    The article explores aspects of the notion of forms of life in the Wittgensteinian tradition especially following Iris Murdoch’s lead. On the one hand, the notion signals the hardness and inexhaustible character of reality, as the background needed in order to make sense of our lives in various ways. On the other, the hardness of reality is the object of a moral work of apprehension and deepening to the point at which its distinctive character dissolves into the family of connections we have gained for ourselves.     Spleen histology in children with sickle cell disease and hereditary spherocytosis: Hints on the disease pathophysiology

    open2Hereditary spherocytosis (HS) and sickle cell disease (SCD) are associated with splenomegaly and spleen dysfunction in pediatric patients. Scant data exist on possible correlations between spleen morphology and function in HS and SCD. This study aimed to assess the histological and morphometric features of HS and SCD spleens, in order to get possible correlations with disease pathophysiology. In a large series of spleens from SCD, HS and control patients the following parameters were considered: (i) macroscopic features; (ii) lymphoid follicle (LF) density; (iii) presence of peri-follicular marginal zones (MZs); (iv) presence of Gamna-Gandy bodies; (v) density of CD8-positive sinusoids; (vi) density of CD34-positive microvessels; (vii) presence/distribution of fibrosis and SMA-positive myoid cells; (viii) density of CD68-positive macrophages. SCD and HS spleens have similar macroscopic features. SCD spleens had lower LF density and fewer MZs than HS spleens and controls. SCD also showed lower CD8-positive sinusoid density, increased CD34-positive microvessel density and SMA-positive myoid cells, and higher prevalence of fibrosis and Gamna-Gandy bodies. HS had lower LF and CD8-positive sinusoid density than controls. No significant differences were noted in red pulp macrophages. By multivariate analysis, the majority of HS spleens clustered with controls, while SCD grouped separately. A multi-parametric score could predict the degree of spleen changes irrespective of the underlying disease. In conclusion, SCD spleens display greater histologic effacement than HS and SCD-related changes suggest impaired function due to vascular damage.     Clinical, radiologic, pathologic, and molecular characteristics of long-term survivors of diffuse intrinsic pontine glioma (DIPG): a collaborative report from the International and European Society for Pediatric Oncology DIPG registries

    Purpose Diffuse intrinsic pontine glioma (DIPG) is a brainstem malignancy with a median survival of < 1 year. The International and European Society for Pediatric Oncology DIPG Registries collaborated to compare clinical, radiologic, and histomolecular characteristics between short-term survivors (STSs) and long-term survivors (LTSs). Materials and Methods Data abstracted from registry databases included patients from North America, Australia, Germany, Austria, Switzerland, the Netherlands, Italy, France, the United Kingdom, and Croatia. Results Among 1,130 pediatric and young adults with radiographically confirmed DIPG, 122 (11%) were excluded. Of the 1,008 remaining patients, 101 (10%) were LTSs (survival ≥ 2 years). Median survival time was 11 months (interquartile range, 7.5 to 16 months), and 1-, 2-, 3-, 4-, and 5-year survival rates were 42.3% (95% CI, 38.1% to 44.1%), 9.6% (95% CI, 7.8% to 11.3%), 4.3% (95% CI, 3.2% to 5.8%), 3.2% (95% CI, 2.4% to 4.6%), and 2.2% (95% CI, 1.4% to 3.4%), respectively. LTSs, compared with STSs, more commonly presented at age < 3 or > 10 years (11% v 3% and 33% v 23%, respectively; P < .001) and with longer symptom duration ( P < .001). STSs, compared with LTSs, more commonly presented with cranial nerve palsy (83% v 73%, respectively; P = .008), ring enhancement (38% v 23%, respectively; P = .007), necrosis (42% v 26%, respectively; P = .009), and extrapontine extension (92% v 86%, respectively; P = .04). LTSs more commonly received systemic therapy at diagnosis (88% v 75% for STSs; P = .005). Biopsies and autopsies were performed in 299 patients (30%) and 77 patients (10%), respectively; 181 tumors (48%) were molecularly characterized. LTSs were more likely to harbor a HIST1H3B mutation (odds ratio, 1.28; 95% CI, 1.1 to 1.5; P = .002). Conclusion We report clinical, radiologic, and molecular factors that correlate with survival in children and young adults with DIPG, which are important for risk stratification in future clinical trials

