    Prognostic Utility of a New Risk Stratification Protocol for Secondary Prevention in Patients Attending Cardiac Rehabilitation

    Several risk scores have been used to predict risk after an acute coronary syndrome (ACS), but none of these risk scores include functional class. The aim was to assess the predictive value of risk stratification (RS), including functional class, and how cardiac rehabilitation (CR) changed RS. Two hundred and thirty-eight patients with ACS from an ambispective observational registry were stratified as low (L) and no-low (NL) risk and classified according to exercise compliance; low risk and exercise (L-E), low risk and control (no exercise) (L-C), no-low risk and exercise (NL-E), and no-low risk and control (NL-C). The primary endpoint was cardiac rehospitalization. Multivariable analysis was performed to identify variables independently associated with the primary endpoint. The L group included 56.7% of patients. The primary endpoint was higher in the NL group (18.4% vs. 4.4%, p < 0.001). After adjustment for age, sex, diabetes, and exercise in multivariable analysis, HR (95% CI) was 3.83 (1.51-9.68) for cardiac rehospitalization. For RS and exercise, the prognosis varied: the L-E group had a cardiac rehospitalization rate of 2.5% compared to 26.1% in the NL-C group (p < 0.001). Completing exercise training was associated with reclassification to low-risk, associated with a better outcome. This easy-to-calculate risk score offers robust prognostic information. No-exercise groups were independently associated with the worst outcomes. Exercise-based CR program changed RS, improving classification and prognosis

    AimsDespite the evidence, lipid-lowering treatment (LLT) in secondary prevention remains insufficient, and a low percentage of patients achieve the recommended LDL cholesterol (LDLc) levels by the guidelines. We aimed to evaluate the efficacy of an intensive, mobile devices-based healthcare lipid-lowering intervention after hospital discharge in patients hospitalized for acute coronary syndrome (ACS).Methods and resultsAmbiespective register in which a mobile devices-based healthcare intervention including periodic follow-up, serial lipid level controls, and optimization of lipid-lowering therapy, if appropriate, was assessed in terms of serum lipid-level control at 12 weeks after discharge. A total of 497 patients, of which 462 (93%) correctly adhered to the optimization protocol, were included in the analysis. At the end of the optimization period, 327 (70.7%) patients had LDLc levels ≤ 70 mg/dL. 40% of patients in the LDLc ≤ 70 mg/dL group were upgraded to very-high intensity lipid-lowering ability therapy vs. 60.7% in the LDLc &gt; 70 mg/dL group, p &lt; 0.001. Overall, 38.5% of patients had at least a change in their LLT. Side effects were relatively infrequent (10.7%). At 1-year follow-up, LDLc levels were measured by the primary care physician in 342 (68.8%) of the whole cohort of 497 patients. In this group, 71.1% of patients had LDLc levels ≤ 70 mg/dL.ConclusionAn intensive, structured, mobile devices-based healthcare intervention after an ACS is associated with more than 70% of patients reaching the LDLc levels recommended by the clinical guidelines. In patients with LDLc measured at 1-year follow-up, 71.1% had LDLc levels ≤ 70 mg/dL

    Age-dependent impact of two exercise training regimens on genomic and metabolic remodeling in skeletal muscle and liver of male mice

    Skeletal muscle adapts to different exercise training modalities with age; however, the impact of both variables at the systemic and tissue levels is not fully understood. Here, adult and old C57BL/6 male mice were assigned to one of three groups: sedentary, daily high-intensity intermittent training (HIIT), or moderate intensity continuous training (MICT) for 4 weeks, compatible with the older group’s exercise capacity. Improvements in body composition, fasting blood glucose, and muscle strength were mostly observed in the MICT old group, while effects of HIIT training in adult and old animals was less clear. Skeletal muscle exhibited structural and functional adaptations to exercise training, as revealed by electron microscopy, OXPHOS assays, respirometry, and muscle protein biomarkers. Transcriptomics analysis of gastrocnemius muscle combined with liver and serum metabolomics unveiled an age-dependent metabolic remodeling in response to exercise training. These results support a tailored exercise prescription approach aimed at improving health and ameliorating age-associated loss of muscle strength and function in the elderly.This work was supported by funding from the Intramural Research Program of the National Institute on Aging/NIH.     Overview of recent TJ-II stellarator results

