    The use of TV series as authentic materials to develop pragmatic competence in EFL contexts: a systematic review

    La presente monografía es una revisión sistemática de la literatura (RSL) que tiene como objetivo revisar cualitativamente la literatura que utiliza series de televisión para desarrollar la competencia pragmática en contextos de inglés como lengua extranjera. Cuando se enseña en un contexto de inglés como lengua extranjera, el desarrollo de la competencia pragmática tiende a pasarse por alto en las aulas. Además, los estudiantes no tienen la necesidad real de usar el lenguaje fuera de clase ya que hay pocas oportunidades de enfrentar una situación comunicativa; por lo tanto, es necesario proponer una estrategia o herramienta que ayude a los educadores a desarrollar la competencia pragmática. En ese sentido, las series de televisión parecen ser un elemento de la realia que satisface estas necesidades debido a su vasta cantidad de comunicación auténtica. Por lo tanto, el presente documento propone una revisión sistemática de la literatura con el fin de revisar los estudios sobre el tema antes mencionado, de modo que sus resultados puedan ser recopilados, comparados y mostrados. Para seleccionar los archivos, se crearon algunos criterios de inclusión y exclusión. Posteriormente, los documentos fueron filtrados mediante una herramienta para valoración de la calidad de estudios y analizados en una matriz analítica. Una vez hecho el análisis, los trece documentos finales fueron divididos en dos capítulos de la monografía en los cuales fueron presentadas las implicaciones pedagógicas y pragmáticas.The present monographic document is a systematic literature review that aims to qualitatively revise the literature that uses TV series in order to develop pragmatic competence in EFL contexts. When teaching in an EFL context, the development of pragmatic competence tends to be overlooked in the classrooms. Additionally, students do not have the real need of using the language outside of class since there are few opportunities of facing a communicative situation; therefore, it is necessary to propose a strategy or tool that helps educators to develop pragmatic competence. In that sense, TV series appear to be an element of realia that fulfills these needs due to their vast amount of authentic communication. Hence, the present document proposes a systematic literature review (SLR) in order to revise studies on the aforementioned subject, so their outcomes can be compiled, compared, and displayed. In order to select the papers, some inclusion and exclusion criteria were created. Then, the documents were filtered through a tool for Study Quality Assessment and analyzed in an Analytical Matrix.     Protein Fold Recognition from Sequences using Convolutional and Recurrent Neural Networks

    The identification of a protein fold type from its amino acid sequence provides important insights about the protein 3D structure. In this paper, we propose a deep learning architecture that can process protein residue-level features to address the protein fold recognition task. Our neural network model combines 1D-convolutional layers with gated recurrent unit (GRU) layers. The GRU cells, as recurrent layers, cope with the processing issues associated to the highly variable protein sequence lengths and so extract a fold-related embedding of fixed size for each protein domain. These embeddings are then used to perform the pairwise fold recognition task, which is based on transferring the fold type of the most similar template structure. We compare our model with several template-based and deep learning-based methods from the state-of-the-art. The evaluation results over the well-known LINDAHL and SCOP_TEST sets,along with a proposed LINDAHL test set updated to SCOP 1.75, show that our embeddings perform significantly better than these methods, specially at the fold level.     Efficacy and Safety of Pegylated Interferon plus Ribavirin in HIV and Hepatitis C Virus–Coinfected Patients with Advanced Immunosuppression

    Background. The aim of this study was to assess the efficacy and safety of pegylated interferon (IFN) plus ribavirin (RBV) in human immunodeficiency virus (HIV) and hepatitis C virus (HCV)–coinfected patients with severe immunodeficiency in a clinical cohort. Methods. A total of 542 HIV-infected patients receiving treatment with pegylated IFN plus RBV from June 2001 through April 2007 were included in this study. The outcome variables were sustained virologic response (SVR) rate and the emergence of AIDS-defining events during HCV infection therapy. SVR rates among patients with a CD4 cell count 250 cells/mm3 at baseline were compared with those among patients with CD4 cell counts 1250 cells/mm3. The association between SVR and potential predictors was analyzed. Results. Ten (26%) of 39 individuals with a baseline CD4 cell count 250 cells/mm3 and 198 (39%) of 503 with baseline CD4 cell counts 1250 CD4 cells/mm3 achieved SVR (Pp.09). In a nested case-control study with populations matched at a 1:2 ratio, the SVR rate was 26% in the CD4 cell count 250 cells/mm3 group and 32% in the CD4 cell count 1250 cells/mm3 group (Pp.5). Baseline CD4 cell count ( 250 cells/mm3 vs 1250 cells/ mm3) was not associated with SVR in the multivariate analysis. Two (5%) individuals in the CD4 cell count 250 cells/mm3 group experienced opportunistic events during follow-up. In the CD4 cell count 250 cells/mm3 group, severe hematological toxicity and pegylated IFN or RBV dosage reductions occurred in 16 (41%) and 12 (31%) patients, respectively. In the CD4 cell count 1250 cells/mm3 group, severe hematological toxicity and pegylated IFN or RBV dosage reductions occurred in 29% (Pp.1) and 20% (Pp.1) of patients, respectively. Conclusions. The efficacy of pegylated IFN plus RBV in HIV-HCV–coinfected patients with advanced immunosuppression is substantial and not significantly different to that observed in the overall coinfected population. HCV therapy is generally safe in the population of coinfected patients with advanced immunosuppression

