    Fencing influence on overgrazed ranges of the Sierra del Segura is studied, over two fenced plots, during three years. An important increment of cover in herbaceous plants of elevated pastoral value, mainly perennial legumes, is observated. Uncovered soil ratio decreased by effect of fencing. Woody species of elevated pastoral value increment their cover. An important increment in floral richness and diversity is produced during the first year after fencing. Appearance species ratio decreased after the first year.Se estudia la influencia del acotamiento al pastoreo en pastizales esquilmados de la Sierra del Segura sobre dos parcelas acotadas y muestreadas tres años post-acotamiento. Se observa un importante incremento de cobertura en especies herbáceas de interés pastoral, principalmente de leguminosas vivaces, así como de la cobertura de suelo recubierto de vegetación. Las leñosas incrementan ligeramente su cobertura apareciendo diversas especies de interés pastoral. Se produce un importante incremento en la riqueza y diversidad florísticas durante el primer año post-acotamiento, ralentizándose posteriormente la tasa de aparición de especies

    Challenges in Diabetic Macular Edema Management: An Expert Consensus Report

    Purpose: This paper aimed to present daily-practice recommendations for the management of diabetic macular edema (DME) patients based on available scientific evidence and the clinical experience of the consensus panel. Methods: A group of Spanish retina experts agreed to discuss different aspects related with the clinical management of DME patients. Results: Panel was mainly focused on therapeutic objectives in DME management; defini-tion terms; and role of biomarkers as prognostic and predictive factors to intravitreal treatment response. The panel recommends to start DME treatment as soon as possible in those eyes with a visual acuity less than 20/25 (always according to the retina unit capacity). Naive patient was defined, in a strict manner, as a patient who, up to that moment, had never received any treatment. A refractory DME patient may be defined as the one who did not achieve a complete resolution of the disease, regardless of the treatment administered. Different optical coherence tomography biomarkers, such as disorganization of the retinal inner layers, hyperreflective dots, and cysts, have been identified as prognostic factors. Conclusion: This document has sought to lay down a set of recommendations and to identify key issues that may be useful for the daily management of DME patients

    Very low-mass members of the Lupus 3 cloud

    We report on a multi-band survey for very low-mass stars and brown dwarfs in the Lupus 3 cloud with the Wide Field Imager (WFI) at the ESO/MPG 2.2m telescope on La Silla Observatory (Chile). Our multiband optical photometry is combined with available 2MASS JHK photometry to identify 19 new young stars and 3 brown dwarf candidates as probable members of this star forming region. Our objects are mostly clustered around the cloud core. Stars and brown dwarfs have similar levels of H-alpha emission, probably a signature of accretion. One object, a brown dwarf candidate, exhibits a near-infrared excess, which may indicate the presence of a disk, but its H-alpha emission cannot be confirmed due to its faintness in the optical passbands. We also find two visual pairs of probable Lupus 3 members that may be wide binaries

    The very low-mass population of the Corona Australis and Chamaeleon II star forming regions

    We present the results of a deep optical survey in the Corona Australis and Chamaeleon II star forming regions. Our optical photometry is combined with available near- and mid-infrared photometry to identify very low-mass candidate members in these dark clouds. In our Chamaeleon II field, only one object exhibits clear H-alpha emission, but the discrepancy between its optical and near-infrared colours suggests that it might be a foreground star. We also identify two objects without H-alpha emission that could be planetary mass members of Chamaeleon II. In Corona Australis, we find ten stars and three brown dwarf candidates in the Coronet cluster. Five of our new members are identified with ISOCAM sources. Only two of them have a mid-infrared excess, indicating the presence of an accretion disk. On the other hand, one brown dwarf candidate has a faint close companion, seen only in our deepest I-band image. For many of the candidates in both clouds, membership could not be inferred from their H-alpha emission or near-infrared colours; these objects need spectroscopic confirmation of their status

    Systemic Effects of Repeated Intraocular Dexamethasone Intravitreal Implant in Diabetic Patients: A Retrospective Study.

    The objective of this study is to evaluate the influence of repeated intraocular dexamethasone implant (Ozurdex) injections on metabolic control in type 2 diabetic patients. Retrospective study of 165 type 2 diabetic patients starting Ozurdex treatment who received no less than three consecutive injections. Glycated hemoglobin (HbA1c), serum creatinine, total cholesterol, high-density lipoprotein (HDL) cholesterol, low-density lipoprotein (LDL) cholesterol, and triglycerides (TGs) were evaluated during 15 months of follow-up after Ozurdex treatment onset. Fifty-seven patients met inclusion criteria. Mean baseline values for HbA1c, creatinine, total cholesterol, HDL cholesterol, and TGs before treatment (7.1%, 1.3, 176.7, 51.1, and 125.6 mg/dl, respectively) were similar to mean values after Ozurdex onset (Wilcoxon test p values were 0.68, 0.41, 0.06, 0.87, and 0.33, respectively) and remained stable during the follow-up period. Mean LDL cholesterol levels increased slightly after Ozurdex treatment onset (90.1 vs 88.2 mg/dl, p = 0.04) but after 15 months of follow-up they had returned to baseline values. Transient increase in LDL cholesterol was remarkable in the group of 24 bilaterally treated patients (96.8 vs 88.4 mg/dl, p = 0.03). A third of these patients increased their baseline LDL values by more than 20%. Even with continuous injections of Ozurdex, LDL cholesterol levels also declined back to baseline by month 15. Ozurdex injections had no influence on HbA1c or renal function. Lipid profile changes were mild and transient. However, a significant temporary increase has been found in LDL cholesterol levels in patients receiving simultaneous bilateral injections. Lipid levels should be monitored in patients starting with bilateral Ozurdex injections especially in those with recent history of acute myocardial infarction

