    Anatomical study and reanalysis of the nomenclature of the anterolateral complex of the knee focusing on the distal iliotibial band: identification and description of the condylar strap

    Background: The capsulo-osseous layer, short lateral ligament, mid-third lateral capsular ligament, lateral capsular ligament and anterolateral ligament are terms that have been used interchangeably to describe what is probably the same structure. This has resulted in confusion regarding the anatomy and function of the anterolateral complex of the knee and its relation to the distal iliotibial band. Purpose: To characterize the macroscopic anatomy of the anterolateral complex of the knee, in particular the femoral condylar attachment of the distal iliotibial band (ITB). We identified a specific and consistent anatomical structure that was not accurately described previously, connects the deep surface of the ITB to the condylar area, and is distinct from the anterolateral ligament, the capsulo-osseous layer and the Kaplan fibers. Study Design: Descriptive laboratory study. Methods: Sixteen fresh-frozen human cadaveric knees were used to study the anterolateral complex of the knee. Standardized dissections were performed that included a qualitative and quantitative assessment of the anatomy through both anterior (n=5) and posterior (n=11) approaches. Results: The femoral condylar attachment of the distal ITB was not reliably identified by anterior dissection but was in all posterior dissections. A distinct anatomical structure, hereafter termed condylar strap (CS), was identified between the femur and the lateral gastrocnemius on one side and the deep surface of the ITB on the other, in all posteriorly dissected specimens. The structure had a mean thickness of 0.88 mm, and its femoral insertion was located between the distal Kaplan fibers and the epicondyle. The proximal femoral attachment of the structure had a mean width of 15.82 mm and the width of the distal insertion of the structure on the ITB was 13.27 mm. The mean length of the structure was 26.33 mm on its distal border and 21.88 mm on its proximal border. Qualitative evaluation of behavior in internal rotation revealed that this anatomical structure became tensioned and created a tenodesis effect on the ITB. Conclusions: There is a consistent structure that attaches to the deep ITB and the femoral epicondylar area. The orientation of fibers suggest that it may have a role in anterolateral knee stability. Clinical Relevance: This new anatomical description may help surgeons to optimize technical aspects of lateral extra-articular procedures in cases of anterolateral knee laxity

There are several ways to address the cartilage injuries when encountered during ACL reconstruction. The favorable treatment of cartilage injuries during ACL reconstruction is controversial. Indeed, the treatment of cartilage injuries depends on multiple factors including patient variables and severity of lesion. It is unclear whether cartilage lesions affect the recovery after ACL reconstruction and vice versa. Whether ACL reconstruction has a preventive effect on further progression of cartilage lesions is also unclear.     In vitro production of bovine embryos derived from individual donors in the Corral® dish

    Background: Since the identity of the embryo is of outmost importance during commercial in vitro embryo production, bovine oocytes and embryos have to be cultured strictly per donor. Due to the rather low yield of oocytes collected after ovum pick-up (OPU) per individual cow, oocyte maturation and embryo culture take place in small groups, which is often associated with inferior embryo development. The objective of this study was to improve embryonic development in small donor groups by using the Corral (R) dish. This commercial dish is designed for human embryo production. It contains two central wells that are divided into quadrants by a semi-permeable wall. In human embryo culture, one embryo is placed per quadrant, allowing individual follow-up while embryos are exposed to a common medium. In our study, small groups of oocytes and subsequently embryos of different bovine donors were placed in the Corral (R) dish, each donor group in a separate quadrant. Results: In two experiments, the Corral (R) dish was evaluated during in vitro maturation (IVM) and/or in vitro culture (IVC) by grouping oocytes and embryos of individual bovine donors per quadrant. At day 7, a significantly higher blastocyst rate was noted in the Corral (R) dish used during IVM and IVC than when only used during IVM (12.9% +/- 2.10 versus 22.8% +/- 2.67) (P < 0.05). However, no significant differences in blastocyst yield were observed anymore between treatment groups at day 8 post insemination. Conclusions: In the present study, the Corral (R) dish was used for in vitro embryo production (IVP) in cattle; allowing to allocate oocytes and/or embryos per donor. As fresh embryo transfers on day 7 have higher pregnancy outcomes, the Corral (R) dish offers an added value for commercial OPU/IVP, since a higher blastocyst development at day 7 is obtained when the Corral (R) dish is used during IVM and IVC

    Genome stability of bovine in vivo-conceived cleavage-stage embryos is higher compared to in vitro-produced embryos.

