The Delphi method: methodological issues arising from a study examining factors influencing the publication or non-publication of mental health nursing research
Purpose â The paper describes how the classic Delphi method can be adapted and structured to ensure that specific research questions are clearly addressed.
Design/methodology/approach â As part of a larger mixed method project, a modified Delphi study was undertaken to explore factors influencing publication and non-publication of mental health nursing research.
Findings - This paper reports brief findings from the Delphi study. However, its main focus is the methodological issues arising from the Delphi method.
Implications - The paper argues that the classic Delphi method can be adapted and structured to ensure that specific research questions are able to be clearly answered. The adaptations are pragmatic in approach and in keeping with the general principles underpinning the Delphi method, while successfully addressing the problems of attrition and previous criticism of homogenous panels.
Originality/value - This paper offers some practical solutions to issue arising from undertaking research using the Delphi method
Coronary atherectomy: report of the first experience in Hawaii.
Since Andreas Gruentzig first introduced percutaneous transluminal coronary angioplasty (PTCA) in 1977, the ability to revascularize occluded coronary vessels with a catheter has enjoyed an explosive and unimaginable growth. As the equipment and operator experience improved, the possibilities appeared boundless. However, balloon angioplasty is hampered by a significant restenosis rate in the dilated vessel (approximately 30%), which is higher in selected locations (up to 60% in the proximal left anterior descending artery), even in the best of hands. This fundamental limitation may in part be due to the actual nature of the technique itself--stretching the vessel and fissuring the plaque causing remodeling without removal. The uneven, exposed vessel surface post-plaque rupture may contribute to activation of the hemostatic system, with acute thrombosis and release of various platelet and endothelial-derived growth factors, leading to long-term tissue proliferation and restenosis. Atherectomy, the mechanical removal of plaque from the vessel wall, appears to be an answer. This process actually debulks the culprit tissue and leaves behind a smoother, presumably less thrombogenic surface. We wish to report our first experience with a specific form of this technique in 4 consecutive patients, with a brief discussion of its promises and limitations

Identifying malnutrition in emergency general surgery:systematic review
Background: Emergency general surgery practice is high risk. Surgery is a key part of treatment, with resultant catabolic stress and frequent need for nutritional support. The aim of this study was to examine the current methods of defining and determining malnutrition in emergency general surgery. This included examining the use of nutrition screening and assessment tools and other measures of malnutrition.Methods: MEDLINE, Embase, Cumulative Index to Nursing and Allied Health Literature, trial registries, and relevant journals published between January 2000 and January 2022 were searched for studies of adult patients with any emergency general surgery diagnosis, managed conservatively or operatively, with an assessment of nutritional status. Mixed populations were included if more than 50 per cent of patients were emergency general surgery patients or emergency general surgery results could be separately extracted. Studies in which patients had received nutritional support were excluded. The protocol was registered with PROSPERO, the international prospective register of systematic reviews (CRD42021285897).Results: From 6700 studies screened, 324 full texts were retrieved and 31 were included in the analysis. A definition of malnutrition was provided in 23 studies (75 per cent), with nutritional status being determined by a variety of methods. A total of seven nutrition screening tools and a total of nine âassessmentâ tools were reported. To define malnutrition, the most commonly used primary or secondary marker of nutritional status was BMI, followed by albumin level.Conclusion: Wide variation exists in approaches to identify malnutrition risk in emergency general surgery patients, using a range of tools and nutrition markers. Future studies should seek to standardize nutrition screening and assessment in the emergency general surgery setting as two discrete processes. This will permit better understanding of malnutrition risk in surgical patients
Global assessment of nitrogen deposition effects on terrestrial plant diversity : a synthesis
