Treatment of Vascular Parkinsonism: A Systematic Review

    Get PDF
Background and aims: Although the distinction between vascular parkinsonism (VP) and idiopathic Parkinson’s disease (IPD) is widely described, it is not uncommon to find parkinsonisms with overlapping clinical and neuroimaging features even in response to levodopa treatment. In addition, several treatments have been described as possible adjuvants in VP. This study aims to update and analyze the different treatments and their efficacy in VP. Methods: A literature search was performed in PubMed, Scopus and Web of Science for studies published in the last 15 years until April 2022. A systematic review was performed. No meta-analysis was performed as no new studies on response to levodopa in VP were found since the last systematic review and meta-analysis in 2017, and insufficient studies on other treatments were located to conduct it in another treatment subgroup. Results: Databases and other sources yielded 59 publications after eliminating duplicates, and a total of 12 original studies were finally included in the systematic review. The treatments evaluated included levodopa, vitamin D, repetitive transcranial magnetic stimulation (rTMS) and intracerebral transcatheter laser photobiomodulation therapy (PBMT). The response to levodopa was lower in patients with VP with respect to IPD. Despite this, there has been described a subgroup of patients with good response, it being possible to identify them by means of neuroimaging techniques and the olfactory identification test. Other therapies showed encouraging results in studies with some risk of bias. Conclusions: The response of VP to different therapeutic strategies is modest. However, there is evidence that a subgroup of patients can be identified as more responsive to L-dopa based on clinical and neuroimaging criteria. This subgroup should be treated with L-dopa at appropriate doses. New therapies such as vitamin D, rTMS and PBMT warrant further studies to demonstrate their efficacy

Paisaje, territorio y sostenibilidad: algunas reflexiones aplicables al ámbito rural

    Get PDF

    Get PDF
The future value of organizations depends on the economic performance linked to their level of response to social needs and interestgroups, as well as the type of relationship established with the environment in which it develops and influences. Thus, achieving high levels ofcorporate social responsibility (CSR) becomes a necessity for business success. This type of research is empirical. The research was aimed at identifying and validating the variables that condition and are related to CSR. The study considered in its methodology the design of an instrument based on referential research, which was applied to 778 workers. In the analysis, a structural equation model was proposed to identify associations and relationships for CSR. The evaluation of this model of structural equations and its validation through the confirmatory factorial analysis allowed to verify, as main conclusions, that corporate social responsibility presents a development conditioned by a multidimensional approach, where internal variables of the organization are manifested that show a significant level of correlation with the CSR that the organization’scustomers recognize, such as leadership, orientation strategy, organizational design, communication, the development of labor competences, organizational values and job satisfaction, keeping a close relationship with previous research.

Applicability of Milne-Eddington inversions to high spatial resolution observations of the quiet Sun

The physical conditions of the solar photosphere change on very small spatial scales both horizontally and vertically. Such a complexity may pose a serious obstacle to the accurate determination of solar magnetic fields. We examine the applicability of Milne-Eddington (ME) inversions to high spatial resolution observations of the quiet Sun. Our aim is to understand the connection between the ME inferences and the actual stratifications of the atmospheric parameters. We use magnetoconvection simulations of the solar surface to synthesize asymmetric Stokes profiles such as those observed in the quiet Sun. We then invert the profiles with the ME approximation. We perform an empirical analysis of the heights of formation of ME measurements and analyze the uncertainties brought about by the ME approximation. We also investigate the quality of the fits and their relationship with the model stratifications. The atmospheric parameters derived from ME inversions of high-spatial resolution profiles are reasonably accurate and can be used for statistical analyses of solar magnetic fields, even if the fit is not always good. We also show that the ME inferences cannot be assigned to a specific atmospheric layer: different parameters sample different ranges of optical depths, and even the same parameter may trace different layers depending on the physical conditions of the atmosphere. Despite this variability, ME inversions tend to probe deeper layers in granules as compared with intergranular lanes.Comment: Accepted for publication in Astronomy and Astrophysic

