    Este artículo analiza las páginas webs de las principales compañías operantes en el sector de la energía (electricidad y gas) de los países más desarrollados. La situación actual del mercado es muy significativa. Por primera vez en la historia, las administraciones centrales de estos países han consentido liberalizar el mercado energético. Y, aunque este proceso avanza con gran lentitud, comienzan a notarse cambios que anticipan un proceso de concentración, a través de compras, fusiones y acuerdos operativos. En este panorama, juega un importante papel la imagen de las compañías y la comunicación con sus usuarios, y esto sucede también en los medios digitales. En este artículo estudiamos cómo las diferentes compañías utilizan su página web para reforzar su posicionamiento e imagen de cara a los consumidores y también para comenzar a incorporar servicios, buscando una mayor interactividad entre compañía y usuarios. Para realizar este artículo los autores han estudiado a fondo todas las páginas webs de las principales compañías del sector, en el curso de una investigación modalidad Artículo 83 LOU realizada recientemente por los autores en la Universidad Complutense de Madrid.This article analyzes the web pages of the major companies in the energy market (electricity and gas) of the more developed countries (EU and North America). The present situation of the market is very significant: For the first time, the Government of these countries have given permission to liberalize the power market. This process goes on very slowly but we can see now some changes that leads to a process of concentration, through acquisitions, merges and commercial agreements between the companies. In this situation, the image of the companies and the communication with the consumers plays an important role and this happens also in the digital media. In this article we study how the different companies uses their web page to reinforce their positioning, and image. And also how they start incorporating new services looking for a better interactivity between the company and the consumers.     Este artículo estudia desde un punto de vista cuantitativo y cualitativo la publicidad de las empresas del sector energético de nuestro país en un momento peculiar: justo antes de que la liberalización del sector provoque cambios en el panorama de los operadores del sector (compras, fusiones o uniones operativas) y en consecuencia de sus estrategias de comunicación persuasiva.El artículo enmarca la situación del sector energético en la Unión Europea y su posible evolución durante los próximos años, utilizando estudios de la Unión Europea, la Agencia Internacional de Energía y el Ministerio de Industria. A continuación, examina la inversión publicitaria del sector energético español, comparando sus diferentes peculiaridades y su evolución en los últimos años. Por último, analiza las campañas de las principales empresas anunciantes, profundizando en los valores emitidos y su posicionamiento competitivo ante los consumidores, tratando de obtener un espacio diferencial y definido, pero también congruente con la sensibilidad de la ciudadanía por el crecimiento sostenible. De esta forma, se estudian los cambios experimentados en estas campañas durante los últimos años, apreciando una marcada evolución desde las campañas ostentosas y corporativas hacia un tipo de comunicación basado en los registros emocionales y socioconscientes.This article studies -from a quantitative and qualitative point of view- the advertising of the companies of the Spanish energy market in a very interesting moment: just before the liberalization of this market brings about important changes in the structure of the companies (acquisitions, merges, operative unions…) and their strategies of persuasive commercial communication.First of all, this article observes the situation of the energy market in the European Union and its evolution, studying reports from the European Union, the International Agency of Energy and the Spanish Ministry of Industry. Next, it studies the global investment in advertising o the Spanish power sector, by comparing its different peculiarities and their evolution in the last years. And finally, this article analyzes the campaigns of the main companies, evaluating their core values and their competitive positioning, trying to obtain a differential space sharply defined, but also alligned with the philosophies of the sustainable growth. It also studies the changes in these campaigns during the last years, appreciating a strong evolution from the campaigns based in the corporate values towards a type of communication based in the emotional and socioconscients values

