    Paradoxes of the environmental behavior of university students in different academic disciplines

    The objective of the present study was to establish the characteristics of the pro environmental behavior of Colombian university students; that is, to identify the attitudes, beliefs, valuations, practices and environmental knowledge of Colombian students from different university careers. For this purpose, a cross-sectional descriptive-correlational study was carried out with a sample of 1503 students from different university careers in five Colombian cities, with an incidence probability of .5, 95 % confidence and an error of 2.5 %. The participants were randomly selected within each university and the questionnaires applied were the Pro environmental Behaviour Inventory (ICP) and the Environmental Knowledge Test (TCA); the latter was developed specifically for the purposes of this research. The students surveyed were found to have positive environmental attitudes and beliefs in most dimensions. The level of environmental awareness of students in the five selected cities is low. Although no significant differences were observed between the careers with respect to beliefs, valuations, actions and environmental knowledge, paradoxically the students of environmental sciences occupied inferior places to those who study economic and administrative sciences. Thus, in the analysis of specific items it was evidenced that the students of environmental sciences develop diverse anti environmental actions, in a similar way to the behavior patterns of the students of the other researched careers. With respect to the region, the results show that it is possible to conclude that there are differences according to the city with respect to the beliefs, valuation, knowledge and environmental actions of the students surveyed. Among the findings it is worth highlighting the results of Bucaramanga in terms of the report of environmental actions and the highest average in the TCA observed in that city. Similarly, Cali stands out along with Bucaramanga in its results, while Medellín obtained the lowest scores in environmental knowledge. Finally, in the report on environmental actions in university students, contradictory results are observed, given that it presents a controversial profile because people perform pro-environmental and anti-environmental behaviors at the same time, with positive environmental attitudes and beliefs in accordance with these attitudes. Regarding the knowledge about the environment, in the descriptive results the average of the test was 12 points, which means that the students failed the test. The multivariate analysis showed that the theoretical model on which the construction of the ICP was based and the instruments from which items were selected for its conformation, apparently has great predictive power with respect to the verbal report of environmental actions; however, the TCA, a test constructed specifically for this research, would covariate with almost all the factors of the ICP but does not predict causally the actions reported by the students. In general, this study shows that university institutions and the Colombian government have the challenge of ensuring that environmental education achieves the expected results for the fulfillment of both the country and the world’s environmental goals. The results also demonstrates the need to articulate policies, actions and regulations to improve the quality of environmental education in universities, as well as to implement citizen behavior programs that promote responsible consumption, given that knowledge does not correlate with effective actions in terms of public services, green brands and solid urban waste management, among other areas.El objetivo del presente estudio fue establecer las características del comportamiento proambiental de los estudiantes universitarios colombianos, esto es, identificar las actitudes, creencias, valoraciones, prácticas y conocimientos ambientales de estudiantes colombianos de diferentes carreras universitarias. Para ello se realizó un estudio descriptivo-correlacional de corte transversal con una muestra de 1503 estudiantes de diferentes carreras universitarias en cinco ciudades colombianas, con una probabilidad de incidencia de .5, 95 % de confianza y un error del 2.5 %. Los participantes fueron seleccionados aleatoriamente al interior de cada universidad y se les aplicaron el Inventario de Comportamiento Proambiental (ICP) y el Test de Conocimiento Ambiental (TCA), este último desarrollado específicamente para propósitos de la presente investigación. Se encontró que los estudiantes encuestados poseen actitudes y creencias ambientales positivas en la mayoría de las dimensiones. El nivel de conocimiento ambiental de los estudiantes en las cinco ciudades seleccionadas es bajo. Si bien no se observaron diferencias significativas entre las carreras respecto a las creencias, valoraciones, acciones y conocimientos ambientales, paradójicamente los estudiantes de ciencias ambientales ocuparon lugares inferiores a los que estudian ciencias económicas y administrativas. Se puede concluir que existen diferencias según la ciudad en las creencias, valoración, conocimiento y acciones ambientales de los estudiantes encuestados. El reporte de acciones ambientales presenta un perfil controversial pues al mismo tiempo las personas realizan comportamientos proambientales y antiambientales. Las instituciones universitarias y el gobierno tienen el desafío de lograr que la educación ambiental muestre los resultados esperados para el cumplimiento de las metas ambientales del país y del mundo

    Older Adult Patients in the Emergency Department: Which Patients should be Selected for a Different Approach?

