    Habilidades lectoras a partir de textos multimodales : Una secuencia didáctica desde la lectura comprensiva en estudiantes del grado tercero de básica primaria

    La investigación tuvo lugar en la institución educativa Félix Henao de Medellín, lugar donde las autoras laboramos como maestras. Se desarrolló con un grupo de estudiantes del grado tercero y su aplicación se dio en el año 2017. El problema a resolver se orientó hacia el fortalecimiento de habilidades lectoras en los estudiantes, dado los bajos resultados obtenidos en las Pruebas Saber de Lenguaje durante los últimos cuatro años. Metodológicamente se privilegió el enfoque cualitativo donde la revisión documental y el trabajo en el aula se combinaron dando lugar a la construcción de la secuencia didáctica: -Nuestra ciudad más que palabras?, en la cual se incluyeron estrategias como: sonidos e imágenes de la ciudad, gesticuentos, construyendo murales, círculos de palabras, nos vemos en el blog, entre otras; mediante el uso de diversidad de textos como el barrio, fotografías, videos, cuentos, murales, retahílas, versos, trovas, refranes, canciones, etc. Con ello, se hizo posible el fortalecimiento de la comprensión lectora expresada en habilidades como: la elaboración de explicaciones e inferencias sobre lo observado y/o leído, desarrollo de comparaciones estableciendo similitudes y diferencias, consulta del diccionario, realización de resúmenes, entre otras, así como el incremento del 21 % de los estudiantes del grado tercero, ubicados en el nivel avanzado y el 46 % en el nivel satisfactorio de las pruebas saber 2017. Para destacar en los estudiantes, el valor otorgado a los recorridos por el barrio, esos espacios por ellos habitados, cuya lectura les permitió construir nuevos significados y, por los docentes, el reconocer en el grupo participante de la investigación, una mayor disponibilidad para el aprendizaje."The research took place at the Félix Henao educational institution in Medellín, where the authors work as teachers, and took place during 2017 with group of third graders. The goal of the project was the strengthening of the students reading skills, considering the low scores obtained in the national Language Knowledge Tests (Pruebas Saber) during the last four years. Methodologically, a qualitative approach was privileged, where the document review and classroom work were combined to construct the didactic sequence: ""Our city more than words"" (-Nuestra ciudad más que palabras?), in which strategies such as sonidos e imágenes de la ciudad, gesticuentos, construyendo murales, círculos de palabras, nos vemos en el blog, among others. Using the diversity of texts such as: el barrio, fotografías, videos, cuentos, murales, retahílas, versos, trovas, refranes,and canciones. With the listed strategies, it became possible to strengthen students reading comprehension, substantiated by skill improvement, such as the elaboration of explanations and inferences about what I have seen and/or read, comparisons development, establishing similarities and differences, dictionary consultation, summaries realization, to mention some. Moreover, the group of third graders improved their results within 21% located in the advanced level and 46% in the adequate level of the 2017 knowledge tests. To emphasize in the students, the value given to the neighborhood tours, those spaces inhabited by them, whose reading allowed them to construct new meanings and, by the teachers, by recognizing in the participant research a greater availability for learning."Magíster en EducaciónMaestrí

    Predictors of positive (18) F-FDG PET/CT-scan for large vessel vasculitis in patients with persistent polymyalgia rheumatica

    Objective: Polymyalgia rheumatica (PMR) is often the presenting manifestation of giant cell arteritis (GCA). Fluorine-18-fluorodeoxyglucose positron emission tomography/computed tomography (PET/CT) scan often discloses the presence of large vessel vasculitis (LVV) in PMR patients. We aimed to identify predictive factors of a positive PET/CT scan for LVV in patients classified as having isolated PMR according to well-established criteria. Methods: A set of consecutive patients with PMR from a single hospital were assessed. All of them underwent PET/CT scan between January 2010 and February 2018 based on clinical considerations. Patients with PMR associated to other diseases, including those with cranial features of GCA, were excluded. The remaining patients were categorized in classic PMR (if fulfilled the 2012 EULAR/ACR classification criteria at disease diagnosis; n=84) or atypical PMR (who did not fulfill these criteria; n=16). Only information on patients with classic PMR was assessed. Results: The mean age of the 84 patients (51 women) with classic PMR was 71.4±9.2 years. A PET/CT scan was positive in 51(60.7%). Persistence of classic PMR symptoms was the most common reason to perform a PET/CT scan. Nevertheless, patients with positive PET/CT scan often had unusual symptoms. The best set of predictors of a positive PET/CT scan were bilateral diffuse lower limb pain (OR=8.8, 95% CI 1.7-46.3; p=0.01), pelvic girdle pain (OR=4.9, 95% CI 1.50-16.53; p=0.01) and inflammatory low back pain (OR=4.7, 95% CI 1.03-21.5; p=0.04). Conclusion: Inflammatory low back pain, pelvic girdle and diffuse lower limb pain are predictors of positive PET/CT scan for LVV in PMR

