, destaca la necesidad del futuro psicólogo de atenerse y de profundizar los referidos conceptos como una manera de instrumentalizarse para el ejercicio profesional crítico destinado a romper la hegemonía de la perspectiva individualizante que pone sobre el sujeto y/o a su familia la responsabilidad por los procesos que los mantienen en situación de riesgo.O presente artigo propõe uma reflexão teórica acerca das relações entre as políticas públicas e a situação de vulnerabilidade social a que estão submetidas crianças e adolescentes, a partir de uma experiência de estágio supervisionado de alunos do Curso Psicologia da UNESP/ Campus de Assis. Trata-se de um estudo teórico proveniente de uma experiência de inserção dos estudantes de Psicologia em situações de campo, efetivada por meio das atividades do estágio de Núcleo Básico que priorizaram a observação. A experiência suscitou a necessidade de analisar as implicações que os conceitos de vulnerabilidade social e políticas públicas assumem nesse campo de atenção profissional. O estudo indicou a importância de se problematizar a produção da condição de vulnerabilidade social e assinalou também as possíveis contribuições que a Psicologia pode oferecer no campo social, com destaque para a necessidade de o futuro psicólogo se ater e aprofundar tais conceitos como forma de instrumentalizar-se para um exercício profissional crítico visando romper com a hegemonia do olhar individualizante que coloca sobre o sujeito e/ou sua família a responsabilidade pelos processos que os mantém em situação de risco.<br /
