The role of ARNT in liver and myeloid cell function
Aryl hydrocarbon receptor nuclear translocator (ARNT) is a transcription factor which acts as a general partner for members of the bHLH/PAS family of transcription factors. To investigate the effect of long term ARNT deletion in hepatocyte and myeloid cells, we created 2 lines of mice with ARNT deletion in these cells. Mice lacking hepatocyte ARNT had impaired glucose tolerance, increased gluconeogenesis, decreased ATP and increased post-prandial serum triglycerides. However, in contrast to type 2 diabetes (T2D) hepatic ARNT deletion resulted in decreased liver steatosis. Importantly, these changes became non-significant after high fat diet (HFD). Decreased ARNT in myeloid cells led to decreased cytokine expression, decreased phagocytosis, decreased bactericidal activity, impaired response to infection, and impaired wound healing. Again, the phenotype of impaired wound healing equilibrated in a diabetic milieu. In addition mice lacking ARNT in myeloid cells displayed impaired glucose tolerance on HFD and paradoxically increased liver inflammation. In human monocytes ARNT mRNA correlated negatively with serum cytokine levels of IL-6, IL-8, MCP-1 and TNF-α. This data demonstrates that ARNT has important roles in hepatocyte and myeloid cell function and suggests that modulation of this transcription factor could be used in future therapy for diabetes and disorders of immune function
Antibody response to Haemophilus influenzae type-b conjugate vaccine in children and young adults with congenital asplenia or after undergoing splenectomy
Absence of the spleen constitutes a risk of infection caused by encapsulated bacteria. The aim of our study was to determine the immune response to Haemophilus influenzae type-b (Hib) conjugate vaccine (HibCV) in asplenic individuals, considering the cause of asplenia, the age when splenectomy was carried out, and previous Hib vaccinations. Twenty asplenic patients, aged five to 25 years, were immunized with a single dose of HibCV. The specific antibody concentrations against HibCV were measured by enzyme-linked immunosorbent assay. Before vaccinations, the geometric mean antibody concentration (GMC) had an average value of 3.21 μg/ml and was comparable for all of the patients, regardless of the causes of asplenia. After vaccinations, the GMC was significantly higher, with an average of 6.78 μg/ml. Further, 4.5 years after vaccinations, the GMC was comparable to that of previously unvaccinated children. Moreover, 17/20 patients had GMC ≥ 1.0 μg/ml, which included all of the children with congenital asplenia, children splenectomized before the age of six years, and only 57% of children splenectomized after that age. HibCV gives asplenic patients long-term protection. Hence, HibCV should be administered regardless of previous vaccinations and time from splenectomy, even if antibody evaluation is not available
Higher Doses of Subcutaneous IgG Reduce Resource Utilization in Patients with Primary Immunodeficiency
The recommended dose of IgG in primary immunodeficiency (PID) has been increasing since its first use. This study aimed to determine if higher subcutaneous IgG doses resulted in improved patient outcomes by comparing results from two parallel clinical studies with similar design. One patient cohort received subcutaneous IgG doses that were 1.5 times higher than their previous intravenous doses (mean 213 mg/kg/week), whereas the other cohort received doses identical to previous subcutaneous or intravenous doses (mean 120 mg/kg/week). While neither cohort had any serious infections, the cohort maintained on higher mean IgG dose had significantly lower rates of non-serious infections (2.76 vs. 5.18 episodes/year, P < 0.0001), hospitalization (0.20 vs. 3.48 days/year, P < 0.0001), antibiotic use (48.50 vs. 72.75 days/year, P < 0.001), and missed work/school activity (2.10 vs. 8.00 days/year, P < 0.001). The higher-dose cohort had lower health care utilization and improved indices of well being compared to the cohort treated with traditional IgG doses
Wiskott-Aldrich Syndrome protein deficiency perturbs the homeostasis of B-cell compartment in humans
Wiskott-Aldrich Syndrome protein (WASp) regulates the cytoskeleton in hematopoietic cells and mutations in its gene cause the Wiskott-Aldrich Syndrome (WAS), a primary immunodeficiency with microthrombocytopenia, eczema and a higher susceptibility to develop tumors. Autoimmune manifestations, frequently observed in WAS patients, are associated with an increased risk of mortality and still represent an unsolved aspect of the disease. B cells play a crucial role both in immune competence and self-tolerance and defects in their development and function result in immunodeficiency and/or autoimmunity. We performed a phenotypical and molecular analysis of central and peripheral B-cell compartments in WAS pediatric patients. We found a decreased proportion of immature B cells in the bone marrow correlating with an increased presence of transitional B cells in the periphery. These results could be explained by the defective migratory response of WAS B cells to SDF-1α, essential for the retention of immature B cells in the BM. In the periphery, we observed an unusual expansion of CD21low B-cell population and increased plasma BAFF levels that may contribute to the high susceptibility to develop autoimmune manifestations in WAS patients. WAS memory B cells were characterized by a reduced in vivo proliferation, decreased somatic hypermutation and preferential usage of IGHV4-34, an immunoglobulin gene commonly found in autoreactive B cells. Patients with primary immunodeficiencies are a reservoir of poliovirus and a risk to polio eradication
ABSTARCT: Immunodeficiency-associated vaccine-derived polioviruses (iVDPVs) have been isolated from primary immunodeficiency (PID) patients exposed to oral poliovirus vaccine (OPV). Patients may excrete poliovirus strains for months or years; the excreted viruses are frequently highly divergent from the parental OPV and have been shown to be as neurovirulent as wild virus. Thus, these patients represent a potential reservoir for transmission of neurovirulent polioviruses in the post-eradication era. In support of WHO recommendations to better estimate the prevalence of poliovirus excreters among PIDs and characterize genetic evolution of these strains, 635 patients including 570 with primary antibody deficiencies and 65 combined immunodeficiencies were studied from 13 OPV-using countries. Two stool samples were collected over 4 days, tested for enterovirus, and the poliovirus positive samples were sequenced. Thirteen patients (2%) excreted polioviruses, most for less than 2 months following identification of infection. Five (0.8%) were classified as iVDPVs (only in combined immunodeficiencies and mostly poliovirus serotype 2). Non-polio enteroviruses were detected in 30 patients (4.7%). Patients with combined immunodeficiencies had increased risk of delayed poliovirus clearance compared to primary antibody deficiencies. Usually, iVDPV was detected in subjects with combined immunodeficiencies in a short period of time after OPV exposure, most for less than 6 months. Surveillance for poliovirus excretion among PID patients should be reinforced until polio eradication is certified and the use of OPV is stopped. Survival rates among PID patients are improving in lower and middle income countries, and iVDPV excreters are identified more frequently. Antivirals or enhanced immunotherapies presently in development represent the only potential means to manage the treatment of prolonged excreters and the risk they present to the polio endgame. Stress and Strain Concentrations in Steel Angle Tension Members Connected by One Leg
The paper presents the numerical simulations results of net section failure in tensioned angles. Angles are made of structural steel with nominal grade S235. Simulation takes into account ductile fracture initiation, by application of GursonTvergaard-Needleman (GTN) material model. Parametrical analysis of ultimate resistance was carried out. The finite elements analyses were conducted by ABAQUS computer program. Shear lag effect in considered joint was observed, as a non uniform tensile stress distribution in angles in the vicinity of a connection. Stress concentration areas and stress concentration factors have been predicted, both in elastic and ultimate behaviour of joint. Especially change of non-uniform stress distribution in net cross-section was observed, during increase of loading, until the ultimate resistance was reached
