    The micro-minerals composition in serum of rabbits (Oryctolagus cuniculus) infected with Trypanosoma congolense

    Sixteen (16) rabbits aged between 6 to 12 months were infected with fresh stock of Trypanosoma congolense (Gboko strain) intravenously at the rate of 1.0 × 106ml. Animals were classified into two groups; groups A were infected, while group B served as uninfected controls. Samples from the infected and the uninfected controls showed a significant increase in the levels of sodium (Na+), calcium (Ca2+), phosphate (PO4 2-) and blood urea nitrogen (BUN) (P < 0.05) and a significant decline in the levels of potassium (K+) and bicarbonate (HCO3 2-) (P < 0.05). Therefore, the alterations in the compositions of these micro-minerals in the serum of rabbits may suggest that, they could have a role in the pathogenesis of trypanosomosis due to T. congolense infection.Key words: Rabbits, micro-minerals, Trypanosoma congolense, pathogenesis

    Quantitative and Qualitative Assessment of Phytochemicals in Methanolic Extracts of Hurricane Weed ( Phyllanthus amarus Schumach. &Thonn) Plant

    The phytocomponents of the leaf and root extracts of Phyllanthus amarus (Euphorbiaceae) were studied. The constituents of the leaves and roots were identified and quantified by using GC-MS. Result from the phytochemical analyses showed the presence of saponins, tannins, phenolics, anthocyanins, and glycosides in both leaves and root of the plant. Alkaloids and triterpenoids were, however only absent in the root of P. amarus but present in the leaves of the plant. Flavonoids, coumarins and steroids were absent in the leaves but present in the root part. The mean concentration of the phytochemicals investigated in leaves are 0.73\ub10.01%, 1.85\ub10.03%, 1.12\ub10.01%, 1.80\ub10.01%, 1.59\ub10.50%, 0.13\ub10.10%, and 0.86\ub10.01%, for saponins, tannins, phenolics, anthocyanins, alkaloids, triterpenoids, and glycosides respectively, while the mean concentration of the phytochemicals investigated in roots are 0.91\ub10.01%, 1.53\ub10.03%, 0.70\ub10.01%, 2.97\ub10.10%, 2.47\ub10.03%, 0.62\ub10.01%, 0.90\ub10.01% and 2.02\ub10.10% for saponins, tannins, phenolics, steroids, flavonoids, anthocyanins, glycosides and coumarins respectively. Furthermore, the GC-MS analysis of methanol extracts of the leaves and root of P. amarus revealed the presence of three medicinally important bioactive compounds, which are: 9-Octadecenoic acid which has percentage of abundance of 92.23% and 82.46% in leaves and roots of the plant respectively, followed by n-Hexadecanoic acid and Tetradecanoic acid with their corresponding percentage of abundance of 7.7% and 17.54% for leaves and root. These analytical results suggest the plantto possess a significant role in phyto-medicine. The importance of this plant was discussed in line with the role they play in ethnomedicinal life of the people

    Effect of changes over time in the performance of a customized SAPS-II model on the quality of care assessment

    Purpose: The aim of our study was to explore, using an innovative method, the effect of temporal changes in the mortality prediction performance of an existing model on the quality of care assessment. The prognostic model (rSAPS-II) was a recalibrated Simplified Acute Physiology Score-II model developed for very elderly Intensive Care Unit (ICU) patients. Methods: The study population comprised all 12,143 consecutive patients aged 80 years and older admitted between January 2004 and July 2009 to one of the ICUs of 21 Dutch hospitals. The prospective dataset was split into 30 equally sized consecutive subsets. Per subset, we measured the model's discrimination [area under the curve (AUC)], accuracy (Brier score), and standardized mortality ratio (SMR), both without and after repeated recalibration. All performance measures were considered to be stable if 1 without and after repeated recalibration for the year 2009. Results: For all subsets, the AUCs were stable, but the Brier scores and SMRs were not. The SMR was downtrending, achieving levels significantly below 1. Repeated recalibration rendered it stable again. The proportions of hospitals with SMR>1 and SMR <1 changed from 15 versus 85% to 35 versus 65%. Conclusions: Variability over time may markedly vary among different performance measures, and infrequent model recalibration can result in improper assessment of the quality of care in many hospitals.     Characterization of a fluvial aquifer at a range of depths and scales: the Triassic St Bees Sandstone Formation, Cumbria, UK

