    This report summarises the results of market and production surveys carried out in both the dry and wet seasons in Cameroon and Uganda during 1997/98 as part of the DFID fimded project "Opportunities and constraints in the subsistence production and marketing of indigenous vegetables in East and Central Africa (A0699)". The main objective of the study was to establish the socio-economic significance of indigenous vegetables compared to exotic ones. This project is a follow up to the strategy paper on indigenous vegetables and the subsequent workshop held in Lirnbe, Cameroon

    Domestic market potential for tree products from farms and rural communities: Experience from Cameroon

    This publication summarizes the results of research carried out by NRI in Cameroon during 1995-96 on the domestic market potential for tree products from farms and rural communities. The study arose from concern that agroforestry projects, which generally aim to reduce the pressure on natural forests by planting trees on farmland, were inadequately addressing the associated marketing needs. As a consequence, many of the projects were failing. The research reported here sought to characterize the markets for non-timber tree products (NTTPs) and highlight constraints to the development of markets for traditional and emerging NTTPs. The research in Cameroon was paralleled by a study in the Brazilian Amazon carried out by IFPRI; ODA provided funding for both studies. The work began with an inventory of products and subsequent identification of marketing chains. Four products were selected as case studies and were used to explore issues relating to valued added, domestication and the role of NTTPs as sources of income. The marketing of the four products was well-established and appeared to be relatively competitive, in spite of some concerns over lack of information at farmer level. There was growing demand for the products, apparently matched by increasing supply. The wholesalers, who source the products in the forest fringe communities and sell them on to retailers, were the most dynamic links in the marketing chains. The impetus to domesticate NTTPs seemed always to be farmer-driven, and occurred when a product important for subsistence and income needs was not readily available from the forest. However, the role of NTTPs within the farming system is important; they must fit into a complex strategy which includes seasonal smoothing of income, production and labour needs. Also, tree-planting is a long-term investment which is unlikely to be undertaken if land tenure is insecure. In conclusion, areas are highlighted for future research. These include: technical aspects of cultivation, processing and storage; identification of products which will be in greater demand as a result of urbanization; the role of NTTPs in the livelihood strategies of particularly vulnerable groups (including the landless) in the forest fringe communities

    Experience of Robotic Exoskeleton Use at Four Spinal Cord Injury Model Systems Centers

    Background and Purpose: Refinement of robotic exoskeletons for overground walking is progressing rapidly. We describe clinicians\u27 experiences, evaluations, and training strategies using robotic exoskeletons in spinal cord injury rehabilitation and wellness settings and describe clinicians\u27 perceptions of exoskeleton benefits and risks and developments that would enhance utility. Methods: We convened focus groups at 4 spinal cord injury model system centers. A court reporter took verbatim notes and provided a transcript. Research staff used a thematic coding approach to summarize discussions. Results: Thirty clinicians participated in focus groups. They reported using exoskeletons primarily in outpatient and wellness settings; 1 center used exoskeletons during inpatient rehabilitation. A typical episode of outpatient exoskeleton therapy comprises 20 to 30 sessions and at least 2 staff members are involved in each session. Treatment focuses on standing, stepping, and gait training; therapists measure progress with standardized assessments. Beyond improved gait, participants attributed physiological, psychological, and social benefits to exoskeleton use. Potential risks included falls, skin irritation, and disappointed expectations. Participants identified enhancements that would be of value including greater durability and adjustability, lighter weight, 1-hand controls, ability to navigate stairs and uneven surfaces, and ability to balance without upper extremity support. Discussion and Conclusions: Each spinal cord injury model system center had shared and distinct practices in terms of how it integrates robotic exoskeletons into physical therapy services. There is currently little evidence to guide integration of exoskeletons into rehabilitation therapy services and a pressing need to generate evidence to guide practice and to inform patients\u27 expectations as more devices enter the market.     Scalar Decay in Chaotic Mixing

    I review the local theory of mixing, which focuses on infinitesimal blobs of scalar being advected and stretched by a random velocity field. An advantage of this theory is that it provides elegant analytical results. A disadvantage is that it is highly idealised. Nevertheless, it provides insight into the mechanism of chaotic mixing and the effect of random fluctuations on the rate of decay of the concentration field of a passive scalar.Comment: 35 pages, 15 figures.     Western Australian public opinions of a minimum pricing policy for alcohol: study protocol

    Background: Excessive alcohol consumption has significant adverse economic, social, and health outcomes. Recent estimates suggest that the annual economic costs of alcohol in Australia are up to AUD $36 billion. Policies influencing price have been demonstrated to be very effective in reducing alcohol consumption and alcohol-related harms. Interest in minimum pricing has gained traction in recent years. However, there has been little research investigating the level of support for the public interest case of minimum pricing in Australia. Objective: This article describes protocol for a study exploring Western Australian (WA) public knowledge, understanding, and reaction to a proposed minimum price policy per standard drink. Methods: The study will employ a qualitative methodological design. Participants will be recruited from a wide variety of backgrounds, including ethnic minorities, blue and white collar workers, unemployed, students, and elderly/retired populations to participate in focus groups. Focus group participants will be asked about their knowledge of, and initial reactions to, the proposed policy and encouraged to discuss how such a proposal may affect their own alcohol use and alcohol consumption at the population level. Participants will also be asked to discuss potential avenues for increasing acceptability of the policy. The focus groups will adopt a semi-structured, open-ended approach guided by a question schedule. The schedule will be based on feedback from pilot samples, previous research, and a steering group comprising experts in alcohol policy and pricing. Results: The study is expected to take approximately 14 months to complete. Conclusions: The findings will be of considerable interest and relevance to government officials, policy makers, researchers, advocacy groups, alcohol retail and licensed establishments and organizations, city and town planners, police, and other stakeholder organizations