    Background: While multidimensional and interdisciplinary assessment of older adult patients improves their short-term outcomes after evaluation in the emergency department (ED), this assessment is time-consuming and ill-suited for the busy environment. Thus, identifying patients who will benefit from this strategy is challenging. Therefore, this study aimed to identify older adult patients suitable for a different ED approach as well as independent variables associated with poor short-term clinical outcomes. Methods: We included all patients >= 65 years attending 52 EDs in Spain over 7 days. Sociodemographic, comorbidity, and baseline functional status data were collected. The outcomes were 30 -day mortality, re -presentation, hospital readmission, and the composite of all outcomes. Results: During the study among 96,014 patients evaluated in the ED, we included 23,338 patients >= 65 years-mean age, 78.4 +/- 8.1 years; 12,626 (54.1%) women. During follow-up, 5,776 patients (24.75%) had poor outcomes after evaluation in the ED: 1,140 (4.88%) died, 4,640 (20.51) returned to the ED, and 1,739 (7.69%) were readmitted 30 days after discharge following the index visit. A model including male sex, age >= 75 years, arrival by ambulance, Charlson Comorbidity Index >= 3, and functional impairment had a C -index of 0.81 (95% confidence interval, 0.80-0.82) for 30 -day mortality. Conclusion: Male sex, age >= 75 years, arrival by ambulance, functional impairment, or severe comorbidity are features of patients who could benefit from approaches in the ED different from the common triage to improve the poor short-term outcomes of this population

    Dendritic cell deficiencies persist seven months after SARS-CoV-2 infection

    Severe Acute Respiratory Syndrome Coronavirus (SARS-CoV)-2 infection induces an exacerbated inflammation driven by innate immunity components. Dendritic cells (DCs) play a key role in the defense against viral infections, for instance plasmacytoid DCs (pDCs), have the capacity to produce vast amounts of interferon-alpha (IFN-α). In COVID-19 there is a deficit in DC numbers and IFN-α production, which has been associated with disease severity. In this work, we described that in addition to the DC deficiency, several DC activation and homing markers were altered in acute COVID-19 patients, which were associated with multiple inflammatory markers. Remarkably, previously hospitalized and nonhospitalized patients remained with decreased numbers of CD1c+ myeloid DCs and pDCs seven months after SARS-CoV-2 infection. Moreover, the expression of DC markers such as CD86 and CD4 were only restored in previously nonhospitalized patients, while no restoration of integrin β7 and indoleamine 2,3-dyoxigenase (IDO) levels were observed. These findings contribute to a better understanding of the immunological sequelae of COVID-19

    Effectiveness of an intervention for improving drug prescription in primary care patients with multimorbidity and polypharmacy:Study protocol of a cluster randomized clinical trial (Multi-PAP project)

