    Influence of a single regional list for corneal transplantation on the eye bank of a university hospital

    PURPOSE: To evaluate how the implantation of a single regional list for corneal transplants influenced an eye bank of a university hospital. To analyze the influence of the regional list on the corneas (procurement and destiny), the number of transplantations made and the average waiting time for surgery. METHODS: A retrospective study was made to compare the records of patients who underwent penetrating keratoplasty and also the data of the eye bank of the Medical School of São José do Rio Preto, São Paulo, Brazil. The study compared the data of the service one year before and one year after the creation of the single regional list. RESULTS: The number of transplantations increased from 60 to 92 procedures. The monthly average of directly acquired corneas increased from 13.83 + 6.57 to 18.16 + 4.8 (p-value = 0.07). The number of corneas sent to other institutions was greater than the number of corneas received from other eye banks (p-value = 0.003). There was no significant difference between the waiting time for surgery before and after the creation of the list except for the period of adaptation. CONCLUSIONS: This eye bank served as a supplier of corneas for other institutions. By the end of the first year of operation of the single regional list, there was no alteration in the waiting time as compared to that before its creation. Nonetheless, a tendency of homogenization in the waiting time for penetrating keratoplasty among the patients was observed.OBJETIVO: Avaliar como a implantação do sistema de lista única para transplantes de córnea influenciou um Banco de Olhos vinculado a um hospital escola. Analisar sua interferência nas córneas (captação e destino), no número de transplantes realizados e também na média de tempo de espera pela cirurgia. MÉTODOS: Foi realizado estudo retrospectivo, avaliando os prontuários dos pacientes submetidos a ceratoplastia penetrante e também os dados do Banco de Olhos da Faculdade de Medicina de São José do Rio Preto - SP. O estudo comparou dados relativos ao funcionamento do serviço por um ano antes e após a criação da lista única. RESULTADOS: O número de cirurgias aumentou de 60 para 92 cirurgias. A média mensal de córneas retiradas aumentou de 13,83 ± 6,57 para 18,16 ± 4,80 (p=0,07). O número de córneas enviadas por esta instituição foi maior que o número de córneas recebidas de outros serviços (p=0,003). Não houve diferença significativa entre o tempo de espera pela cirurgia antes e após a criação da fila única (desconsiderando o período de cadastramento). CONCLUSÕES: Este Banco de Olhos funcionou como fornecedor de córneas para outras instituições. Após seu primeiro ano de funcionamento, a implantação da lista única não alterou o tempo de espera dos pacientes pela cirurgia.     The use of wooden troughs on stream beds, artificially colonized by blackfly larvae, is proposed for larvicide evaluations. Mortality was recorded 3 or 4 hours after treatment. Larval susceptibility was also evaluated utilizing the LT50 criterion. In there field assays Simulium (C.) pertinax populations from the litoral of S. Paulo and Rio de Janeiro States were shown to be resistant to temephos, even when subjected to high concentrations. Vectobac 12 AS, a Bacillus thuringiensis var. israelensis product, was shown to be more potent against late instar larvae and efficient in concentrations higher than 7,200 ITU/1 (10 min). The LT50 to 3,744 ITU/1 (10 min) was calculated as 70.9 min.O uso de pequenas calhas de madeira no leito de riachos, manualmente colonizadas com borrachudos, é proposto para avaliações da eficiência de larvicidas 3 ou 4 horas após as aplicações.     Widespread forest vertebrate extinctions induced by a mega hydroelectric dam in lowland Amazonia