    Evaluación de signos tomográficos no exudativos en casos de degeneración macular asociada a la edad de tipo exudativo

    Producción CientíficaABSTRACT. OBJECTIVE: To analyze the prevalence of non-exudative tomographic signs (onion sign, pseudoswelling, external retinal tubulation, pseudocysts, subretinal clefts and macular atrophy) in patients with neovascular age-related macular degeneration. MATERIAL AND METHODS: A total of 174 eyes of patients with neovascular age-related macular degeneration who had not received previous treatment were included in the study. Visual acuity, neovascularization activity, and the appearance or not of the different signs under study were assessed at times 0 (initial visit), 4 months, one year, year and a half, and at 2 and 3 years of follow-up. The following were also evaluated: age, sex, affected eye and type of neovascularization (1, 2, 3, polypoid or mixed). The analysis were performed using the statistical software R (version 3.3.2) and the glmmADMB package (version 0.8.3.3). RESULTS: The presence of pseudocysts and external retinal tubulation increases throughout the follow-up. The onion sign begins with an ascending frequency up to 12 months, then decreases at 18 months and increases again at 24 months. Regarding pseudowelling, it maintains an increase until 18 months to finally decrease. Subretinal clefts is the rarest sign, presenting in 1.1% on the first visit. Finally, macular atrophy, present in 12.6% of the eyes initially, is found in 25% after 2 years. CONCLUSION: Pseudocysts, external retinal tubulation and macular atrophy were the most prevalent signs, while subretinal clefts were the most infrequent

    Using Data-mining Techniques for the prediction of the severity of road crashes in Cartagena, Colombia

    Objective: Analyze the road crashes in Cartagena (Colombia) and the factors associated with the collision and severity. The aim is to establish a set of rules for defining countermeasures to improve road safety. Methods: Data mining and machine learning techniques were used in 7894 traffic accidents from 2016 to 2017. The severity was determined between low (84%) and high (16%). Five classification algorithms to predict the accident severity were applied with WEKA Software (Waikato Environment for Knowledge Analysis). Including Decision Tree (DT-J48), Rule Induction (PART), Support Vector Machines (SVMs), Naïve Bayes (NB), and Multilayer Perceptron (MLP). The effectiveness of each algorithm was implemented using cross-validation with 10-fold. Decision rules were defined from the results of the different methods. Results: The methods applied are consistent and similar in the overall results of precision, accuracy, recall, and area under the ROC curve. Conclusions: 12 decision rules were defined based on the methods applied. The rules defined show motorcyclists, cyclists, including pedestrians, as the most vulnerable road users. Men and women motorcyclists between 20–39 years are prone in accidents with high severity. When a motorcycle or cyclist is not involved in the accident, the probable severity is low

    A young hierarchical triple system harbouring a candidate debris disc

    We report the detection of a wide young hierarchical triple system where the primary has a candidate debris disc. The primary, TYC 5241-986-1 A, is a known Tycho star which we classify as a late-K star with emission in the X-ray, near- and far-ultraviolet (UV) and Hα suggestive of youth. Its proper motion, photometric distance (65–105 pc) and radial velocity lead us to associate the system with the broadly defined Local Association of young stars but not specifically with any young moving group. The presence of weak lithium absorption and X-ray and calcium H and K emission support an age in the 20 to ∼125 Myr range. The secondary is a pair of M4.5 ± 0.5 dwarfs with near- and far-UV and Hα emission separated by approximately 1 arcsec (∼65–105 au projected separation) which lie of 145 arcsec (9200–15200 au) from the primary. The primary has a Wide-field Infrared Survey Explorer (WISE) 22 μm excess and follow-up Herschel observations also detect an excess at 70 μm. The excess emissions are indicative of a 100–175 K debris disc. We also explore the possibility that this excess could be due to a coincident background galaxy and conclude that this is unlikely. Debris discs are extremely rare around stars older than 15 Myr, hence if the excess is caused by a disc this is an extremely novel system

    Cut-offs and response criteria for the Hospital Universitario la Princesa Index (HUPI) and their comparison to widely-used indices of disease activity in rheumatoid arthritis

    Objective To estimate cut-off points and to establish response criteria for the Hospital Universitario La Princesa Index (HUPI) in patients with chronic polyarthritis. Methods Two cohorts, one of early arthritis (Princesa Early Arthritis Register Longitudinal PEARL] study) and other of long-term rheumatoid arthritis (Estudio de la Morbilidad y Expresión Clínica de la Artritis Reumatoide EMECAR]) including altogether 1200 patients were used to determine cut-off values for remission, and for low, moderate and high activity through receiver operating curve (ROC) analysis. The areas under ROC (AUC) were compared to those of validated indexes (SDAI, CDAI, DAS28). ROC analysis was also applied to establish minimal and relevant clinical improvement for HUPI. Results The best cut-off points for HUPI are 2, 5 and 9, classifying RA activity as remission if =2, low disease activity if >2 and =5), moderate if >5 and <9 and high if =9. HUPI''s AUC to discriminate between low-moderate activity was 0.909 and between moderate-high activity 0.887. DAS28''s AUCs were 0.887 and 0.846, respectively; both indices had higher accuracy than SDAI (AUCs: 0.832 and 0.756) and CDAI (AUCs: 0.789 and 0.728). HUPI discriminates remission better than DAS28-ESR in early arthritis, but similarly to SDAI. The HUPI cut-off for minimal clinical improvement was established at 2 and for relevant clinical improvement at 4. Response criteria were established based on these cut-off values. Conclusions The cut-offs proposed for HUPI perform adequately in patients with either early or long term arthritis
