    Powering IoT Sensors with RF Energy Harvesting

    There is a need to power Internet of Things (IoT) applications that require frequent, expensive, and/or dangerous battery replacements. Radio-frequency energy harvesting (RFEH) is a possible alternative source of power for select IoT sensor applications. In comparison to other methods of energy harvesting, RFEH has the smallest incident power densities and therefore comes with many design challenges. In this project we implement a novel RFEH system powered via a dedicated transmitter. A planar inverted-F antenna (PIFA) and voltage doubler circuit form the designed rectenna (rectifier + antenna) and the system is implemented on a custom PCB to carry out RF-to-DC conversion. The system’s feasibility is demonstrated by powering a commercial power management unit (PMU) and temperature sensor over a test duration of eight hours

    An electronic healthcare record server implemented in PostgreSQL

    This paper describes the implementation of an Electronic Healthcare Record server inside a PostgreSQL relational database without dependency on any further middleware infrastructure. The five-part international standard for communicating healthcare records (ISO EN 13606) is used as the information basis for the design of the server. We describe some of the features that this standard demands that are provided by the server, and other areas where assumptions about the durability of communications or the presence of middleware lead to a poor fit. Finally, we discuss the use of the server in two real-world scenarios including a commercial application

    An Invertible Transform for Efficient String Matching in Labeled Digraphs

    Let G = (V, E) be a digraph where each vertex is unlabeled, each edge is labeled by a character in some alphabet ?, and any two edges with both the same head and the same tail have different labels. The powerset construction gives a transform of G into a weakly connected digraph G\u27 = (V\u27, E\u27) that enables solving the decision problem of whether there is a walk in G matching an arbitrarily long query string q in time linear in |q| and independent of |E| and |V|. We show G is uniquely determined by G\u27 when for every v_? ? V, there is some distinct string s_? on ? such that v_? is the origin of a closed walk in G matching s_?, and no other walk in G matches s_? unless it starts and ends at v_?. We then exploit this invertibility condition to strategically alter any G so its transform G\u27 enables retrieval of all t terminal vertices of walks in the unaltered G matching q in O(|q| + t log |V|) time. We conclude by proposing two defining properties of a class of transforms that includes the Burrows-Wheeler transform and the transform presented here

    Systematic design approach for optimized resonantly enhanced Mach-Zehnder modulators

    A systematic design approach using the developed numerical model for the investigation of any arbitrary electrooptic modulator configuration is described, and its application to the simulation and synthesis of resonantly enhanced Mach-Zehnder modulators (RE-MZMs) is demonstrated. The tool is implemented using equivalent circuit model using transmission lines, lumped elements, and N-port S-parameters. The numerical tool is used to simulate the modulation enhancement factor and radio frequency (RF) return loss of a number of theoretically and experimentally demonstrated examples. Finally, the design tool is used to synthesize a new optimized RE-MZM. This RE-MZM is fabricated and measured, and predicted results are compared

    TREATMENT RECOMMENDATION IN VIETNAMESE MEDICAL CONSULTATIONS

    This study investigated the doctor’s recommendation of treatment to their adult patients in primary-care visits in the Vietnamese context. Data was gathered from 55 audio-recorded consultations at two public hospitals, and examined from a conversation-analytic perspective. We demonstrate that the participating doctors used two main approaches to treatment recommendation with their patients: general and detailed. In the latter case, the doctor recommended a treatment regime, sought the patient’s agreement, or offered choices regarding aspects of the treatment. Our overall contention is that, in the Vietnamese public hospital system, the doctor’s organisation of talk in the course of recommending treatment tends to be shaped by the institutional and cultural context in which it occurs, regardless of which type of treatment approach is being used

    Eliciting patients’ health concerns in consulting rooms and wards in Vietnamese public hospitals

    This article examines the doctor’s elicitation of the patient’s presenting health concern in two clinical settings in the Vietnamese public hospital system: the consulting room and the ward. The data were taken from 66 audio-recorded consultations. Our analysis shows that the elicitors used by the doctor in the consulting room often communicate a weak epistemic stance towards the patient’s health issue, while those used in the ward tend to signal a strong epistemic stance. In addition, this contrast between the elicitors employed in the consulting room and the ward is evident in our data regardless of whether the consultation is a first visit or a same follow-up (in which the doctor is the same one that treated the patient on their last visit), though the contrast is less clear for different follow-ups (in which the doctor has not treated the patient before). An additional finding is that the clinical setting has some bearing on the use of inappropriate elicitation formats (in which the doctor opens the visit with an elicitor which is more appropriate for another type of visit). The precise way in which each of the consulting room and the ward operates is, of course, a feature of the Vietnamese public hospital system itself. Hence, the overall contrast between the elicitors and elicitation formats used in these two settings illustrates how, on a more general level, the institutional context can have an impact on doctor-patient communication