Convective motions and net circular polarization in sunspot penumbrae

We have employed a penumbral model, that includes the Evershed flow and convective motions inside penumbral filaments, to reproduce the azimuthal variation of the net circular polarization (NCP) in sunspot penumbrae at different heliocentric angles for two different spectral lines. The theoretical net circular polarization fits the observations as satisfactorily as penumbral models based on flux-tubes. The reason for this is that the effect of convective motions on the NCP is very small compared to the effect of the Evershed flow. In addition, the NCP generated by convective upflows cancels out the NCP generated by the downflows. We have also found that, in order to fit the observed NCP, the strength of the magnetic field inside penumbral filaments must be very close to 1000 G. In particular, field-free or weak-field filaments fail to reproduce both the correct sign of the net circular polarization, as well as its dependence on the azimuthal and heliocentric angles.Comment: Accepted for publication in the Astrophysical Journal. 10 pages, 7 figures (3 in color). Antibody-Capped Mesoporous Nanoscopic Materials:Design of a Probe for the Selective Chromo-FluorogenicDetection of Finasteride

[EN] The synthesis of capped mesoporous silica nanoparticles (MSN) conjugated with an antibody (AB) as a gatekeeper has been carried out in order to obtain a delivery system able to release an entrapped cargo (dye) in the presence of a target molecule (antigen) to which the conjugated antibody binds selectively. In particular, MSN loaded with rhodamine B and functionalized on the external surface with a suitable derivative of N-(t-butyl)- 3-oxo-(5a,17b)-4-aza-androst-1-ene-17-carboxamide (finasteride) have been prepared (S1). The addition of polyclonal antibodies against finasteride induced capping of the pores due to the interaction with the anchored hapten-like finasteride derivative to give a MSN¿hapten¿AB nanoparticle S1-AB. It was found that the addition of capped material S1-AB to water solutions containing finasteride resulted in displacement of the antibody, pore uncapping and entrapped-dye release. The response of the gated material is highly selective, and only finasteride, among other steroids, was able to induce a significant uncapping process. Compared with finasteride, the finasteride metabolite was able to release 17% of the dye, whereas the exogen steroids testosterone, metenolone and 16-b-hydroxystanozolol only induced very little release of rhodamine B (lower than 10%) from aqueous suspensions containing sensing solid S1-AB. A detection limit as low as 20 ppb was found for the fluorimetric detection of finasteride. In order to evaluate a possible application of the material for label-free detection of finasteride, the capped material was isolated and stored to give final sensing solid S1-AB-i. It was found to display a similar behavior towards finasteride as to that shown by freshly prepared S1-AB; even after a period of two months, no significant loss of selectivity or sensitivity was noted. Moreover, to study the application for the detection of finasteride in biological samples, this ¿aged¿ material, S1-AB-i, was tested using commercially available blank urine as matrix. Samples containing 70 and 90% blank urine were spiked with a defined amount of finasteride, and the concentration was determined using capped S1-AB-i. Recovery ranges from 94% to 118% were reached.Financial support from the Spanish Government (project MAT2009-14564-C04-01) and the Generalitat Valenciana (Spain) (projects PROMETEO/2009/016 and PROMETEO/2010/008) is gratefully acknowledged. A design environment for synthesis of embedded fuzzy controllers on FPGAs

    Get PDF
This paper presents a design environment for the synthesis of embedded fuzzy controllers on FPGAs. It provides a novel implementation technique that allows accelerating the exploration of the design space of fuzzy control modules, as well as a codesign flow that eases their integration into complex control systems and the joint development of hardware and software components. The set of CAD tools supporting this environment includes specific fuzzy logic design tools provided by Xfuzzy, FPGA synthesis and implementation tools from Xilinx, and modeling and simulation facilities from Matlab. As demonstrated by the analyzed design examples, the described development strategy takes advantage of flexibility and ease of configuration offered by the different tools to dramatically speed up the stages of description, synthesis, and functional verification of embedded fuzzy control system
