    Mandibular trigeminal schwannoma: case report

    We report one case of peripheral trigeminal schwannoma originated from the mandibular branch (V3) and located inside the infratemporal and pterygopalatine fossae with slight intracranial extension. The tumor was completelly removed by a transmaxilar approach. The revision of the literature showed to be a very rare tumor in this location.Relatamos um caso de schwanoma do ramo mandibular do nervo trigêmeo, localizado na fossa infratemporal e ptérigo-palatina, com pequena extensão intracraniana e que foi totalmente extirpado por via transmaxilar.     Crédito Rural e Valor Bruto da Produção Agropecuária: uma análise dos estados brasileiros

    One of the main mechanisms of agricultural policy over the years has been rural credit, with significant contributions to the development and expansion of Brazilian agricultural production. Thus, this study analyzes the impact of total rural credit in the forms of financing, investment, and commercialization on the gross value of agricultural production in Brazilian states from 2005 to 2020. Methodologically, the panel data model and Granger causality test proposed by Dumitrescu and Hurlin (2012) were used. The results obtained, through the panel data model, indicated that only rural credit for investment and harvested area had a positive and significant impact on the gross value of agricultural production. Regarding the Granger causality tests, they identified different causal relationships for the distinct credit modalities and for different lag levels, leading to the conclusion that there is a significant temporal precedence between rural credit and the gross value of agricultural production in Brazilian states - regardless of the number of lags included in the model, rejecting the null hypothesis of homogeneous non-causality.Uno de los principales mecanismos de la política agrícola a lo largo de los años ha sido el crédito rural, con importantes contribuciones al desarrollo y expansión de la producción agropecuaria brasileña. Por lo tanto, este estudio analiza el impacto del crédito rural total en las modalidades de financiamiento, inversión y comercialización sobre el valor bruto de la producción agropecuaria de los estados brasileños, de 2005 a 2020. Metodológicamente, se utilizó el modelo de datos en panel y la prueba de causalidad de Granger propuesta por Dumitrescu y Hurlin (2012). Los resultados obtenidos, a través del modelo de datos en panel, indicaron que solo el crédito rural para inversión y el área cosechada tuvieron un impacto positivo y significativo en el valor bruto de la producción agropecuaria. En cuanto a las pruebas de causalidad de Granger, estas identificaron diferentes relaciones causales para las distintas modalidades de crédito y para diferentes niveles de rezagos, lo que llevó a la conclusión de que existe una precedencia temporal significativa entre el crédito rural y el valor bruto de la producción agropecuaria de los estados brasileños, independientemente del número de rezagos incluidos en el modelo, rechazando la hipótesis nula de no causalidad homogénea.Um dos principais mecanismos da política agrícola ao longo dos anos é o crédito rural, com importantes contribuições para o desenvolvimento e expansão da produção agropecuária brasileira. Desta forma, este estudo analisa o impacto do crédito rural total nas modalidades custeio, investimento e comercialização sobre o valor bruto da produção agropecuária dos estados brasileiros, de 2005 a 2020. Metodologicamente, utilizou-se o modelo de dados em painel e o teste de causalidade de Granger proposto por Dumitrescu e Hurlin (2012). Os resultados obtidos, por meio do modelo de dados em painel, indicaram que apenas o crédito rural para investimento e a área colhida exerceram impacto positivo e significativo sobre o valor bruto da produção agropecuária. Quanto aos testes de causalidade de Granger, estes identificaram diferentes relações de causalidade para as distintas modalidades de crédito e para os diferentes níveis de defasagens, levando a concluir que existe uma precedência temporal significativa entre o crédito rural e o valor bruto da produção agropecuária dos estados brasileiros - independentemente do número de defasagens incluídas no modelo, rejeitando a hipótese nula de não causalidade homogênea

