    Assessing Feasibility and Readiness to Address Obesity through Policy in American Indian Reservations

    The Institute of Medicine and Centers for Disease Control and Prevention (CDC) have identified policy and environmental strategies as critical to the prevention and control of obesity. However such strategies are rare in American Indian communities despite significant obesity-related disparities. Tribal policymaking processes differ by tribal nation and are often poorly understood by researchers and public health practitioners, hindering the dissemination, implementation, and successful scale-up of evidence-base obesity strategies in tribal communities. To address these gaps in knowledge we surveyed 138 diverse stakeholders in two American Indian reservations to assess the feasibility of and readiness to implement CDC-recommended obesity policy strategies within their communities. We assessed general community readiness to address obesity using 18 questions from the Community Readiness Handbook. Means and standard deviations were evaluated and scores ranged from 1 (no readiness) to 9 (high readiness). We then assessed stakeholder attitudes regarding the feasibility of implementing specific strategies given tribal culture, infrastructure, leadership, and funding support. Average scores were calculated and mean values ranked from highest (best strategy) to lowest. Despite significant differences in their geographic and sociodemographic characteristics, both communities identified increasing the availability of healthy foods in tribal venues as the most feasible strategy and scored in the “preplanning” readiness stage. The survey design, implementation process, and findings generated significant community interest and discussion. Health planners in one of the communities used the survey findings to provide tribal decision-makers with measurable information to prioritize appropriate strategies for implementation

    Self-reported health and cardiovascular reactions to psychological stress in a large community sample: Cross-sectional and prospective associations

    Exaggerated cardiovascular reactions to acute psychological stress have been implicated in a number of adverse health outcomes. This study examined, in a large community sample, the cross-sectional and prospective associations between reactivity and self-reported health. Blood pressure and heart rate were measured at rest and in response to an arithmetic stress task. Self-reported health was assessed concurrently and five years later. In cross-sectional analyses, those with excellent/good self-reported health exhibited larger cardiovascular reactions than those with fair/poor subjective health. In prospective analyses, participants who had larger cardiovascular reactions to stress were more likely to report excellent/good health five years later, taking into account their reported health status at the earlier assessment. The findings suggest that greater cardiovascular reactivity may not always be associated with negative health outcomes

    A new genus and two new species of Thyasiridae associated with methane seeps off Svalbard, Arctic Ocean

    This is an Accepted Manuscript of an article published by Taylor & Francis in Marine Biology Research on May 16th 2017, available online: http://www.tandfonline.com/10.1080/17451000.2016.1272699 Bivalves have been found in unique benthic assemblages associated with active methane seeps and mounds along the western and southern margins of the Svalbard shelf (75–79°N) at 350–380 m depth. Among the samples collected were a number of shells of Thyasiridae that are distinct from any species previously described. Here we describe one new genus Rhacothyas gen. nov. and two new species Thyasira capitanea sp. nov. and Rhacothyas kolgae sp. nov., including their distinguishing characteristics and the environmental setting where they were found. Thyasira capitanea sp. nov. is large compared with many other thyasirids, has an equilateral shell and demarcated zones on the median and anterior areas along with a distinct posterior sulcus. Rhacothyas kolgae sp. nov. is unique among other thyasirid genera and species regarding its characteristic outline, sunken lunule, lack of submarginal sulcus and wrinkled surface. Furthermore, we discuss their present occurrence in the context of the glaciomarine history of the Svalbard margin. We posit that these new species, after the deglaciation of the Barents Sea Ice Sheet, may have originated from other chemosynthetic or reducing environments along the Atlantic shelf margin or the southern Barents Sea shelf by following the net transport of the North Atlantic Current rather than having evolved in situ

    Leading and Sustaining Curricular Change: Workshop Proceedings from the 2018 Sex and Gender Health Education Summit

    The education of health science professionals must balance the incorporation of new and essential content against the current curriculum density. Scientific evidence documenting the impact of sex and gender on health outcomes establishes the need for incorporation of these topics into the health science curriculum. An interprofessional workshop was designed to provide participants with the knowledge and skills to effectively champion curricular change. Surveys before and after the workshop assessed the participants' perception of curriculum change. Introductory presentations addressed topics of organizational readiness and characteristics of change agents. This was followed by role-play activities in groups of 8 to 10, utilizing two scenarios. The first scenario involved a faculty champion advocating for change to the school curriculum leadership, and the second scenario involved the curriculum leadership advocating for change to the teaching faculty. After the role-play, participants shared the important points discovered by their groups, and the same information was collected by survey. After the workshop, 95% participants reported an increased ability to advocate for the inclusion of sex and gender topics in the curriculum. The most important aspect of the workshop was the providing of resources related to the teaching of sex and gender topics. We conclude that a workshop format balancing didactic information and role-playing scenarios is an effective tool for empowering faculty to introduce changes in health sciences curricula in areas that may be new to faculty or health science school leadership, such as the impact and role of sex and gender on health outcomes

    Phenome-wide association study identifies marked increased in burden of comorbidities in African Americans with systemic lupus erythematosus.

