Les latrines dans l’architecture domestique et palatiale de Palerme, un marqueur social ?
Le peu d’informations disponibles sur l’architecture domestique de Palerme islamique ne permet pas de comprendre si le nombre très réduit d’attestations de latrines est dû à une véritable absence de celles-ci ou à la difficulté à en reconnaître les vestiges. Quelques indices semblent confirmer toutefois leur existence, montrant de plus qu’elles étaient associées à des fosses. Quant aux sources textuelles, elles suggèrent que le tissu urbain n’était pas homogène et que les résidences des élites, plus sûrement dotées d’installations sanitaires, étaient concentrées dans des quartiers que nous connaissons peu, car les données archéologiques publiées sont rares et leur interprétation reste difficile. Pour l’époque normande, en revanche, le palais de la Zisa conserve un remarquable ensemble de latrines qui doivent être rapprochées de celles connues dans l’architecture palatiale islamique. Mais le caractère exceptionnel de ce palais, construit dans la seconde moitié du xiie siècle, interdit de généraliser ces observations à l’architecture domestique palermitaine antérieure ou contemporaine. Le lien entre lieux d’aisance et résidences des élites est réaffirmé par la redécouverte de latrines tant au château de Ségeste que dans l’édifice inférieur du palais fortifié d’Entella – ville que les sources islamiques considèrent comme le siège du prétendu « émirat des Montagnes ». Des études récentes, enfin, ont mis en relation les latrines extrêmement raffinées des châteaux fédériciens (Castel Maniace à Syracuse, Castello di Augusta) avec certains exemples de l’architecture résidentielle islamique.The little information that we have on domestic architecture in Islamic Palermo makes it impossible to understand whether the reduced number of latrines is due to a problem of recognition of the archaeological datum or to the fact that there really were few such structures. Some evidences allow us to affirm that there were latrines and that they were connected to cesspits, while the sources indicate that Palermo was a city with a heterogeneous fabric in which the residences of the elites, presumably fitted out with sanitation, were concentrated in some districts that are little known because published archaeological data are few and difficult to interpret. By contrast, for the Norman age in the Zisa palace an exceptional system of latrines is preserved that is comparable to the latrines in Islamic palatial architecture and nevertheless, because of its exceptional character, cannot be considered indicative of the type of sanitation present in domestic architecture in Islamic and Norman Palermo. The connection between latrines and residences of elites is confirmed by the recovery of latrines both in Segesta Castle and in the lower building of the fortified palace at Entella, a city that Islamic sources identify as the center of the so-called “emirate of the Mountains”. Lastly, recent studies have connected to examples attested in Islamic residential architecture the very refined latrines of the castles of Frederick II (Castel Maniace in Syracuse, Augusta Castle)
Supportive treatment for cast nephropathy in patients with multiple myeloma; a pilot study
Introduction: Cast nephropathy is a prevalent cause of acute kidney injury (AKI) in patients with myeloma. Objectives: The aim of this study is to define the outcome of a standardized supportive therapy for cast nephropathy. Patients and Methods: Retrospective analysis of the outcome of cast nephropathy in a University hospital for a period of five years. Data analysed; serum creatinine, estimated glomerular filtration rate (eGFR; mL/min/1.73 m2 BSA) and need for dialysis. Standardized therapy with the aim of preventing/removing tubular casts; fluid administration and mannitol to increase urine flow, sodium bicarbonate to alkalize the urine and low dose steroid to reduce peritubular inflammation. Statistical analysis: Student's t-test or the Mann-Whitney test according to data distribution. A two-tailed P value <0.05 was considered statistically significant. Survival curve was drawn according to Kaplan and Meier. Results: Twenty-seven cases were reviewed. Upon admission, mean serum creatinine was 7.1±4.9 mg/dL and mean eGFR 6±4 mL/min/1.73 m2 BSA; 30% of patients had oligo-anuria. Diagnosis of cast nephropathy was presumptive in 23 patients, and renal biopsy proven in four. Hemodialysis was required by 10 (37%) patients, two of whom continued dialysis after discharge. At discharge, serum creatinine was 3.7±2.5 mg/dL and eGFR 20±13 mL/min/1.73 m2 BSA (P=0.002), and after a median of 3.4 months, the values were 2.9±2.1 mg/dL and 35±32 mL/min/1.73 m2 BSA, respectively. Patient survival was 60% after 24 months. Conclusion: Administration of fluid, mannitol, sodium bicarbonate and low-dose steroid may improve the outcome of cast nephropathy. Despite the fact that the study has many limitations, its findings could be the base for prospective controlled trials on cast nephropathy and could be useful in those countries where the expensive extracorporeal treatments are not available

New insights into early medieval Islamic cuisine : Organic residue analysis of pottery from rural and urban Sicily
Sicily, during the 9th-12th century AD, thrived politically, economically, and culturally under Islamic political rule and the capital of Palermo stood as a cultural and political centre in the Mediterranean Islamic world. However, to what extent the lifeways of the people that experienced these regimes were impacted during this time is not well understood, particularly those from lesser studied rural contexts. This paper presents the first organic residue analysis of 134 cooking pots and other domestic containers dating to the 9th -12th century in order to gain new insights into the culinary practices during this significant period. Ceramics from three sites in the urban capital of Palermo and from the rural town of Casale San Pietro were analysed and compared. The multi-faceted organic residue analysis identified a range of commodities including animal products, vegetables, beeswax, pine and fruit products in the ceramics, with a complex mixing of resources observed in many cases, across all four sites and ceramic forms. Alongside the identification of commodities and how they were combined, new light has been shed on the patterning of resource use between these sites. The identification of dairy products in calcite wares from the rural site of Casale San Pietro and the absence of dairy in ceramics from the urban centre of Palermo presents interesting questions regarding the role of rural sites in food consumption and production in Islamic Sicily. This is the first time organic residue analysis of ceramics has been used to explore foodways in a medieval multi-faith society and offers new pathways to the understanding of pottery use and resources that were prepared, consumed and combined, reflecting cuisine in different socio-economic environments within the pluralistic population of medieval Sicily
Towards the genetic basis of cerebral venous thrombosis-the BEAST Consortium: a study protocol.