    PRÁTICAS INTEGRATIVAS E COMPLEMENTARES NO TRATAMENTO DO CÂNCER SOB A PERSPECTIVA DO ENFERMEIRA: REVISÃO INTEGRATIVA

    Este estudo teve como objetivo avaliar as evidências disponíveis na literatura acerca do uso das terapias complementares por enfermeiros para o tratamento do câncer. Estudo de revisão integrativa, cuja busca ocorreu no Portal Regional da Biblioteca Virtual em Saúde, na Literatura Latino-Americana e do Caribe em Ciências da Saúde, na National Library of Medicine, National Institutes of Health e na Scientific Electronic Library Online, no mês de outubro de 2019. Foram incluídos artigos nos idiomas português, espanhol ou inglês, publicados entre 2006 a 2018. Foram encontrados 121 artigos e após a aplicação dos critérios de inclusão e exclusão compuseram o corpus da revisão 11 artigos. A análise originou cinco categorias: “Crenças”; “Percepções e experiências”; “Conhecimentos”; “Atitudes e práticas” e “Intervenções”. Os resultados evidenciaram que o uso das terapias complementares por enfermeiros para o tratamento do câncer são indicadas, na maioria das vezes, com base nas suas crenças

    Prevention and management of osteoporotic fractures by non-physician health professionals: a systematic literature review to inform EULAR points to consider

    Objective To perform a systematic literature review (SLR) about the effect of non-pharmacological interventions delivered by non-physician health professionals to prevent and manage osteoporotic fractures. Methods Eight clinical questions based on two criteria guided the SLR: (1) adults >= 50 years at high risk of osteoporotic fracture and (2) interventions delivered by non-physician health professionals to prevent and manage osteoporotic fractures. Interventions focused on diagnostic procedures to identify risk of falling, therapeutic approaches and implementation strategies. Outcomes included fractures, falls, risk of falling and change in bone mineral density. Systematic reviews and randomised controlled trials were preferentially selected. Data were synthesised using a qualitative descriptive approach. Results Of 15 917 records, 43 articles were included. Studies were clinically and methodologically diverse. We identified sufficient evidence that structured exercise, incorporating progressive resistance training delivered to people who had undergone hip fracture surgery, and multicomponent exercise, delivered to people at risk of primary fracture, reduced risk of falling. The effectiveness of multidisciplinary fracture liaison services in reducing refracture rate was confirmed. There was insufficient evidence found to support the effectiveness of nutrients and falls prevention programmes in this patient population. Conclusion Despite study heterogeneity, our SLR showed beneficial effects of some interventions delivered by non-physician health professionals and the positive impact of multidisciplinary team working and patient educational approaches to prevent and manage osteoporotic fractures. These results informed a EULAR taskforce that developed points to consider for non-physician health professionals to prevent and manage osteoporotic fractures.This study was funded by the EULAR.     Valproic Acid Causes Proteasomal Degradation of DICER and Influences miRNA Expression

    Valproic acid (VPA) is a commonly used drug to treat epilepsy and bipolar disorders. Known properties of VPA are inhibitions of histone deacetylases and activation of extracellular signal regulated kinases (ERK), which cannot fully explain VPA\u27s clinical features. We found that VPA induces the proteasomal degradation of DICER, a key protein in the generation of micro RNAs. Unexpectedly, the concentration of several micro RNAs increases after VPA treatment, which is caused by the upregulation of their hosting genes prior to DICER degradation. The data suggest that a loss of DICER protein and changes in micro RNA concentration contributes to the clinical properties of VPA. VPA can be used experimentally to down regulate DICER protein levels, which likely reflects a natural regulation of DICER

    Molecular Characterization of a Patient Presumed to Have Prader-Willi Syndrome

    Prader-Willi syndrome (PWS) is caused by the loss of RNA expression from an imprinted region on chromosome 15 that includes SNRPN, SNORD115, and SNORD116. Currently, there are no mouse models that faithfully reflect the human phenotype and investigations rely on human post-mortem material. During molecular characterization of tissue deposited in a public brain bank from a patient diagnosed with Prader-Willi syndrome, we found RNA expression from SNRPN, SNORD115, and SNORD116 which does not support a genetic diagnosis of Prader-Willi syndrome. The patient was a female, Caucasian nursing home resident with history of morbid obesity (BMI 56.3) and mental retardation. She died at age of 56 from pulmonary embolism. SNORD115 and SNORD116 are unexpectedly stable in post mortem tissue and can be used for post-mortem diagnosis. Molecular characterization of PWS tissue donors can confirm the diagnosis and identify those patients that have been misdiagnosed