    Detection of activating estrogen receptor gene (ESR1) mutations in single circulating tumor cells

    Purpose: Early detection is essential for treatment plans before onset of metastatic disease. Our purpose was to demonstrate feasibility to detect and monitor estrogen receptor 1 (ESR1) gene mutations at the single circulating tumor cell (CTC) level in metastatic breast cancer (MBC). Experimental Design: We used a CTC molecular characterization approach to investigate heterogeneity of 14 hotspot mutations in ESR1 and their correlation with endocrine resistance. Combining the CellSearch and DEPArray technologies allowed recovery of 71 single CTCs and 12 WBC from 3 ER-positive MBC patients. Forty CTCs and 12 WBC were subjected to whole genome amplification by MALBAC and Sanger sequencing. Results: Among 3 selected patients, 2 had an ESR1 mutation (Y537). One showed two different ESR1 variants in a single CTC and another showed loss of heterozygosity. All mutations were detected in matched cell-free DNA (cfDNA). Furthermore, one had 2 serial blood samples analyzed and showed changes in both cfDNA and CTCs with emergence of mutations in ESR1 (Y537S and T570I), which has not been reported previously. Conclusions: CTCs are easily accessible biomarkers to monitor and better personalize management of patients with previously demonstrated ER-MBC who are progressing on endocrine therapy. We showed that single CTC analysis can yield important information on clonal heterogeneity and can be a source of discovery of novel and potential driver mutations. Finally, we also validate a workflow for liquid biopsy that will facilitate early detection of ESR1 mutations, the emergence of endocrine resistance and the choice of further target therapy. ©2017 AACR

    Follow-up visits in doctor-patient communication: the Vietnamese case

    In a 'follow-up visit', a patient seeks medical attention for an existing health problem. Using data from the Vietnamese public hospital system, we present a more nuanced analysis of follow-ups in health communication than the one currently available. To be specific, we discriminate between 'same follow-ups', in which the doctor is the same one as in the last visit, and 'different follow-ups', in which the doctor has not treated the patient for their problem before. We then extend existing research on 'inappropriate follow-ups', in which the problem solicitation is more suitable for another type of visit, by teasing out additional typological distinctions within this category of follow-up. We go on to show that same and different follow-ups contrast with each other in terms of the format used for the problem solicitation. The broader implication of our findings is that the structure of a medical visit is not invariant, but is shaped by the cultural context in which it occurs

    Trade-offs in absorption and scattering by nanophotonic structures

    Trade-offs between feasible absorption and scattering cross sections of obstacles confined to an arbitrarily shaped volume are formulated as a multi-objective optimization problem solvable by Lagrangian-dual methods. Solutions to this optimization problem yield a Pareto-optimal set, the shape of which reveals the feasibility of achieving simultaneously extremal absorption and scattering. Two forms of the trade-off problems are considered involving both loss and reactive material parameters. Numerical comparisons between the derived multi-objective bounds and several classes of realized structures are made. Additionally, low-frequency (electrically small, long wavelength) limits are examined for certain special cases.Comment: 16 pages, 9 figure

    The diagnostic validity and reliability of an internet-based clinical assessment program for mental disorders

    BACKGROUND: Internet-based assessment has the potential to assist with the diagnosis of mental health disorders and overcome the barriers associated with traditional services (eg, cost, stigma, distance). Further to existing online screening programs available, there is an opportunity to deliver more comprehensive and accurate diagnostic tools to supplement the assessment and treatment of mental health disorders. OBJECTIVE: The aim was to evaluate the diagnostic criterion validity and test-retest reliability of the electronic Psychological Assessment System (e-PASS), an online, self-report, multidisorder, clinical assessment and referral system. METHODS: Participants were 616 adults residing in Australia, recruited online, and representing prospective e-PASS users. Following e-PASS completion, 158 participants underwent a telephone-administered structured clinical interview and 39 participants repeated the e-PASS within 25 days of initial completion. RESULTS: With structured clinical interview results serving as the gold standard, diagnostic agreement with the e-PASS varied considerably from fair (eg, generalized anxiety disorder: &kappa;=.37) to strong (eg, panic disorder: &kappa;=.62). Although the e-PASS\u27 sensitivity also varied (0.43-0.86) the specificity was generally high (0.68-1.00). The e-PASS sensitivity generally improved when reducing the e-PASS threshold to a subclinical result. Test-retest reliability ranged from moderate (eg, specific phobia: &kappa;=.54) to substantial (eg, bulimia nervosa: &kappa;=.87). CONCLUSIONS: The e-PASS produces reliable diagnostic results and performs generally well in excluding mental disorders, although at the expense of sensitivity. For screening purposes, the e-PASS subclinical result generally appears better than a clinical result as a diagnostic indicator. Further development and evaluation is needed to support the use of online diagnostic assessment programs for mental disorders. <br /