    Mega hydropower projects in tropical forests pose a major emergent threat to terrestrial and freshwater biodiversity worldwide. Despite the unprecedented number of existing, underconstruction and planned hydroelectric dams in lowland tropical forests, long-term effects on biodiversity have yet to be evaluated. We examine how medium and large-bodied assemblages of terrestrial and arboreal vertebrates (including 35 mammal, bird and tortoise species) responded to the drastic 26-year post-isolation history of archipelagic alteration in landscape structure and habitat quality in a major hydroelectric reservoir of Central Amazonia. The Balbina Hydroelectric Dam inundated 3,129 km2 of primary forests, simultaneously isolating 3,546 land-bridge islands. We conducted intensive biodiversity surveys at 37 of those islands and three adjacent continuous forests using a combination of four survey techniques, and detected strong forest habitat area effects in explaining patterns of vertebrate extinction. Beyond clear area effects, edge-mediated surface fire disturbance was the most important additional driver of species loss, particularly in islands smaller than 10 ha. Based on species-area models, we predict that only 0.7% of all islands now harbor a species-rich vertebrate assemblage consisting of ≥80% of all species. We highlight the colossal erosion in vertebrate diversity driven by a man-made dam and show that the biodiversity impacts of mega dams in lowland tropical forest regions have been severely overlooked. The geopolitical strategy to deploy many more large hydropower infrastructure projects in regions like lowland Amazonia should be urgently reassessed, and we strongly advise that long-term biodiversity impacts should be explicitly included in pre-approval environmental impact assessments

    Further investigation of confirmed urinary tract infection (UTI) in children under five years: a systematic review.

    Background: Further investigation of confirmed UTI in children aims to prevent renal scarring and future complications. Methods: We conducted a systematic review to determine the most effective approach to the further investigation of confirmed urinary tract infection (UTI) in children under five years of age. Results: 73 studies were included. Many studies had methodological limitations or were poorly reported. Effectiveness of further investigations: One study found that routine imaging did not lead to a reduction in recurrent UTIs or renal scarring. Diagnostic accuracy: The studies do not support the use of less invasive tests such as ultrasound as an alternative to renal scintigraphy, either to rule out infection of the upper urinary tract (LR- = 0.57, 95%CI: 0.47, 0.68) and thus to exclude patients from further investigation or to detect renal scarring (LR+ = 3.5, 95% CI: 2.5, 4.8). None of the tests investigated can accurately predict the development of renal scarring. The available evidence supports the consideration of contrast-enhanced ultrasound techniques for detecting vesico-ureteric reflux (VUR), as an alternative to micturating cystourethrography (MCUG) (LR+ = 14.1, 95% CI: 9.5, 20.8; LR- = 0.20, 95%CI: 0.13, 0.29); these techniques have the advantage of not requiring exposure to ionising radiation. Conclusion: There is no evidence to support the clinical effectiveness of routine investigation of children with confirmed UTI. Primary research on the effectiveness, in terms of improved patient outcome, of testing at all stages in the investigation of confirmed urinary tract infection is urgently required

    Mapeamento ambiental participativo e matriz SWOT/FOFA enquanto práticas de gestão e educação ambiental com ênfase em recursos hídricos

    Water is one of the most degradated natural resource. Solution comes through technical and educational aspects. Environmental diagnosis, land use planning and degraded area recovery are technical actions. It is necessary the stablishment of an active and collective conscience besides environmental information and knowledgement. The objective of this work is to apply two tools for environmental education and diagnosis in four cities of Pontal do Triângulo Mineiro region. The tools were the Participative Environmental Mapping and the SWOT matrix. Results revealed that community participants had critical perception of the environmental reality. Maps with wealth environmental information and action plans towards weakness improvement and maintenance of the strengths were proposed. The use of the tools showed to be efficient towards the promotion of environmental consciousness besides data generated for the water resource public management of thecities.A água é um dos recursos naturais mais degradados. A solução deste problema envolve questões técnicas e educacionais. O diagnóstico de cenários e o planejamento do uso do solo além da recuperação de áreas degradadas são algumas das ações técnicas.  Do ponto de vista educacional, depende não só de informação e conhecimento mas também de uma consciência ambiental ativa e coletiva. O objetivo do presente trabalho é a aplicação de duas ferramentas para realizar atividades de educação e diagnóstico ambiental junto à comunidade de quatro municípios do Pontal do Triângulo Mineiro. As ferramentas utilizadas foram o Mapeamento Ambiental Participativo e a Matriz SWOT/FOFA. Os resultados relevaram a percepção e visão críticas da comunidade participante em termos ambientais. Foram gerados mapas com grande riqueza de informações sobre aspectos ambientais relacionados aos recursos hídricos dos municípios e planos de ação focando tanto a melhoria de fragilidades ambientais como a manutenção das fortalezas elencadas pelos participantes. O uso dessas ferramentas se mostrou eficiente tanto para a promoção da consciência ambiental como também gerou dados para contribuir com a gestão dos recursos hídricos do município

    How does study quality affect the results of a diagnostic meta-analysis?