    The main results obtained in the TJ-II stellarator in the last two years are reported. The most important topics investigated have been modelling and validation of impurity transport, validation of gyrokinetic simulations, turbulence characterisation, effect of magnetic configuration on transport, fuelling with pellet injection, fast particles and liquid metal plasma facing components. As regards impurity transport research, a number of working lines exploring several recently discovered effects have been developed: the effect of tangential drifts on stellarator neoclassical transport, the impurity flux driven by electric fields tangent to magnetic surfaces and attempts of experimental validation with Doppler reflectometry of the variation of the radial electric field on the flux surface. Concerning gyrokinetic simulations, two validation activities have been performed, the comparison with measurements of zonal flow relaxation in pellet-induced fast transients and the comparison with experimental poloidal variation of fluctuations amplitude. The impact of radial electric fields on turbulence spreading in the edge and scrape-off layer has been also experimentally characterized using a 2D Langmuir probe array. Another remarkable piece of work has been the investigation of the radial propagation of small temperature perturbations using transfer entropy. Research on the physics and modelling of plasma core fuelling with pellet and tracer-encapsulated solid-pellet injection has produced also relevant results. Neutral beam injection driven Alfvénic activity and its possible control by electron cyclotron current drive has been examined as well in TJ-II.     Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Effects of Exercise in Patients with Obstructive Sleep Apnoea

    Obstructive sleep apnoea (OSA) constitutes a public health problem, with various systemic consequences that can increase cardiovascular morbidity and mortality as well as increase healthcare expenditure. This review discusses the rationale and effects of using general physical exercise, oropharyngeal exercises, and respiratory muscle training as an adjunctive treatment for patients with sleep apnoea. The recommended treatment for OSA is the use of continuous positive airway pressure, which is a therapy that prevents apnoea events by keeping the airways open. In the last decade, coadjuvant treatments that aim to support weight loss (including diet and physical exercise) and oropharyngeal exercises have been proposed to lower the apnoea/hypopnoea index among patients with OSA. Based on the available evidence, health professionals could decide to incorporate these therapeutic strategies to manage patients with sleep apnoea