Atmospheric nitrogen (N) deposition is it recognized threat to plant diversity ill temperate and northern parts of Europe and North America. This paper assesses evidence from field experiments for N deposition effects and thresholds for terrestrial plant diversity protection across a latitudinal range of main categories of ecosystems. from arctic and boreal systems to tropical forests. Current thinking on the mechanisms of N deposition effects on plant diversity, the global distribution of G200 ecoregions, and current and future (2030) estimates of atmospheric N-deposition rates are then used to identify the risks to plant diversity in all major ecosystem types now and in the future. This synthesis paper clearly shows that N accumulation is the main driver of changes to species composition across the whole range of different ecosystem types by driving the competitive interactions that lead to composition change and/or making conditions unfavorable for some species. Other effects such its direct toxicity of nitrogen gases and aerosols long-term negative effects of increased ammonium and ammonia availability, soil-mediated effects of acidification, and secondary stress and disturbance are more ecosystem, and site-specific and often play a supporting role. N deposition effects in mediterranean ecosystems have now been identified, leading to a first estimate of an effect threshold. Importantly, ecosystems thought of as not N limited, such as tropical and subtropical systems, may be more vulnerable in the regeneration phase. in situations where heterogeneity in N availability is reduced by atmospheric N deposition, on sandy soils, or in montane areas. Critical loads are effect thresholds for N deposition. and the critical load concept has helped European governments make progress toward reducing N loads on sensitive ecosystems. More needs to be done in Europe and North America. especially for the more sensitive ecosystem types. including several ecosystems of high conservation importance. The results of this assessment Show that the Vulnerable regions outside Europe and North America which have not received enough attention are ecoregions in eastern and Southern Asia (China, India), an important part of the mediterranean ecoregion (California, southern Europe). and in the coming decades several subtropical and tropical parts of Latin America and Africa. Reductions in plant diversity by increased atmospheric N deposition may be more widespread than first thought, and more targeted Studies are required in low background areas, especially in the G200 ecoregions
QED vertex form factors at two loops
We present the closed analytic expression of the form factors of the two-loop
QED vertex amplitude for on-shell electrons of finite mass and arbitrary
momentum transfer . The calculation is carried out within the
continuous -dimensional regularization scheme, with a single continuous
parameter , the dimension of the space-time, which regularizes at the same
time UltraViolet (UV) and InfraRed (IR) divergences. The results are expressed
in terms of 1-dimensional harmonic polylogarithms of maximum weight 4.Comment: 53 pages, 3 figure
Two-Loop Bhabha Scattering in QED
In the context of pure QED, we obtain analytic expressions for the
contributions to the Bhabha scattering differential cross section at order
alpha^4 which originate from the interference of two-loop photonic vertices
with tree-level diagrams and from the interference of one-loop photonic
diagrams amongst themselves. The ultraviolet renormalization is carried out.
The IR-divergent soft-photon emission corrections are evaluated and added to
the virtual cross section. The cross section obtained in this manner is valid
for on-shell electrons and positrons of finite mass, and for arbitrary values
of the center of mass energy and momentum transfer. We provide the expansion of
our results in powers of the electron mass, and we compare them with the
corresponding expansion of the complete order alpha^4 photonic cross section,
recently obtained in hep-ph/0501120. As a by-product, we obtain the
contribution to the Bhabha scattering differential cross section of the
interference of the two-loop photonic boxes with the tree-level diagrams, up to
terms suppressed by positive powers of the electron mass. We evaluate
numerically the various contributions to the cross section, paying particular
attention to the comparison between exact and expanded results.Comment: 35 pages, 18 figure
Master Integrals for the 2-loop QCD virtual corrections to the Forward-Backward Asymmetry
We present the Master Integrals needed for the calculation of the two-loop
QCD corrections to the forward-backward asymmetry of a quark-antiquark pair
produced in electron-positron annihilation events. The abelian diagrams
entering in the evaluation of the vector form factors were calculated in a
previous paper. We consider here the non-abelian diagrams and the diagrams
entering in the computation of the axial form factors, for arbitrary space-like
momentum transfer Q^2 and finite heavy quark mass m. Both the UV and IR
divergences are regularized in the continuous D-dimensional scheme. The Master
Integrals are Laurent-expanded around D=4 and evaluated by the differential
equation method; the coefficients of the expansions are expressed as

Decays of Scalar and Pseudoscalar Higgs Bosons into Fermions: Two-loop QCD Corrections to the Higgs-Quark-Antiquark Amplitude
As a first step in the aim of arriving at a differential description of
neutral Higgs boson decays into heavy quarks, , to second
order in the QCD coupling , we have computed the
amplitude at the two-loop level in QCD for a general neutral Higgs boson which
has both scalar and pseudoscalar couplings to quarks. This amplitude is given
in terms of a scalar and a pseudoscalar vertex form factor, for which we
present closed analytic expressions in terms of one-dimensional harmonic
polylogarithms of maximum weight 4. The results hold for arbitrary
four-momentum squared, , of the Higgs boson and of the heavy quark mass,
. Moreover we derive the approximate expressions of these form factors near