    BACKGROUND: African Americans with systemic lupus erythematosus (SLE) have increased renal disease compared to Caucasians, but differences in other comorbidities have not been well-described. We used an electronic health record (EHR) technique to test for differences in comorbidities in African Americans compared to Caucasians with SLE. METHODS: We used a de-identified EHR with 2.8 million subjects to identify SLE cases using a validated algorithm. We performed phenome-wide association studies (PheWAS) comparing African American to Caucasian SLE cases and African American SLE cases to matched non-SLE controls. Controls were age, sex, and race matched to SLE cases. For multiple testing, a false discovery rate (FDR) p value of 0.05 was used. RESULTS: We identified 270 African Americans and 715 Caucasians with SLE and 1425 matched African American controls. Compared to Caucasians with SLE adjusting for age and sex, African Americans with SLE had more comorbidities in every organ system. The most striking included hypertension odds ratio (OR) = 4.25, FDR p = 5.49 × 10 CONCLUSIONS: African Americans with SLE have an increased comorbidity burden compared to Caucasians with SLE and matched controls. This increase in comorbidities in African Americans with SLE highlights the need to monitor for cardiovascular and infectious complications

    Independent Review of Charity Regulation in Northern Ireland

    The Department for Communities has published the Report on the Independent Review of Charity Regulation, commissioned by the Minister for Communities in January 2021. The Independent Review Panel was chaired by Dr Oonagh Breen, Professor of Law at the Sutherland School of Law, University College Dublin, whose research focuses on comparative charity law regulation and governance. Dr Breen was supported by Noel Lavery, former Permanent Secretary of a number of NICS Departments and Fellow of the Institute of Chartered Accountants in Ireland, and the Reverend Dr Lesley Carroll, the NI Prisoner Ombudsman and former Deputy Chief Commissioner of the Equality Commission. In fulfilling its Terms of Reference, the Panel’s comprehensive review report contains findings and recommendations in relation to the regulatory framework, the Commission’s performance, the Department’s leadership and oversight of the regulatory framework and best practice in relation to all of these, taking lessons from across the UK, Ireland and elsewhere. Through its inclusive engagement process, it heard from 306 community engagement webinar attendees, and attendees at 20 other meetings with other regulators and their responsible government bodies, the Department for Communities, the Charity Commission and other interested stakeholders. It received 145 written submissions. The report finds that the Charity Commission NI was constrained from its earliest days as a result of flawed legislation made by the former Department, and as a result, the sequencing and focusing of its work was disproportionately focused on compliance, at the cost of completion of the charity register, with a need for a reset of the Commission’s communications and engagement approach to recalibrate its culture and relationship with the charity sector. The Report makes 93 recommendations, all of which the Panel see as important and includes, as a matter of priority, the completion of the charity register. In addition, it recommends changes to accounting and reporting requirements, and the commencement of the provisions for Charitable Incorporated Organisations in NI.     The Postglacial response of Arctic Ocean gas hydrates to climatic amelioration

    Seafloor methane release due to the thermal dissociation of gas hydrates is pervasive across the continental margins of the Arctic Ocean. Furthermore, there is increasing awareness that shallow hydrate-related methane seeps have appeared due to enhanced warming of Arctic Ocean bottom water during the last century. Although it has been argued that a gas hydrate gun could trigger abrupt climate change, the processes and rates of subsurface/atmospheric natural gas exchange remain uncertain. Here we investigate the dynamics between gas hydrate stability and environmental changes from the height of the last glaciation through to the present day. Using geophysical observations from offshore Svalbard to constrain a coupled ice sheet/gas hydrate model, we identify distinct phases of subglacial methane sequestration and subsequent release on ice sheet retreat that led to the formation of a suite of seafloor domes. Reconstructing the evolution of this dome field, we find that incursions of warm Atlantic bottom water forced rapid gas hydrate dissociation and enhanced methane emissions during the penultimate Heinrich event, the B?lling and Aller?d interstadials, and the Holocene optimum. Our results highlight the complex interplay between the cryosphere, geosphere, and atmosphere over the last 30,000 y that led to extensive changes in subseafloor carbon storage that forced distinct episodes of methane release due to natural climate variability well before recent anthropogenic warmingauthorsversionPeer reviewe