    Caracterização de cardiopatias congênitas em Manizales, Colômbia, 2010-2016

    Congenital heart defects are the third cause of congenital malformations and one of the main causes of mortality in the neonatal period in the world; they originate from alterations in embryonic development. The incidence is variable; in Colombia, it is estimated that they affect 2 to 3% of live newborns. In 2015, 20.8% of children under the age of five affected with congenital heart defects died from these disorders. This is a descriptive and retrospective study. Data of 252 clinical records of patients diagnosed with congenital heart defect between 2010 and 2016 were included, showing that 56% of this population were female. Acyanotic Congenital Heart Diseases predominated (87%). The highest prevalence was patent ductus arteriosus (35.3%). Doppler echocardiography was used in 98.4% of cases; regarding management, 47.2% of patients were in follow-up, this being more common than medical or surgical alternatives. In conclusion patent ductus arteriosus, ventricular septal defect and interatrial septal defect are the most frequent acyanotic heart diseases in the population under study and they are associated with pulmonary hypertension that require higher follow-up. Tetralogy of Fallot was the most common cyanotic congenital heart disease.Las cardiopatías congénitas son la tercera causa de malformaciones congénitas y una de las principales responsables de la mortalidad en periodo neonatal en el mundo; se originan a partir de alteraciones en el desarrollo embrionario. La incidencia es variable; en Colombia, se estima que afectan del 2 al 3 % de los recién nacidos vivos. En 2015, 20,8 % de los niños menores de cinco años afectados con cardiopatías congénitas fallecieron a causa de estas alteraciones. El presente es un estudio descriptivo y retrospectivo. Se incluyeron los datos de 252 historias clínicas de pacientes con diagnóstico de cardiopatía congénita entre los años 2010 y 2016, el 56 % de la población encontrada fue de sexo femenino. Predominaron las cardiopatías congénitas no cianosantes (87 %). La mayor revalencia fue el ductus arterioso permeable (35,3 %). La ecocardiografía Doppler se utilizó en 98,4 % de los casos; con respecto al manejo, el 47,2 % de los pacientes estuvieron en seguimiento, siendo este más común que las alternativas médicas o quirúrgicas. En conclusión, el ductus arterioso permeable, la comunicación interventricular y la comunicación interatrial son las cardiopatías no cianosantes más frecuentes en la población estudiada y se asocian a hipertensión pulmonar que ameritan mayor seguimiento. La tetralogía de Fallot fue la cardiopatía congénita cianosante más común.As cardiopatias congênitas são a terceira causa de malformações congênitas e uma das principais responsáveis pela mortalidade no período neonatal no mundo. Em 2015, 20,8 % das crianças menores de cinco anos afetadas com cardiopatias congênitas faleceram por causa dessas alterações. Este estudo é descritivo e retrospectivo. Foram incluídos dados de 252 prontuários de pacientes com diagnóstico de cardiopatia congênita entre 2010 e 2016, 56 % da população encontrada foi do sexo feminino. Predominaram as cardiopatias congênitas não  ianóticas (87 %). A maior persistência foi o ducto arterioso permeável (35,3 %). A ecocardiografia Doppler foi utilizada em 98,4 % dos casos; quanto ao manejo, 47,2 % dos pacientes estiveram em seguimento, o que foi mais comum do que as alternativas médicas ou cirúrgicas. Em conclusão, o ducto arterioso permeável, a comunicação interventricular e a comunicação interatrial são as cardiopatias não cianóticas mais frequentes na população estudada e são associadas com a hipertensão pulmonar que merecem maior seguimento.     Inter-Rater Variability in the Evaluation of Lung Ultrasound in Videos Acquired from COVID-19 Patients

