    Self-interference cancellation in underwater acoustic communications systems using orthogonal pilots in IBFD

    This paper proposes a Self-interference (SI) cancellation system model of Underwater acoustic (UWA) communication for in-band full-duplex (IBFD) technology. The SI channel is separated from the Far channel by exploiting a concurrently orthogonal pilot channel estimation technique using two orthogonal frequency-division multiplexing (OFDM) blocks to establish orthogonality between them based on a unitary matrix. Compared to the half-duplex channel estimator, the mean squared error (MSE) and the bit error rate (BER) provided strong evidence for the efficiency of the proposed SI cancellation. Since full-duplex systems are more efficient than half-duplex ones, the proposed approach might be seen as a viable option for them. The proposed method proved effective when used with a fixed full-duplex (FD) position and FD shifting of up to 4°. Different channel lengths and distances are adopted to evaluate the proposed method. Initial findings indicate that MSE for the SI channel minimum mean-square error (MMSE) estimator at 20 dB is 0.118 · 10−3, for fixed FD. In addition, this paper presents a geometry channel model for the Far channel in the IBFD underwater communication system that describes the propagation delay of the multipath reflection. The simulation results for the multipath propagation delay spread are similar to the traditional results, with the delay spread of the suggested model reaching (79 ms), which is close to the Bellhop simulator result (78 ms)

    Time Delay Estimation in Mobile Sensors for Underwater Networking

    The time synchronization between any two sensor nodes in an Ad-hoc Underwater Sensor Networks (UWSNs) could be destroyed due to motion of these wireless sensors which induced Doppler shift. This synchronization obstacle can be sorted out by exploiting the mobility between sensor nodes. In the proposed system, the time delay between sensor nodes in both divergence and convergence scenarios are estimated based on estimating the time scaling factor. An improvement is introduced in terms of packet structure in order to challenge the channel effect and accurate estimation over the speed up to ±2 m/s. To verify the proposed system robustness, different levels of the nodes speeds have been considered in the simulation.     Design and Implementation of an Embedded System for Software Defined Radio

    In this paper, developing high performance software for demanding real-time embedded systems is proposed. This software-based design will enable the software engineers and system architects in emerging technology areas like 5G Wireless and Software Defined Networking (SDN) to build their algorithms. An ADSP-21364 floating point SHARC Digital Signal Processor (DSP) running at 333 MHz is adopted as a platform for an embedded system. To evaluate the proposed embedded system, an implementation of frame, symbol and carrier phase synchronization is presented as an application. Its performance is investigated with an on line Quadrature Phase Shift keying (QPSK) receiver. Obtained results show that the designed software is implemented successfully based on the SHARC DSP which can utilized efficiently for such algorithms. In addition, it is proven that the proposed embedded system is pragmatic and capable of dealing with the memory constraints and critical time issue due to a long length interleaved coded data utilized for channel coding

    Global economic burden of unmet surgical need for appendicitis

    Background: There is a substantial gap in provision of adequate surgical care in many low-and middle-income countries. This study aimed to identify the economic burden of unmet surgical need for the common condition of appendicitis. Methods: Data on the incidence of appendicitis from 170 countries and two different approaches were used to estimate numbers of patients who do not receive surgery: as a fixed proportion of the total unmet surgical need per country (approach 1); and based on country income status (approach 2). Indirect costs with current levels of access and local quality, and those if quality were at the standards of high-income countries, were estimated. A human capital approach was applied, focusing on the economic burden resulting from premature death and absenteeism. Results: Excess mortality was 4185 per 100 000 cases of appendicitis using approach 1 and 3448 per 100 000 using approach 2. The economic burden of continuing current levels of access and local quality was US 92492millionusingapproach1and92 492 million using approach 1 and 73 141 million using approach 2. The economic burden of not providing surgical care to the standards of high-income countries was 95004millionusingapproach1and95 004 million using approach 1 and 75 666 million using approach 2. The largest share of these costs resulted from premature death (97.7 per cent) and lack of access (97.0 per cent) in contrast to lack of quality. Conclusion: For a comparatively non-complex emergency condition such as appendicitis, increasing access to care should be prioritized. Although improving quality of care should not be neglected, increasing provision of care at current standards could reduce societal costs substantially

    Laparoscopy in management of appendicitis in high-, middle-, and low-income countries: a multicenter, prospective, cohort study.