    Fluvial sedimentary successions represent porous media that host groundwater and geothermal resources. Additionally, they overlie crystalline rocks hosting nuclear waste repositories in rift settings. The permeability characteristics of an arenaceous fluvial succession, the Triassic St Bees Sandstone Formation in England (UK), are described, from core-plug to well-test scale up to ~1 km depth. Within such lithified successions, dissolution associated with the circulation of meteoric water results in increased permeability (K~10−1–100 m/day) to depths of at least 150 m below ground level (BGL) in aquifer systems that are subject to rapid groundwater circulation. Thus, contaminant transport is likely to occur at relatively high rates. In a deeper investigation (> 150 m depth), where the aquifer has not been subjected to rapid groundwater circulation, well-test-scale hydraulic conductivity is lower, decreasing from K~10−2 m/day at 150–400 m BGL to 10−3 m/day down-dip at ~1 km BGL, where the pore fluid is hypersaline. Here, pore-scale permeability becomes progressively dominant with increasing lithostatic load. Notably, this work investigates a sandstone aquifer of fluvial origin at investigation depths consistent with highly enthalpy geothermal reservoirs (~0.7–1.1 km). At such depths, intergranular flow dominates in unfaulted areas with only minor contribution by bedding plane fractures. However, extensional faults represent preferential flow pathways, due to presence of high connective open fractures. Therefore, such faults may (1) drive nuclear waste contaminants towards the highly permeable shallow (< 150 m BGL) zone of the aquifer, and (2) influence fluid recovery in geothermal fields

    Laparoscopy in management of appendicitis in high-, middle-, and low-income countries: a multicenter, prospective, cohort study.

    BACKGROUND: Appendicitis is the most common abdominal surgical emergency worldwide. Differences between high- and low-income settings in the availability of laparoscopic appendectomy, alternative management choices, and outcomes are poorly described. The aim was to identify variation in surgical management and outcomes of appendicitis within low-, middle-, and high-Human Development Index (HDI) countries worldwide. METHODS: This is a multicenter, international prospective cohort study. Consecutive sampling of patients undergoing emergency appendectomy over 6 months was conducted. Follow-up lasted 30 days. RESULTS: 4546 patients from 52 countries underwent appendectomy (2499 high-, 1540 middle-, and 507 low-HDI groups). Surgical site infection (SSI) rates were higher in low-HDI (OR 2.57, 95% CI 1.33-4.99, p = 0.005) but not middle-HDI countries (OR 1.38, 95% CI 0.76-2.52, p = 0.291), compared with high-HDI countries after adjustment. A laparoscopic approach was common in high-HDI countries (1693/2499, 67.7%), but infrequent in low-HDI (41/507, 8.1%) and middle-HDI (132/1540, 8.6%) groups. After accounting for case-mix, laparoscopy was still associated with fewer overall complications (OR 0.55, 95% CI 0.42-0.71, p < 0.001) and SSIs (OR 0.22, 95% CI 0.14-0.33, p < 0.001). In propensity-score matched groups within low-/middle-HDI countries, laparoscopy was still associated with fewer overall complications (OR 0.23 95% CI 0.11-0.44) and SSI (OR 0.21 95% CI 0.09-0.45). CONCLUSION: A laparoscopic approach is associated with better outcomes and availability appears to differ by country HDI. Despite the profound clinical, operational, and financial barriers to its widespread introduction, laparoscopy could significantly improve outcomes for patients in low-resource environments.     The dominant Anopheles vectors of human malaria in the Asia-Pacific region: occurrence data, distribution maps and bionomic précis

    <p>Abstract</p> <p>Background</p> <p>The final article in a series of three publications examining the global distribution of 41 dominant vector species (DVS) of malaria is presented here. The first publication examined the DVS from the Americas, with the second covering those species present in Africa, Europe and the Middle East. Here we discuss the 19 DVS of the Asian-Pacific region. This region experiences a high diversity of vector species, many occurring sympatrically, which, combined with the occurrence of a high number of species complexes and suspected species complexes, and behavioural plasticity of many of these major vectors, adds a level of entomological complexity not comparable elsewhere globally. To try and untangle the intricacy of the vectors of this region and to increase the effectiveness of vector control interventions, an understanding of the contemporary distribution of each species, combined with a synthesis of the current knowledge of their behaviour and ecology is needed.</p> <p>Results</p> <p>Expert opinion (EO) range maps, created with the most up-to-date expert knowledge of each DVS distribution, were combined with a contemporary database of occurrence data and a suite of open access, environmental and climatic variables. Using the Boosted Regression Tree (BRT) modelling method, distribution maps of each DVS were produced. The occurrence data were abstracted from the formal, published literature, plus other relevant sources, resulting in the collation of DVS occurrence at 10116 locations across 31 countries, of which 8853 were successfully geo-referenced and 7430 were resolved to spatial areas that could be included in the BRT model. A detailed summary of the information on the bionomics of each species and species complex is also presented.</p> <p>Conclusions</p> <p>This article concludes a project aimed to establish the contemporary global distribution of the DVS of malaria. The three articles produced are intended as a detailed reference for scientists continuing research into the aspects of taxonomy, biology and ecology relevant to species-specific vector control. This research is particularly relevant to help unravel the complicated taxonomic status, ecology and epidemiology of the vectors of the Asia-Pacific region. All the occurrence data, predictive maps and EO-shape files generated during the production of these publications will be made available in the public domain. We hope that this will encourage data sharing to improve future iterations of the distribution maps.</p
