    Purification of Uranium-based Endohedral Metallofullerenes (EMFs) by Selective Supramolecular Encapsulation and Release

    Supramolecular nanocapsule 1⋅(BArF) is able to sequentially and selectively entrap recently discovered U@C and unprecedented ScCU@C, simply by soaking crystals of 1⋅(BArF) in a toluene solution of arc-produced soot. These species, selectively and stepwise absorbed by 1⋅(BArF), are easily released, obtaining highly pure fractions of U@C and ScCU@C in one step. ScCU@C represents the first example of a mixed metal actinide-based endohedral metallofullerene (EMF). Remarkably, the host-guest studies revealed that 1⋅(BArF) is able to discriminate EMFs with the same carbon cage but with different encapsulated cluster and computational studies provide support for these observations

    Natural History of MYH7-Related Dilated Cardiomyopathy

    BACKGROUND Variants in myosin heavy chain 7 (MYH7) are responsible for disease in 1% to 5% of patients with dilated cardiomyopathy (DCM); however, the clinical characteristics and natural history of MYH7-related DCM are poorly described. OBJECTIVES We sought to determine the phenotype and prognosis of MYH7-related DCM. We also evaluated the influence of variant location on phenotypic expression. METHODS We studied clinical data from 147 individuals with DCM-causing MYH7 variants (47.6% female; 35.6 +/- 19.2 years) recruited from 29 international centers. RESULTS At initial evaluation, 106 (72.1%) patients had DCM (left ventricular ejection fraction: 34.5% +/- 11.7%). Median follow-up was 4.5 years (IQR: 1.7-8.0 years), and 23.7% of carriers who were initially phenotype-negative developed DCM. Phenotypic expression by 40 and 60 years was 46% and 88%, respectively, with 18 patients (16%) first diagnosed at <18 years of age. Thirty-six percent of patients with DCM met imaging criteria for LV noncompaction. During follow-up, 28% showed left ventricular reverse remodeling. Incidence of adverse cardiac events among patients with DCM at 5 years was 11.6%, with 5 (4.6%) deaths caused by end-stage heart failure (ESHF) and 5 patients (4.6%) requiring heart transplantation. The major ventricular arrhythmia rate was low (1.0% and 2.1% at 5 years in patients with DCM and in those with LVEF of <= 35%, respectively). ESHF and major ventricular arrhythmia were significantly lower compared with LMNA-related DCM and similar to DCM caused by TTN truncating variants. CONCLUSIONS MYH7-related DCM is characterized by early age of onset, high phenotypic expression, low left ventricular reverse remodeling, and frequent progression to ESHF.     La importancia del empleo de la simulación clínica a escala real reside en que permite realizar actividades que conducen a un aprendizaje efectivo preservando la seguridad clínica. El objetivo de la actividad fue incrementar el desarrollo de las habilidades tanto técnicas como no técnicas y fomentar la autoconfianza de los alumnos mediante la simulación a escala real. Se realizaron unas jornadas intensivas de simulación clínica durante la semana anterior a la incorporación de los alumnos al periodo de prácticas clínicas hospitalarias, para los alumnos inscritos en el Prácticum II y Prácticum III. Se incluyeron en la realización de la actividad un total de 60 alumnos, 30 alumnos de cada curso académico. La simulación ha permitido a los alumnos alcanzar las competencias propuestas fomentando el autoaprendizaje y los alumnos consideran totalmente de acuerdo o de acuerdo que su experiencia con la simulación a escala real ha sido satisfactoria.SIN FINANCIACIÓNNo data 2013UE

    SCORE study: quality indicators for rheumatology nursing clinics

    Nursing clinics in rheumatology (NCR) are organizational care models that provide care centred within the scope of nurses abilities. To analyse patients differences in the knowledge of the disease, adherence to the treatment, quality indicators of the Rheumatology Departments included quality perceived by the patients with and without NCR. National multicenter observational prospective cohort study 1 year follow-up, comparing patients attending rheumatology services with and without NCR. NCR was defined by the presence of: (1) office itself; (2) at least one dedicated nurse; (3) its own appointment schedule, and (4) phone. Variables included were (baseline and 12 months) Batalla, Haynes–Sackett, Morisky–Green and quality perceived tests. In addition, another specific questionnaire was drawn up to collect the healthcare, teaching and research activities of each Rheumatology Department. A total of 393 patients were included; 181 NCR and 212 not NCR, corresponding to 39 units, 21 with NCR and 18 without NCR (age 53 ± 11.8 vs 56 ± 13.5 years). Significant differences in favour of the NCR group were found in Haynes–Sackett (p = 0.033) and Morisky–Green (p = 0.03) tests in the basal visit. Significant differences were found in questions about “the courtesy and/or kindness received by the nurse”, being “good or very good” in greater proportion in the NCR group. The publications from the last 5 years were significantly higher in the NCR group in both, national (p = 0.04) and international (p = 0.03) journals.
