    The Design of a Portable Municipal Waste Incinerator with Fuzzy Logic Based Support for Emission Estimation

    A fuzzy logic interface system to estimate oxygen requirement for complete combustion as well as the level of pollution from incinerator gas flue in order to manage solid waste from domestic, institutional, medical and industrial sources was designed. The designed incinerator is double chambered operating with a maximum temperature of 760 °C in the lower chamber and 1000°C in the upper chamber. The insulating wall is made up of a refractory brick of 55mm in thickness having a 2mm thickness low carbon steel as the outer wall. Hydrogen Chloride (HCl) and Nitrous oxides (NO)x are the gases was used to demonstrate the Fuzzy Inference System (FIS) model. The FIS was built with five input variables (Food, PVC, Polythene, Paper and Textile) and three input variables with two membership functions. The FIS was developed to estimation the degree of possibility distribution of pollution that should be expected when a certain composition of waste is incinerated. The plots of composition of waste high in food against oxygen require for combustion gives a possibility distribution of about 0.9 which is high according to the fuzzy set definition while the plot of waste composition high in PVC against HCL shows linearity

    Este artículo analiza las características de la autonomía local de las entidades locales estadounidenses y cómo las iniciativas de política progresista de estos municipios se están viendo coartadas a través del mecanismo de la prevalencia de las normas de cada estado, que frenan consciente y activamente la aplicación o avance de dichas políticas. Para ello, se estudian tanto la prevalencia estatal como mecanismo constitucional de desplazamiento de normas locales, como las nuevas tendencias que pretenden no solo neutralizar la actuación local de progreso, sino castigar a aquellos municipios y cargos públicos locales cuya acción político-normativa sea más progresista que la del estado al que pertenece.     Measuring oxygen access: lessons from health facility assessments in Lagos, Nigeria

    The COVID-19 pandemic has highlighted global oxygen system deficiencies and revealed gaps in how we understand and measure ‘oxygen access’. We present a case study on oxygen access from 58 health facilities in Lagos state, Nigeria. We found large differences in oxygen access between facilities (primary vs secondary, government vs private) and describe three key domains to consider when measuring oxygen access: availability, cost, use. Of 58 facilities surveyed, 8 (14%) of facilities had a functional pulse oximeter. Oximeters (N=27) were typically located in outpatient clinics (12/27, 44%), paediatric ward (6/27, 22%) or operating theatre (4/27, 15%). 34/58 (59%) facilities had a functional source of oxygen available on the day of inspection, of which 31 (91%) facilities had it available in a single ward area, typically the operating theatre or maternity ward. Oxygen services were free to patients at primary health centres, when available, but expensive in hospitals and private facilities, with the median cost for 2 days oxygen 13 000 (US36)and27 500(US36) and 27 500 (US77) Naira, respectively. We obtained limited data on the cost of oxygen services to facilities. Pulse oximetry use was low in secondary care facilities (32%, 21/65 patients had SpO2 documented) and negligible in private facilities (2%, 3/177) and primary health centres (<1%, 2/608). We were unable to determine the proportion of hypoxaemic patients who received oxygen therapy with available data. However, triangulation of existing data suggested that no facilities were equipped to meet minimum oxygen demands. We highlight the importance of a multifaceted approach to measuring oxygen access that assesses access at the point-of-care and ideally at the patient-level. We propose standard metrics to report oxygen access and describe how these can be integrated into routine health information systems and existing health facility assessment tools

    A Clinicopathological Review of Colonic Polyps in a Tertiary Hospital in North Central Nigeria

    Background: Colorectal polyps, especially adenomas, are known as precursors of colorectal carcinomas. This study was aimed at determining the prevalence and histopathologic characteristics of colonic polyps among Nigerians that underwent colonoscopy at the University of Ilorin Teaching Hospital, Ilorin, Nigeria. The authors also determined the polyp detection rate and adenoma detection rate. Materials and Methods: This was a hospital-based cross-sectional study of all colonoscopy examinations performed at the University of Ilorin Teaching Hospital, Ilorin from March 2013 to September 2017. The endoscopy register was reviewed, and patients’ biodata, indications for colonoscopy and colonoscopic findings were recorded on a proforma. The histopathologic reports of the polyps were also retrieved and their diagnosis confirmed. Results: A total of 289 patients had colonoscopy carried out on them. The age of the patients ranged from 14 to 90 years with a mean ±SD of 57.0 ± 15.3 years and a median age of 58 years. There were 178 (61.6%) males and 111 (38.4%) females giving a M:F ratio of 1.6:1. The indications for colonoscopy were rectalbleeding 124 (42.9%); suspected colorectal cancer 67 (23.2%); abdominal pain 22 (7.6%); chronic diarrhea 18 (6.2%); surveillance colonoscopy 12 (4.2%); constipation 11 (3.8%); change in bowel habit 9 (3.1%); occult bleeding 6 (2.1%); others 20 (6.9%). Endoscopic findings were normal findings 89 (30.8%); hemorrhoids 88 (30.4%); colonic polyps 50 (17.3%); diverticulosis 34 (11.8%); rectal cancer 29 (10.0%); colon cancer 24 (8.3%); colitis 19 (6.6%); others 13 (4.5%). The findings were not mutually exclusive. The diagnostic yield was 69.2%. The overall polyp detection rate was 17.3%. Of the 50 patients with polyps, 33 (66.0%) were males (p = 0.481). Theadenoma detection rate was 4.8%. Conclusion: The authors conclude that their observed polyp detection rate is consistent with the estimated rates for Africa. Rectal bleeding and adenomatous polyps were the commonest presentation and histopathologic finding, respectively