    This study was funded by the Fondo de Investigaciones Sanitarias ISCIII (Grant Numbers PI15/00276, PI15/00572, PI15/00996), REDISSEC (Project Numbers RD12/0001/0012, RD16/0001/0005), and the European Regional Development Fund ("A way to build Europe").Background: Multimorbidity is associated with negative effects both on people's health and on healthcare systems. A key problem linked to multimorbidity is polypharmacy, which in turn is associated with increased risk of partly preventable adverse effects, including mortality. The Ariadne principles describe a model of care based on a thorough assessment of diseases, treatments (and potential interactions), clinical status, context and preferences of patients with multimorbidity, with the aim of prioritizing and sharing realistic treatment goals that guide an individualized management. The aim of this study is to evaluate the effectiveness of a complex intervention that implements the Ariadne principles in a population of young-old patients with multimorbidity and polypharmacy. The intervention seeks to improve the appropriateness of prescribing in primary care (PC), as measured by the medication appropriateness index (MAI) score at 6 and 12months, as compared with usual care. Methods/Design: Design:pragmatic cluster randomized clinical trial. Unit of randomization: family physician (FP). Unit of analysis: patient. Scope: PC health centres in three autonomous communities: Aragon, Madrid, and Andalusia (Spain). Population: patients aged 65-74years with multimorbidity (≥3 chronic diseases) and polypharmacy (≥5 drugs prescribed in ≥3months). Sample size: n=400 (200 per study arm). Intervention: complex intervention based on the implementation of the Ariadne principles with two components: (1) FP training and (2) FP-patient interview. Outcomes: MAI score, health services use, quality of life (Euroqol 5D-5L), pharmacotherapy and adherence to treatment (Morisky-Green, Haynes-Sackett), and clinical and socio-demographic variables. Statistical analysis: primary outcome is the difference in MAI score between T0 and T1 and corresponding 95% confidence interval. Adjustment for confounding factors will be performed by multilevel analysis. All analyses will be carried out in accordance with the intention-to-treat principle. Discussion: It is essential to provide evidence concerning interventions on PC patients with polypharmacy and multimorbidity, conducted in the context of routine clinical practice, and involving young-old patients with significant potential for preventing negative health outcomes. Este proyecto sombrilla, se desarrolla bajo la sub-línea de Procesos educativos para la diversidad y la transformación cultural se realiza bajo un enfoque cualitativo descriptivo a partir de una selección aleatoria de 160 tesis de la MEVI. En el capítulo 1 se presenta una contextualización sobre el proceso investigativo dentro de la MEVI.     Un examen actualizado de la percepción de las barreras para la implementación de la farmacogenómica y la utilidad de los pares fármaco/gen en América Latina y el Caribe