    La problemática abordada en el presente trabajo está relacionada con la salud mental de los adultos mayores y la poca información de nuestra sociedad sobre cómo mantener una mente activa y cuáles son las consecuencias de no hacerlo, además de las escazas soluciones que actualmente se les puede ofrecer. Dicha problemática se determinó mediante el análisis del usuario meta, a través de la metodología de design thinking, la cual nos permitió plantear soluciones a esta necesidad con la creación de la aplicación digital “Abueléate”. La propuesta que se planteó para solucionar esta problemática fue promover a través de juegos virtuales, el bienestar y salud mental de los adultos mayores, lo cual va a permitir mejorar su calidad de vida, compartir nuevas experiencias en el proceso y afianzar sus lazos familiares. Los resultados se sustentan debido a una gran escalabilidad del negocio por la proyección de incremento mundial del 100% de la población del adulto mayor hacia el año 2050. Además, debido a que, en el Perú, una de las principales deficiencias en el sistema de Salud es la falta de atención y seguimiento en el tratamiento del deterioro de la salud mental del adulto mayor, la propuesta impacta positivamente en los Objetivos de Desarrollo Sostenible (ODS) N°3, además de las otras ODS N°8 y N°9. El promedio de la relación entre el VTVC y el CAC en los escenarios planteados es de 3.91. Esta cifra es un indicador positivo y significativo, ya que implica que el plan de marketing diseñado generará más ingresos que pérdidas a lo largo de los 5 años de operación. Finalmente, este modelo de negocio espera alcanzar en cinco años un VAN Social de S/ 5,407,871. Por ello, la implementación de este negocio está recomendada por ser de triple impacto.The problem addressed in this work is related to the mental health of older adults and the lack of information in our society about how to keep an active mind and the consequences of not doing so, as well as the scarce solutions that can currently be offered to them. This problem was determined through the analysis of the target user, through the design thinking methodology, which allowed us to propose solutions to this need with the creation of the digital application "Abueléate". The proposal put forward to solve this problem was to promote the well-being and mental health of older adults through virtual games, which will allow them to improve their quality of life, share new experiences in the process and strengthen their family ties. As part of the evaluation of the proposal, it was verified that the business model is viable and profitable through the financial viability evaluation, which determined an NPV of 864,828 and a rate of return with IRR 158.23%. The results are supported by the high scalability of the business due to the projected global increase of 100% of the elderly population by the year 2050. Furthermore, given that in Peru, one of the main deficiencies in the health system is the lack of attention and follow-up in the treatment of the deterioration of the mental health of the elderly, the proposal has a positive impact on the Sustainable Development Goals (SDG) N°3, in addition to the other SDGs N°8 and N°9. The average VTVC to CAC ratio in the scenarios is 3.91. This figure is a positive and significant indicator, as it implies that the marketing plan designed will generate more income than losses over the 5 years of operation. Finally, this business model expects to achieve a Social NPV of S/. 5,407,871 in five years. Therefore, the implementation of this business is recommended as it has a triple impact

    Étude du marché du travail des diplômés de Médicine Familière à Tabasco, Mexique

    The study of the labor market and the perception of the postgraduate studies of graduates from the specialty of family medicine of the Mexican Social Security Institute (IMSS) in Tabasco, allowed a cross-sectional descriptive investigation. From the universe of 201 graduates of the Specialty of Family Medicine, a survey sample of 103 graduates (51%) was determined.Among the results obtained, 59% of the graduates were between 30 and 39 years of age. The group of women predominated with 57%, compared to the group of men with 43%. The majority of the graduated doctors were married (57%), and the Juárez Autonomous University of Tabasco (UJAT) was found as the main university institution, with 66% provenance. 90% worked with clinical care functions in their current job and 96% were very satisfied with the training received. 48% consider that the knowledge acquired has served them to a high degree for professional performance, with the profile of graduates mostly married women between 30 and 39 years of age, coming from the UJAT degree in Surgery, labor-related to the health sector in clinical-care operative activities and currently satisfied with the work they carry out and with the academic training they received.El estudio del mercado laboral y la percepción sobre los estudios de posgrado de los egresados de la especialidad de medicina familiar del Instituto Mexicano del Seguro Social (IMSS) en Tabasco permitió una investigación de tipo descriptivo, transversal. El universo fueron 201 egresados de la Especialidad de Medicina Familiar, Se determinó una muestra a encuestar de 103egresados (51%).Dentro de los resultados obtenidos el 59% de los egresados tuvieron entre 30 y 39 años de edad. El grupo de las mujeres predominó con un 57%, en relación al grupo de los hombres que fue de 43 %. La mayoría de los médicos egresados estaban casados 57%, y se encontró a la Universidad Juárez Autónoma de Tabasco (UJAT) como principal institución universitaria de procedencia 66%. El 90% se desempeñaron con funciones clínica asistenciales en su empleo actual y el 96% estuvieron muy satisfechos con la formación recibida. El 48% considera que los conocimientos adquiridos le han servido en un alto grado para el desempeño profesional, siendo el perfil de los egresados en su mayoría mujerescasadas entre 30 y 39 años, procedentes de la licenciatura de médico cirujanode la UJAT, laboralmente vinculadas al sector salud en actividades operativasclínico-asistenciales y actualmente satisfechas con el trabajo que realizan y con la formación académica recibida.L’étude du marché du travail et la perception des études du troisième cycle des diplômés de la spécialité de Médecine Familiale de l’Institut Mexicain de SécuritéSociale (IMSS) à Tabasco, ont permis de mener une recherche descriptive transversale.     Spirometry and respiratory oscillometry: Feasibility and concordance in schoolchildren with asthma