    Assessment of Decentralized Electricity Production from Hybrid Renewable Energy Sources for Sustainable Energy Development in Nigeria

    This paper presents technical and economic assessment of a hybrid energy system for electricity generation in rural communities in the six geopolitical zones of Nigeria. The study was based on a 500 rural household model having an electric load of 493 kWh per day. To simulate long-term continuous implementation of the hybrid system, 21 years (1992 – 2012) hourly mean global solar radiation and wind speed data for the selected sites were used. The mean annual wind speed and solar radiation for the locations ranged from 2.31 m/s for Warri to 3.52 m/ s for Maiduguri and 4.53 kWh/m2 for Warri to 5.92 kWh/m2 for Maiduguri, respectively. These weather data were used for simulation with the Micro-power Optimization Model software HOMER. From the optimum results of the hybrid system,Warri has the highest NPC and COE of 2,441,222and2,441,222 and 0.721/kWh, respectively while Maiduguri has the least NPC and COE of 2,225,387and2,225,387 and 0.658/kWh, respectively for the 21 years project lifespan. The high value of COE for Warri is due to its low renewable energy resource while low COE for Maiduguri is due to its high renewable energy resource. The Northern part of the country has ample renewable energy resource availability and with a strong political will, optimal utilization of these renewable resources (solar and wind) can be actualized. Researchers, Industrialists, Policy Makers and the Nigerian government should therefore seize this opportunity in developing a sustainable energy through utilization of abundant renewable energy resources in the country

    Preparing for the spread of patient-reported outcomes (PROs) data collection from primary care to community pharmacy: stakeholder insights

    Medication non-adherence is a significant public health problem. Patient-reported outcomes (PROs) offer a rich data source to facilitate resolution of medication non-adherence. PatientTocâ„¢ is an electronic PRO data collection software originally implemented at primary care practices in California, United States (US). Currently, the use of standardized PRO data collection systems in US community pharmacies is limited. Thus, we are conducting a two-phase evaluation of the spread and scale of PatientTocâ„¢ to US Midwestern community pharmacies. This report focuses on the first phase of the evaluation. The objective of this phase was to prepare for implementation of PatientTocâ„¢ in community pharmacies by conducting a pre-implementation developmental formative evaluation to (1) identify potential barriers, facilitators, and actionable recommendations to PatientTocâ„¢ implementation and (2) create a draft implementation toolkit

    Background Medication non-adherence is a problem of critical importance, affecting approximately 50% of all persons taking at least one regularly scheduled prescription medication and costing the United States more than $100 billion annually. Traditional data sources for identifying and resolving medication non-adherence in community pharmacies include prescription fill histories. However, medication possession does not necessarily mean patients are taking their medications as prescribed. Patient-reported outcomes (PROs), measuring adherence challenges pertaining to both remembering and intention to take medication, offer a rich data source for pharmacists and prescribers to use to resolve medication non-adherence. PatientTocâ„¢ is a PROs collection software developed to facilitate collection of PROs data from low-literacy and non-English speaking patients in Los Angeles. Objectives This study will evaluate the spread and scale of PatientTocâ„¢ from primary care to community pharmacies for the collection and use of PROs data pertaining to medication adherence. Methods The following implementation and evaluation steps will be conducted: 1) a pre-implementation developmental formative evaluation to determine community pharmacy workflow and current practices for identifying and resolving medication non-adherence, potential barriers and facilitators to PatientTocâ„¢ implementation, and to create a draft implementation toolkit, 2) two plan-do-study-act cycles to refine an implementation toolkit for spreading and scaling implementation of PatientTocâ„¢ in community pharmacies, and 3) a comprehensive, theory-driven evaluation of the quality of care, implementation, and patient health outcomes of spreading and scaling PatientTocâ„¢ to community pharmacies. Expected impact This research will inform long-term collection and use of PROs data pertaining to medication adherence in community pharmacies

    Experiences Applying Technology to Overcome Common Challenges in Pharmacy Practice-Based Research in the United States

    Despite the importance of pharmacy practice-based research in generating knowledge that results in better outcomes for patients, health systems and society alike, common challenges to PPBR persist. Herein, we authors describe PPBR challenges our research teams have encountered, and our experiences using technology-driven solutions to overcome such challenges. Notably, limited financial resources reduce the time available for clinicians and researchers to participate in study activities; therefore, resource allocation must be optimized. We authors have also encountered primary data collection challenges due to unique data needs and data access/ownership issues. Moreover, we have experienced a wide geographic dispersion of study practices and collaborating researchers; a lack of trained, on-site research personnel; and the identification and enrollment of participants meeting study eligibility criteria. To address these PPBR challenges, we authors have begun to turn to technology-driven solutions, as described here

    The exploitation of new hydrocarbon discoveries in meeting the present global energy demand is a function of the availability and application of new technologies. The relevance of new technologies is borne out of the complex subsurface architecture and conditions of offshore petroleum plays. Conventional techniques, from drilling to production, for exploiting these discoveries may require adaption for such subsurface conditions as they fail under conditions of high pressure and high temperature. The oil and gas industry over the past decades has witnessed increased research into the use of nanotechnology with great promise for drilling operations, enhanced oil recovery, reservoir characterization, production, etc. The prospect for a paradigm shift towards the application of nanotechnology in the oil and gas industry is constrained by evolving challenges with its progression.