    La farmacogenómica (PGx) se considera un campo emergente en los países en desarrollo. La investigación sobre PGx en la región de América Latina y el Caribe (ALC) sigue siendo escasa, con información limitada en algunas poblaciones. Por lo tanto, las extrapolaciones son complicadas, especialmente en poblaciones mixtas. En este trabajo, revisamos y analizamos el conocimiento farmacogenómico entre la comunidad científica y clínica de ALC y examinamos las barreras para la aplicación clínica. Realizamos una búsqueda de publicaciones y ensayos clínicos en este campo en todo el mundo y evaluamos la contribución de ALC. A continuación, realizamos una encuesta regional estructurada que evaluó una lista de 14 barreras potenciales para la aplicación clínica de biomarcadores en función de su importancia. Además, se analizó una lista emparejada de 54 genes/fármacos para determinar una asociación entre los biomarcadores y la respuesta a la medicina genómica. Esta encuesta se comparó con una encuesta anterior realizada en 2014 para evaluar el progreso en la región. Los resultados de la búsqueda indicaron que los países de América Latina y el Caribe han contribuido con el 3,44% del total de publicaciones y el 2,45% de los ensayos clínicos relacionados con PGx en todo el mundo hasta el momento. Un total de 106 profesionales de 17 países respondieron a la encuesta. Se identificaron seis grandes grupos de obstáculos. A pesar de los continuos esfuerzos de la región en la última década, la principal barrera para la implementación de PGx en ALC sigue siendo la misma, la "necesidad de directrices, procesos y protocolos para la aplicación clínica de la farmacogenética/farmacogenómica". Las cuestiones de coste-eficacia se consideran factores críticos en la región. Los puntos relacionados con la reticencia de los clínicos son actualmente menos relevantes. Según los resultados de la encuesta, los pares gen/fármaco mejor clasificados (96%-99%) y percibidos como importantes fueron CYP2D6/tamoxifeno, CYP3A5/tacrolimus, CYP2D6/opioides, DPYD/fluoropirimidinas, TMPT/tiopurinas, CYP2D6/antidepresivos tricíclicos, CYP2C19/antidepresivos tricíclicos, NUDT15/tiopurinas, CYP2B6/efavirenz y CYP2C19/clopidogrel. En conclusión, aunque la contribución global de los países de ALC sigue siendo baja en el campo del PGx, se ha observado una mejora relevante en la región. La percepción de la utilidad de las pruebas PGx en la comunidad biomédica ha cambiado drásticamente, aumentando la concienciación entre los médicos, lo que sugiere un futuro prometedor en las aplicaciones clínicas de PGx en ALC.Pharmacogenomics (PGx) is considered an emergent field in developing countries. Research on PGx in the Latin American and the Caribbean (LAC) region remains scarce, with limited information in some populations. Thus, extrapolations are complicated, especially in mixed populations. In this paper, we reviewed and analyzed pharmacogenomic knowledge among the LAC scientific and clinical community and examined barriers to clinical application. We performed a search for publications and clinical trials in the field worldwide and evaluated the contribution of LAC. Next, we conducted a regional structured survey that evaluated a list of 14 potential barriers to the clinical implementation of biomarkers based on their importance. In addition, a paired list of 54 genes/drugs was analyzed to determine an association between biomarkers and response to genomic medicine. This survey was compared to a previous survey performed in 2014 to assess progress in the region. The search results indicated that Latin American and Caribbean countries have contributed 3.44% of the total publications and 2.45% of the PGx-related clinical trials worldwide thus far. A total of 106 professionals from 17 countries answered the survey. Six major groups of barriers were identified. Despite the region’s continuous efforts in the last decade, the primary barrier to PGx implementation in LAC remains the same, the “need for guidelines, processes, and protocols for the clinical application of pharmacogenetics/pharmacogenomics”. Cost-effectiveness issues are considered critical factors in the region. Items related to the reluctance of clinicians are currently less relevant. Based on the survey results, the highest ranked (96%–99%) gene/drug pairs perceived as important were CYP2D6/tamoxifen, CYP3A5/tacrolimus, CYP2D6/opioids, DPYD/fluoropyrimidines, TMPT/thiopurines, CYP2D6/tricyclic antidepressants, CYP2C19/tricyclic antidepressants, NUDT15/thiopurines, CYP2B6/efavirenz, and CYP2C19/clopidogrel. In conclusion, although the global contribution of LAC countries remains low in the PGx field, a relevant improvement has been observed in the region. The perception of the usefulness of PGx tests in biomedical community has drastically changed, raising awareness among physicians, which suggests a promising future in the clinical applications of PGx in LAC

Comprende el análisis y diagnóstico de la industria risaraldense, sus necesidades y tendencias, con enfoque a la industria 4.0. Se provee información para: identificar tecnologías y ocupaciones emergentes que permitan anticipar la definición de perfiles de instructores, determinar requerimientos de modernización de infraestructura física y tecnológica del Centro de formación, actualizar, crear o eliminar programas de formación, establecer el tipo de formación, servicios tecnológicos e innovación que el centro de formación ofrecerá en un horizonte de 10 años e identificar los proyectos y actores estratégicos para el centro de formación.The technological plan of the SENA Industrial Technological Design and Innovation Center for the period 2002-2030 is presented.     First Results of the 140^{140}Ce(n,γ)141^{141}Ce Cross-Section Measurement at n_TOF

    An accurate measurement of the 140^{140}Ce(n,γ) energy-dependent cross-section was performed at the n_TOF facility at CERN. This cross-section is of great importance because it represents a bottleneck for the s-process nucleosynthesis and determines to a large extent the cerium abundance in stars. The measurement was motivated by the significant difference between the cerium abundance measured in globular clusters and the value predicted by theoretical stellar models. This discrepancy can be ascribed to an overestimation of the 140^{140}Ce capture cross-section due to a lack of accurate nuclear data. For this measurement, we used a sample of cerium oxide enriched in 140^{140}Ce to 99.4%. The experimental apparatus consisted of four deuterated benzene liquid scintillator detectors, which allowed us to overcome the difficulties present in the previous measurements, thanks to their very low neutron sensitivity. The accurate analysis of the p-wave resonances and the calculation of their average parameters are fundamental to improve the evaluation of the 140^{140}Ce Maxwellian-averaged cross-section