    Tesis (Profesor de Educación Física, Licenciado en Educación)A través de los años la humanidad se ha visto "bombardeada" con información cada vez más compleja, referida en gran medida al aspecto genético, llevando al ser humano en general a abrir sus mentes a un mundo distinto al que conocíamos siglos atrás; y a los científicos a expandir sus investigaciones y llevar todos estos sucesos a completos análisis Uno de esos sucesos es el que será presentado a través de esta investigación: El Síndrome de Down. Muchas cosas se dicen sobre él, incluso en la antigüedad fue llamada la "raza de los idiotas" o también "mongólicos" por asemejarse su fisonomía a las personas de ese país. A través de innumerables investigaciones realizadas por autores como Siegfried, Pueschel, entre otros, podemos darnos cuentas que este estado es más complejo de lo que se pensaba y al mismo tiempo que no podemos sacar conclusiones certeras acerca del comportamiento de estos niños. En esta investigación se pudo comprobar como el astmto genético tiene la mayor incidencia en que los niños puedan contraer el Síndrome de Down, dejando en segundo plano, la edad en que la madre fecunda a ese bebé, llevándole a una relación directamente proporcional con esta posibilidad: A mayor edad de la madre, mayor posibilidad de concebir un hijo con Síndrome de Down. Con el dato anterior podemos insinuar algunas de las complicaciones que se pueden presentar de manera tenue, o bien marcada, en los nacidos con esta anomalía, pronosticando un difícil pero no imposible pasar por la vida. Complicaciones cervicales, auditivas, visuales, gastrointestinales, motrices, físicas e incluso a nivel del sistema nervioso y cardiaco se presentan ante nosotros como dificultades de gran importancia; Estos limitantes nos permiten conocer en mayor medida las reacciones de los niños con Síndrome de Down y a su vez nos alerta y prepara para actuar de forma adecuada. Mediante el presente estudio se mostrará cada característica de los niños con Síndrome de Down, desde su nacimiento hasta su etapa de entrada a la adolescencia, y se podrá comprobar que se debe tener un cuidado especial en cada etapa de su vida. El presente trabajo se llevó a cabo teniendo como eje central, que el tema de niños con síndrome de Down no es muy abordado en el plano de la actividad física, y que muchas veces los futuros profesores del área educacional de la educación física no se sienten preparados para trabajar con esta clase de niños. El tema de la actividad física en los niños Down es muy importante, tanto por el tema motriz como por la tendencia a la obesidad. Podemos darnos cuenta que estos niños tienen la necesidad de desarrollar habilidades y destrezas motrices, lo cual le ayuda en la integración con sus pares y a desenvolverse en situaciones de patrones motores básicos, como también que conozcan los beneficios que otorga la práctica del adiestramiento físico para el mejoramiento de la salud y calidad de vida, lo que conducirá al desarrollo pleno del niño, tanto en el área física como psicológica y social. La investigación se realizó en distintos sitios de Internet y libros educacionales, para lograr tener una referencia real de la vida de los niños con Síndrome de Down y así poder llegar a la matriz de todos los problemas y virtudes de los afectados por esta complicación. En el desarrollo de la investigación se pudo constatar el rol que cumplen los educadores de estos niños, donde se postula un modelo constructivista, que a través de los distintos aprendizajes estructurados por los docentes construyen su personalidad y forma de desenvolverse frente a un obstáculo; es acá donde podemos ver la importancia de los profesores y sus innovaciones pedagógicas.La finalidad de la investigación fue crear un manual de ejercicios para niños con síndrome de Down, y que éste fuera un apoyo al docente para que pueda trabajar de una manera óptima, sin provocar un daño físico y/o psicológico a estos niños; es también muy importante la estimulación que se le debe entregar día tras día a niños con esta complicación e incentivar la convicción en ellos que son normales y pueden hacer todo lo que se propongan. Para realizar este manual fue necesario además conocer el contexto educacional en el cual se desenvuelven los niños Down, la gestión educativa, sus implicancias y actualizaciones frente a las deficiencias de cada uno

    The conventional isoproterenol-induced heart failure model does not consistently mimic the diaphragmatic dysfunction observed in patients

    Heart failure (HF) impairs diaphragm function. Animal models realistically mimicking HF should feature both the cardiac alterations and the diaphragmatic dysfunction characterizing this disease. The isoproterenol-induced HF model is widely used, but whether it presents diaphragmatic dysfunction is unknown. However, indirect data from research in other fields suggest that isoproterenol could increase diaphragm function. The aim of this study was to test the hypothesis that the widespread rodent model of isoproterenol-induced HF results in increased diaphragmatic contractility. Forty C57BL/6J male mice were randomized into 2 groups: HF and healthy controls. After 30 days of isoproterenol infusion to establish HF, in vivo diaphragmatic excursion and ex vivo isolated diaphragm contractibility were measured. As compared with healthy controls, mice with isoproterenol-induced HF showed the expected changes in structural and functional echocardiographic parameters and lung edema. isoproterenol-induced HF increased in vivo diaphragm excursion (by ≈30%, p<0.01) and increased by ≈50% both ex vivo peak specific force (p<0.05) and tetanic force (p<0.05) at almost all 10-100 Hz frequencies (p<0.05), with reduced fatigue resistance (p<0.01) when compared with healthy controls. Expression of myosin genes encoding the main muscle fiber types revealed that Myh4 was higher in isoproterenol-induced HF than in healthy controls (p<0.05), suggesting greater distribution of type IIb fibers. These results show that the conventional isoproterenol-induced HF model increases diaphragm contraction, a finding contrary to what is observed in patients with HF. Therefore, this specific model seems limited for translational an integrative HF research, especially when cardio-respiratory interactions are investigated