    Association of Functional Polymorphisms of KIR3DL1/DS1 With Behçet's Disease

    Behçet's disease (BD) is an immune-mediated vasculitis related to imbalances between the innate and adaptive immune response. Infectious agents or environmental factors may trigger the disease in genetically predisposed individuals. HLA-B51 is the genetic factor stronger associated with the disease, although the bases of this association remain elusive. NK cells have also been implicated in the etiopathogenesis of BD. A family of NK receptors, Killer-cell Immunoglobulin-like Receptor (KIR), with a very complex organization, is very important in the education and control of the NK cells by the union to their ligands, most of them, HLA class I molecules. This study aimed to investigate the contribution of certain KIR functional polymorphisms to the susceptibility to BD. A total of 466 BD patients and 444 healthy individuals were genotyped in HLA class I (A, B, and C). The set of KIR genes and the functional variants of KIR3DL1/DS1 and KIR2DS4 were also determined. Frequency of KIR3DL1004 was lower in patients than in controls (0.15 vs. 0.20, P = 0.005, Pc = 0.015; OR = 0.70; 95% CI 0.54-0.90) in both B51 positive and negative individuals. KIR3DL1004, which encodes a misfolded protein, is included in a common telomeric haplotype with only one functional KIR gene, KIR3DL2. Both, KIR3DL1 and KIR3DL2 sense pathogen-associated molecular patterns but they have different capacities to eliminate them. The education of the NK cells depending on the HLA, the balance of KIR3DL1/KIR3DL2 licensed NK cells and the different capacities of these receptors to eliminate pathogens could be involved in the etiopathogenesis of BD

    Tocilizumab in giant cell arteritis. Observational, open-label multicenter study of 134 patients in clinical practice

    OBJECTIVE: Tocilizumab (TCZ) has shown efficacy in clinical trials on giant cell arteritis (GCA). Real-world data are scarce. Our objective was to assess efficacy and safety of TCZ in unselected patients with GCA in clinical practice Methods: Observational, open-label multicenter study from 40 national referral centers of GCA patients treated with TCZ due to inefficacy or adverse events of previous therapy. Outcomes variables were improvement of clinical features, acute phase reactants, glucocorticoid-sparing effect, prolonged remission and relapses. A comparative study was performed: (a) TCZ route (SC vs. IV); (b) GCA duration (?6 vs. >6 months); (c) serious infections (with or without); (d) ?15 vs. >15 mg/day at TCZ onset. RESULTS: 134 patients; mean age, 73.0 ± 8.8 years. TCZ was started after a median [IQR] time from GCA diagnosis of 13.5 [5.0-33.5] months. Ninety-eight (73.1%) patients had received immunosuppressive agents. After 1 month of TCZ 93.9% experienced clinical improvement. Reduction of CRP from 1.7 [0.4-3.2] to 0.11 [0.05-0.5] mg/dL (p < 0.0001), ESR from 33 [14.5-61] to 6 [2-12] mm/1st hour (p < 0.0001) and decrease in patients with anemia from 16.4% to 3.8% (p < 0.0001) were observed. Regardless of administration route or disease duration, clinical improvement leading to remission at 6, 12, 18, 24 months was observed in 55.5%, 70.4%, 69.2% and 90% of patients. Most relevant adverse side-effect was serious infections (10.6/100 patients-year), associated with higher doses of prednisone during the first three months of therapy. CONCLUSION: In clinical practice, TCZ yields a rapid and maintained improvement of refractory GCA. Serious infections appear to be higher than in clinical trials

    En este PAP se integran objetivos que buscan incidir para la implementación de entornos incluyentes, el desarrollo de la educación sobre la salud y cultura auditiva, las herramientas necesarias para la defensa del confort acústico y la propagación de información sobre la discapacidad auditiva. Para lograr esto, se desenvuelven dentro del PAP seis proyectos, los cuales son, Coordinación de eventos, Cruzada Contra el Ruido, Material Didáctico, Mariana Anaya, Redes sociales PAP Escucha México y Observatorio interdisciplinario del ruido. Cada proyecto se desarrolla en diferentes áreas de trabajo, como puede ser la organización de paneles informativos y encuentros, el manejo de redes sociales, la elaboración de talleres y cursos.     Tocilizumab in refractory Caucasian Takayasu's arteritis: a multicenter study of 54 patients and literature review