    STUDY QUESTION Is the rate and nature of chromosome instability (CIN) similar between bovine in vivo-derived and in vitro-cultured cleavage-stage embryos? SUMMARY ANSWER There is a major difference regarding chromosome stability of in vivo-derived and in vitro-cultured embryos, as CIN is significantly lower in in vivo-derived cleavage-stage embryos compared to in vitro-cultured embryos. WHAT IS KNOWN ALREADY CIN is common during in vitro embryogenesis and is associated with early embryonic loss in humans, but the stability of in vivo-conceived cleavage-stage embryos remains largely unknown. STUDY DESIGN, SIZE, DURATION Because human in vivo preimplantation embryos are not accessible, bovine (Bos taurus) embryos were used to study CIN in vivo. Five young, healthy, cycling Holstein Friesian heifers were used to analyze single blastomeres of in vivo embryos, in vitro embryos produced by ovum pick up with ovarian stimulation (OPU-IVF), and in vitro embryos produced from in vitro matured oocytes retrieved without ovarian stimulation (IVM-IVF). PARTICIPANTS/MATERIALS, SETTING, METHODS Single blastomeres were isolated from embryos, whole-genome amplified and hybridized on Illumina BovineHD BeadChip arrays together with the bulk DNA from the donor cows (mothers) and the bull (father). DNA was also obtained from the parents of the bull and from the parents of the cows (paternal and maternal grandparents, respectively). Subsequently, genome-wide haplotyping and copy-number profiling was applied to investigate the genomic architecture of 171 single bovine blastomeres of 16 in vivo, 13 OPU-IVF and 13 IVM-IVF embryos. MAIN RESULTS AND THE ROLE OF CHANCE The genomic stability of single blastomeres in both of the in vitro-cultured embryo cohorts was severely compromised (P < 0.0001), and the frequency of whole chromosome or segmental aberrations was higher in embryos produced in vitro than in embryos derived in vivo. Only 18.8% of in vivo-derived embryos contained at least one blastomere with chromosomal anomalies, compared to 69.2% of OPU-IVF embryos (P < 0.01) and 84.6% of IVM-IVF embryos (P < 0.001). LARGE SCALE DATA Genotyping data obtained in this study has been submitted to NCBI Gene Expression Omnibus (GEO; accession number GSE95358) LIMITATIONS REASONS FOR CAUTION There were two main limitations of the study. First, animal models may not always reflect the nature of human embryogenesis, although the use of an animal model to investigate CIN was unavoidable in our study. Second, a limited number of embryos were obtained, therefore more studies are warranted to corroborate the findings. WIDER IMPLICATIONS OF THE FINDINGS Although CIN is also present in in vivo-developed embryos, in vitro procedures exacerbate chromosomal abnormalities during early embryo development. Hence, the present study highlights that IVF treatment compromises embryo viability and should be applied with care. Additionally, our results encourage to refine and improve in vitro culture conditions and assisted reproduction technologies.     Physiological and Movement Demands of Rugby League Referees: Influence on Penalty Accuracy.

    Research into the physiological and movement demands of Rugby League (RL) referees is limited, with only one study in the European Super League (SL). To date, no studies have considered decision-making in RL referees. The purpose of this study was to quantify penalty accuracy scores of RL referees and determine the relationship between penalty accuracy and total distance covered (TD), high-intensity running (HIR) and heart rate per 10-min period of match-play. Time motion analysis was undertaken on 8 referees over 148 European SL games during the 2012 season using 10Hz GPS analysis and heart rate monitors. The number and timing of penalties awarded was quantified using Opta Stats. Referees awarded the correct decision on 74 ± 5% of occasions. Lowest accuracy was observed in the last 10-minute period of the game (67 ± 13%), with a moderate drop (ES= 0.86) in accuracy observed between 60-70 minutes and 70-80 minutes. Despite this, there were only small correlations observed between HRmean, total distance, HIR efforts and penalty accuracy. Although a moderate correlation was observed between maximum velocity and accuracy. Despite only small correlations observed, it would be rash to assume that physiological and movement demands of refereeing have no influence on decision-making. More likely, other confounding variables influence referee decision-making accuracy, requiring further investigation. Findings can be used by referees and coaches to inform training protocols, ensuring training is specific to both cognitive and physical match demands