INTRODUCTION:
Cerebral venous thrombosis (CVT) is a rare cerebrovascular condition accounting for <1% of all stroke cases and mainly affects young adults. Its genetic aetiology is not clearly elucidated.
METHODS AND ANALYSIS:
To better understand the genetic basis of CVT, we have established an international biobank of CVT cases, Biorepository to Establish the Aetiology of Sinovenous Thrombosis (BEAST) which aims to recruit highly phenotyped cases initially of European descent and later from other populations. To date we have recruited 745 CVT cases from 12 research centres. As an initial step, the consortium plans to undertake a genome-wide association analysis of CVT using the Illumina Infinium HumanCoreExome BeadChip to assess the association and impact of common and low-frequency genetic variants on CVT risk by using a case-control study design. Replication will be performed to confirm putative findings. Furthermore, we aim to identify interactions of genetic variants with several environmental and comorbidity factors which will likely contribute to improve the understanding of the biological mechanisms underlying this complex disease.
ETHICS AND DISSEMINATION:
BEAST meets all ethical standards set by local institutional review boards for each of the participating sites. The research outcomes will be published in international peer-reviewed open-access journals with high impact and visibility. The results will be presented at national and international meetings to highlight the contributions into improving the understanding of the mechanisms underlying this uncommon but important disease. This international DNA repository will become an important resource for investigators in the field of haematological and vascular disorders
Long-term organoid culture of a small intestinal neuroendocrine tumor
Gastroenteropancreatic neuroendocrine tumors (GEP-NETs) are rare and highly heterogeneous neoplasms whose incidence has markedly increased over the last decades. A grading system based on the tumor cells’ proliferation index predicts high-risk for G3 NETs. However, low-to-intermediate grade (G1/G2) NETs have an unpredictable clinical course that varies from indolent to highly malignant. Cultures of human cancer cells enable to perform functional perturbation analyses that are instrumental to enhance our understanding of cancer biology. To date, no tractable and reliable long-term culture of G1/G2 NET has been reported to permit disease modeling and pharmacological screens. Here, we report of the first long-term culture of a G2 metastatic small intestinal NET that preserves the main genetic drivers of the tumor and retains expression patterns of the endocrine cell lineage. Replicating the tissue, this long-term culture showed a low proliferation index, and yet it could be propagated continuously without dramatic changes in the karyotype. The model was readily available for pharmacological screens using targeted agents and as expected, showed low tumorigenic capacity in vivo. Overall, this is the first long-term culture of NETs to faithfully recapitulate many aspects of the original neuroendocrine tumor
Coma in adult cerebral venous thrombosis:The BEAST study
Background and purpose: Coma is an independent predictor of poor clinical outcomes in cerebral venous thrombosis (CVT). We aimed to describe the association of age, sex, and radiological characteristics of adult coma patients with CVT. Methods: We used data from the international, multicentre prospective observational BEAST (Biorepository to Establish the Aetiology of Sinovenous Thrombosis) study. Only positively associated variables with coma with <10% missing data in univariate analysis were considered for the multivariate logistic regression model. Results: Of the 596 adult patients with CVT (75.7% women), 53 (8.9%) patients suffered coma. Despite being a female-predominant disease, the prevalence of coma was higher among men than women (13.1% vs. 7.5%, p = 0.04). Transverse sinus thrombosis was least likely to be associated with coma (23.9% vs. 73.3%, p < 0.001). The prevalence of superior sagittal sinus thrombosis was higher among men than women in the coma sample (73.6% vs. 37.5%, p = 0.01). Men were significantly older than women, with a median (interquartile range) age of 51 (38.5–60) versus 40 (33–47) years in the coma (p = 0.04) and 44.5 (34–58) versus 37 (29–48) years in the non-coma sample (p < 0.001), respectively. Furthermore, an age-and superior sagittal sinus-adjusted multivariate logistic regression model found male sex (odds ratio = 1.8, 95% confidence interval [CI] = 1.0–3.4, p = 0.04 to be an independent predictor of coma in CVT, with an area under the receiver operating characteristic curve of 0.61 (95% CI = 0.52–0.68, p = 0.01). Conclusions: Although CVT is a female-predominant disease, men were older and nearly twice as likely to suffer from coma than women