    BACKGROUND: Appendicitis is the most common abdominal surgical emergency worldwide. Differences between high- and low-income settings in the availability of laparoscopic appendectomy, alternative management choices, and outcomes are poorly described. The aim was to identify variation in surgical management and outcomes of appendicitis within low-, middle-, and high-Human Development Index (HDI) countries worldwide. METHODS: This is a multicenter, international prospective cohort study. Consecutive sampling of patients undergoing emergency appendectomy over 6 months was conducted. Follow-up lasted 30 days. RESULTS: 4546 patients from 52 countries underwent appendectomy (2499 high-, 1540 middle-, and 507 low-HDI groups). Surgical site infection (SSI) rates were higher in low-HDI (OR 2.57, 95% CI 1.33-4.99, p = 0.005) but not middle-HDI countries (OR 1.38, 95% CI 0.76-2.52, p = 0.291), compared with high-HDI countries after adjustment. A laparoscopic approach was common in high-HDI countries (1693/2499, 67.7%), but infrequent in low-HDI (41/507, 8.1%) and middle-HDI (132/1540, 8.6%) groups. After accounting for case-mix, laparoscopy was still associated with fewer overall complications (OR 0.55, 95% CI 0.42-0.71, p < 0.001) and SSIs (OR 0.22, 95% CI 0.14-0.33, p < 0.001). In propensity-score matched groups within low-/middle-HDI countries, laparoscopy was still associated with fewer overall complications (OR 0.23 95% CI 0.11-0.44) and SSI (OR 0.21 95% CI 0.09-0.45). CONCLUSION: A laparoscopic approach is associated with better outcomes and availability appears to differ by country HDI. Despite the profound clinical, operational, and financial barriers to its widespread introduction, laparoscopy could significantly improve outcomes for patients in low-resource environments.     Pooled analysis of WHO Surgical Safety Checklist use and mortality after emergency laparotomy

    Background The World Health Organization (WHO) Surgical Safety Checklist has fostered safe practice for 10 years, yet its place in emergency surgery has not been assessed on a global scale. The aim of this study was to evaluate reported checklist use in emergency settings and examine the relationship with perioperative mortality in patients who had emergency laparotomy. Methods In two multinational cohort studies, adults undergoing emergency laparotomy were compared with those having elective gastrointestinal surgery. Relationships between reported checklist use and mortality were determined using multivariable logistic regression and bootstrapped simulation. Results Of 12 296 patients included from 76 countries, 4843 underwent emergency laparotomy. After adjusting for patient and disease factors, checklist use before emergency laparotomy was more common in countries with a high Human Development Index (HDI) (2455 of 2741, 89.6 per cent) compared with that in countries with a middle (753 of 1242, 60.6 per cent; odds ratio (OR) 0.17, 95 per cent c.i. 0.14 to 0.21, P <0001) or low (363 of 860, 422 per cent; OR 008, 007 to 010, P <0.001) HDI. Checklist use was less common in elective surgery than for emergency laparotomy in high-HDI countries (risk difference -94 (95 per cent c.i. -11.9 to -6.9) per cent; P <0001), but the relationship was reversed in low-HDI countries (+121 (+7.0 to +173) per cent; P <0001). In multivariable models, checklist use was associated with a lower 30-day perioperative mortality (OR 0.60, 0.50 to 073; P <0.001). The greatest absolute benefit was seen for emergency surgery in low- and middle-HDI countries. Conclusion Checklist use in emergency laparotomy was associated with a significantly lower perioperative mortality rate.     Palladium Nanoparticles Loaded on Carbon Modified TiO2 Nanobelts for Enhanced Methanol Electrooxidation