    Producción CientíficaObjective:The purpose of this study was to describe the feasibility of respiratory oscillometry (RO) in schoolchildren with asthma, and the concordance of its results with those of spirometry, to determine its clinical usefulness. Methods:RO and spirometry were performed in 154 children (6 to 14-year-old) with asthma, following strict quality criteria for the tests. Their feasibility (probability of valid test, time of execution, number of maneuvers needed to achieve a valid test, and perceived difficulty) was compared. The factors that influence feasibility were analyzed with multivariate methods. FEV1, FEV1/FVC, FVC and FEF25-75 for spirometry, and R5, AX and R5-19 for RO, were converted into z-scores and their concordance was investigated through intraclass correlation coefficients (ICC) and kappa indices for normal/abnormal values. Results:There were no differences in the probability of obtaining a valid RO or spirometry (83.1% vs. 81.8%, p = 0.868). RO required a lower number of maneuvers [mean (SD) 4.2 (1.8) versus 6.0 (1.6), p < 0.001] and less execution time [5.1 (2.7) versus 7.6 (2.4) minutes, p < 0.001], and patients considered it less difficult. Age increased the probability of obtaining valid RO and spirometry. The concordance of results between RO and spirometry was low, and only between zFEV1 and zAX could it be considered moderate (ICC = 0.412, kappa = 0.427). Conclusion:RO and spirometry are feasible in children with asthma.     A Clustering Study of Sociodemographic Data, Dietary Patterns, and Gut Microbiota in Healthy and Breast Cancer Women Participating in the MICROMA Study

    Scope: This work is part of the clinical study NCT03885648 registered in ClinicalTrials.gov, aimed at studying the relationship among breast cancer, microbiota, and exposure to environmental pollutants. As a first step, we characterized and evaluated risk factors of the participants. Methods and results: A case–control study was designed with breast cancer (cases, n = 122) and healthy women (controls, n = 56) recruited in two hospitals of Andalusia (Southern Spain). Participants answered questionnaires of Mediterranean diet adherence and food frequency. Data were collected from medical histories and microbiota was analyzed on stool samples. Most cases (78.2%) were diagnosed as stages I and II. Cases had higher age, body mass index (BMI), glucose, cholesterol, and potassium values than controls. Cases exhibited higher adherence to the Mediterranean diet and their food consumption was closer to that dietary pattern. A hierarchical cluster analysis revealed that the Bacillota/Bacteroidota ratio was the most relevant variable in women with breast cancer, which was higher in this group compared with controls.     Relation between coliphages presence in drinking water, rain and acute diarrhea in Costa Rica

    La calidad del agua de consumo es una de las preocupaciones más grandes a las que se enfrentan los entes de salud. En Costa Rica, a pesar de la buena calidad de las fuentes de agua y del agua de consumo, se siguen registrando numerosos casos de diarrea anualmente. En este estudio se realizó un análisis de indicadores de contaminación virales y bacterianos, en dos comunidades del Valle Central con acueducto propio. Se analizaron en total 24 muestras de agua por cada comunidad, durante un año, 12 en la fuente de agua y 12 en el agua tratada. La frecuencia de diarreas en cada comunidad fue comparada con la calidad microbiológica del agua consumida, encontrándose ausencia de indicadores bacterianos en muestras de agua tratada, mientras que la presencia de indicadores virales en el agua de consumo (con cloro residual) se relaciona con un incremento en el número de casos de diarreas, adicionalmente se analizó el comportamiento de las diarreas de acuerdo a la precipitación promedio con el fin de observar posibles patrones de estacionalidad. Estos hallazgos respaldan la necesidad de ampliar el monitoreo de calidad microbiológica del agua con otro tipo de indicadores de contaminación que correlacionen mejor con otros agentes patógenos de transmisión hídrica, principalmente en época seca, periodo en que se observan más casos de diarrea.The quality of potable water is one of the most important issues of public health. In Costa Rica, in spite of having good sources of water and good quality of water for consumption, there continues to be a significant number of diarrhea cases annually. In this investigation, viral and bacterial analyses of indicators of contamination were conducted in two rural communities with their own aqueducts in the Central Valley of Costa Rica. Twenty - four samples were taken from each community during the year; 12 fro m the water sources and 12 from the treated waters. The frequency of diarrhea in each community was compared to the microbiological quality of the water being consumed, noting an absence of bacterial indicators in samples of treated water, while the presen ce of viral indicators in the water (with residual chlorine) were associated with an incremental increase in the number of cases of diarrhea. The analysis also included measurements of average monthly rainfall with the aim of observing possible association s between seasonal conditions and the incidence of diarrhea cases. These findings indicate the need to increase the monitoring of the microbiological quality of the water with other types of indicators of contamination that correlate better with other path ogens transmitted by water, especially in the dry seasons when a greater number of cases of diarrhea are observed.UCR::Vicerrectoría de Investigación::Unidades de Investigación::Ciencias de la Salud::Instituto de Investigaciones en Salud (INISA

