Higher cost of implementing Xpert(®) MTB/RIF in Ugandan peripheral settings: implications for cost-effectiveness.
SETTING: Initial cost-effectiveness evaluations of Xpert(®) MTB/RIF for tuberculosis (TB) diagnosis have not fully accounted for the realities of implementation in peripheral settings. OBJECTIVE: To evaluate costs and diagnostic outcomes of Xpert testing implemented at various health care levels in Uganda. DESIGN: We collected empirical cost data from five health centers utilizing Xpert for TB diagnosis, using an ingredients approach. We reviewed laboratory and patient records to assess outcomes at these sites and10 sites without Xpert. We also estimated incremental cost-effectiveness of Xpert testing; our primary outcome was the incremental cost of Xpert testing per newly detected TB case. RESULTS: The mean unit cost of an Xpert test was US16-58) and was primarily determined by testing volume. Total diagnostic costs were 2.4-fold higher in Xpert clinics than in non-Xpert clinics; however, Xpert only increased diagnoses by 12%. The diagnostic costs of Xpert averaged US885 at the center with the lowest volume of tests. CONCLUSION: Xpert testing can detect TB cases at reasonable cost, but may double diagnostic budgets for relatively small gains, with cost-effectiveness deteriorating with lower testing volumes
Equality of Participation Online Versus Face to Face: Condensed Analysis of the Community Forum Deliberative Methods Demonstration
Online deliberation may provide a more cost-effective and/or less inhibiting
environment for public participation than face to face (F2F). But do online
methods bias participation toward certain individuals or groups? We compare F2F
versus online participation in an experiment affording within-participants and
cross-modal comparisons. For English speakers required to have Internet access
as a condition of participation, we find no negative effects of online modes on
equality of participation (EoP) related to gender, age, or educational level.
Asynchronous online discussion appears to improve EoP for gender relative to
F2F. Data suggest a dampening effect of online environments on black
participants, as well as amplification for whites. Synchronous online voice
communication EoP is on par with F2F across individuals. But individual-level
EoP is much lower in the online forum, and greater online forum participation
predicts greater F2F participation for individuals. Measured rates of
participation are compared to self-reported experiences, and other findings are

Fluctuations of a holographic quantum Hall fluid
We analyze the neutral spectrum of the holographic quantum Hall fluid
described by the D2-D8' model. As expected for a quantum Hall state, we find
the system to be stable and gapped and that, at least over much of the
parameter space, the lowest excitation mode is a magneto-roton. In addition, we
find magneto-rotons in higher modes as well. We show that these magneto-rotons

Rotational master equation for cold laser-driven molecules
The equations of motion for the molecular rotation are derived for
vibrationally cold dimers that are polarized by off-resonant laser light. It is
shown that, by eliminating electronic and vibrational degrees of freedom, a
quantum master equation for the reduced rotational density operator can be
obtained. The coherent rotational dynamics is caused by stimulated Raman
transitions, whereas spontaneous Raman transitions lead to decoherence in the
motion of the quantized angular momentum. As an example the molecular dynamics
for the optical Kerr effect is chosen, revealing decoherence and heating of the

Hybrid Model for the Analysis of Human Gait: A Non-linear Approach
In this work, a generalization of the study of the human gait was made from already
existent models in the literature, like models of Keller and Kockshenev. In this hybrid model, a
strategy of metabolic energy minimization is combined in a race process, with a non-linear
description of the movement of the mass center’s libration, trying to reproduce the behavior of
the walk-run transition. The results of the experimental data, for different speed regimes, indicate
that the perimeter of the trajectory of the mass center is a relevant quantity in the quantification
of this dynamic. An experimental procedure was put into practice in collaboration with the
research group in Biomedical Engineering, Basic Sciences and Laboratories of the Manuela Beltrán
University in Bogotá, Colombia
IL8 polymorphisms and overall survival in pazopanib- or sunitinib-treated patients with renal cell carcinoma.
BACKGROUND: We evaluated germline single nucleotide polymorphisms (SNPs) for association with overall survival (OS) in pazopanib- or sunitinib-treated patients with advanced renal cell carcinoma (aRCC). METHODS: The discovery analysis tested 27 SNPs within 13 genes from a phase III pazopanib trial (N=241, study 1). Suggestive associations were then pursued in two independent datasets: a phase III trial (COMPARZ) comparing pazopanib vs sunitinib (N=729, study 2) and an observational study of sunitinib-treated patients (N=89, study 3). RESULTS: In study 1, four SNPs showed nominally significant association (P≤0.05) with OS; two of these SNPs (rs1126647, rs4073) in IL8 were associated (P≤0.05) with OS in study 2. Because rs1126647 and rs4073 were highly correlated, only rs1126647 was evaluated in study 3, which also showed association (P≤0.05). In the combined data, rs1126647 was associated with OS after conservative multiple-test adjustment (P=8.8 × 10(-5); variant vs reference allele hazard ratio 1.32, 95% confidence interval: 1.15-1.52), without evidence for heterogeneity of effects between studies or between pazopanib- and sunitinib-treated patients. CONCLUSIONS: Variant alleles of IL8 polymorphisms are associated with poorer survival outcomes in pazopanib- or sunitinib-treated patients with aRCC. These findings provide insight in aRCC prognosis and may advance our thinking in development of new therapies
Neurospora from natural populations: Population genomics insights into the Life history of a model microbial Eukaryote
The ascomycete filamentous fungus Neurospora crassa played a historic role in experimental biology and became a model system for genetic research. Stimulated by a systematic effort to collect wild strains initiated by Stanford geneticist David Perkins, the genus Neurospora has also become a basic model for the study of evolutionary processes, speciation, and population biology. In this chapter, we will first trace the history that brought Neurospora into the era of population genomics. We will then cover the major contributions of population genomic investigations using Neurospora to our understanding of microbial biogeography and speciation, and review recent work using population genomics and genome-wide association mapping that illustrates the unique potential of Neurospora as a model for identifying the genetic basis of (potentially adaptive) phenotypes in filamentous fungi. The advent of population genomics has contributed to firmly establish Neurospora as a complete model system and we hope our review will entice biologists to include Neurospora in their research
The gray matter volume of the amygdala is correlated with the perception of melodic intervals: a voxel-based morphometry study
Music is not simply a series of organized pitches, rhythms, and timbres, it is capable of evoking emotions. In the present study, voxel-based morphometry (VBM) was employed to explore the neural basis that may link music to emotion. To do this, we identified the neuroanatomical correlates of the ability to extract pitch interval size in a music segment (i.e., interval perception) in a large population of healthy young adults (N = 264). Behaviorally, we found that interval perception was correlated with daily emotional experiences, indicating the intrinsic link between music and emotion. Neurally, and as expected, we found that interval perception was positively correlated with the gray matter volume (GMV) of the bilateral temporal cortex. More important, a larger GMV of the bilateral amygdala was associated with better interval perception, suggesting that the amygdala, which is the neural substrate of emotional processing, is also involved in music processing. In sum, our study provides one of first neuroanatomical evidence on the association between the amygdala and music, which contributes to our understanding of exactly how music evokes emotional responses