Publication available at Springer via http://dx.doi.org/10.5101/nml.v5i3.p202-212Carbon modified TiO2 nanobelts (TiO2-C) were synthesized using a hydrothermal growth method, as a support material for palladium (Pd) nanoparticles (Pd/TiO2-C) to improve the electrocatalytic performance for methanol electrooxidation by comparison to Pd nanoparticles on bare TiO2 nanobelts (Pd/TiO2) and activated carbon (Pd/AC). Cyclic voltammetry characterization was conducted with respect to saturated calomel electrode (SCE) in an alkaline methanol solution, and the results indicate that the specific activity of Pd/TiO2-C is 2.2 times that of Pd/AC and 1.5 times that of Pd/TiO2. Chronoamperometry results revealed that the TiO2-C support was comparable in stability to activated carbon; but possesses an enhanced current density for methanol oxidation at a potential of -0.2 V vs. SCE.     SARS-CoV-2 vaccination modelling for safe surgery to save lives: data from an international prospective cohort study

    Background Preoperative SARS-CoV-2 vaccination could support safer elective surgery. Vaccine numbers are limited so this study aimed to inform their prioritization by modelling. Methods The primary outcome was the number needed to vaccinate (NNV) to prevent one COVID-19-related death in 1 year. NNVs were based on postoperative SARS-CoV-2 rates and mortality in an international cohort study (surgical patients), and community SARS-CoV-2 incidence and case fatality data (general population). NNV estimates were stratified by age (18–49, 50–69, 70 or more years) and type of surgery. Best- and worst-case scenarios were used to describe uncertainty. Results NNVs were more favourable in surgical patients than the general population. The most favourable NNVs were in patients aged 70 years or more needing cancer surgery (351; best case 196, worst case 816) or non-cancer surgery (733; best case 407, worst case 1664). Both exceeded the NNV in the general population (1840; best case 1196, worst case 3066). NNVs for surgical patients remained favourable at a range of SARS-CoV-2 incidence rates in sensitivity analysis modelling. Globally, prioritizing preoperative vaccination of patients needing elective surgery ahead of the general population could prevent an additional 58 687 (best case 115 007, worst case 20 177) COVID-19-related deaths in 1 year. Conclusion As global roll out of SARS-CoV-2 vaccination proceeds, patients needing elective surgery should be prioritized ahead of the general population

    The use of watershed geomorphic data in flash flood susceptibility zoning: a case study of the Karnaphuli and Sangu river basins of Bangladesh

    The occurrence of heavy rainfall in the south-eastern hilly region of Bangladesh makes this area highly susceptible to recurrent flash flooding. As the region is the commercial capital of Bangladesh, these flash floods pose a significant threat to the national economy. Predicting this type of flooding is a complex task which requires a detailed understanding of the river basin characteristics. This study evaluated the susceptibility of the region to flash floods emanating from within the Karnaphuli and Sangu river basins. Twenty-two morphometric parameters were used. The occurrence and impact of flash floods within these basins are mainly associated with the volume of runoff, runoff velocity, and the surface infiltration capacity of the various watersheds. Analysis showed that major parts of the basin were susceptible to flash flooding events of a ‘moderate’-to-‘very high’ level of severity. The degree of susceptibility of ten of the watersheds was rated as ‘high’, and one was ‘very high’. The flash flood susceptibility map drawn from the analysis was used at the sub-district level to identify populated areas at risk. More than 80% of the total area of the 16 sub-districts were determined to have a ‘high’-to-‘very-high’-level flood susceptibility. The analysis noted that around 3.4 million people reside in flash flood-prone areas, therefore indicating the potential for loss of life and property. The study identified significant flash flood potential zones within a region of national importance, and exposure of the population to these events. Detailed analysis and display of flash flood susceptibility data at the sub-district level can enable the relevant organizations to improve watershed management practices and, as a consequence, alleviate future flood risk