    Background: The use of systematic literature review to inform evidence based practice in diagnostics is rapidly expanding. Although the primary diagnostic literature is extensive, studies are often of low methodological quality or poorly reported. There has been no rigorously evaluated, evidence based tool to assess the methodological quality of diagnostic studies. The primary objective of this study was to determine the extent to which variations in the quality of primary studies impact the results of a diagnostic meta-analysis and whether this differs with diagnostic test type. A secondary objective was to contribute to the evaluation of QUADAS, an evidence-based tool for the assessment of quality in diagnostic accuracy studies. Methods: This study was conducted as part of large systematic review of tests used in the diagnosis and further investigation of urinary tract infection (UTI) in children. All studies included in this review were assessed using QUADAS, an evidence-based tool for the assessment of quality in systematic reviews of diagnostic accuracy studies. The impact of individual components of QUADAS on a summary measure of diagnostic accuracy was investigated using regression analysis. The review divided the diagnosis and further investigation of UTI into the following three clinical stages: diagnosis of UTI, localisation of infection, and further investigation of the UTI. Each stage used different types of diagnostic test, which were considered to involve different quality concerns. Results: Many of the studies included in our review were poorly reported. The proportion of QUADAS items fulfilled was similar for studies in different sections of the review. However, as might be expected, the individual items fulfilled differed between the three clinical stages. Regression analysis found that different items showed a strong association with test performance for the different tests evaluated. These differences were observed both within and between the three clinical stages assessed by the review. The results of regression analyses were also affected by whether or not a weighting (by sample size) was applied. Our analysis was severely limited by the completeness of reporting and the differences between the index tests evaluated and the reference standards used to confirm diagnoses in the primary studies. Few tests were evaluated by sufficient studies to allow meaningful use of meta-analytic pooling and investigation of heterogeneity. This meant that further analysis to investigate heterogeneity could only be undertaken using a subset of studies, and that the findings are open to various interpretations. Conclusion: Further work is needed to investigate the influence of methodological quality on the results of diagnostic meta-analyses. Large data sets of well-reported primary studies are needed to address this question. Without significant improvements in the completeness of reporting of primary studies, progress in this area will be limited

    The role of Prenatal Care and Social Risk Factors in the relationship between immigrant status and neonatal morbidity: A retrospective cohort study

    Background and Aim Literature evaluating association between neonatal morbidity and immigrant status presents contradictory results. Poorer compliance with prenatal care and greater social risk factors among immigrants could play roles as major confounding variables, thus explaining contradictions. We examined whether prenatal care and social risk factors are confounding variables in the relationship between immigrant status and neonatal morbidity. Methods Retrospective cohort study: 231 pregnant African immigrant women were recruited from 2007–2010 in northern Spain. A Spanish population sample was obtained by simple random sampling at 1:3 ratio. Immigrant status (Spanish, Sub-Saharan and Northern African), prenatal care (Kessner Index adequate, intermediate or inadequate), and social risk factors were treated as independent variables. Low birth weight (LBW < 2500 grams) and preterm birth (< 37 weeks) were collected as neonatal morbidity variables. Crude and adjusted odds ratios (OR) were estimated by unconditional logistic regression with 95% confidence intervals (95% CI). Results Positive associations between immigrant women and higher risk of neonatal morbidity were obtained. Crude OR for preterm births in Northern Africans with respect to nonimmigrants was 2.28 (95% CI: 1.04–5.00), and crude OR for LBW was 1.77 (95% CI: 0.74–4.22). However, after adjusting for prenatal care and social risk factors, associations became protective: adjusted OR for preterm birth = 0.42 (95% CI: 0.14–1.32); LBW = 0.48 (95% CI: 0.15–1.52). Poor compliance with prenatal care was the main independent risk factor associated with both preterm birth (adjusted OR inadequate care = 17.05; 95% CI: 3.92–74.24) and LBW (adjusted OR inadequate care = 6.25; 95% CI: 1.28–30.46). Social risk was an important independent risk factor associated with LBW (adjusted OR = 5.42; 95% CI: 1.58– 18.62). Conclusions Prenatal care and social risk factors were major confounding variables in the relationship between immigrant status and neonatal morbidity