    Clinical and ultrasound thyroid nodule characteristics and their association with cytological and histopathological outcomes: A retrospective multicenter study in high-resolution thyroid nodule clinics

    Introduction: Thyroid nodules are a common finding. A high-resolution thyroid nodule clinic (HR-TNC) condenses all tests required for the evaluation of thyroid nodules into a single appointment. We aimed to evaluate the clinical outcomes at HR-TNCs. Design and Methods: A retrospective cross-sectional multicenter study including data from four HR-TNCs in Spain. We evaluated fine-needle aspiration (FNA) indications and the association between clinical and ultrasound characteristics with cytological and histopathological outcomes. Results: A total of 2809 thyroid nodules were included; FNA was performed in 82.1%. Thyroid nodules that underwent FNA were more likely larger, isoechoic, with microcalcifications, and in younger subjects. The rate of nondiagnostic FNA was 4.3%. A solid component, irregular margins or microcalcifications, significantly increased the odds of Bethesda IV-V-VI (vs. Bethesda II). Irregular margins and a solid component were independently associated with increased odds of malignancy. Thyroid nodules <20 mm and ≥20–<40 mm had a 6.5-fold and 3.3-fold increased risk for malignancy respectively in comparison with those ≥40 mm. Conclusion: In this large multicenter study, we found that the presence of a solid component and irregular margins are factors independently related to malignancy in thyroid nodules. Since nodule size ≥40 mm was associated with the lowest odds of malignancy, this cut-off should not be a factor leading to indicate thyroid surgery.     Drug-refractory myasthenia gravis : Clinical characteristics, treatments, and outcome

This observational retrospective cross-sectional multicenter study was based on data from the Spanish MG Registry (NMD-ES). Patients were considered refractory when their MG Foundation of America post-interventional status (MGFA-PIS) was unchanged or worse after corticosteroids and two or more other immunosuppressive agents. Clinical and immunologic characteristics of drug-refractory patients, efficiency and toxicity of drugs used, and outcome (MGFA-PIS) at end of follow-up were studied. We included 990 patients from 15 hospitals. Eighty-four patients (68 of 842 anti-acetylcholine receptor [AChR], 5 of 26 anti-muscle-specific tyrosine kinase [MusK], 10 of 120 seronegative, and 1 of 2 double-seropositive patients) were drug refractory. Drug-refractory patients were more frequently women (p < 0.0001), younger at onset (p < 0.0001), and anti-MuSK positive (p = 0.037). Moreover, they more frequently presented a generalized form of the disease, bulbar symptoms, and life-threatening events (p < 0.0001; p = 0.018; and p = 0.002, respectively) than non-drug-refractory patients. Mean follow-up was 9.8 years (SD 4.5). Twenty-four (50%) refractory patients had side effects to one or more of the drugs. At the end of follow-up, 42.9% of drug-refractory patients (42.6% of anti-AChR, 100% of anti-MuSK, and 10% of seronegative patients) and 79.8% of non-drug-refractory patients (p < 0.0001) achieved remission or had minimal manifestations. Eighty percent of drug-refractory-seronegative patients did not respond to any drug tested. In this study, 8.5% of MG patients were drug-refractory. New more specific drugs are needed to treat drug-refractory MG patients