    Designing for designers: Towards the development of accessible ICT products and services using the VERITAS framework

    Among key design practices which contribute to the development of inclusive ICT products and services is user testing with people with disabilities. Traditionally, this involves partial or minimal user testing through the usage of standard heuristics, employing external assisting devices, and the direct feedback of impaired users. However, efficiency could be improved if designers could readily analyse the needs of their target audience. The VERITAS framework simulates and systematically analyses how users with various impairments interact with the use of ICT products and services. Findings show that the VERITAS framework is useful to designers, offering an intuitive approach to inclusive design.The work presented in this article forms part of VERITAS, which is funded by the European Commission's 7th Framework Programme (FP7) (grant agreement # 247765 FP7-ICT-2009.7.2)

    How was it for you? Experiences of participatory design in the UK health service

    Improving co-design methods implies that we need to understand those methods, paying attention to not only the effect of method choices on design outcomes, but also how methods affect the people involved in co-design. In this article, we explore participants' experiences from a year-long participatory health service design project to develop ‘Better Outpatient Services for Older People’. The project followed a defined method called experience-based design (EBD), which represented the state of the art in participatory service design within the UK National Health Service. A sample of participants in the project took part in semi-structured interviews reflecting on their involvement in and their feelings about the project. Our findings suggest that the EBD method that we employed was successful in establishing positive working relationships among the different groups of stakeholders (staff, patients, carers, advocates and design researchers), although conflicts remained throughout the project. Participants' experiences highlighted issues of wider relevance in such participatory design: cost versus benefit, sense of project momentum, locus of control, and assumptions about how change takes place in a complex environment. We propose tactics for dealing with these issues that inform the future development of techniques in user-centred healthcare design

    The shifting nature of women’s experiences and perceptions of ductal carcinoma in situ

    Aim: This paper is a report of a descriptive qualitative study of the evolution of women’s perceptions and experiences of ductal carcinoma in situ from the period near to diagnosis to one year later. Background: Ductal carcinoma in situ is a non-invasive breast condition where cancer cells are detected but confined to the ducts of the breast. With treatment, the condition has a positive prognosis but ironically patients undergo treatment similar to that for invasive breast cancer. There is a lack of longitudinal qualitative research studying women’s experiences of ductal carcinoma in situ, especially amongst newly diagnosed patients and how experiences change over time. Methods: Forty-five women took part in an initial interview following a diagnosis of ductal carcinoma in situ and twenty-seven took part in a follow-up interview 9-13 months later. Data were collected between January 2007 and October 2008. Transcripts were analysed using a hybrid approach to thematic analysis. Findings: Women’s early perceptions of ductal carcinoma in situ merged and sometimes conflicted with their lay beliefs of breast cancer. Perceptions and experiences of the condition shifted over time. These overriding aspects were evident within four themes identified across the interviews: 1) perceptions of DCIS versus breast cancer, 2) from paradox to acceptance, 3) personal impact, and 4) support and interactions with others. Conclusion: This study represents one of the few longitudinal qualitative studies with newly diagnosed patients, capturing women’s initial and shifting experiences and perceptions of the condition. The issues identified need to be recognised in clinical practice and supported appropriately

    Global meteorological influences on the record UK rainfall of winter 2013-14

    The UK experienced record average rainfall in winter 2013–14, leading to widespread and prolonged flooding. The immediate cause of this exceptional rainfall was a very strong and persistent cyclonic atmospheric circulation over the North East Atlantic Ocean. This was related to a very strong North Atlantic jet stream which resulted in numerous damaging wind storms. These exceptional meteorological conditions have led to renewed questions about whether anthropogenic climate change is noticeably influencing extreme weather. The regional weather pattern responsible for the extreme UK winter coincided with highly anomalous conditions across the globe. We assess the contributions from various possible remote forcing regions using sets of ocean–atmosphere model relaxation experiments, where winds and temperatures are constrained to be similar to those observed in winter 2013–14 within specified atmospheric domains. We find that influences from the tropics were likely to have played a significant role in the development of the unusual extra-tropical circulation, including a role for the tropical Atlantic sector. Additionally, a stronger and more stable stratospheric polar vortex, likely associated with a strong westerly phase of the stratospheric Quasi-Biennial Oscillation (QBO), appears to have contributed to the extreme conditions. While intrinsic climatic variability clearly has the largest effect on the generation of extremes, results from an analysis which segregates circulation-related and residual rainfall variability suggest that emerging climate change signals made a secondary contribution to extreme rainfall in winter 2013–14