    Objective: To assess the efficacy and safety of tocilizumab (TCZ) in Caucasian patients with refractory Takayasu's arteritis (TAK) in clinical practice. Methods: A multicenter study of Caucasian patients with refractory TAK who received TCZ. The outcome variables were remission, glucocorticoid-sparing effect, improvement in imaging techniques, and adverse events. A comparative study between patients who received TCZ as monotherapy (TCZMONO) and combined with conventional disease modifying anti-rheumatic drugs (cDMARDs) (TCZCOMBO) was performed. Results: The study comprised 54 patients (46 women/8 men) with a median [interquartile range (IQR)] age of 42.0 (32.5-50.5) years. TCZ was started after a median (IQR) of 12.0 (3.0-31.5) months since TAK diagnosis. Remission was achieved in 12/54 (22.2%), 19/49 (38.8%), 23/44 (52.3%), and 27/36 (75%) patients at 1, 3, 6, and 12 months, respectively. The prednisone dose was reduced from 30.0 mg/day (12.5-50.0) to 5.0 (0.0-5.6) mg/day at 12 months. An improvement in imaging findings was reported in 28 (73.7%) patients after a median (IQR) of 9.0 (6.0-14.0) months. Twenty-three (42.6%) patients were on TCZMONO and 31 (57.4%) on TCZCOMBO: MTX (n = 28), cyclosporine A (n = 2), azathioprine (n = 1). Patients on TCZCOMBO were younger [38.0 (27.0-46.0) versus 45.0 (38.0-57.0)] years; difference (diff) [95% confidence interval (CI) = -7.0 (-17.9, -0.56] with a trend to longer TAK duration [21.0 (6.0-38.0) versus 6.0 (1.0-23.0)] months; diff 95% CI = 15 (-8.9, 35.5), and higher c-reactive protein [2.4 (0.7-5.6) versus 1.3 (0.3-3.3)] mg/dl; diff 95% CI = 1.1 (-0.26, 2.99). Despite these differences, similar outcomes were observed in both groups (log rank p = 0.862). Relevant adverse events were reported in six (11.1%) patients, but only three developed severe events that required TCZ withdrawal.     Epistatic interaction of ERAP1 and HLA-B in Behçet disease: a replication study in the Spanish population

    Behçet's disease (BD) is a multifactorial disorder associated with the HLA region. Recently, the ERAP1 gene has been proposed as a susceptibility locus with a recessive model and with epistatic interaction with HLA-B51. ERAP1 trims peptides in the endoplasmic reticulum to optimize their length for MHC-I binding. Polymorphisms in this gene have been related with the susceptibility to other immune-mediated diseases associated to HLA class I. Our aim was, the replication in the Spanish population of the association described in the Turkish population between ERAP1 (rs17482078) and BD. Additionally, in order to improve the understanding of this association we analyzed four additional SNPs (rs27044, rs10050860, rs30187 and rs2287987) associated with other diseases related to HLA class I and the haplotype blocks in this gene region. According to our results, frequencies of the homozygous genotypes for the minor alleles of all the SNPs were increased among patients and the OR values were higher in the subgroup of patients with the HLA-B risk factors, although differences were not statistically significant. Moreover, the presence of the same mutation in both chromosomes increased the OR values from 4.51 to 10.72 in individuals carrying the HLA-B risk factors. Therefore, although they were not statistically significant, our data were consistent with an association between ERAP1 and BD as well as with an epistatic interaction between ERAP1 and HLA-B in the Spanish population

    Teniendo en cuenta la incidencia de las nuevas tecnologías en la educación de hoy, el propósito de la presente investigación radica en implementar las tecnologías de la información y la comunicación TIC en el aula de clase para fomentar el interés académico en los estudiantes de la IED Fundación. Metodológicamente la investigación se orientó desde una mirada Cualitativa, bajo el paradigma sociocritico; la unidad de análisis estuvo constituida por cuarenta (40) estudiantes del grado octavo entre las edades de catorce (14) quince (15) y dieciséis (16) años; como técnica de recolección de información se utilizó la observación participante, como instrumento de recolección, el diario de campo; a partir de las actividades desarrolladas con la implementación de las TIC en el aula de clase se encontró que los estudiantes presentan mayores niveles de motivación e interés por aprender, mejorando significativamente su promedio de notas cuándo se trabajan los contenidos curriculares articulados con estasTaking into account the incidence of new information and communication technologies in education today, the purpose of research in the implementation of information technology and communication students of the IED Foundation.
