    WHAT IS THE LENGTH OF A SNAKE?

    The way that herpetologists have traditionally measuredlive snakes is by stretching them on a ruler andrecording the total length (TL). However, due to the thinconstitution of the snake, the large number of intervertebraljoints, and slim muscular mass of most snakes,it is easier to stretch a snake than it is to stretch anyother vertebrate. The result of this is that the length ofa snake recorded is infl uenced by how much the animalis stretched. Stretching it as much as possible is perhapsa precise way to measure the length of the specimenbut it might not correspond to the actual length ofa live animal. Furthermore, it may seriously injure a livesnake. Another method involves placing the snake in aclear plexiglass box and pressing it with a soft materialsuch as rubber foam against a clear surface. Measuringthe length of the snake may be done by outlining itsbody with a string (Fitch 1987; Frye 1991). However, thismethod is restricted to small animals that can be placedin a box, and in addition, no indications of accuracy of thetechnique are given. Measuring the snakes with a fl exibletape has also been reported (Blouin-Demers 2003)but when dealing with a large animals the way the tapeis positioned can produce great variance on the fi nal outcome.In this contribution we revise alternative ways tomeasuring a snake and propose a method that offers repeatableresults. We further analyze the precision of thismethod by using a sample of measurements taken fromwild populations of green anacondas (Eunectes murinus)with a large range of sizes

    Evaluating the Possibility of Translating Technological Advances in Non-Invasive Continuous Lactate Monitoring into Critical Care

    Lactate is widely measured in critically ill patients as a robust indicator of patient deterioration and response to treatment. Plasma concentrations represent a balance between lactate production and clearance. Analysis has typically been performed with the aim of detecting tissue hypoxia. However, there is a diverse range of processes unrelated to increased anaerobic metabolism that result in the accumulation of lactate, complicating clinical interpretation. Further, lactate levels can change rapidly over short spaces of time, and even subtle changes can reflect a profound change in the patient’s condition. Hence, there is a significant need for frequent lactate monitoring in critical care. Lactate monitoring is commonplace in sports performance monitoring, given the elevation of lactate during anaerobic exercise. The desire to continuously monitor lactate in athletes has led to the development of various technological approaches for non-invasive, continuous lactate measurements. This review aims firstly to reflect on the potential benefits of non-invasive continuous monitoring technology within the critical care setting. Secondly, we review the current devices used to measure lactate non-invasively outside of this setting and consider the challenges that must be overcome to allow for the translation of this technology into intensive care medicine. This review will be of interest to those developing continuous monitoring sensors, opening up a new field of research

    Molecularly imprinted polymer based electrochemical biosensors: Overcoming the challenges of detecting vital biomarkers and speeding up diagnosis

    Electrochemical biosensors for the detection of vital biomarkers is a well-established technology that utilises a transducer and recognition element in tandem to determine the presence of an analyte. There is growing interest in using Molecularly Imprinted Polymers (MIPs) as recognition elements in a wide range of sensing devices due to their economic viability and scalability. The inherent properties of polymer platforms, alongside the vast array of monomeric options, synthetic routes and incorporation strategies allow for the production of a multitude of sensitive and selective recognition elements that have significant advantages over classically utilised biological entities. MIPs exhibit superior chemical and thermal stability offering a wider variety of immobilization/incorporation strategies, virtually unlimited ambient shelf-life and a longer product lifetime, whilst the vast array of monomers available offer flexibility to their synthesis. Even though some sensor platforms have been reported for the detection of vital biomarkers, the use of MIPs has a number of challenges and drawbacks that need to be overcome in order to produce sensing platforms with the required sensitivity and specificity for clinical use. In this review, we will provide an overview of the reasoning behind using MIPs as recognition elements in electrochemical biosensors for vital biomarkers, discuss the problems synergizing MIPs and electrochemical read-out strategies and offer insights into the future perspectives of this promising and innovative technology

    Epidemiology of Neonatal Acute Respiratory Distress Syndrome:Prospective, Multicenter, International Cohort Study

    OBJECTIVES: Age-specific definitions for acute respiratory distress syndrome (ARDS) are available, including a specific definition for neonates (the "Montreux definition"). The epidemiology of neonatal ARDS is unknown. The objective of this study was to describe the epidemiology, clinical course, treatment, and outcomes of neonatal ARDS. DESIGN: Prospective, international, observational, cohort study. SETTING: Fifteen academic neonatal ICUs. PATIENTS: Consecutive sample of neonates of any gestational age admitted to participating sites who met the neonatal ARDS Montreux definition criteria. MEASUREMENTS AND MAIN RESULTS: Neonatal ARDS was classified as direct or indirect, infectious or noninfectious, and perinatal (≤ 72 hr after birth) or late in onset. Primary outcomes were: 1) survival at 30 days from diagnosis, 2) inhospital survival, and 3) extracorporeal membrane oxygenation (ECMO)-free survival at 30 days from diagnosis. Secondary outcomes included respiratory complications and common neonatal extrapulmonary morbidities. A total of 239 neonates met criteria for the diagnosis of neonatal ARDS. The median prevalence was 1.5% of neonatal ICU admissions with male/female ratio of 1.5. Respiratory treatments were similar across gestational ages. Direct neonatal ARDS (51.5% of neonates) was more common in term neonates and the perinatal period. Indirect neonatal ARDS was often triggered by an infection and was more common in preterm neonates. Thirty-day, inhospital, and 30-day ECMO-free survival were 83.3%, 76.2%, and 79.5%, respectively. Direct neonatal ARDS was associated with better survival outcomes than indirect neonatal ARDS. Direct and noninfectious neonatal ARDS were associated with the poorest respiratory outcomes at 36 and 40 weeks' postmenstrual age. Gestational age was not associated with any primary outcome on multivariate analyses. CONCLUSIONS: Prevalence and survival of neonatal ARDS are similar to those of pediatric ARDS. The neonatal ARDS subtypes used in the current definition may be associated with distinct clinical outcomes and a different distribution for term and preterm neonates

    Friedrich Hayek and his visits to Chile

    F. A. Hayek took two trips to Chile, the first in 1977, the second in 1981. The visits were controversial. On the first trip he met with General Augusto Pinochet, who had led a coup that overthrew Salvador Allende in 1973. During his 1981 visit, Hayek gave interviews that were published in the Chilean newspaper El Mercurio and in which he discussed authoritarian regimes and the problem of unlimited democracy. After each trip, he complained that the western press had painted an unfair picture of the economic situation under the Pinochet regime. Drawing on archival material, interviews, and past research, we provide a full account of this controversial episode in Hayek’s life

    Abstract Admission procalcitonin measurements and microbiology results were available for 1040 hospitalized adults with coronavirus disease 2019 (from 48 902 included in the International Severe Acute Respiratory and Emerging Infections Consortium World Health Organization Clinical Characterisation Protocol UK study). Although procalcitonin was higher in bacterial coinfection, this was neither clinically significant (median [IQR], 0.33 [0.11–1.70] ng/mL vs 0.24 [0.10–0.90] ng/mL) nor diagnostically useful (area under the receiver operating characteristic curve, 0.56 [95% confidence interval, .51–.60]).</jats:p