    Transmission of natural scene images through a multimode fibre

    The optical transport of images through a multimode fibre remains an outstanding challenge with applications ranging from optical communications to neuro-imaging. State of the art approaches either involve measurement and control of the full complex field transmitted through the fibre or, more recently, training of artificial neural networks that however, are typically limited to image classes belong to the same class as the training data set. Here we implement a method that statistically reconstructs the inverse transformation matrix for the fibre. We demonstrate imaging at high frame rates, high resolutions and in full colour of natural scenes, thus demonstrating general-purpose imaging capability. Real-time imaging over long fibre lengths opens alternative routes to exploitation for example for secure communication systems, novel remote imaging devices, quantum state control processing and endoscopy

    Two Varieties of Moral Exemplarism

    References to moral exemplars run deep into the history of philosophy, as we find them featured in rather disparate context and approaches which span from virtue ethics to moral perfectionism, from existentialism to moral particularism. In the varied and growing contemporary literature on moral exemplarism, we find a number of options that can be brought down to the two rather broad yet distinctive categories of theoretical and anti-theoretical approaches. In the paper, I showcase and contrast these two varieties by taking the views of Zagzebski and Rorty as representative of, respectively, the reference to exemplars as most perfect beings to aspire to and get guidance from, and the use of them as next yet foreign beings to experiment with and get provocation from. Finally, I will draw some consequences for a conception of moral education hinged on unsettlement and transformation rather than on imitation and reproduction

    Changes in body composition and psychological profile when overcoming four Everesting bike challenges

    Problem Statement: During ultra-endurance races, given the long duration of the competitions, athletes can experience variations in body composition and moods. These elements can greatly affect the athlete's performance. Purpose: To evaluate the effects of an ultra-endurance race (4 consecutive Everesting Bike Challenges) on the body composition and moods of an adult athlete. Material and Methods: A well-trained amateur cyclist (male; 46 years; 64 kg; 1.69 cm; BMI 22.4 kg/m2) was monitored during the 4 Everesting Bike Challenges. This test is an ultra-endurance challenge that involves overcoming 8848 meters by climbing a single peak several times. The changes in body composition and hydration, calculated by bio-impedentiometry, and the changes in moods, obtained by administering the Profile of Mood States (POMS), in addition to Rating of Perceived Exertion (RPE) and Visual Analogic Scale, were measured at the beginning, during or at the end of each Everesting passed. Results: The resting heart rate was 42 beats per minute. The estimated theoretical maximal heart rate was 174 bpm. The monitored athlete overcame the 4 Everesting Bike Challenges covering a total of 904.79 km. The time taken to complete the race was 113 hours and 18 minutes. The total height difference exceeded was 35395 m. During the race the athlete pedaled with an average heart rate of 97 bpm. Body mass dropped from 64.0 to 63.1 kg between the start and end of the test. Wide variations in the athlete's Vigor (T0=16:T5=6), fatigue (T0=0:T5=6) and Sleep quality (T0=100:T5= ≈0) were found during the competition. Regarding the Rating of Perceived Exertion scale, the results obtained indicate a medium-low value (RP=3). Conclusion: The results of this study showed negligible reduction in body mass in the athlete who performed an ultra-endurance challenge. During and at the end of the climbing challenge, a significant reduction in Vigor and an important increase in Fatigue levels was highlighted, as well as a very evident reduction in Sleep quality. From the analysis of the RPE scale, medium-low values emerge at the end of each EB

    Lower limb posture and joint mobility in young Soccer players

    none8noSoccer practice can induce marked changes in the lower limbs with dreaded short and long term consequences. We evaluated the possible effects of sport practice on lower limb posture and their relationships with ankle joint mobility (AJM). In 61 male Soccer players mean age 11.6±1.8 years, and in 50 Volleyball and Basketball players matched for age, sex and body mass index, lower limb posture and AJM in both plantar flexion and dorsiflexion were evaluated on the sagittal plane in supine position using, respectively, images analysis and an inclinometer. A multivariate analysis of variance was carried out to assess if the practice of different sports affects the leg posture (FP: angle between foot and leg) and foot posture (LP: angle between the foot and the line perpendicular to the ground). The sport practiced showed a significant multivariate effect on the lower limb posture. Soccer players showed a greater LP angle (169.2±4.3° vs 164.9±4.5°; p<0.001) and a lesser FP angle in both Basketball and Volleyball players (298.0±12.6° vs 305.6±10.9°; p<0.002). These differences were not present between the Basketball and Volleyball players. Soccer players showed a reduced AJM (127.6±15.7° vs. 138.8±21.6°; p<0.002) compared to the other subjects evaluated. The AJM was found directly correlated to the FP angle (p<0.005). The results of this study indicate that young Soccer players could show an altered posture of the leg and foot and a reduced AJM.