    12 páginas, 7 figuras, 1 tablaLung ultrasound (LUS) allows for the detection of a series of manifestations of COVID-19, such as B-lines and consolidations. The objective of this work was to study the inter-rater reliability (IRR) when detecting signs associated with COVID-19 in the LUS, as well as the performance of the test in a longitudinal or transverse orientation. Thirty-three physicians with advanced experience in LUS independently evaluated ultrasound videos previously acquired using the ULTRACOV system on 20 patients with confirmed COVID-19. For each patient, 24 videos of 3 s were acquired (using 12 positions with the probe in longitudinal and transverse orientations). The physicians had no information about the patients or other previous evaluations. The score assigned to each acquisition followed the convention applied in previous studies. A substantial IRR was found in the cases of normal LUS (κ = 0.74), with only a fair IRR for the presence of individual B-lines (κ = 0.36) and for confluent B-lines occupying 50% (κ = 0.50). No statistically significant differences between the longitudinal and transverse scans were found.     Contribution of cardio-vascular risk factors to depressive status in the PREDIMED-PLUS Trial. A cross-sectional and a 2-year longitudinal study

    Background Cardio-vascular disease and depression are thought to be closely related, due to shared risk factors. The aim of the study was to determine the association between cardio-vascular risk (CVR) factors and depressive status in a population (55-75 years) with metabolic syndrome (MetS) from the PREDIMED-Plus trial. Methods and findings Participants were classified into three groups of CVR according to the Framingham-based REGICOR function: (1) low (LR), (2) medium (MR) or (3) high/very high (HR). The Beck Depression Inventory-II (BDI-II) was used to assess depressive symptoms at baseline and after 2 years. The association between CVR and depressive status at baseline (n = 6545), and their changes after 2 years (n = 4566) were evaluated through multivariable regression models (logistic and linear models). HR women showed higher odds of depressive status than LR [OR (95% CI) = 1.78 (1.26, 2.50)]. MR and HR participants with total cholesterol <160 mg/mL showed higher odds of depression than LR [OR (95% CI) = 1.77 (1.13, 2.77) and 2.83 (1.25, 6.42) respectively)] but those with total cholesterol ¿280 mg/mL showed lower odds of depression than LR [OR (95% CI) = 0.26 (0.07, 0.98) and 0.23 (0.05, 0.95), respectively]. All participants decreased their BDI-II score after 2 years, being the decrease smaller in MR and HR diabetic compared to LR [adjusted mean±SE = -0.52±0.20, -0.41 ±0.27 and -1.25±0.31 respectively). MR and HR participants with total cholesterol between 240-279 mg/mL showed greater decreases in the BDI-II score compared to LR (adjusted mean±SE = -0.83±0.37, -0.77±0.64 and 0.97±0.52 respectively). Conclusions Improving cardiovascular health could prevent the onset of depression in the elderly.     Dietary diversity and depression: cross-sectional and longitudinal analyses in Spanish adult population with metabolic syndrome. Findings from PREDIMED-Plus trial

    Objective: To examine the cross-sectional and longitudinal (2-year follow-up) associations between dietary diversity (DD) and depressive symptoms. Design: An energy-adjusted dietary diversity score (DDS) was assessed using a validated FFQ and was categorised into quartiles (Q). The variety in each food group was classified into four categories of diversity (C). Depressive symptoms were assessed with Beck Depression Inventory-II (Beck II) questionnaire and depression cases defined as physician-diagnosed or Beck II >= 18. Linear and logistic regression models were used. Setting: Spanish older adults with metabolic syndrome (MetS). Participants: A total of 6625 adults aged 55–75 years from the PREDIMED-Plus study with overweight or obesity and MetS. Results: Total DDS was inversely and statistically significantly associated with depression in the cross-sectional analysis conducted; OR Q4 v. Q1 = 0·76 (95 % CI (0·64, 0·90)). This was driven by high diversity compared to low diversity (C3 v. C1) of vegetables (OR = 0·75, 95 % CI (0·57, 0·93)), cereals (OR = 0·72 (95 % CI (0·56, 0·94)) and proteins (OR = 0·27, 95 % CI (0·11, 0·62)). In the longitudinal analysis, there was no significant association between the baseline DDS and changes in depressive symptoms after 2 years of follow-up, except for DD in vegetables C4 v. C1 = (β = 0·70, 95 % CI (0·05, 1·35)). Conclusions: According to our results, DD is inversely associated with depressive symptoms, but eating more diverse does not seem to reduce the risk of future depression.
